European Position Paper on Rhinosinusitis and Nasal Polyps 2020
The European Position Paper on Rhinosinusitis and Nasal Polyps 2020 is the update of similar evidence based position papers published in 2005 and 2007 and 2012. The core objective of the EPOS2020 guideline is to provide revised, up-to-date and clear evidence-based recommendations and integrated care pathways in ARS and CRS. EPOS2020 provides an update on the literature published and studies undertaken in the eight years since the EPOS2012 position paper was published and addresses areas not extensively covered in EPOS2012 such as paediatric CRS and sinus surgery. EPOS2020 also involves new stakeholders, including pharmacists and patients, and addresses new target users who have become more involved in the management and treatment of rhinosinusitis since the publication of the last EPOS document, including pharmacists, nurses, specialised care givers and indeed patients themselves, who employ increasing self-management of their condition using over the counter treatments. The document provides suggestions for future research in this area and offers updated guidance for definitions and outcome measurements in research in different settings. EPOS2020 contains chapters on definitions and classification where we have defined a large number of terms and indicated preferred terms. A new classification of CRS into primary and secondary CRS and further division into localized and diffuse disease, based on anatomic distribution is proposed. There are extensive chapters on epidemiology and predisposing factors, inflammatory mechanisms, (differential) diagnosis of facial pain, allergic rhinitis, genetics, cystic fibrosis, aspirin exacerbated respiratory disease, immunodeficiencies, allergic fungal rhinosinusitis and the relationship between upper and lower airways. The chapters on paediatric acute and chronic rhinosinusitis are totally rewritten. All available evidence for the management of acute rhinosinusitis and chronic rhinosinusitis with or without nasal polyps in adults and children is systematically reviewed and integrated care pathways based on the evidence are proposed. Despite considerable increases in the amount of quality publications in recent years, a large number of practical clinical questions remain. It was agreed that the best way to address these was to conduct a Delphi exercise. The results have been integrated into the respective sections. Last but not least, advice for patients and pharmacists and a new list of research needs are included.Peer reviewe
Implementing baseline ecological and human health field assessments in the Revitalizing Informal Settlements and their Environments (RISE) programme in Makassar, Indonesia: an interdisciplinary study

The Revitalizing Informal Settlements and their Environments (RISE) programme aims to assess the health, wellbeing, and ecological impacts of a water-sensitive-cities approach to improving urban informal settlements. Incorporating water-cycle management and green technology sanitation strategies, we aim to reduce flood risk and improve sanitation and waste water treatment leading to cleaner and healthier environments. Here we present the initial design pre-intervention for evaluation in the first 12 settlements in Makassar, Indonesia.
Methods
Initial environmental, and wellbeing and human health assessments were implemented starting in October, 2018, in 12 settlements in Makassar, Indonesia. Ecological assessments include measuring of temperature and humidity via iButtons installed in select homes, recording of bio-acoustic to measure biodiversity within settlement boundaries, and trapping disease vectors quarterly. Implemented evaluation of environmental contamination includes sampling water and soil sources for total coliforms as well as collecting soil via bootsocks by walking predefined transects. Human assessment includes an annual baseline survey of all settlement households, assessing self-reported symptoms, health-care system utilisation, and subjective wellbeing. Additionally, children younger than 5 years are surveyed quarterly for caregiver reported symptoms of diarrhoea and febrile illness, blood samples and anthropometry are being collected annually, and faeces samples are requested quarterly.
Findings
Ecological assessments have provided more than a million temperature data points. 21 000 mosquitos have been captured and identified. A total of 114 water samples, 84 bootsocks, and 91 soil samples have been collected, with sampling prior to and during the wet season. We have identified over 600 households within the 12 settlements. Health assessments of children under the age of 5 years have revealed 282 children with collection of 234 faeces samples and 188 blood samples.
Interpretation
We have successfully implemented baseline ecological and human health and wellbeing assessment tools in all 12 settlements, which will allow for the evaluation of water-sensitive-cities approach in RISE programme.https://doi.org/10.1016/S2542-5196(19)30151-23Suppl

Diverse secondary outcomes include changes in microbial contamination; abundance and diversity of pathogens and AMR genes in environmental samples; impacts on ecological biodiversity and microclimates; mosquito vector abundance; anthropometric assessments, nutrition markers and systemic inflammation in children; caregiver-reported and self-reported health symptoms and healthcare utilisation; and measures of individual and community psychological, emotional and economic well-being. A planetary health model for reducing exposure to faecal contamination in urban informal settlements: Baseline findings from Makassar, Indonesia

The intense interactions between people, animals and environmental systems in urban informal settlements compromise human and environmental health. Inadequate water and sanitation services, compounded by exposure to flooding and climate change risks, expose inhabitants to environmental contamination causing poor health and wellbeing and degrading ecosystems. However, the exact nature and full scope of risks and exposure pathways between human health and the environment in informal settlements are uncertain. Existing models are limited to microbiological linkages related to faecal-oral exposures at the individual level, and do not account for a broader range of human-environmental variables and interactions that affect population health and wellbeing.
Methods
We undertook a 12-month health and environmental assessment in 12 flood-prone informal settlements in Makassar, Indonesia. We obtained caregiver-reported health data, anthropometric measurements, stool and blood samples from children < 5 years, and health and wellbeing data for children 5–14 years and adult respondents. We collected environmental data including temperature, mosquito and rat species abundance, and water and sediment samples. Demographic, built environment and household asset data were also collected. We combined our data with existing literature to generate a novel planetary health model of health and environment in informal settlements.
Results
Across the 12 settlements, 593 households and 2764 participants were enrolled. Two-thirds (64·1%) of all houses (26·3–82·7% per settlement) had formal land tenure documentation. Cough, fever and diarrhoea in the week prior to the survey were reported among an average of 34.3%, 26.9% and 9.7% of children aged < 5 years, respectively; although proportions varied over time, prevalence among these youngest children was consistently higher than among children 5–14 years or adult respondents. Among children < 5 years, 44·3% experienced stunting, 41·1% underweight, 12.4% wasting, and 26.5% were anaemic. There was self- or carer-reported poor mental health among 16.6% of children aged 5–14 years and 13.9% of adult respondents. Rates of potential risky exposures from swimming in waterways, eating uncooked produce, and eating soil or dirt were high, as were exposures to flooding and livestock. Just over one third of households (35.3%) had access to municipal water, and contamination of well water with E. coli and nitrogen species was common. Most (79·5%) houses had an in-house toilet, but no houses were connected to a piped sewer network or safe, properly constructed septic tank. Median monthly settlement outdoor temperatures ranged from 26·2 °C to 29.3 °C, and were on average, 1·1 °C warmer inside houses than outside. Mosquito density varied over time, with Culex quinquefasciatus accounting for 94·7% of species. Framed by a planetary health lens, our model includes four thematic domains: (1) the physical/built environment; (2) the ecological environment; (3) human health; and (4) socio-economic wellbeing, and is structured at individual, household, settlement, and city/beyond spatial scales.
Conclusions
Our planetary health model includes key risk factors and faecal-oral exposure pathways but extends beyond conventional microbiological faecal-oral enteropathogen exposure pathways to comprehensively account for a wider range of variables affecting health in urban informal settlements. It includes broader ecological interconnections and planetary health-related variables at the household, settlement and city levels. Unpaving the Way to Creek Restoration in Lower Sausal Creek Watershed: Applying the EU Water Framework Directive to a US Urban Watershed
The European Union Water Framework Directive (WFD) provides a strategy for the protection, improvement and restoration of water bodies across Europe. However, in urbanized areas where the drainage network has been engineered for flood conveyance and floodplains have been densely developed, the cost of restoration is usually disproportionate to the ecological benefits such restoration would provide. This project applies the EU WFD to the densely urbanized Lower Sausal Creek Watershed in Oakland, California. While the WFD provides economic insight, recent popularity of stormwater intervention strategies in US urban areas offer alternatives to in-stream creek restoration with additional community benefits. Our Lower Sausal Creek Watershed Stormwater Management Plan addresses stormwater pollution and detention through small, cost-effective landscape features at the lot level. The strategies include retention/ conveyance swales with trees, neighborhood trees, large lot interventions and the use of native plants, which altogether would deal with 100% of 2-year storm events in the lower watershed as well as allow partial detention of larger storms

Blackberry Creek Daylighting Project, Berkeley : Ten-Year Post-Project Appraisal
Blackberry Creek drains a 0.3-square-mile watershed, flowing from the northeastern hills of Berkeley, California into the Marin Creek culvert and then to the San Francisco Bay. A 200-foot reach running under Thousand Oaks Elementary School was daylighted in 1995 by Wolfe Mason Associates in collaboration with the Urban Creeks Council. The goals were to provide an outdoor science lab for the school and an alternative to a culvert with a history of flooding.Post-project appraisals conducted in 1996 and 2000 focused on geomorphic and biological aspects, and found sufficient flood control capacity and greater density of riparian vegetation than envisioned in project design. We conducted a PPA ten years after project completion, surveying the longitudinal profile and two cross sections of the creek. We also looked at historical rainfall data and identified a 10-year event in 2002. Comparing our data to previous PPAs, channel flood capacity and gradient appear stable although the channel itself may have migrated within the high bankfull. Bank vegetation has become even denser, reflecting a lack of maintenance.Previous PPAs documented tension relating to perceived use and design among diverse groups such as the School, the Neighborhood Association, and a Tai Chi group that used the park and tot lot. To get a sense of community perception and use ten years post-project, we interviewed the Thousand Oaks science teacher, past and current presidents of the Thousand Oaks Neighborhood Association, and the chair of the Urban Creeks Council at the time of daylighting. Today, the school is using the site as a regular science lab and the initial tension seems to have dissipated into general public acceptance
Boundaries as Spaces of Knowledge Integration: Learning from transdisciplinary collaboration on planetary health in Indonesia
Introduction: Deepening global inequalities in the health impacts of climate change highlight the need for transformative solutions through international and transdisciplinary collaborations. While the emerging field of planetary health provides a unique lens for recognizing interlinkages across a broader range of knowledge systems, a deeper understanding is needed about the processes through which such knowledge systems can be developed and integrated. Existing transdisciplinarity scholarship offers useful concepts of integration across boundaries; however, such understanding predominantly reflects the perspectives of Global North academic stakeholders, conceivably due to systemic power imbalance as an enduring colonial legacy. This study aims to identify opportunities for learning from the experiences of Global South stakeholders in transdisciplinary collaboration. Methods: We empirically explore the process of transdisciplinary collaboration in a case study of a large-scale planetary health research project. Through multi-method thematic analysis, this study seeks to understand Global South stakeholders’ contributions, motivations, and interactions on transdisciplinary collaboration, through their experiences in the case study context. Results & Discussion: The study found that Global South stakeholders contributed a plethora of disciplinary and non-disciplinary knowledge and other resources, guided by strong cultural inclinations for collaboration. The opening up of boundary spaces was key to multi-directional knowledge integration. Analysis revealed concepts of interdependence and complementarity towards a common vision, and provides insight into stakeholders’ motivations for initial and continuing engagement. Conclusion: Recognizing interdependence provides strong motivation for transdisciplinary collaboration and can help revalorize contributions from historically disadvantaged knowledge systems and stakeholders
