    Epidemija pandemijskog gripa H1N1 u Vojnoj gimnaziji

    Background/Aim. The first cases of the pandemic pH1N1 influenza virus infection was observed in the United States and Mexico in April 2009 and the first laboratory confirmed case in Serbia was registered in June 2009. The aim of this paper was to report on the investigation of the first confirmed outbreak of the 2009 pandemic H1N1 influenza in Serbia and to describe the clinical and epidemiologic findings from this investigation. Methods. Descriptive and analytical epidemiological methods were used. Data were collected from medical records of the Military School students and epidemiological questionnaire. Pandemic H1N1 infection was initially confirmed by the RT-PCR assay in nasopharyngeal and oropharyngeal swabs and subsequently by the complement fixation test in serum samples. Results. The attack rate of acute respiratory illness was 70.8% (204/288). Pandemic H1N1 virus infection was confirmed in 44 of 82 tested cases of acute respiratory illness (53.7%) The most common clinical manifestations of pandemic influenza H1N1 were fever (88.6%), cough (61.4%), malaise (38.6%), runny nose (36.4%), headache (29.6%), sore throat (20.5%) and muscle pain (15.9%). Conclusion. The findings from this investigation suggest that pandemic H1N1 influenza in a high military school was widespread but did not cause severe illness.Uvod/Cilj. Prvi slučajevi pandemijske infekcije virusom influence pH1N1 ustanovljeni su u Americi i Meksiku u aprilu 2009. a u Srbiji prvi laboratorijski potvrđen slučaj registrovan je u junu 2009. godine. Cilj rada bio je da se prikaže istraživanje prve dokazane epidemije pandemijske influence H1N1 u Srbiji i opišu klinički i epidemiološki nalazi iz ovog istraživanja. Metode. Primenjen je deskriptivni i analitički epidemiološki metod. Izvor podataka bila je medicinska dokumentacija učenika Vojne gimnazije i epidemiološki upitnik. Infekcija pandemijskim virusom H1N1 prvo je dokazana pomoću RT-PCR u nazofaringealnim i orofaringealnim brisevima, a zatim i reakcijom vezivanja komlementa u uzorcima seruma. Rezultati. Stopa javljanja akutnog respiratornog oboljenja iznosila je 70.8% (204/288). Infekcija pandemijskim H1N1 virusom potvrđena je kod 44 od 82 testiranih slučajeva akutnog respiratornog oboljenja (53,7%). Najčešće kliničke manifestacije pandemijske influence H1N1 bile su povišena temperatura (88,6%), kašalj (61,4%), malaksalost (38,6%), rinitis (36,4%), glavobolja (29,6%), gušobolja (20,5%) i bol u mišićima (15,9%). Zaključak. Rezultati ovog istraživanja ukazuju da je pandemijska influenca H1N1 bila raširena u Vojnoj gimnaziji, ali da nije izazvala teške forme oboljenja

    Gender Differences in Relation to Knowledge and Risky Behavior among Students of Medical Colleges in Serbia: Study of Prevalence

    The aim of this study was to determine the level of knowledge about sexually transmitted diseases and the frequency of behavior presenting risk for acquiring those diseases among students of Medical colleges in relation to gender. The study is based on a cross-sectional survey conducted in two Medical colleges in Belgrade in December 2016. A total of 670 students (96%) completed an epidemiological questionnaire. Respondents were selected through random sampling, they took part in the study voluntarily and filled in the questionnaire anonymously and independently. The questionnaire included 18 questions on demographic characteristics, knowledge and risky behavior. Female students were significantly older than male students (p= 0.014). Male students were more often single, while female students were mostly in the relationship or married (p< 0.001). Female students have more knowledge about symptoms of sexually transmitted diseases (p= 0.008) and more than males are familiar with the causality of HPV infection and cervical cancer (p= 0.002). Males students earlier had the first sexual intercourse (p< 0.001), had sex on the first date (p< 0.001), changed sexual partners during their lives (p< 0.001), and paid for sex (p< 0.001). Male students have specific risk factors for unhealthy sexual behavior. It is necessary to upgrade the education programs and implement population-targeted prevention and control measures for sexually transmitted infections

    HIV/AIDS and other sexually transmitted infections among military members of the armed forces of Serbia

    Background/Aim. Military personnel is a population group at special risk of exposure to sexually transmitted diseases (STD). In peacetime, STD infection rates among service members are generally 2 to 5 times higher than among civilian population. In time of conflict, the differences can be 50 or more times greater. This study describes sexual behavior as a risk factor for STD in the Armed Forces of Serbia. Methods. The sample of 5 617 voluntary blood donors from the Armed Forces of Serbia gave blood and filled World Health Organization Questionnaire about sexual behavior within January 2007 - December 2008 period. The mandatory testing of voluntary blood donors was performed in the Institute of Transfusiology Military Medical Academy in Belgrade, by the specific immunoenzyme tests and polymerasa chain reaction tests for HIV, hepatitis B, C and syphilis. Statistical analysis of data was done using State for Windows 93, USA, 1996. Results. We identified 36 soldiers with some form of STDs. This study showed that 1 668 (29.7%) tested soldiers reported always using condoms, 1 725 (30.72%) almost always, 1 238 (20.04%) sometimes, 495 (8.81%) almost never and 490 (8.73%) never. Among the sample, 449 (7.99%) soldiers reported sexual contacts with partners with high risk of sexual behavior, whilst 22 (0.37%) of them reported homosexual and bisexual contacts. Conclusion. This study reported STDs found in voluntary blood donors among the service members of the Armed Forces of Serbia, but none of them was identified to be HIV positive. Soldiers with the most frequent risk behavior were reported to be those with inconsistent condom use. In the future, the STD Control and Prevention Program should be more intensively conducted among the members of the Armed Forces of Serbia

    The impact of an educational film on promoting knowledge and attitudes toward HIV in soldiers of the Serbian armed forces

    Background/Aim. Millions of soldiers around the world represent one of the most vulnerable populations regarding exposure to human immunodeficiency virus (HIV) infection. The programs for HIV prevention remain the most viable approach to reducing the spread of HIV infection. Very few studies have tested the effectiveness of HIV preventive interventions undertaken in military population. The aim of this study was to determine the effectiveness of educational film to transfer knowledge about HIV infection to soldiers. Methods. We performed a quasi-experimental study among 102 soldiers of the Serbian Armed Forces. The experimental intervention consisted of the HIV knowledge pre-questionnaire, watching a film on HIV knowledge, then the post-HIV knowledge questionnaire. The results of pre-and post-HIV knowledge questionnaires were compared. Results. There were 23 questions in the test. The average total score on the questionnaire before watching the film was 18.23 and after watching it was 20.14, which was statistically significant difference (p < 0.001). Conclusions. The results of the study show that viewing a film on HIV infection is an effective method of transferring knowledge about HIV to the Serbian military population

    <it>In vitro</it> antitumor actions of extracts from endemic plant <it>Helichrysum zivojinii</it>

    Abstract Background The aim of this research was to determine the intensity and mechanisms of the cytotoxic actions of five extracts isolated from the endemic plant species Helichrysum zivojinii Černjavski & Soška (family Asteraceae) against specific cancer cell lines. In order to evaluate the sensitivity of normal immunocompetent cells implicated in the antitumor immune response, the cytotoxicity of extracts was also tested against healthy peripheral blood mononuclear cells (PBMC). Methods The aerial parts of the plants were air-dried, powdered, and successively extracted with solvents of increasing polarity to obtain hexane, dichloromethane, ethyl-acetate, n-butanol and methanol extracts. The cytotoxic activities of the extracts against human cervix adenocarcinoma HeLa, human melanoma Fem-x, human myelogenous leukemia K562, human breast adenocarcinoma MDA-MB-361 cells and PBMC were evaluated by the MTT test. The mode of HeLa cell death was investigated by morphological analysis. Changes in the cell cycle of HeLa cells treated with the extracts were analyzed by flow cytometry. The apoptotic mechanisms induced by the tested extracts were determined using specific caspase inhibitors. Results The investigated Helichrysum zivojinii extracts exerted selective dose-dependent cytotoxic actions against selected cancer cell lines and healthy immunocompetent PBMC stimulated to proliferate, while the cytotoxic actions exerted on unstimulated PBMC were less pronounced. The tested extracts exhibited considerably stronger cytotoxic activities towards HeLa, Fem-x and K562 cells in comparison to resting and stimulated PBMC. It is worth noting that the cytotoxicity of the extracts was weaker against unstimulated PBMC in comparison to stimulated PBMC. Furthermore, each of the five extracts induced apoptosis in HeLa cells, through the activation of both intrinsic and extrinsic signaling pathways. Conclusion Extracts obtained from the endemic plant Helichrysum zivojinii may represent an important source of novel potential antitumor agents due to their pronounced and selective cytotoxic actions towards malignant cells.</p

    Catheter-associated urinary tract infection in a surgical intensive care unit

    Background/Aim. Because patients in intensive care units usually have an urinary catheter, the risk of urinary tract infection for these patients is higher than in other patients. The aim of this study was to identify risk factors and causative microrganisms in patients with catheter-associated urinary tract infection (CAUTI) in the Surgical Intensive Care Unit (SICU) during a 6-year period. Methods. All data were collected during prospective surveillance conducted from 2006 to 2011 in the SICU, Military Medical Academy, Belgrade, Serbia. This case control study was performed in patients with nosocomial infections recorded during surveillance. The cases with CAUTIs were identified using the definition of the Center for Disease Control and Prevention. The control group consisted of patients with other nosocomial infections who did not fulfill criteria for CAUTIs according to case definition. Results. We surveyed 1,369 patients representing 13,761 patient days. There were a total of 226 patients with nosocomial infections in the SICU. Of these patients, 64 had CAUTIs as defined in this study, and 162 met the criteria for the control group. Multivariate logistic regression analysis identified two risk factors independently associated to CAUTIs: the duration of having an indwelling catheter (OR = 1.014; 95% CI 1.005-1.024; p = 0.003) and female gender (OR = 2.377; 95%CI 1.278-4.421; p = 0.006). Overall 71 pathogens were isolated from the urine culture of 64 patients with CAUTIs. Candida spp. (28.2%), Pseudomonas aeruginosa (18.3%) and Klebsiella spp. (15.5%) were the most frequently isolated microorganisms. Conclusions. The risk factors and causative microrganisms considering CAUTIs in the SICU must be considered in of planning CAUTIs prevention in this setting

    Prevalence and risk factors of Chlamydia trachomatis genital infection among military personnel of the Armed Forces of Serbia: A cross-sectional study

    Background/Aim. Chlamydia trachomatis infection (CTI) is an increasing public health problem worldwide and is the most frequent sexually transmitted infection. Studies conducted in many armed forces worldwide showed that CTI is common within military population and generate significant healthcare costs. The aim of this study was to estimate the prevalence of CTI among members of the Serbian Armed Forces (SAF) and to determine risk factors for this infection. Methods. The study was designed as a cross-sectional survey and consisted of completing a questionnaire and chlamydial testing. The questionnaire was divided into two question groups: one was about demographic/service related characteristics and the other was about behavioral patterns/ habits. Chlamydia trachomatis real time polymerase chain reaction (Real-TM PCR) was used for detection of pathogen genome specific sequence in the male urethral swabs and in the female endocervical swabs. All data collected were used to compare military personnel with and without CTI. Risk factors independently associated with CTI were identified by the stepwise multivariate logistic regression analysis (MLRA) of variables selected by the univariate logistic regression analysis (ULRA), with a limit for entering and removing variables from the model at 0.05. Results. The overall prevalence of CTI was 55 of 356 respondents (15.4%; 95% CI 0.5–2.7%). The ULRA identified that CTI was significantly associated with several characteristics: number of sexual partners during previous year (p = 0.008), knowledge about symptoms of CTI (p = 0.035), tattooing (p = 0.035) and married or in stable relationship (p = 0.022). The MLRA revealed that number of different sexual partners during last year was independent risk factor of CTI (p = 0.026; OR : 0.344; 95% CI: 0.13–0.88). Conclusion. CTI is significant problem in male and female military personnel in the SAF. The number of different sexual partners during previous year was independently associated with CTI. These finding indicates that screening for CTI should be undertaken in the SAF, to reduce rates of CTI in the SAF and to prevent morbidity due to this infection
