    Contemporary Women’s Adaptation to Motherhood: The First 3 to 6 Weeks Postpartum

    A better understanding of the process of adaptation to motherhood should enhance nurses’ ability to prepare women for the transition to motherhood and to provide care following childbirth. Knowledge about women’s adaptation to motherhood was developed primarily from the 1960s to the 1990s. Cesarean birthing was a special focus of research throughout the late 1970s and the 1980s, followed by functional status, and more recently, depression and stress associated with birth and postpartum. Adaptation to motherhood in the context of the early 21st century has received limited attention in nursing research, creating an assumption that the process of adaptation is universal and context-free rather than evolving within the life and societal context of women across generations. Although becoming and being a mother has been described as a normative transition rather than a stressor by some, knowledge development about adaptation to motherhood also has been constrained by the limited use of a unified perspective of transition as a process of adaptation. Therefore, the purposes of this exploratory study were to describe contemporary women’s physical, emotional, functional, and social adaptation to motherhood and to examine the relations of selected demographic and perinatal variables to adaptation to motherhood in the first 3 to 6 weeks of the postpartum

    Teaching, Practice, and Research: An Integrative Approach Benefiting Students and Faculty

The project involved using information gathered as part of baccalaureate nursing students\u27 assessments of women\u27s perceptions of and responses to cesarean birth to extend a program of nursing research. Benefits to student learning and faculty research programs are identified. The project serves as a prototype for the teaching, practice, and research activities of all students and faculty in nursing and other professional disciplines

    Adaptation to Cesarean Birth: Implementation of an International Multisite Study

    The purpose of this column is to describe the implementation of an international multisite Roy adaptation model-based study of women’s perceptions of and responses to cesarean birth. The need for the study arose from the concern that women’s childbearing needs may not be met to their full satisfaction, especially if the infant is born by cesarean. Serendipity and networking played a part in the selection of four study sites in the United States (Boston, Milwaukee, Norfolk, Oklahoma City) and two in other countries (Finland, Australia). Data were collected by nursing students and staff nurses. Post-hoc consideration of the diversity of study sites revealed opportunities for examination of the influence of the contextual stimuli of culture and geographic region on the women’s adaptation to cesarean birth. Strategies used to foster integration of teaching, practice, and research are discussed

    Women’s Perceptions of Caesarean Birth: A Roy International Study

    The purpose of this Roy adaptation model-based multi-site international mixed method study was to examine the relations of type of caesarean birth (unplanned/planned), number of caesarean births (primary/repeat), and preparation for caesarean birth to women’s perceptions of and responses to caesarean birth. The sample included 488 women from the United States (n = 253), Finland (n = 213), and Australia (n = 22). Path analysis revealed direct effects for type of and preparation for caesarean birth on responses to caesarean birth, and an indirect effect for preparation on responses to caesarean birth through perception of birth the experience

    Bacterial Cell Envelope Lysis and Hemotoxicity of Peptides Previously isolated From African Catfish, Clarias gariepinus

    Background: The skin mucus layer of fish is endowed with biologics including, Antimicrobial peptides (AMPs) that offer a first line of defence against pathogens. Such peptides can either inhibit bacterial growth or completely kill the bacteria and hence are regarded as a viable alternative to traditional antibiotics, in addressing the ever-increasing incidences of antimicrobial resistance. However, one of the major hurdles to AMPs use is their poor haemolytic profile. As a result, a thorough evaluation of prospective AMPs’ bacterial cell membrane disruption and hemolytic potentials in the early phases of drug discovery is critical. The current study presented cell membrane destruction as well as hemo-compatibility of antimicrobial peptides previously isolated from skin mucus of African catfish, Clarias gariepinus.Methods: A previously isolated antimicrobial peptide in the skin mucus of African catfish, C. gariepinus were profiled using 15% Sodium Dodecyl Sulfate-Polyacrylamide Gel Electrophoresis (SDS-PAGE). The electrical conductivity and alkaline phosphatase assays were utilised to measure bacterial cell envelope lysis activity as a classical mode of action of the antimicrobial peptides. Afterwards, fresh Rabbit blood cells were then utilised for in vitro hemolytic assay.Results: The peptides were found to be about 5 kDa molecular weight with, ability to damage the bacterial cell envelope causing significant leakage in periplasmic alkaline phosphatase enzyme and cytoplasmic electrolytes. Even at the highest peptide extract concentration of 100 μg/mL, no significant hemolysis was observed on the fresh rabbit blood cells [3.63%;P>.05], signifying their safety on normal mammalian cells.Conclusion: The findings of this study pointed out that antimicrobial peptides in skin mucus of C. gariepinus are potentially safe source of antimicrobial drug leads; however, further studies are still required to search for possibly maximum dose that is safe to host cells but still effective against infecting bacteria

Even so, poor pharmacokinetic profiles of certain AMPs impede their utility necessitating, a careful assessment of potential AMPs’ absorption, distribution, metabolism, excretion, and toxicity (ADMET) properties during novel lead exploration. Accordingly, the present study utilized ADMET scores to profile seven previously isolated African catfish antimicrobial peptides (ACAPs). After profiling, the peptides were docked against approved bacterial protein targets to gain insight into their possible mode of action. Promising ACAPs were then chemically synthesized, and their antibacterial activity was validated in vitro utilizing the broth dilution method. All seven examined antimicrobial peptides passed the ADMET screening, with two (ACAP-IV and ACAP-V) exhibiting the best ADMET profile scores. The ACAP-V had a higher average binding energy (−8.47 kcal/mol) and average global energy (−70.78 kcal/mol) compared to ACAP-IV (−7.60 kcal/mol and −57.53 kcal/mol), with the potential to penetrate and disrupt bacterial cell membrane (PDB Id: 2w6d). Conversely, ACAP-IV peptide had higher antibacterial activity against E. coli and S. aureus (Minimum Inhibitory Concentration, 520.7 ± 104.3 μg/ml and 1666.7 ± 416.7 μg/ml, respectively) compared to ACAP-V. Collectively, the two antimicrobial peptides (ACAP-IV and ACAP-V) are potential novel leads for the food, cosmetic and pharmaceutical industries. Future research is recommended to optimize the expression of such peptides in biological systems for extended evaluation

    Effects of lay support for pregnant women with social risk factors on infant development and maternal psychological health at 12 months postpartum

    Background The ELSIPS (Evaluation of Lay Support in Pregnant Women with Social Risk) RCT showed that lay support for women with social risk had a positive effect on maternal mental health and mother-infant bonding. This exploratory study examined whether these observed benefits would impact infant development at 1 year. Methods A sub-sample of women whose infants were under one year who had participated in the ELSIPS RCT which randomised women to receive either standard care or the services of a Pregnancy Outreach Worker (POW), and who were contactable, were eligible to participate in the follow up. At home visits, the Bayley Scales of Infant Development (3rd Edition) and standardised measures of depression, self efficacy, mind-mindedness and bonding were completed. Results 486 women were eligible for follow up, of whom 154 agreed to participate. 61/273 were successfully followed up in the standard maternity care arm and 51/213 in the POW arm. Women who completed follow up were less depressed and had higher selfefficacy scores at 8–12 weeks postpartum than those who did not complete follow up. There were no significant differences in maternal outcomes, infant cognitive development, receptive communication, expressive communication, fine motor development or social/emotional functioning between groups at 12 month follow up. Infants of mothers who received the POW intervention had significantly better gross motor development than infants whose mothers received standard care (p<0.03)

    A review of Serenoa serrulata: A potential monotherapy medicinal plant for prostatic disease in combination with other therapies

    Background: Prostatic diseases which include prostatitis, benign prostatic hyperplasia (BPH) and prostate cancer are the benign or malignant disorders that affect the prostate. Phytotherapies have been adopted as the alternative treatment/ management option especially for BPH since the current modern methods of treatment presents a lot of adverse effects. Methodology: The literature was searched using different databases including Medline/PubMed, Cochrane library, Scopus, Proquest library, Embase, EBooks and Google Scholar for relevant records for a period from 1988 to 2021 to identify all the published articles of S. serrulata regarding treatment of prostatic diseases. The key search terms were Serenoa serrulata, S. repens, Saw palmetto, Prostate cancer treatment with Serenoa serrulata, treatment of Benign Prostatic Hyperplasia with Serenoa serrulata,&nbsp; phytochemicals of Serenoa serrulata, ethnobotanical uses of Serenoa serrulata, toxicity of Serenoa serrulata, pharmacological activities of Serenoa serrulata and also traditional management and treatment of prostatic diseases&nbsp; using Serenoa serrulata and also clinical trials on treatment of prostatic diseases&nbsp; with Serenoa serrulata. The retrieved articles were reviewed, synthesized and analyzed qualitatively. The reference list of the retrieved articles was also reviewed and synthesized. The original research articles which reported an investigation of S. serrulata of any study design, original published research articles, any time of publication and grey literature (conference papers, reported articles, academic thesis) were included. The articles whose full texts were not freely available by the time of search and those without clear information about methodology and study design were excluded. Results: This review reported that Serenoa serrulata belonging to the Arecaceae family commonly known as saw palmetto is used traditionally for treating prostatic disease conditions and other infertility conditions in both men and women. Phytochemical screening of hexanic and ethanolic extracts of S. serrulata comprised of free fatty acids and phytosterols which together contribute to their antiprostatic activities. These extracts of S. serrulata exhibited antiandrogenic, anti-inflammatory and anti-proliferative activities through inhibition of both isoenzymes 5α- reductase and inhibition of binding of dihydrotestosterone (DHT) to the cytosolic androgen receptors. This is a similar mechanism exhibited by finasteride and Tamsulosin both antiprostatic conventional drugs though the plant phytochemicals do not interfere with PSA secretion. S. serrulata has also been reported to be non-toxic in both non-clinical and clinical trial studies. The medicinal plants reported by this review to be used in combination include; stinging nettle (Urtica dioca), Zingiber officinalis, Echinacea angustifolia and pumpkin (Cucurbita pepo). The antiprostatic conventional drugs reported include Finasteride and Tamsulosin. Conclusion and Recommendation: The results showed that S. serrulata is effective in treating prostatic diseases. The potency and safety are improved when used in combination with Urtica dioca, Cucurbita pepo, Zingiber officinalis and Echinacea angustifolia as compared with anti-prostatic conventional drugs Finasteride and Tamsulosin alone. The plant combination has also been shown to have improvement in the quality of life and as well enhancing the synergy of Finasteride and Tamsulon and their adverse effects. Effective medicinal plant combinations should be formulated into products and integrated into the usual treatment for prostatic diseases

    Effects of chronic nitrogen amendment on dissolved organic matter and inorganic nitrogen in soil solution

    Increased atmospheric deposition of N to forests is an issue of global concern, with largely undocumented long-term effects on soil solution chemistry. In contrast to bulk soil properties, which are typically slow to respond to a chronic stress, soil solution chemistry may provide an early indication of the long-term changes in soils associated with a chronic stress. At the Harvard Forest, soil solution was collected beneath the forest floor in zero tension lysimeters for 10 years (1993-2002) as part of an N saturation experiment. The experiment was begun in 1988 with 5 or 15 g N m(-2) per year added to hardwood and pine forest plots, and our samples thus characterize the long-term response to N fertilization. Samples were routinely analyzed for inorganic nitrogen, dissolved organic nitrogen (DON), and dissolved organic carbon (DOC); selected samples were also analyzed to determine qualitative changes in the composition of dissolved organic matter. Fluxes of DOC, DON, and inorganic N were calculated based on modeled water loss from the forest floor and observed concentrations in lysimeter samples. The concentration and flux of inorganic N lost from the forest floor in percolating soil solution are strongly affected by N fertilization and have not shown any consistent trends over time. On average, inorganic N fluxes have reached or exceeded the level of fertilizer application in most plots. Concentrations of DOC were unchanged by N fertilization in both the hardwood and pine stands, with long-term seasonal averages ranging from 31-57 mg l(-1) (hardwood) and 36-93 mg l(-1) (pine). Annual fluxes of DOC ranged from 30-50 g m(-2) per year. DON concentrations more than doubled, resulting in a shift toward N-rich organic matter in soil solution percolating from the plots, and DON fluxes of 1-3 g m(-2) per year. The DOC:DON ratio of soil solution under high N application (10-20) was about half that of controls. The organic chemistry of soil solution undergoes large qualitative changes in response to N addition. With N saturation, there is proportionally more hydrophilic material in the total DON pool, and a lower C:N ratio in the hydrophobic fraction of the total DOM pool. Overall, our data show that fundamental changes in the chemistry of forest floor solution have occurred in response to N fertilization prior to initiation of our sampling. During the decade of this study (years 5-14 of N application) both inorganic N and dissolved organic matter concentrations have changed little despite the significant biotic changes that have accompanied N saturation.
