    A case report of Vagus nerve stimulation for intractable hiccups

    Introduction: Intractable hiccups frequently result from an underlying pathology and can cause considerable illness in the patients. Initial remedies such as drinking cold water, induction of emesis, carotid sinus massage or Valsalva manoeuvre all seem to work by over stimulating the Vagus nerve. Pharmacotherapy with baclofen, gabapentin and other centrally and peripherally acting agents such as chlorpromazine and metoclopramide are reserved as second line treatment. Medical refractory cases even indulge in unconventional therapies such as hypnosis, massages and acupuncture. Surgical intervention, although undertaken very rarely, predominantly revolves around phrenic nerve crushing, blockade or pacing. A novel surgical strategy is emerging in the form of Vagus nerve stimulator (VNS) placement with three cases cited in literature to date with varying degrees of success. Here the authors report a case of VNS placement for intractable hiccups with partial success, in accordance with SCARE-2018 guidelines. Presentation of the case: An 85-year-old gentleman with a 9-year history of intractable hiccups secondary to pneumonia came to our hospital. The hiccups were symptomatic causing anorexia, insomnia, irritability, depression, exhaustion, muscle wasting and weight loss. The patient underwent countless medical evaluations. All examinations and investigations yielded normal results. The patient underwent aggressive pharmacotherapy, home remedies and unconventional therapies for intractable hiccups but to no avail. He also underwent left phrenic nerve blocking and resection without therapeutic success. The patient presented to our hospital and decision for VNS insertion was taken for compassionate reasons considering patient morbidity. The patient demonstrated significant improvement in his symptoms following VNS insertion. Discussion: A temporary hiccup is an occasional happening experienced by everyone. However, intractable hiccups are associated with significant morbidity and often mortality. Several medical, pharmacological, surgical and novel treatment options are available for intractable hiccups. Conclusion: VNS insertion is a novel surgical option for the treatment of intractable hiccups

    Bi-local Construction of Sp(2N)/dS Higher Spin Correspondence

    We derive a collective field theory of the singlet sector of the Sp(2N) sigma model. Interestingly the hamiltonian for the bilocal collective field is the same as that of the O(N) model. However, the large-N saddle points of the two models differ by a sign. This leads to a fluctuation hamiltonian with a negative quadratic term and alternating signs in the nonlinear terms which correctly reproduces the correlation functions of the singlet sector. Assuming the validity of the connection between O(N) collective fields and higher spin fields in AdS, we argue that a natural interpretation of this theory is by a double analytic continuation, leading to the dS/CFT correspondence proposed by Anninos, Hartman and Strominger. The bi-local construction gives a map into the bulk of de Sitter space-time. Its geometric pseudospin-representation provides a framework for quantization and definition of the Hilbert space. We argue that this is consistent with finite N grassmanian constraints, establishing the bi-local representation as a nonperturbative framework for quantization of Higher Spin Gravity in de Sitter space.Comment: 1 figur

    Four small puzzles that Rosetta doesn't solve

    A complete macromolecule modeling package must be able to solve the simplest structure prediction problems. Despite recent successes in high resolution structure modeling and design, the Rosetta software suite fares poorly on deceptively small protein and RNA puzzles, some as small as four residues. To illustrate these problems, this manuscript presents extensive Rosetta results for four well-defined test cases: the 20-residue mini-protein Trp cage, an even smaller disulfide-stabilized conotoxin, the reactive loop of a serine protease inhibitor, and a UUCG RNA tetraloop. In contrast to previous Rosetta studies, several lines of evidence indicate that conformational sampling is not the major bottleneck in modeling these small systems. Instead, approximations and omissions in the Rosetta all-atom energy function currently preclude discriminating experimentally observed conformations from de novo models at atomic resolution. These molecular "puzzles" should serve as useful model systems for developers wishing to make foundational improvements to this powerful modeling suite.Comment: Published in PLoS One as a manuscript for the RosettaCon 2010 Special Collectio

    Validity of a tool designed to assess the preventability of potentially preventable hospitalizations for chronic conditions

    Background: Potentially preventable hospitalizations (PPH) are defined as unplanned hospital admissions which could potentially have been prevented with the provision of effective, timely outpatient care. To better understand and ultimately reduce rates of PPH, a means of identifying those which are actually preventable is required. The Preventability Assessment Tool (PAT) was designed for use by hospital clinicians to assess the preventability of unplanned admissions for chronic conditions. Objective: The present study examined the ability of the PAT to distinguish between those unplanned admissions which are preventable and those which are not, compared to the assessments of an Expert Panel. Methods: Data were collected between November 2014 and June 2017 at three hospitals in NSW, Australia. Participants were community-dwelling patients with unplanned hospital admissions for congestive heart failure, chronic obstructive pulmonary disease, diabetes complications or angina pectoris. A nurse and a doctor caring for the patient made assessments of the preventability of the admission using the PAT. Expert Panels made assessments of the preventability of each admission based on a comprehensive case report and consensus process. Results: There was little concordance between the hospital doctors and nurses regarding the preventability of admissions, nor between the assessments of the Expert Panel and the hospital nurse or the Expert Panel and the hospital doctor. Conclusions: The PAT demonstrated poor concurrent validity and is not a valid tool for assessing the preventability of unplanned hospital admissions. The use of Expert Panels provides a more rigorous approach to assessing the preventability of such admissions

    Prompt and accurate diagnosis of ventricular arrhythmias with a novel index based on phase space reconstruction of ECG

The final version is available from Elsevier via the DOI in this record.Aim To develop a statistical index based on the phase space reconstruction (PSR) of the electrocardiogram (ECG) for the accurate and timely diagnosis of ventricular tachycardia (VT) and ventricular fibrillation (VF). Methods Thirty-two ECGs with sinus rhythm (SR) and 32 ECGs with VT/VF were analyzed using the PSR technique. Firstly, the method of time delay embedding were employed with the insertion of delay “τ” in the original time-series X(t), which produces the Y(t) = X(t − τ). Afterwards, a PSR diagram was reconstructed by plotting Y(t) against X(t). The method of box counting was applied to analyze the behavior of the PSR trajectories. Measures as mean (μ), standard deviation (σ) and coefficient of variation (CV = σ/μ), kurtosis (β) for the box counting of PSR diagrams were reported. Results During SR, CV was always 0.05. A similar pattern was observed with β, where < 6 was considered as the cut-off point between SR and VT/VF. Therefore, the upper threshold for SR was considered CVth = 0.05 and βth < 6. For optimisation of the accuracy, a new index (J) was proposed: J=wCVCVth+1−wββth. During SR the upper limit of J was the value of 1. Furthermore CV, β and J crossed the cut-off point timely before the onset of arrhythmia (average time: 4 min 31 s; SD: 2 min 30 s); allowing sufficient time for preventive therapy. Conclusion The J index improved ECG utility for arrhythmia monitoring and detection utility, allowing the prompt and accurate diagnosis of ventricular arrhythmias

    Stringy effects in black hole decay

    We compute the low energy decay rates of near-extremal three(four) charge black holes in five(four) dimensional N=4 string theory to sub-leading order in the large charge approximation. This involves studying stringy corrections to scattering amplitudes of a scalar field off a black hole. We adapt and use recently developed techniques to compute such amplitudes as near-horizon quantities. We then compare this with the corresponding calculation in the microscopic configuration carrying the same charges as the black hole. We find perfect agreement between the microscopic and macroscopic calculations; in the cases we study, the zero energy limit of the scattering cross section is equal to four times the Wald entropy of the black hole.Comment: 32 page

    Meson Thermalization in Various Dimensions

    In gauge/gravity duality framework the thermalization of mesons in strongly coupled (p+1)-dimensional gauge theories is studied for a general Dp-Dq system, q>=p, using the flavour Dq-brane as a probe. Thermalization corresponds to the horizon formation on the flavour Dq-brane. We calculate the thermalization time-scale due to a time-dependent change in the baryon number chemical potential, baryon injection in the field theory. We observe that for such a general system it has a universal behaviour depending only on the t'Hooft coupling constant and the two parameters which describe how we inject baryons into the system. We show that this universal behaviour is independent of the details of the theory whether it is conformal and/or supersymmetric.Comment: 26 pages, 2 figure

    A frequentist framework of inductive reasoning

    Reacting against the limitation of statistics to decision procedures, R. A. Fisher proposed for inductive reasoning the use of the fiducial distribution, a parameter-space distribution of epistemological probability transferred directly from limiting relative frequencies rather than computed according to the Bayes update rule. The proposal is developed as follows using the confidence measure of a scalar parameter of interest. (With the restriction to one-dimensional parameter space, a confidence measure is essentially a fiducial probability distribution free of complications involving ancillary statistics.) A betting game establishes a sense in which confidence measures are the only reliable inferential probability distributions. The equality between the probabilities encoded in a confidence measure and the coverage rates of the corresponding confidence intervals ensures that the measure's rule for assigning confidence levels to hypotheses is uniquely minimax in the game. Although a confidence measure can be computed without any prior distribution, previous knowledge can be incorporated into confidence-based reasoning. To adjust a p-value or confidence interval for prior information, the confidence measure from the observed data can be combined with one or more independent confidence measures representing previous agent opinion. (The former confidence measure may correspond to a posterior distribution with frequentist matching of coverage probabilities.) The representation of subjective knowledge in terms of confidence measures rather than prior probability distributions preserves approximate frequentist validity.Comment: major revisio
