Differences in the association between childhood trauma and BMI in black and white South African women
Objective: Childhood trauma has previously been associated with adult obesity. The aim of this study was to determine if ethnicity altered the relationship between childhood trauma and obesity in South African women. Methods: Forty-four normalweight (BMI<25kg/m2) and obese (BMI>30kg/m2), black and white premenopausal women completed the Childhood Trauma Questionnaire (CTQ), which retrospectively assessed emotional and physical neglect, and emotional, physical and sexual abuse in childhood. Results: Body composition did not differ by ethnicity in the normal-weight and obese groups. However,independent of BMI group, there were significant differences in socioeconomic status (SES) between black and white women (P<0.01). Total CTQ score, as well as the sub-scales, physical and emotional neglect, and physical and sexual abuse were higher in black than white women (all P<0.05), but these scores did not differ between BMI groups. Apart from the sexual abuse score, the differences in physical and emotional neglect and physical abusescores were no longer significant after adjusting for ethnic differences in age and SES. For sexual abuse, there was a significant interaction between ethnicity and BMI group(P=0.04), with scores in normal weight women being higher in black than white women, but scores in obese women not differing by ethnicity. Conclusion: Ethnicity alters the association between childhood sexual abuse and BMI status. Larger studies are required to verify this finding, including measures of body image and body size satisfaction that may explain these findings.Keywords: Childhood trauma questionnaire; Body fat; Body image; Sexual abuse; Ethnicity, Socioeconomic statu
Towards an Accurate Identification of Pyloric Neuron Activity with VSDi
Voltage-sensitive dye imaging (VSDi) which enables simultaneous optical recording of many neurons in the pyloric circuit of the stomatogastric ganglion is an important technique to supplement electrophysiological recordings. However, utilising the technique to identify pyloric neurons directly is a computationally exacting task that requires the development of sophisticated signal processing procedures to analyse the tri-phasic pyloric patterns generated by these neurons. This paper presents our work towards commissioning such procedures. The results achieved to date are most encouraging

Nevertheless, there is evidence of important differences between these two disorders. Some authors have conceptualized the disorders as lying on an OCD spectrum of conditions. Methods Two hundred and seventy eight OCD patients (n = 278: 148 male; 130 female) and 54 TTM patients (n = 54; 5 male; 49 female) of all ages were interviewed. Female patients were compared on select demographic and clinical variables, including comorbid axis I and II disorders, and temperament/character profiles. Results OCD patients reported significantly more lifetime disability, but fewer TTM patients reported response to treatment. OCD patients reported higher comorbidity, more harm avoidance and less novelty seeking, more maladaptive beliefs, and more sexual abuse. OCD and TTM symptoms were equally likely to worsen during menstruation, but OCD onset or worsening was more likely associated with pregnancy/puerperium. Conclusions These findings support previous work demonstrating significant differences between OCD and TTM. The classification of TTM as an impulse control disorder is also problematic, and TTM may have more in common with conditions characterized by stereotypical self-injurious symptoms, such as skin-picking. Differences between OCD and TTM may reflect differences in underlying psychobiology, and may necessitate contrasting treatment approaches.Published versio
Low-cost reusable instrumentation for laparoendoscopic single-site nephrectomy: assessment in a porcine model
PURPOSE:
To test different sets of prebent instruments and a new reusable access device for laparoendoscopic single-site (LESS) surgery.
MATERIALS AND METHODS:
Three surgeons with previous experience in LESS performed 12 nephrectomies in six pigs. In all procedures, a multichannel access device (X-CONE) and a 5-mm extra-long telescope were used. Four sets of prebent instruments with different profiles (S-portal) were tested: Standard (one straight scissors and one curved grasper), Cuschieri, Carus, and Leroy set (each of them consisting of two curved instruments with different configurations). Assessment was performed based on both objective (procedure time; time to manage the pedicle; time to free kidney) and subjective parameters (entry/exit of instruments; triangulation; dissection up/down; dissection lateral; retraction; interdependence). The subjective assessment tool used was a Likert type scale (1 = easy to 5 = prohibitive). The access device was assessed by using objective (time to complete insertion of device after skin incision) and subjective (significant air leakage, movement constraint) parameters.
RESULTS:
Time to insertion of the X-CONE was <1 minute in all the cases. Surgeons reported significant insufflant leakage in 58% of cases. The procedure was completed in 10/12 (83%) cases. Mean operative time was 8.3 ± 4.2 minutes, being lower for the Carus group (4.5 min) and higher for the standard group (13 min). Among the different sets, the standard one obtained the best mean scores for all subjective parameters.
CONCLUSIONS:
X-CONE allows easy abdominal access, and its reusable properties represent cost savings for LESS compared with disposable devices. Prebent instruments might also represent attractive low-cost tools for LESS

A Personalized Self-Management Rehabilitation System for Stroke Survivors: A Quantitative Gait Analysis Using a Smart Insole.
Background: In the United Kingdom, stroke is the single largest cause of adult disability and results in a cost to the economy of £8.9 billion per annum. Service needs are currently not being met; therefore, initiatives that focus on patient-centered care that promote long-term self-management for chronic conditions should be at the forefront of service redesign. The use of innovative technologies and the ability to apply these effectively to promote behavior change are paramount in meeting the current challenges.
Objective: Our objective was to gain a deeper insight into the impact of innovative technologies in support of home-based, self-managed rehabilitation for stroke survivors. An intervention of daily walks can assist with improving lower limb motor function, and this can be measured by using technology. This paper focuses on assessing the usage of self-management technologies on poststroke survivors while undergoing rehabilitation at home.
Methods: A realist evaluation of a personalized self-management rehabilitation system was undertaken in the homes of stroke survivors (N=5) over a period of approximately two months. Context, mechanisms, and outcomes were developed and explored using theories relating to motor recovery. Participants were encouraged to self-manage their daily walking activity; this was achieved through goal setting and motivational feedback. Gait data were collected and analyzed to produce metrics such as speed, heel strikes, and symmetry. This was achieved using a “smart insole” to facilitate measurement of walking activities in a free-living, nonrestrictive environment.
Results: Initial findings indicated that 4 out of 5 participants performed better during the second half of the evaluation. Performance increase was evident through improved heel strikes on participants’ affected limb. Additionally, increase in performance in relation to speed was also evident for all 5 participants. A common strategy emerged across all but one participant as symmetry performance was sacrificed in favor of improved heel strikes. This paper evaluates compliance and intensity of use.
Conclusion: Our findings suggested that 4 out of the 5 participants improved their ability to heel strike on their affected limb. All participants showed improvements in their speed of gait measured in steps per minute with an average increase of 9.8% during the rehabilitation program. Performance in relation to symmetry showed an 8.5% average decline across participants, although 1 participant improved by 4%. Context, mechanism, and outcomes indicated that dual motor learning and compensatory strategies were deployed by the participants
Obsessive compulsive disorder- prevalence in Xhosaspeaking schizophrenia patients
Obsessive compulsive disorder (OCD) has been reported in up to 31% of schizophrenia sufferers. This study evaluated the presence of OCD in a Xhosa-speaking schizophrenia group. Xhosa patients (N = 509, including 100 sibships) with schizophrenia were recruited from hospital and community settings. The patients underwent a structured clinical interview for the presence of lifetime co-morbid schizo-phrenia and OCD. Only 3 patients (0.5%) fulfilled criteria for OCD. No concordance for OCD was noted in the sibship group. Our findings differ from those in other parts of the world, and if replicated, might suggest unique protective environmental or genetic factors for OCD in certain ethnic groups
Convection and the Origin of Evershed Flows
Numerical simulations have by now revealed that the fine scale structure of
the penumbra in general and the Evershed effect in particular is due to
overturning convection, mainly confined to gaps with strongly reduced magnetic
field strength. The Evershed flow is the radial component of the overturning
convective flow visible at the surface. It is directed outwards -- away from
the umbra -- because of the broken symmetry due to the inclined magnetic field.
The dark penumbral filament cores visible at high resolution are caused by the
'cusps' in the magnetic field that form above the gaps. Still remaining to be
established are the details of what determines the average luminosity of
penumbrae, the widths, lengths, and filling factors of penumbral filaments, and
the amplitudes and filling factors of the Evershed flow. These are likely to
depend at least partially also on numerical aspects such as limited resolution
and model size, but mainly on physical properties that have not yet been
adequately determined or calibrated, such as the plasma beta profile inside
sunspots at depth and its horizontal profile, the entropy of ascending flows in

Quantum resource estimates for computing elliptic curve discrete logarithms
We give precise quantum resource estimates for Shor's algorithm to compute
discrete logarithms on elliptic curves over prime fields. The estimates are
derived from a simulation of a Toffoli gate network for controlled elliptic
curve point addition, implemented within the framework of the quantum computing
software tool suite LIQ. We determine circuit implementations for
reversible modular arithmetic, including modular addition, multiplication and
inversion, as well as reversible elliptic curve point addition. We conclude
that elliptic curve discrete logarithms on an elliptic curve defined over an
-bit prime field can be computed on a quantum computer with at most qubits using a quantum circuit of at most Toffoli gates. We are able to classically simulate the
Toffoli networks corresponding to the controlled elliptic curve point addition
as the core piece of Shor's algorithm for the NIST standard curves P-192,
P-224, P-256, P-384 and P-521. Our approach allows gate-level comparisons to
recent resource estimates for Shor's factoring algorithm. The results also
support estimates given earlier by Proos and Zalka and indicate that, for
current parameters at comparable classical security levels, the number of
qubits required to tackle elliptic curves is less than for attacking RSA,
