    Pseudophaco-anaphylactic endophthalmitis?

    The pathology of subacute granulomatous endophthalmitis centered on a posterior chamber lens implant placed at extracapsular cataract surgery in the eye of a 72 year-old diabetic with cystoid macular edema following intracapsular extraction and an anterior chamber implant in the other eye is described in detail 3 months after surgery. The features of the severe granulomatous inflammation do not fit the usual pattern of sympathetic or phacoanaphylactic uveitis. The possibility of a specific hypersensitivity to the substance of the implant is considered. Die Pathologie von subakuter Endophthalmitis in einem Fall von einem Hinterkammerlinsenimplant nach extrakapsulaerer Kataraktexctraktion im Auge einer 72jährigen Diabetikerin mit zystoidem Makulaödem nach intrakapsulärer Kataraktoperation und Vorderkammerlinsenimplant im anderen Auge wird drei Monate post-operativ in allen Einzelheiten beschrieben. Das Bild einer schweren granulomatösen Entzündung paßt nicht zu den Diagnosen von symphatischer oder phakoanaphylaktischer Uveitis.     Three basic types of foveal involvement in choroidal melanomas

    Three cases are used to demonstrate the main types of cystoid macular edema, which have been seen to occur in association with choroidal melanomas: 1) direct involvement in cases, where the neoplasm is located under the foveal retina, 2) indirect involvement due to a subfoveal exudate in choroidal melanomas distant to the fovea and 3) indirect foveolar involvement without associated subfoveal tumor or exudate. A recording of the pathology of these three basic reaction patterns of the central retina in choroidal melanomas is considered important. It is emphasized, that combinations of the reactions are common and may result in more complex situations. Drei Fälle werden benutzt, um die Haupttypen von zystoidem Maculaödem zu demonstrieren, die zusammen mit malignen Aderhautmelanomen gesehen werden: 1) direkte Foveabeteiligung in Fällen, in denen der Aderhauttumor im Foveabereich liegt, 2) indirekte Foveabeteiligung in Fällen mit subfovealem Exudat und extrafovealen Melanom und 3) indirekte Foveabeteiligung in Fällen, in denen im Foveabereich weder ein Melanom noch retroretinales Exudat gefunden werden. Ein Verständnis dieser drei fundamental verschiedenen Reaktionstypen der zentralen Netzhaut bei Aderhautmelanomen ist wichtig.     Cystoid macular edema associated with limbal melanoma

    Early cystoid macular edema causing some loss of central vision in an eye with an 8-year history of limbal melanoma was studied histopathologically.     Bilateral attachment of corneal endothelium to lens implants in corneal collapse at death: a pseudophakic eye with a Fuchs' epithelioma

    The pathological situation of bilateral attachment of corneal endothelium to iris plane lens implants, due to collapse of the anterior chambers, is recorded in a patient who died of heart failure.     Syneresis and vitreous hemorrhage in pseudophacia

    The eye of a 60-year-old man with a clinically very successful lens implant became available for pathological study after a very sudden and uncomplicated death about 6 months following cataract surgery. The following changes were found: slight non-granulomatous uveitis associated with foreign-body reaction to the lens implant and its supporting iris suture, absence of the anterior vitreous face, advanced syneresis of the central vitreous, vitreous hemorrhage with diffuse layering of erythrocytes on the inner aspect of the remaining vitreous crust, vitreous traction on peripheral and central retina with tenting of foveal internal limiting membrane, and some foveal edema without cystoid changes and without foveolar detachment.     Expulsive hemorrhage: a study of histopathological details

    Cataract surgery in the eye of a 71 year-old male was complicated by an expulsive hemorrhage. The eye became blind and was removed 21 days later. Histopathological evaluation revealed facts of outstanding interest: rupture of the long posterior ciliary artery on the nasal side was the cause of the massive bleeding and cystoid macular edema with vertical infolding of the central cones was found in the totally detached and distorted retina. Eine Cataractoperation am Auge eines 71 Jahre alten Mannes wurde durch eine expulsive Blutung kompliziert. Das Auge wurde blind und wurde 21 Tage später entfernt.     Morphology of the capsule-like portion of the reactive membranes on intraocular lens implants

    The glass-membrane-like capsules probably are the practically most important part of the separating membranes that typically form on intraocular lens implants in the human eye. With time in the eye, these clear capsules become tougher, more firmly adherent, and less eosinophilic. Their formation occurs early after implantation and is associated with the reactions of intraocular macrophages. Examples of well developed capsules are presented to show that only few cells are needed for their maintenance and that they can be of irregular thickness. Cell life on the film-like capsules is demonstrated using the examples of blood-eating macrophages, epitheliod cells, and melanocytes. Attachment of fibrous structures containing zonular fibers to the haptics is recorded. Die glasmembran-ähnlichen Kapseln sind wahrscheinlich der wichtigste Teil der trennenden Membranen, die sich typischerweise auf intraokulaeren Linsenprothesen bilden. Im Auge werden diese klaren Kapseln mit der Zeit zäher, fester anhaftend und weniger eosinophil. Ihre Bildung geschieht im Auge bald nach der Implantation und ist mit den Reaktionen der intraokulären Makrophagen verbunden. Beispiele von voll entwickelten Kapseln werden gezeigt, um zu zeigen, daß nur wenige Zellen zu ihrer Instandhaltung notwendig sind und daß sie von sehr unregelmäßiger Dicke sein können. Das Zelleben auf den filmartigen Kapseln wird am Beispiel von blutfressenden Makrophagen, Epithelioidzellen und Melanozyten demonstriert.     Reactive membrane on a lens implant: three months after implantation

    Implant cytology study of a Leiske anterior chamber lens implant removed after three months because of corneal edema due to suspected endothelial damage during surgery revealed evidence of a fully developed membrane covering all of the implant at this early date. The cells in this membrane represented all the predominant cell elements seen in membranes on implants removed after years: foreign body giant cells, epithelioid cells, and fibroblast-like cells. However, the early stages of differentiation seen in the cells of the present membrane gave an indication that they are interrelated and have their origin in free-floating aqueous humor histiocytes.     Abdominal implantation of intraocular lenses resulting in the formation of reactive membranes

    For a better understanding of the cytological principles involved in the formation of reactive membranes on lens implants in the human eye, two kinds of modern implants were placed into the peritoneal space of mice. Continous membranes mainly composed of macrophages were observed after 5 days. The membrane on a polymethylmethacrylate implant contained numerous large giant cells, while there were no giant cells on a glass implant.
