    Swedish social insurance officers' experiences of difficulties in assessing applications for disability pensions – an interview study

    <p>Abstract</p> <p>Background</p> <p>In this study the focus is on social insurance officers judging applications for disability pensions. The number of applications for disability pension increased during the late 1990s, which has resulted in an increasing number of disability pensions in Sweden. A more restrictive attitude towards the clients has however evolved, as societal costs have increased and governmental guidelines now focus on reducing costs. As a consequence, the quantitative and qualitative demands on social insurance officers when handling applications for disability pensions may have increased. The aim of this study was therefore to describe the social insurance officers' experiences of assessing applications for disability pensions after the government's introduction of stricter regulations.</p> <p>Methods</p> <p>Qualitative methodology was employed and a total of ten social insurance officers representing different experiences and ages were chosen. Open-ended interviews were performed with the ten social insurance officers. Data was analysed with inductive content analysis.</p> <p>Results</p> <p>Three themes could be identified as problematic in the social insurance officers' descriptions of dealing with the applications in order to reach a decision on whether the issue qualified applicants for a disability pension or not: 1. Clients are heterogeneous. 2. Ineffective and time consuming waiting for medical certificates impede the decision process. 3. Perspectives on the issue of work capacity differed among different stakeholders. The backgrounds of the clients differ considerably, leading to variation in the quality and content of applications. Social insurance officers had to make rapid decisions within a limited time frame, based on limited information, mainly on the basis of medical certificates that were often insufficient to judge work capacity. The role as coordinating actor with other stakeholders in the welfare system was perceived as frustrating, since different stakeholders have different goals and demands. The social insurance officers experience lack of control over the decision process, as regulations and other stakeholders restrict their work.</p> <p>Conclusion</p> <p>A picture emerges of difficulties due to disharmonized systems, stakeholder-bound goals causing some clients to fall between two stools, or leading to unnecessary waiting times, which may limit the clients' ability to take an active part in a constructive process. Increased communication with physicians about how to elaborate the medical certificates might improve the quality of certificates and thereby reduce the clients waiting time.</p

    The effects of shoulder load and pinch force on electromyographic activity and blood flow in the forearm during a pinch task

    The object of the current study was to determine whether static contraction of proximal musculature has an effect on the blood flow more distally in the upper extremity. Static contractions of muscles in the neck shoulder region at three levels (relaxed, shoulders elevated and shoulders elevated loaded with 4.95 kg each) were combined with intermittent pinch forces at 0, 10 and 25% of the maximum voluntary contraction (MVC). Blood flow to the forearm was measured with Doppler ultrasound. Myoelectric activity of the forearm and neck-shoulder muscles was recorded to check for the workload levels. Across all levels of shoulder load, blood flow increased significantly with increasing pinch force (21% at 10% MVC and by 44% at 25% MVC). Blood flow was significantly affected by shoulder load, with the lowest blood flow at the highest shoulder load. Interactions of pinch force and shoulder load were not significant. The myoelectric activity of forearm muscles increased with increasing pinch force. The activation of the trapezius muscle decreased with increasing pinch force and increased with increasing shoulder load. The precise mechanisms accounting for the influence of shoulder load remains unclear. The results of this study indicate that shoulder load might influence blood flow to the forearm

    Use of a Poroelastic Model to Predict Intramuscular Pressure

    Measurement of individual muscle tension in a clinical setting has yet to be achieved. Previous investigators have suggested that the tension in skeletal muscle, comprised of approximately 70% fluid, could be determined using interstitial muscle fluid pressure (IMP). A computational model is needed to aid in understanding IMP distribution in muscles of varying geometry and contractile states without exhaustive testing. The first aim of this study was to determine a set of transversely isotropic material properties (i.e., permeability, relaxed modulus, and drained Poisson\u27s ratio) for excised skeletal muscle using inverse finite element analysis with a poroelastic constitutive formulation on tension data from either longitudinal or transverse uniaxial load-relaxation tests of skeletal muscle tissue. The second aim was to compare pore pressure estimated from a model to experimental pressure measurements to assess its ability to accurately predict IMP. Results of this study indicated that skeletal muscle was transversely isotropic under load-relaxation as demonstrated by significant differences in the drained Poisson\u27s ratio. It was also noted that the drained Poisson\u27s ratios under both longitudinal and transverse loading were negative in these tests of excised muscle tissue. Pore pressure calculated with this model provided a good prediction of the development of IMP. These results point to the benefit of using a poroelastic model of skeletal muscle to predict IMP. © 2013 American Society of Civil Engineers
