A community pharmacy weight management programme: an evaluation of effectiveness
<p>Background: Community pharmacies may offer an accessible way of delivering weight-management programmes but there have been few trials that use clinically significant weight loss outcomes, objective measures of weight and follow-up to 12 months. We aimed to evaluate weight change among patients who used the Counterweight weight management programme delivered by community pharmacies.</p>
<p>Methods: The Counterweight Programme was introduced into community pharmacies in Fife, Scotland in 2009 for patients with a BMI ≥ 30 kg/m2 or a BMI ≥ 28 kg/m2 with a co-morbidity in localities in which Counterweight was not available at GP practices. The aim was to achieve an energy deficit of 500-600 kcal per day. Counterweight specialist dietitians delivered training, support and patient information materials to community pharmacies. Patient weight was measured by pharmacy staff at each weight management session. Weight data recorded at each weight management session were used to estimate weight change and attendance at 3, 6 and 12 months.</p>
<p>Results: Between March 2009 and July 2012, 458 patients were enrolled by the community pharmacies. Three-quarters of patients were women, mean age was 54 (SD 7.4) years and mean BMI 36.1 (SD 5.9) kg/m2. Of 314 patients enrolled for at least 12 months, 32 (10.2% on an intention to treat basis) had achieved the target weight loss of ≥5%; this was 41.6% of those who attended at 12 months representing a mean weight loss of 4.1 kg. Using Last Observation Carried Forward, 15.9% achieved the target weight loss within 12 months of enrolling. There was no significant effect of sex, baseline BMI or age on weight loss.</p>
<p>Conclusions: The Counterweight pharmacy programme has a similar effectiveness to other primary care based weight management programmes and should be considered as part of a range of services available to a community to manage overweight and obesity.</p>
Galaxy Luminosity Functions from Deep Spectroscopic Samples of Rich Clusters
Using a new spectroscopic sample and methods accounting for spectroscopic
sampling fractions that vary in magnitude and surface brightness, we present
R-band galaxy luminosity functions (GLFs) for six nearby galaxy clusters with
redshifts 4000 < cz < 20000 km/s and velocity dispersions 700 < sigma < 1250
km/s. In the case of the nearest cluster, Abell 1060, our sample extends to
M_R=-14 (7 magnitudes below M*), making this the deepest spectroscopic
determination of the cluster GLF to date. Our methods also yield composite GLFs
for cluster and field galaxies to M_R=-17 (M*+4), including the GLFs of
subsamples of star forming and quiescent galaxies. The composite GLFs are
consistent with Schechter functions (M*_R=-21.14^{+0.17}_{-0.17},
alpha=-1.21^{+0.08}_{-0.07} for the clusters, M*_R=-21.15^{+0.16}_{-0.16},
alpha=-1.28^{+0.12}_{-0.11} for the field). All six cluster samples are
individually consistent with the composite GLF down to their respective
absolute magnitude limits, but the GLF of the quiescent population in clusters
is not universal. There are also significant variations in the GLF of quiescent
galaxies between the field and clusters that can be described as a steepening
of the faint end slope. The overall GLF in clusters is consistent with that of
field galaxies, except for the most luminous tip, which is enhanced in clusters
versus the field. The star formation properties of giant galaxies are more

This study uses data from a 2001 survey of Great Plains farmers to explore the adoption, usage patterns, and perceived benefits of computers and the Internet. Adoption results suggest that exposure to the technology through college, outside employment, friends, and family is ultimately more influential than farmer age and farm size. Notably, about half of those who use the Internet for farm-related business report zero economic benefits from it. Colour Relations in Form
The orthodox monadic determination thesis holds that we represent colour relations by virtue of representing colours. Against this orthodoxy, I argue that it is possible to represent colour relations without representing any colours. I present a model of iconic perceptual content that allows for such primitive relational colour representation, and provide four empirical arguments in its support. I close by surveying alternative views of the relationship between monadic and relational colour representation
Ghrelin in the human myometrium
<p>Abstract</p> <p>Background</p> <p>Ghrelin is a 28-amino acid octanolyated peptide, synthesised primarily in the stomach. It stimulates growth hormone release, food intake and exhibits many other diverse effects. Our group have previously determined that ghrelin inhibited human contractility in <it>vitro</it>. The aim of this study therefore, was to investigate the expression of ghrelin, its receptor, the growth hormone secretagogue receptor type 1 (GHS-R1), ghrelin O-acyltransferase (GOAT) which catalyses ghrelin octanoylation, prohormone convertase 1/3 (PC1/3) responsible for pro-ghrelin processing, in human myometrium, during pregnancy prior to labour, during labour and in the non-pregnant state. Modulation of ghrelin and ghrelin receptor expression in cultured myometrial cells was also investigated.</p> <p>Methods</p> <p>mRNA and protein were isolated from human myometrium and the myometrial smooth muscle cell line hTERT-HM; and real-time fluorescence RT-PCR, western blotting and fluorescence microscopy performed. The effects of β-Estradiol and bacterial lipopolysaccharide (LPS) on hTERT-HM gene expression were evaluated by western blotting.</p> <p>Results</p> <p>We have reported for the first time the expression and processing of ghrelin, GHS-R1, GOAT and PC1/3 expression in human myometrium, and also the down-regulation of ghrelin mRNA and protein expression during labour. Furthermore, GHS-R1 protein expression significantly decreased at labour. Myometrial GOAT expression significantly increased during term non-labouring pregnancy in comparison to both non-pregnant and labouring myometrium. Mature PC1/3 protein expression was significantly decreased at term pregnancy and labour in comparison to non-pregnant myometrium. Ghrelin, GHS-R1, GOAT and PC1/3 mRNA and protein expression was also detected in the hTERT-HM cells. Ghrelin protein expression decreased upon LPS treatment in these cells while β-Estradiol treatment increased GHS-R1 expression.</p> <p>Conclusions</p> <p>Ghrelin processing occurred in the human myometrium at term pregnancy and in the non-pregnant state. GOAT expression which increased during term non-labouring pregnancy demonstrating a similar expression pattern to prepro-ghrelin and GHS-R1, decreased at labour, signifying possible myometrial ghrelin acylation. Moreover, the presence of PC1/3 may contribute to pro-ghrelin processing. These results along with the previous <it>in vitro </it>data suggest that myometrially-produced and processed ghrelin plays a significant autocrine or paracrine role in the maintenance of relaxation in this tissue during pregnancy. Furthermore, the significant uterine modulators LPS and β-Estradiol are involved in the regulation of ghrelin and ghrelin receptor expression respectively, in the human myometrium.</p
Pre-emptive intervention and its effect on student attainment and retention
This paper describes how a small educational technology (edtech) company worked with academics, combining technological expertise, science content expertise, pedagogy and social research methodology to develop and evaluate the effect of video feedback on learners’ ability to answer science questions correctly. The investigation was carried out by the research team in Tassomai as part of their involvement with the EDUCATE programme. The Tassomai team worked with the research mentors in EDUCATE to find the best ways of helping students both to understand science concepts and to help them correctly answer science questions in exams. Findings indicated that, as expected, the video feedback helped learners to answer the question correctly, but also that, after a delay of around one week, a higher proportion of those students were still able to answer the question correctly compared to those in a control group of learners who did not have access to the related instructive video. The collaborative work between the Tassomai research team and the EDUCATE business and research mentors provided an environment to share expertise and channel it to improve Tassomai’s offering to learners. As a result of this study, Tassomai is now investing in the production of more instructive videos to help students understand difficult science concepts, and students will be offered these videos if they are having difficulty in answering the questions correctly
The clinical value of immunohistochemically demonstrable CEA in breast cancer: a possible method of selecting patients for adjuvant chemotherapy.
The production of carcinoembryonic antigen (CEA) by human breast cancer tissue has been studied in relation to the prognosis of patients with breast cancer. All of the patients were in a controlled trial of adjuvant chemotherapy for the treatment of operable breast cancer. CEA was studied in primary tumours and axillary node metastases from these patients using an immunoperoxidase (PAP) method. Sections of 290 primary carcinomas and 217 axillary metastases were examined for CEA. The CEA status of the primary tumours was of no value as a prognostic indicator nor in the selection of patients for chemotherapy. In contrast, patients could be divided into 3 groups on the basis of the CEA results in the axillary nodes. In one group, in which cases were strongly positive for CEA (24% of the total) the prognosis, as reflected by recurrence free survival, was relatively good and chemotherapy produced no further advantage. In another group in which cases were weakly positive for CEA (18% of the total) the prognosis was poor but chemotherapy produced significant improvement. In a third group, in which cases were negative for CEA (58% of the total) the prognosis was poor and was not improved by chemotherapy, at least in the short term. Thus, the CEA status of axillary metastases may be clinically useful
