    Hydraulic Efficiency of Flood Detention Pools Evaluated by Means of Mathematical Simulation

    The hydraulic efficiency to secure optimization of the flood detention pool system will be treated through mathematical simulation. The mathematical model is approximated by the point-sink virtually located at the center of the deversoir section. Numerical computations show that the optimization of the system in hydraulic efficiency will be given through a combination of the deversoir length and the initial diversion discharge. Furthermore, it is seen the whole pool volume will be nearly constant for any possible combination

    Toward Understanding the Origin of Turbulence in Molecular Clouds: Small Scale Structures as Units of Dynamical Multi-Phase Interstellar Medium

    In order to investigate the origin of the interstellar turbulence, detailed observations in the CO J=1--0 and 3--2 lines have been carried out in an interacting region of a molecular cloud with an HII region. As a result, several 1,000 to 10,000 AU scale cloudlets with small velocity dispersion are detected, whose systemic velocities have a relatively large scatter of a few km/s. It is suggested that the cloud is composed of small-scale dense and cold structures and their overlapping effect makes it appear to be a turbulent entity as a whole. This picture strongly supports the two-phase model of turbulent medium driven by thermal instability proposed previously. On the surface of the present cloud, the turbulence is likely to be driven by thermal instability following ionization shock compression and UV irradiation. Those small scale structures with line width of ~ 0.6 km/s have a relatively high CO line ratio of J=3--2 to 1--0, 1 < R(3-2/1-0) < 2. The large velocity gradient analysis implies that the 0.6 km/s width component cloudlets have an average density of 10^{3-4} cm^{-3}, which is relatively high at cloud edges, but their masses are only < 0.05 M_{sun}.Comment: 12 pages, 9 figures.     Feasibility and Efficacy of Definitive Radiotherapy with 66 Gy and Concurrent Carboplatin-Paclitaxel Chemotherapy for Stage III Non-Small Cell Lung Cancer.

    Purpose/Objectives : This study was conducted to assess the feasibility and efficacy of definitive radiotherapy (RT) with a total dose of 66 Gy and concurrent carboplatin-paclitaxel chemotherapy for patients (pts) with stage Ⅲ non-small celllung cancer. Materials/Methods : Between April 2007 and December 2013,99 pts with non-small cell lung cancer were treated using RT with concurrent carboplatin-paclitaxel chemotherapy in our hospital. Sixty-eight of them received RT with a total dose of 66 Gy. We analyzed 46 Stage Ⅲ pts who had been treated with RT using three-dimensional radiotherapy treatment planning. The prophylactic mediastinal lymph nodes were included in the clinical target volume for RT. The survival rate after the start of RT was estimated using the Kaplan-Meier method. We estimated the cumulative local failure and distant metastasis rates with the Fine-Gray method. Adverse events were evaluated according to the CTCAE (v.4.0). Results : The median age of the pts was 70.9 (52.8-78.7) years old (y.o.). The performance status (PS) of each pt was fairly good (ECOG PS 0: 25, PS 1: 20, PS 3:1), and their clinical stages (UICC 7th) were twenty-nine Ⅲ A and seventeen Ⅲ B. Diagnoses were pathologically confirmed in 32 pts. The median follow-up period was 35.7 (2.0-82.2) months among all pts, and 55.9 (40.1-82.2) months among survivors. The 3- and 5-year Kaplan-Meier overall survival rates were 52.2 and 34.0%,respectively, and the median survival time was 36.6 months. The 3- and 5-year Kaplan-Meier progression-free survival rates were 29.1 and 21.9%,respectively, and the median progression-free survival time was 9.9 months. The 5-year local failure rate was 37.6%, and the 5-year distant metastasis rate was 49.7%. Sixteen (34.8%) pts required steroid administration because of radiation pneumonitis (CTCAE Grade 2 or higher) and two of them died (Grade 5). No other severe non-hematologic toxicity (Grade 3 or higher) was observed. Conclusion : These results suggest that definitive RT with a total dose of 66 Gy and concurrent carboplatin-paclitaxel chemotherapy is feasible and may be promising for pts with Stage Ⅲ non-small cell lung cancer

    Background: Distal radius fracture surgery is performed under general (GA) or regional anesthesia with brachial plexus block (NB). Whether anesthesia type affects patient outcomes is unclear. This study retrospectively compared patient satisfaction between GA and NB after surgery. Methods: This was a historical cohort study of 80 (34 GA and 46 NB) patients who underwent volar plate fixation of distal radius fractures. Propensity score analysis was used to generate a set of matched cases (NB) and controls (GA), yielding 14 matched patient-pairs. The simplified patient satisfaction scale was compared for primary outcomes. Secondary outcomes were anesthesia and surgery duration, hospital stay length, adverse events, postoperative analgesic requirement, and wrist range of motion (ROM) 2 weeks and 3 months postoperatively. Results: After propensity score matching, patients in almost all cases in both groups were “Satisfied” (effect size: 0.1, p = 0.572), indicating little significant difference. Significant differences in adverse events and postoperative analgesic use observed before matching disappeared after matching. Anesthesia duration and hospital stay length were significantly shorter in the NB group (effect size: -1.27 and -0.77, p = 0.00074 and p = 0.0388, respectively), as was surgery duration (effect size: -0.84, p = 0.0122) after matching. Similar to before matching, wrist ROM significantly improved in the NB group (effect size: 1.11, p = 0.0279) in the early postoperative period, but the difference disappeared at 3 months postoperatively. Conclusions: Patient satisfaction between distal radius fracture surgery under GA and NB was similar.     Behavioral phenotypes of mice lacking purinergic P2X4 receptors in acute and chronic pain assays

    A growing body of evidence indicates that P2X receptors (P2XRs), a family of ligand-gated cation channels activated by extracellular ATP, play an important role in pain signaling. In contrast to the role of the P2X3R subtype that has been extensively studied, the precise roles of others among the seven P2XR subtypes (P2X1R-P2X7R) remain to be determined because of a lack of sufficiently powerful tools to specifically block P2XR signaling in vivo. In the present study, we investigated the behavioral phenotypes of a line of mice in which the p2rx4 gene was disrupted in a series of acute and chronic pain assays. While p2rx4-/- mice showed no major defects in pain responses evoked by acute noxious stimuli and local tissue damage or in motor function as compared with wild-type mice, these mice displayed reduced pain responses in two models of chronic pain (inflammatory and neuropathic pain). In a model of chronic inflammatory pain developed by intraplantar injection of complete Freund's adjuvant (CFA), p2rx4-/- mice exhibited attenuations of pain hypersensitivity to innocuous mechanical stimuli (tactile allodynia) and also of the CFA-induced swelling of the hindpaw. A most striking phenotype was observed in a test of neuropathic pain: tactile allodynia caused by an injury to spinal nerve was markedly blunted in p2rx4-/- mice. By contrast, pain hypersensitivity to a cold stimulus (cold allodynia) after the injury was comparable in wild-type and p2rx4-/- mice. Together, these findings reveal a predominant contribution of P2X4R to nerve injury-induced tactile allodynia and, to the lesser extent, peripheral inflammation. Loss of P2X4R produced no defects in acute physiological pain or tissue damaged-induced pain, highlighting the possibility of a therapeutic benefit of blocking P2X4R in the treatment of chronic pain, especially tactile allodynia after nerve injury

    Ultrabright narrow-band telecom two-photon source for long-distance quantum communication

    We demonstrate an ultrabright narrow-band two-photon source at the 1.5 -\mu m telecom wavelength for long-distance quantum communication. By utilizing a bow-tie cavity, we obtain a cavity enhancement factor of 4.06×1044.06\times 10^4. Our measurement of the second-order correlation function G(2)(τ)G^{(2)} ({\tau}) reveals that the linewidth of 2.42.4 MHz has been hitherto unachieved in the 1.5 -\mu m telecom band. This two-photon source is useful for obtaining a high absorption probability close to unity by quantum memories set inside quantum repeater nodes. Furthermore, to the best of our knowledge, the observed spectral brightness of 3.94×1053.94\times 10^5 pairs/(s\cdotMHz\cdotmW) is also the highest reported over all wavelengths.Comment: 11 pages, 4 figures, 2 table

    Spirulina Effectiveness Study on Child Malnutrition in Zambia

    Ensuring adequate nutrition among vulnerable children has been a serious challenge in Zambia. Chronic child malnutrition is more predominant at 45 per cent while underweight and wasting are at 15 and 5 per cent respectively. This study tested the effectiveness of spirulina on malnourished children in Zambia. The study took place from June 2012 to February 2013. Sixty children were divided into spirulina treatment and control groups. The outcome of taking spirulina was analysed by collecting anthropometric data. The fixed-effect regression result showed that 10g of spirulina dairy intake leads to improvement by producing 0.29 higher points in the height-for-age z-score (HAZ); confidence interval (CI)[0.0404, 0.535]. On the contrary, the weight-for-age z-score (WAZ) and the mid-upper arm circumference z-score (MUACZ) did not show a significant difference, although treated children showed a larger improvement by 0.09 points and 0.38 points, respectively. This study implied the validity of spirulina in reducing chronic malnutrition
