Способ контроля толщины межжелудочковой перегородки при выполнении септальной миэктомии. Экспериментальное исследование

Background: At present, there are no methods for intraoperative monitoring of the interventricular septum (IVS) thickness in a stopped and empty heart. This might be an obvious reason for unsatisfactory results after a number of septal myectomies.Aim: To provide an experimental background for the method to control the IVS thickness (that we had proposed) during septal myectomy.Materials and methods: The proposed technique is based on the transillumination method. The experimental models were cadaveric porcine hearts, as well as fragments of the human myocardium removed during septal myectomies. The thickness of the translucent myocardium was estimated depending on the local illumination value at the entrance to the myocardium and the external illumination of the surgical field. We compared the results of 67 septal myectomies performed in the clinic of the Almazov National Medical Research Centre with the results of 35 similar experimental procedures with cadaveric porcine hearts using the proposed way of measurement.Results: A graph of the illumination at the entrance to the myocardium against the thickness of the translucent myocardium was constructed. After conventionally performed septal myectomies the median variation of the myocardial thickness was 4 [3; 6] mm. In the experiment using the proposed control method, the median variation was 1 [1; 2] mm, i.e. significantly less than with the conventional approach (p = 3 x 10-10).Conclusion: The method to control the IVS thickness when performing septal myectomy makes it possible to achieve the required myocardial thickness the resection area with much greater accuracy than with the conventional one.ОбоснОбоснование. В настоящее время не существует методов интраоперационного контроля толщины межжелудочковой перегородки (МЖП) на остановленном и разгруженном сердце. Этим, по всей видимости, обусловлены неудовлетворительные результаты ряда операций септальной миэктомии.Цель - экспериментально обосновать предложенный авторами способ контроля толщины МЖП при выполнении септальной миэктомии.Материал и методы. В основу способа положен метод трансиллюминации. Экспериментальными моделями служили трупные свиные сердца, а также фрагменты человеческого миокарда, удаленные при операциях септальной миэктомии. Оценивалась толщина просвечиваемого миокарда в зависимости от величины локальной освещенности на входе в миокард и внешней освещенности операционного поля. Сопоставлены результаты 67 операций септальной миэктомии, выполненных в клинике ФГБУ «НМИЦ им. В.А. Алмазова» Минздрава России, и результаты 35 аналогичных экспериментальных операций, проведенных на трупных свиных сердцах с использованием предложенного метода контроля толщины МЖП.Результаты. Построен график зависимости освещенности на входе в миокард и толщины просвечиваемого миокарда. После операций септальной миэктомии, выполненных традиционным способом, медиана разброса значений толщины миокарда составила 4 [3; 6] мм, а в эксперименте с использованием предложенного метода контроля толщины МЖП - 1 [1; 2] мм (р = 3 х 10-10).Заключение. Способ контроля толщины МЖП при выполнении септальной миэктомии позволяет добиться необходимой толщины миокарда в зоне резекции со значительно большей точностью, чем при традиционном варианте исполнения

Predictors of obsessive–compulsive symptomology: mind wandering about the past and future

Obsessive and compulsive tendencies are known to occur in the general population and involve worry around specific concerns (obsessions) and an urge to resolve the concern with thoughts or behaviours (compulsions). Spontaneous, but not deliberate, mind wandering experiences (when attention turns to internal mentation), have been found to predict obsessive-compulsive tendencies [Seli, P., Risko, E.F., Purdon, C. & Smilek, D. Recent cognitive theory suggests a particular role for future-oriented spontaneous thought in obsessive-compulsive (OC) symptoms.
