    Spatial distribution of air-sea heat fluxes over the sub-polar North Atlantic Ocean

The definitive version was published in Geophysical Research Letters 39 (2012): L18806, doi:10.1029/2012GL053097.On a variety of spatial and temporal scales, the energy transferred by air-sea heat and moisture fluxes plays an important role in both atmospheric and oceanic circulations. This is particularly true in the sub-polar North Atlantic Ocean, where these fluxes drive water-mass transformations that are an integral component of the Atlantic Meridional Overturning Circulation (AMOC). Here we use the ECMWF Interim Reanalysis to provide a high-resolution view of the spatial structure of the air-sea turbulent heat fluxes over the sub-polar North Atlantic Ocean. As has been previously recognized, the Labrador and Greenland Seas are areas where these fluxes are large during the winter months. Our particular focus is on the Iceland Sea region where, despite the fact that water-mass transformation occurs, the winter-time air-sea heat fluxes are smaller than anywhere else in the sub-polar domain. We attribute this minimum to a saddle point in the sea-level pressure field, that results in a reduction in mean surface wind speed, as well as colder sea surface temperatures associated with the regional ocean circulation. The magnitude of the heat fluxes in this region are modulated by the relative strength of the Icelandic and Lofoten Lows, and this leads to periods of ocean cooling and even ocean warming when, intriguingly, the sensible and latent heat fluxes are of opposite sign. This suggests that the air-sea forcing in this area has large-scale impacts for climate, and that even modest shifts in the atmospheric circulation could potentially impact the AMOC.GWKM was supported by the Natural Science and Engineering Research Council of Canada.     Buoy observations from the windiest location in the world ocean, Cape Farewell, Greenland

The definitive version was published in Geophysical Research Letters 35 (2008): L18802, doi:10.1029/2008GL034845.Cape Farewell, Greenland's southernmost point, is a region of significant interest in the meteorological and oceanographic communities in that atmospheric flow distortion associated with the high topography of the region leads to a number of high wind speed jets. The resulting large air-sea fluxes of momentum and buoyancy have a dramatic impact on the region's weather and ocean circulation. Here the first in-situ observations of the surface meteorology in the region, collected from an instrumented buoy, are presented. The buoy wind speeds are compared to 10 m wind speeds from the QuikSCAT satellite and the North American Regional Reanalysis (NARR). We show that the QuikSCAT retrievals have a high wind speed bias that is absent from the NARR winds.     An exploration of the experiences and utility of functional electrical stimulation for foot drop in people with multiple sclerosis

    Purpose: Functional electrical stimulation (FES) is effective in improving walking in people with multiple sclerosis (MS) with foot drop. There is limited research exploring people’s experiences of using this device. This study aims to explore the utility, efficacy, acceptability, and impact on daily life of the device in people with MS. Methods: An interpretative phenomenological approach was employed. Ten participants who had used FES for 12 months were interviewed. Transcripts were analysed, and emergent themes identified. Results: Nine participants continued to use the device. Three relevant super-ordinate themes were identified; impact of functional electrical stimulation, sticking with functional electrical stimulation, and autonomy and control. Participants reported challenges using the device; however, all reported positive physical and psychological benefits. Intrinsic and external influences such as; access to professional help, the influence of others, an individual’s ability to adapt, and experiences using the device, influenced their decisions to continue with the device. A thematic model of these factors was developed. Conclusions: This study has contributed to our understanding of people with MS experiences of using the device and will help inform prescribing decisions and support the continued, appropriate use of FES over the longer term

    Local Parasite Lineage Sharing In Temperate Grassland Birds Provides Clues About Potential Origins Of Galapagos Avian Plasmodium

    Oceanic archipelagos are vulnerable to natural introduction of parasites via migratory birds. Our aim was to characterize the geographic origins of two Plasmodium parasite lineages detected in the Galapagos Islands and in North American breeding bobolinks (Dolichonyx oryzivorus) that regularly stop in Galapagos during migration to their South American overwintering sites. We used samples from a grassland breeding bird assemblage in Nebraska, United States, and parasite DNA sequences from the Galapagos Islands, Ecuador, to compare to global data in a DNA sequence registry. Homologous DNA sequences from parasites detected in bobolinks and more sedentary birds (e.g., brown-headed cowbirds Molothrus ater, and other co-occurring bird species resident on the North American breeding grounds) were compared to those recovered in previous studies from global sites. One parasite lineage that matched between Galapagos birds and the migratory bobolink, Plasmodium lineage B, was the most common lineage detected in the global MalAvi database, matching 49 sequences from unique host/site combinations, 41 of which were of South American origin. We did not detect lineage B in brown-headed cowbirds. The other Galapagos-bobolink match, Plasmodium lineage C, was identical to two other sequences from birds sampled in California. We detected a close variant of lineage C in brown-headed cowbirds. Taken together, this pattern suggests that bobolinks became infected with lineage B on the South American end of their migratory range, and with lineage C on the North American breeding grounds. Overall, we detected more parasite lineages in bobolinks than in cowbirds. Galapagos Plasmodium had similar host breadth compared to the non-Galapagos haemosporidian lineages detected in bobolinks, brown-headed cowbirds, and other grassland species. This study highlights the utility of global haemosporidian data in the context of migratory bird–parasite connectivity. It is possible that migratory bobolinks bring parasites to the Galapagos and that these parasites originate from different biogeographic regions representing both their breeding and overwintering sites

    ‘You're kinda passing a test’: A phenomenological study of women's experiences of breastfeeding

    Background: Despite an increasing research base about what helps or hinders breastfeeding, there is a dramatic drop in breastfeeding prevalence within the first 6 weeks. Aim: To explore the experiences of breastfeeding women. Methods: This study used interpretive phenomenology to research the experiences of 22 women who had all breastfed their youngest baby for at least 11 days. Data were collected using in-depth interviews when the women's babies were between 3-6 months of age. Thematic analysis was used to analyse findings. Findings: The women described tensions and mixed messages regarding breastfeeding, and contradictions between public health messages promoting breastfeeding and the support received to continue breastfeeding. The women also described how these approaches and messages affected their breastfeeding experiences, and how they managed breastfeeding as a result. Conclusions: The findings from this study revealed a patriarchal healthcare support system for breastfeeding whereby the women felt under surveillance and expected to perform to a prescribed ideal, but also a lack of support for exclusive breastfeeding after the initial postnatal period. These findings have clear implications for practice and policy

    ‘The final, published version of this article is available at http://www.karger.com/?doi=[insert DOI number] (e.g. http://doi: 10.1159/000368114Cystic and bile duct dysplasia is a rare histological finding, especially when found in the absence of an underlying malignancy. We report a patient who presented with jaundice and weight loss. Clinical and cytological evidence suggested a diagnosis of cholangiocarcinoma and the patient underwent a pancreatico-duodenectomy. Histopathological examination suggested a diagnosis of two foci of biliary dysplasia: cystic duct and lower common bile duct. Fifteen months later, the patient re-presented with signs of obstructive jaundice and biliary sepsis. Although CT scan revealed images highly indicative of metastatic disease, repeated biopsies failed to confirm this. Eventually a liver biopsy did reveal moderately differentiated adenocarcinoma, however oncological interventional was no longer appropriate and the patient was managed palliatively. This case report focuses on the current understanding of progression of biliary dysplasia

    A 20-year study of melt processes over Larsen C Ice Shelf using a high-resolution regional atmospheric model: Part 1, Model configuration and validation

    Following collapses of the neighbouring Larsen A and B ice shelves, Larsen C has become a focus of increased attention. Determining how the prevailing meteorological conditions influence its surface melt regime is of paramount importance for understanding the dominant processes causing melt and ultimately for predicting its future. To this end, a new, high-resolution (4 km grid spacing) Met Office Unified Model (MetUM) hindcast of atmospheric conditions and surface melt processes over the central Antarctic Peninsula is introduced. The hindcast is capable of simulating observed near-surface meteorology and surface melt conditions over Larsen C. In contrast with previous model simulations, the MetUM captures the observed east-west gradient in surface melting associated with foehn winds, as well as the inter-annual variability in melt shown in previous observational studies. As exemplars, we focus on two case studies – the months preceding the collapse of the Larsen B ice shelf in March 2002 and the high-foehn, high-melt period of March-May 2016 - to test the hindcast’s ability to reproduce the atmospheric effects that contributed to considerable melting during those periods. The results suggest that the MetUM hindcast is a useful tool with which to explore the dominant causes of surface melting on Larsen C
