    Proximal tibial osteotomies for the medial compartment arthrosis of the knee: a historical journey

    Several proximal tibial osteotomy techniques for the medial compartment arthrosis of the knee are described and traced in their development. These techniques are of the closed wedge, dome and open wedge types. We detail the differences in planning and surgery as well the need for different fixation devices. This historical and technical description will benefit those surgeons wishing to undertake the procedure as an alternative to joint replacement strategies

    Screw pullout is a very common problem in the fixation of sacrum with pedicle screws. The principal cause of this problem is that the cyclic micro motions in the fixation of sacrum are higher than the other regions of the vertebrae that limit the osteo-integration between bone and screw. In addition to that, the bone quality is very poor at sacrum region. This study investigated a possible solution to the pullout problem without the expandable screws' handicaps. Newly designed poly-ether-ether-ketone expandable shell and classical pedicle screws were biomechanically compared. Torsion test, pullout tests, fatigue tests, flexion/extension moment test, axial gripping capacity tests and torsional gripping capacity tests were conducted in accordance with ASTM F543, F1798 and F1717. Standard polyurethane foam and calf vertebrae were used as embedding medium for pullout tests. Classical pedicle screw pullout load on polyurethane foam was 564.8N compared to the failure load for calf vertebrae's 1264N. Under the same test conditions, expandable poly-ether-ether-ketone shell system's pullout loads from polyurethane foam and calf vertebrae were 1196.3 and 1890N, respectively. The pullout values for expandable poly-ether-ether-ketone shell were 33% and 53% higher than classical pedicle screw on polyurethane foam and calf vertebrae, respectively. The expandable poly-ether-ether-ketone shell exhibited endurance on its 90% of yield load. Contrary to poly-ether-ether-ketone shell, classical pedicle screw exhibited endurance on 70% of its yield load. Expandable poly-ether-ether-ketone shell exhibited much higher pullout performance than classical pedicle screw. Fatigue performance of expandable poly-ether-ether-ketone shell is also higher than classical pedicle screw due to damping the micro motion capacity of the poly-ether-ether-ketone. Expandable poly-ether-ether-ketone shell is a safe alternative to all other expandable pedicle screw systems on mechanical perspective.The author acknowledges Turkish Science Foundation (TUBITAK) for the financial support for the study with project number 111M583

    OBJETIVO: Este estudo tem por finalidade verificar a consolidação da osteotomia valgizante da tíbia com cunha de abertura fixada com placa tipo calço de Anthony® (OVT), no tratamento da osteoartrose medial do joelho varo, a correção da deformidade e a resposta clínica ao tratamento cirúrgico. MÉTODOS: Vinte pacientes (vinte joelhos) com osteoartrose do compartimento medial do joelho, com idade média de 48,4 ± 9,9, foram avaliados por um período mínimo de um ano. Os pacientes foram submetidos a avaliação radiográfica da consolidação e do eixo mecânico no pré e pós operatório, além da avaliação dos critérios de LYSHOLM. RESULTADOS: A consolidação da osteotomia ocorreu após 12 semanas em 100% dos casos sem complicações. A avaliação do LYSHOLM no pós operatório apresentou 80% de excelentes e bons resultados. A correção final média do eixo mecânico foi de 3,4 ± 3,3 graus de valgo. CONCLUSÃO: Concluímos que a consolidação da osteotomia supra-tuberositária da tíbia com cunha de abertura fixada com placa calço de Anthony® e com enxertia óssea tricortical ocorre num intervalo de três meses. A cirurgia é eficaz para a correção da deformidade em varo do joelho, e propicia melhora clínica significante para o paciente.<br>OBJECTIVE: This paper aims to check the proximal tibial valgusing open-wedge osteotomy union with Anthony® plate for the treatment of bowleg with medial osteoarthrosis, final correction of the deformity and clinical improvement. METHODS: Twenty patients (twenty knees) with medial osteoarthrosis of the knee, with mean age of 48.4 years, were evaluated for one year. The patients were submitted to the Lysholm's score, and also to X-ray studies before and after surgery. RESULTS: The osteotomy union occurred after 12 weeks in all cases without complications. The Lysholm's score was regarded as excellent or good in 80% of the cases. The postoperative mechanical alignment was 3.4 ± 3.3 valgus. CONCLUSION: We conclude that the union happened within 3 months with the use of bone grafting and the Anthony® plate to fix the open wedge osteotomy. The open wedge osteotomy is effective in fixing the deformity of the knee providing a significant improvement to patients' lives

    Neurovascular risks of sacral screws with bicortical purchase: an anatomical study

    The aim of this cadaver study is to define the anatomic structures on anterior sacrum, which are under the risk of injury during bicortical screw application to the S1 and S2 pedicles. Thirty formaldehyde-preserved human male cadavers were studied. Posterior midline incision was performed, and soft tissues and muscles were dissected from the posterior part of the lumbosacral region. A 6 mm pedicle screw was inserted between the superior facet of S1 and the S1 foramen. The entry point of the S2 pedicle screw was located between S1 and S2 foramina. S1 and S2 screws were placed on both right and the left sides of all cadavers. Then, all cadavers were turned into supine position. All abdominal and pelvic organs were moved away and carefully observed for any injury. The tips of the sacral screws were marked and the relations with the anatomic structures were defined. The position of the sacral screws relative to the middle and lateral sacral arteries and veins, and the sacral sympathetic trunk were measured. There was no injury to the visceral organs. In four cases, S1 screw tip was in direct contact with middle sacral artery. In two cases, S1 screw tip was in direct contact with middle sacral vein. It was observed that the S1 screw tips were in close proximity to sacral sympathetic trunk on both right and the left sides. The tip of the S2 screw was in contact with middle sacral artery on the left side only in one case. It is found that the tip of the S2 screw was closely located with the middle sacral vein in two cases. The tip of the S2 pedicle screw was in contact with the sacral sympathetic trunk in eight cases on the right side and seven cases on the left side. Lateral sacral vein was also observed to be disturbed by the S1 and S2 screws. As a conclusion, anterior cortical penetration during sacral screw insertion carries a risk of neurovascular injury. The risk of sacral sympathetic trunk and minor vascular structures together with the major neurovascular structures and viscera should be kept in mind
