Organizational Improvement Readiness Assessment (OIRA) Tool Evaluation
Background: Research shows that despite an increase in the number of organizational improvement initiatives there is a lack of consistent, sustained outcomes. Organizations struggle with how to reliably and accurately measure their readiness to drive and sustain outcomes. A search of the literature failed to identify a comprehensive, evidence-based tool that has been developed or evaluated to assess organizational improvement readiness. The objective of this project was to evaluate a newly developed Organizational Improvement Readiness Assessment (OIRA) Tool.
Project Design: Guided by two theoretical models, Delphi-Based Systems Architecting Framework (DB-SAF) and the Rogers Diffusion of Innovation Model, a 3-round, modified Delphi nominal group method was utilized. An evaluation panel of 13 organizational improvement subject matter experts (SMEs) was recruited, with 11 SMEs completing all 3 evaluation rounds. The relevancy and clarity of the OIRA Tool competencies was evaluated using an item-level content validity index (I-CVI) and a scale-level content validity index (S-CVI). Additionally, the tool was evaluated from a usability perspective using Google Analytics.
Results: The OIRA Tool was found to be clear, understandable, and relevant for organizations evaluating their readiness to drive and sustain outcomes improvements (S-CVI index of 0.92 and I-CVI indices ranging from 0.82 to 1.0). The final version of the tool included 22 competencies, modified based on expert consensus from the original 25. Usability test results confirmed the OIRA Tool, a web-based tool, is easy to use and well designed as measured by exit rates (15.44%), bounce rates (51.81%), and conversion rates (14%), all of which were significantly better than industry benchmarks.
Recommendations and Conclusions: Results of this project provide evidence of the content validity and usability of the OIRA Tool. The tool has the potential to help healthcare organizations assess their readiness to sustain organizational improvements and to identify gaps in leadership and culture, processes, technologies, and standards. The OIRA Tool provides the foundation for future analytics modeling and additional studies to test the theory and the advancement of outcomes improvement science
Near Infrared polarimetry of a sample of YSOs
Our goal is to study the physical properties of the circumstellar environment
of young stellar objetcs (YSOs). In particular, the determination of the
scattering mechanism can help to constrain the optical depth of the disk and/or
envelope in the near infrared. We used the IAGPOL imaging polarimeter along
with the CamIV infrared camera at the LNA observatory to obtain near infrared
polarimetry measurements at the H band of a sample of optically visible YSOs,
namely, eleven T Tauri stars and eight Herbig Ae/Be stars. An independent
determination of the disk (or jet) orientation was obtained for twelve objects
from the literature. The circumstellar optical depth could be then estimated
comparing the integrated polarization position angle (PA) with the direction of
the major axis of the disk projected in the plane of the sky. In general,
optically thin disks have polarization PA perpendicular to the disk plane. In
contrast, optically thick disks produce polarization PA parallel to the disks.
Among the T Tauri stars, three are consistent with optically thin disks (AS
353A, RY Tau and UY Aur) and five with optically thick disks (V536 Aql, DG Tau,
DO Tau, HL Tau and LkHalpha 358). Among the Herbig Ae/Be stars, two stars show
evidence of optically thin disk (Hen 3-1191 and VV Ser) and two of optically
thick disks (PDS 453 and MWC 297). Our results seem consistent with the fact
that optically thick disks at near infrared bands are associated more likely
with younger YSOs. Marginal evidence of polarization reversal is found in RY
Tau, RY Ori, WW Vul, and UY Aur. On the first three cases this feature can be
associated to the UXOR phenomenon. Correlations with the IRAS colours and the
spectral index yielded evidence of an evolutionary segregation with the disks

The structure of SgrAI bound to DNA; recognition of an 8 base pair target
The three-dimensional X-ray crystal structure of the ‘rare cutting’ type II restriction endonuclease SgrAI bound to cognate DNA is presented. SgrAI forms a dimer bound to one duplex of DNA. Two Ca2+ bind in the enzyme active site, with one ion at the interface between the protein and DNA, and the second bound distal from the DNA. These sites are differentially occupied by Mn2+, with strong binding at the protein–DNA interface, but only partial occupancy of the distal site. The DNA remains uncleaved in the structures from crystals grown in the presence of either divalent cation. The structure of the dimer of SgrAI is similar to those of Cfr10I, Bse634I and NgoMIV, however no tetrameric structure of SgrAI is observed. DNA contacts to the central CCGG base pairs of the SgrAI canonical target sequence (CR|CCGGYG, | marks the site of cleavage) are found to be very similar to those in the NgoMIV/DNA structure (target sequence G|CCGGC). Specificity at the degenerate YR base pairs of the SgrAI sequence may occur via indirect readout using DNA distortion. Recognition of the outer GC base pairs occurs through a single contact to the G from an arginine side chain located in a region unique to SgrAI
The CCP4 suite: integrative software for macromolecular crystallography
The Collaborative Computational Project No. 4 (CCP4) is a UK-led international collective with a mission to develop, test, distribute and promote software for macromolecular crystallography. The CCP4 suite is a multiplatform collection of programs brought together by familiar execution routines, a set of common libraries and graphical interfaces. The CCP4 suite has experienced several considerable changes since its last reference article, involving new infrastructure, original programs and graphical interfaces. This article, which is intended as a general literature citation for the use of the CCP4 software suite in structure determination, will guide the reader through such transformations, offering a general overview of the new features and outlining future developments. Human and mouse essentiality screens as a resource for disease gene discovery
The identification of causal variants in sequencing studies remains a considerable challenge that can be partially addressed by new gene-specific knowledge. Here, we integrate measures of how essential a gene is to supporting life, as inferred from viability and phenotyping screens performed on knockout mice by the International Mouse Phenotyping Consortium and essentiality screens carried out on human cell lines. We propose a cross-species gene classification across the Full Spectrum of Intolerance to Loss-of-function (FUSIL) and demonstrate that genes in five mutually exclusive FUSIL categories have differing biological properties. Most notably, Mendelian disease genes, particularly those associated with developmental disorders, are highly overrepresented among genes non-essential for cell survival but required for organism development. After screening developmental disorder cases from three independent disease sequencing consortia, we identify potentially pathogenic variants in genes not previously associated with rare diseases. We therefore propose FUSIL as an efficient approach for disease gene discovery. Discovery of causal variants for monogenic disorders has been facilitated by whole exome and genome sequencing, but does not provide a diagnosis for all patients. Here, the authors propose a Full Spectrum of Intolerance to Loss-of-Function (FUSIL) categorization that integrates gene essentiality information to aid disease gene discovery
Diagnosis and management of bone fragility in diabetes: an emerging challenge
Fragility fractures are increasingly recognized as a complication of both type 1 and type 2 diabetes, with fracture risk that increases with disease duration and poor glycemic control. Yet the identification and management of fracture risk in these patients remains challenging. This review explores the clinical characteristics of bone fragility in adults with diabetes and highlights recent studies that have evaluated bone mineral density (BMD), bone microstructure and material properties, biochemical markers, and fracture prediction algorithms (i.e., FRAX) in these patients. It further reviews the impact of diabetes drugs on bone as well as the efficacy of osteoporosis treatments in this population. We finally propose an algorithm for the identification and management of diabetic patients at increased fracture risk
Adjunctive rifampicin for Staphylococcus aureus bacteraemia (ARREST): a multicentre, randomised, double-blind, placebo-controlled trial.
BACKGROUND: Staphylococcus aureus bacteraemia is a common cause of severe community-acquired and hospital-acquired infection worldwide. We tested the hypothesis that adjunctive rifampicin would reduce bacteriologically confirmed treatment failure or disease recurrence, or death, by enhancing early S aureus killing, sterilising infected foci and blood faster, and reducing risks of dissemination and metastatic infection. METHODS: In this multicentre, randomised, double-blind, placebo-controlled trial, adults (≥18 years) with S aureus bacteraemia who had received ≤96 h of active antibiotic therapy were recruited from 29 UK hospitals. Patients were randomly assigned (1:1) via a computer-generated sequential randomisation list to receive 2 weeks of adjunctive rifampicin (600 mg or 900 mg per day according to weight, oral or intravenous) versus identical placebo, together with standard antibiotic therapy. Randomisation was stratified by centre. Patients, investigators, and those caring for the patients were masked to group allocation. The primary outcome was time to bacteriologically confirmed treatment failure or disease recurrence, or death (all-cause), from randomisation to 12 weeks, adjudicated by an independent review committee masked to the treatment. Analysis was intention to treat. This trial was registered, number ISRCTN37666216, and is closed to new participants. FINDINGS: Between Dec 10, 2012, and Oct 25, 2016, 758 eligible participants were randomly assigned: 370 to rifampicin and 388 to placebo. 485 (64%) participants had community-acquired S aureus infections, and 132 (17%) had nosocomial S aureus infections. 47 (6%) had meticillin-resistant infections. 301 (40%) participants had an initial deep infection focus. Standard antibiotics were given for 29 (IQR 18-45) days; 619 (82%) participants received flucloxacillin. By week 12, 62 (17%) of participants who received rifampicin versus 71 (18%) who received placebo experienced treatment failure or disease recurrence, or died (absolute risk difference -1·4%, 95% CI -7·0 to 4·3; hazard ratio 0·96, 0·68-1·35, p=0·81). From randomisation to 12 weeks, no evidence of differences in serious (p=0·17) or grade 3-4 (p=0·36) adverse events were observed; however, 63 (17%) participants in the rifampicin group versus 39 (10%) in the placebo group had antibiotic or trial drug-modifying adverse events (p=0·004), and 24 (6%) versus six (2%) had drug interactions (p=0·0005). INTERPRETATION: Adjunctive rifampicin provided no overall benefit over standard antibiotic therapy in adults with S aureus bacteraemia.
