Timing of Pars Plana Vitrectomy in Management of Gunshot Perforating Eye Injury: Observational Study

The aim of this study is to report the difference in either anatomical or functional outcome of vitreoretinal intervention in cases of gunshot perforating eye injury if done 2–4 weeks or after the 4th week after the original trauma. Patients were treated with pars plana vitrectomy and silicon oil. Surgeries were performed in the period from February 2011 until the end of December 2014. 253 eyes of 237 patients were reviewed. 46 eyes were excluded. 207 eyes of 197 patients were analyzed. The included eyes were classified based on the timing of vitrectomy in relation to the initial trauma into two groups: 149 eyes (the first group) operated on between the 3rd and the 4th week and 58 eyes (the second group) operated on after the 4th week after the trauma. Following one surgical intervention, in the first group, attached retina was achieved in 93.28% of patients. In the second group, attached retina was achieved in 96.55% of patients. All RD cases could be attached by a second surgery. Visual acuity improved in 81.21% of patients, did not change in 15.43% of patients, and declined in 3.35% of patients. In the second group, visual acuity improved in 81.03% of patients, did not change in 12.06% of patients, and worsened in 6.89% of patients. There was no statistically significant difference between the two groups in either anatomical or functional results. We recommend interfering before the 5th week after the trauma as retinal detachment is encountered more in cases operated on after the 4th week. The visual outcome depends on the site of entry and exit (the route of gunshot)

Value of diffusion-weighted MRI in evaluation of solid hepatic focal lesions in patients with renal insufficiency

Objectives: To evaluate the diagnostic accuracy of diffusion weighted MRI in diagnosis of solid hepatic focal lesions in patients with impaired renal functions. Patients and methods: This prospective study included (30) patients with impaired renal function and had solid hepatic focal lesions based on ultrasound examination. All patients subjected to diffusion MRI examination and ADC measurement, and the data obtained were compared with histopathological results of malignant lesions and previously reported appearance of benign lesions. Results: There were 57 solid focal lesions in the included 30 patients. The mean ADC value of hemangiomas was 2.03 × 10−3, lipoma was 0.1 × 10−3, HCC was 1.06 × 10−3 and for metastases was 1.2 × 10−3. Benign lesions have significant higher ADC values than malignant ones (p = 0.003), and in malignant lesions the primary hepatic carcinomas had lower ADC values compared to metastatic lesions which had no significant value. Using a cutoff value of 1.6 × 10−3 for the ADC to differentiate between benign and malignant lesions the AUC was 90% with Sensitivity, Specificity, PPV, NPV and accuracy of 92%, 80%, 98%, 50% and 91% respectively. Conclusion: DW-MRI and ADC value measurements are effective in characterizing solid focal hepatic lesions without contrast injection in patients with renal impairment

Artificial intelligence based optimal coordination of directional overcurrent relay in distribution systems considering vehicle to grid technology

Optimal coordination of overcurrent relays has become a major challenge in power distribution systems due to the increasing participation of distributed generation (DG) and electric vehicle (EV) charging stations, especially vehicle-to-grid (V2G) technology. This paper proposes a novel approach for optimal coordination of directional overcurrent relays (DOCRs) to achieve the minimum operating time and obtain optimal tuning in terms of time dial setting (TDS) and pick-up current (IP) considering V2G integration. The optimization was conducted using three different Artificial Intelligence (AI) techniques: grey wolf optimization (GWO), cuckoo search (CS), and Hunger Games Search (HGS), and was performed on two systems: IEEE 33-bus system and a local grid located in El-Shorouk City–district#8 (ShC-D8), Cairo, Egypt. The proposed methodology is constructed and validated on ETAP and MATLAB software packages. The simulation results demonstrate that the proposed optimization techniques provide high dynamic performance and accurate coordination of DOCRs. The results show that HGS achieves the minimum operating time of all overcurrent relays as compared with GWO and CS. The effect of V2G integration on the coordination methodology is also considered. The proposed approach provides a significant contribution to the field of power system protection and can help improve the reliability and efficiency of power distribution systems with V2G integration

Big Data and Cloud Computing: Trends and Challenges

Big data is currently one of the most critical emerging technologies. Big Data are used as a concept that refers to the inability of traditional data architectures to efficiently handle the new data sets. The 4V’s of big data – volume, velocity, variety and veracity makes the data management and analytics challenging for the traditional data warehouses. It is important to think of big data and analytics together. Big data is the term used to describe the recent explosion of different types of data from disparate sources. Analytics is about examining data to derive interesting and relevant trends and patterns, which can be used to inform decisions, optimize processes, and even drive new business models. Cloud computing seems to be a perfect vehicle for hosting big data workloads. However, working on big data in the cloud brings its own challenge of reconciling two contradictory design principles. Cloud computing is based on the concepts of consolidation and resource pooling, but big data systems (such as Hadoop) are built on the shared nothing principle, where each node is independent and selfsufficient. The integrating big data with cloud computing technologies, businesses and education institutes can have a better direction to the future. The capability to store large amounts of data in different forms and process it all at very large speeds will result in data that can guide businesses and education institutes in developing fast. Nevertheless, there is a large concern regarding privacy and security issues when moving to the cloud which is the main causes as to why businesses and educational institutes will not move to the cloud. This paper introduces the characteristics, trends and challenges of big data. In addition to that, it investigates the benefits and the risks that may rise out of the integration between big data and cloud computing

Is early intervention using Mansoura-VV uterine compression sutures an effective procedure in the management of primary atonic postpartum hemorrhage? : a prospective study

Abstract Background Postpartum hemorrhage is the leading cause of maternal death, uterine atony accounts for 75-90% of primary postpartum hemorrhage. The efficacy of the Uterine compression suture in the treatment of atonic postpartum hemorrhage is time-tested and can be said to be almost established.The aim of this study was to assess the role of the Mansoura-VV uterine compression suture as an early intervention in the management of primary atonic postpartum hemorrhage. Methods This prospective observational study included 108 women with primary atonic PPH over a period of 44 months. Uterine atony was diagnosed when the uterus was soft and failed to respond to ordinary ecbolics. Early intervention by Mansoura-VV uterine compression sutures was carried out within 15 min of the second dose of ecobolics and before progressing to any further surgical procedure. Results Following the Mansoura-VV uterine compression sutures, uterine bleeding was controlled in all except one patient (107/108 cases; 99.07%) who required additional bilateral uterine vessels ligation. Another case (0.93%) was subjected to re-laparotomy due to intraperitoneal hemorrhage. Packed RBC transfusion was needed in 10 cases (9.25%). Admission to ICU was needed in 9 cases (8.33%) because of associated medical conditions. One week following the procedure, 1 case (0.93%) was diagnosed with haematometra. Conclusion Early intervention in cases of primary atonic PPH using the Mansoura-VV uterine compression sutures is an easy, rapid and effective method in controlling PPH in low resource settings. Initial Egyptian ECMO experience

Introduction: Extracorporeal membrane oxygenation (ECMO) is considered a rescue therapy in severe cases of acute cardiac and or respiratory failure. Aim of the work: We describe our initial experience at the first ECMO center in Egypt. Methods: Our adult ECMO program started in January 2014. Since then we supported eleven respiratory failure patients on ECMO indicated according to ELSO guidelines and one case of ECMO CPR. Respiratory failure patients were subjected to VV ECMO when lung injury score (LIS) was above 3 and PaO2/FiO2 <100 on protective lung strategy mechanical ventilation according to ARDS net protocol and or severe hypercapnia with pH < 7.2 with trial of prone positioning in the indicated cases. Percutaneous cannulation was done in all patients using single lumen cannulae, additional cannula was added when needed. Cardiohelp (Maquet, Germany) and Rotaflow (Maquet, Germany) ECMO consoles were used with centrifugal pump. ECMO circuits PLS for Rotaflow and HLS for Cardiohelp were changed when indicated. The ECMO CPR patient was a primary PCI for acute inferior STEMI complicated by left main occlusion, VA ECMO instituted in the cath-lab after 20 min of CPR. Percutaneous (and or surgical) tracheostomy was done after 14 days of mechanical ventilation. Results: A total of twelve patients received ECMO between January 2014 and June 2015. The mean age was 35.9 years. (range 13–65 years), 8 males, with VV ECMO in 10 patients, and VA ECMO in 2 patients. Out of ten patients of VV ECMO, one had H1N1 pneumonia, one had advanced vasculitic lung, four had bacterial pneumonia, two traumatic lung contusions and one with organophosphorus poisoning, and one undiagnosed etiology leading to severe ARDS. Lung injury score range was 3–3.8, PaO2/FiO2 (20–76) mechanical ventilation duration before ECMO 1–14 days, Femoro-jugular cannulation in 7 patients and femoro-femoral in 2 patients and femoro-subclavian in 1 patient; all patients were initially sedated and paralyzed for (2–4 days) and ventilated on pressure controlled ventilation with Pmax of 25 cm H2O and PEEP of 10 cm H2O. In VA ECMO patients were cannulated percutaneously using femoro-femoral approach. One patient showed no neurologic recovery and died after 24 h, the other had CABG on ECMO however the heart didn’t recover and died after 9 days. Heparin intravenous infusion was used initially in all patients and changed to Bivalirudin in 2 patients due to possible HIT. Pump flow ranged from 2.6 to 6.5 L/min. Average support time was 12 days (range 2–24 days). Seven patients (63.3%) were successfully separated from ECMO and survived to hospital discharge. Hospital length of stay ranged from 3 to 42 days, tracheostomy was done percutaneously in 5 patients and surgically in 3. Gastrointestinal bleeding occurred in 6 patients, VAP in 7 patients, neurologic complications in 1 patient with complete recovery, cardiac arrhythmias in 3 patients, pneumothorax in 9 patients, and deep venous thrombosis in 2 patients
