    Good and Bad News from China's New Cooperative Medical Scheme

    The overall goal of this article is to understand the progress in implementing the New Cooperative Medical Scheme, while seeking to assess the strengths and weaknesses of the programme and, in particular, to understand its effects on the incidence of catastrophic medical payment. The study is based on two rounds of nationally representative household survey data collected in 2005 and 2008. The study found that the programme has a very high level of participation, and has increased farmers' use of medical services. However, despite efforts by both central and local governments and high household participation, the programme is only partially achieving its policy objectives. In particular, it has been able to extend to almost all of the rural population, but has failed to cover expenses for catastrophic illness, due to insufficient funds

    School Dropouts and Conditional Cash Transfers: Evidence from a Randomized Controlled Trial in Rural China’s Junior High Schools.

    Recent anecdotal reports suggest that dropout rates may be higher and actually increasing over time in poor rural areas. There are many reasons not to be surprised that there is a dropout problem, given the fact that China has a high level of poverty among the rural population, a highly competitive education system and rapidly increasing wages for unskilled workers. The overall goal of this study is to examine if there is a dropout problem in rural China and to explore the effectiveness that a Conditional Cash Transfer (CCT) program could have on dropouts (and mechanism by which the CCT might affect drop outs). To meet this objective, we conducted a randomized controlled trial (RCT) of a CCT using a sample of 300 junior high school students in a nationally-designated poor county in Northwest China. Using our data, we found that the annual dropout rate in the study county was high, about 7%. We find, however, that a CCT program reduces drop outs by 60%; the dropout rate is 13.3% in the control group and 5.3 % in the treatment group. The program is most effective in the case of girls, younger students and the poorest performing students.

    Do Poor Students Benefit from China’s Merger Program? Transfer Path and Educational Performance.

    Aiming to provide better education facilities and improve the educational attainment of poor rural students, China’s government has been merging remote rural primary schools to centralized village, town, or county schools since the late 1990s. To accompany the policy, boarding facilities have been constructed that allow (mandate) primary school-aged children to live at school rather than at home. More generally, there also have been efforts to improve rural schools, especially those in counties and towns. Unfortunately, little empirical work has been available to evaluate the impact of the new merger and investment programs on the educational performance of students. Drawing on a unique dataset that records both the path by which students navigate their primary school years (i.e., which different types of schools did students attend) as well as math test scores in three poverty-stricken counties, we use descriptive statistics and multivariate analysis (both OLS and covariate matching) to analyze the relationship between different transfer paths and student educational performance. This allows us to examine the costs and benefits of the school merger and investment programs. The results of the analysis show that students who attend county schools perform systematically better than those attend village or town schools. However, completing primary school in town schools seems to have no effect on students’ academic performance. Surprisingly, starting primary education in a teaching point does not hurt rural students; on the contrary, it increases their test scores in some cases. Finally, in terms of the boarding effect, the neutral estimate in OLS and the negative estimate in covariate matching results confirm that boarding at school does not help the students; in some cases it may even reduce their academic performance.

    542 Murine epidermis harbors functionally distinct langerhans cell subsets

    Epidermal Langerhans cells (LCs) derive from embryonic myeloid progenitors at the steady-state and monocyte progenitors under inflammatory conditions. LCs have the capacity to induce both immunity and tolerance in the skin, but how a single population of LCs mediates both these functions has perplexed researchers for decades. We hypothesized that LCs in murine epidermis have functionally heterogenous subpopulations. We employed single-cell RNA sequencing (scRNAseq) and scATACseq to identify transcriptional and epigenetic heterogeneity in LCs during late embryonic development, adult steady-state and inflamed-state. We found three transcriptionally distinct clusters in adult at steady-state: ATF3hiCD207lo (cLC1), ATF3loCD207hi (cLC2), and CD207+ cells expressing keratinocyte (KC) genes (kLCs). Ingenuity pathway analysis showed LC1 had downregulated immunostimulatory pathways and LC2 had upregulated immunostimulatory pathways. LCs from ATF3 knockout mice promoted Th1/2/17 immunity in co-culture experiments, confirming the immunotolerant function of cLC1s. cLC1 and cLC2 clusters had corresponding scATACseq clusters but kLCs did not, suggesting that kLCs may acquire their KC-“fingerprint” through interactions with KCs. scRNAseq analyses of E18.5 pre-LCs and 3 weeks post UVC-treatment also identified ATF3hi and ATF3lo clusters, but kLCs were neither present at E18.5 nor after UVC treatment.     Rethinking the Detection Head Configuration for Traffic Object Detection

    Multi-scale detection plays an important role in object detection models. However, researchers usually feel blank on how to reasonably configure detection heads combining multi-scale features at different input resolutions. We find that there are different matching relationships between the object distribution and the detection head at different input resolutions. Based on the instructive findings, we propose a lightweight traffic object detection network based on matching between detection head and object distribution, termed as MHD-Net. It consists of three main parts. The first is the detection head and object distribution matching strategy, which guides the rational configuration of detection head, so as to leverage multi-scale features to effectively detect objects at vastly different scales. The second is the cross-scale detection head configuration guideline, which instructs to replace multiple detection heads with only two detection heads possessing of rich feature representations to achieve an excellent balance between detection accuracy, model parameters, FLOPs and detection speed. The third is the receptive field enlargement method, which combines the dilated convolution module with shallow features of backbone to further improve the detection accuracy at the cost of increasing model parameters very slightly. The proposed model achieves more competitive performance than other models on BDD100K dataset and our proposed ETFOD-v2 dataset. The code will be available.Comment: 26 pages, 4 figures, 7 table

    Neddylation inhibitor MLN4924 suppresses cilia formation by modulating AKT1

    Abstract The primary cilium is a microtubule-based sensory organelle. The molecular mechanism that regulates ciliary dynamics remains elusive. Here, we report an unexpected finding that MLN4924, a small molecule inhibitor of NEDD8-activating enzyme (NAE), blocks primary ciliary formation by inhibiting synthesis/assembly and promoting disassembly. This is mainly mediated by MLN4924-induced phosphorylation of AKT1 at Ser473 under serum-starved, ciliary-promoting conditions. Indeed, pharmaceutical inhibition (by MK2206) or genetic depletion (via siRNA) of AKT1 rescues MLN4924 effect, indicating its causal role. Interestingly, pAKT1-Ser473 activity regulates both ciliary synthesis/assembly and disassembly in a MLN4924 dependent manner, whereas pAKT-Thr308 determines the ciliary length in MLN4924-independent but VHL-dependent manner. Finally, MLN4924 inhibits mouse hair regrowth, a process requires ciliogenesis. Collectively, our study demonstrates an unexpected role of a neddylation inhibitor in regulation of ciliogenesis via AKT1, and provides a proof-of-concept for potential utility of MLN4924 in the treatment of human diseases associated with abnormal ciliogenesis.https://deepblue.lib.umich.edu/bitstream/2027.42/148214/1/13238_2019_Article_614.pd

    Safety of Spectacles for Children's Vision: A Cluster-Randomized Controlled Trial.

    PURPOSE: To study safety of children's glasses in rural China, where fear that glasses harm vision is an important barrier for families and policy makers. DESIGN: Exploratory analysis from a cluster-randomized, investigator-masked, controlled trial. METHODS: Among primary schools (n = 252) in western China, children were randomized by school to 1 of 3 interventions: free glasses provided in class, vouchers for free glasses at a local facility, or glasses prescriptions only (Control group). The main outcome of this analysis is uncorrected visual acuity after 8 months, adjusted for baseline acuity. RESULTS: Among 19 934 children randomly selected for screening, 5852 myopic (spherical equivalent refractive error <=-0.5 diopters) eyes cif 3001 children (14.7%, mean age 10.5 years) had VA <= 6/12 without glasses correctable to >6/12 with glasses, and were eligible. Among these, 1903 (32.5%), 1798 (30.7%), and 2151 (36.8%) were randomized to Control, Voucher, and Free Glasses, respectively. Intention-to-treat analyses were performed on all 1831 (96.2%), 1699 (94.5%), and 2007 (93.3%) eyes of children with follow-up in Control, Voucher, and Free Glasses groups. Final visual acuity for eyes of children in the treatment groups (Free Glasses and Voucher) was significantly better than for Control children, adjusting only for baseline visual acuity (difference of 0.023 logMAR units [0.23 vision chart lines, 95% CI: 0.03, 0.43]) or for other baseline factors as well (0.025 logMAR units [0.25 lines, 95% CI 0.04, 0.45]). CONCLUSION: We found no evidence that spectacles promote decline in uncorrected vision with aging among children. (C) 2015 by Elsevier Inc.
