Rapid Syphilis Tests as Catalysts for Health Systems Strengthening: A Case Study from Peru.
OBJECTIVES: Untreated maternal syphilis leads to adverse pregnancy outcomes. The use of point of care tests (POCT) offers an opportunity to improve screening coverage for syphilis and other aspects of health systems. Our objective is to present the experience of the introduction of POCT for syphilis in Peru and describe how new technology can catalyze health system strengthening. METHODS: The study was implemented from September 2009-November 2010 to assess the feasibility of the use of a POCT for syphilis for screening pregnant women in Lima, Peru. Outcomes measured included access to syphilis screening, treatment coverage, partner treatment, effect on patient flow and service efficiency, acceptability among providers and patients, and sustainability. RESULTS: Before the introduction of POCT, a pregnant woman needed 6 visits to the health center in 27 days before she received her syphilis result. We trained 604 health providers and implemented the POCT for syphilis as the "two for one strategy", offering with one finger stick both syphilis and HIV testing. Implementation of the POCT resulted in testing and treatment on the first visit. Screening and treatment coverages for syphilis improved significantly compared with the previous year. Implementation of POCT has been scaled up nationally since the study ended, and coverages for screening, treatment and partner treatment have remained over 92%. CONCLUSIONS: Implementation of POCT for syphilis proved feasible and acceptable, and led to improvement in several aspects of health services. For the process to be effective we highlight the importance of: (1) engaging the authorities; (2) dissipating tensions between providers and identifying champions; (3) training according to the needs; (4) providing monitoring, supervision, support and recognition; (5) sharing results and discussing actions together; (6) consulting and obtaining feedback from users; and (7) integrating with other services such as with rapid HIV testing
Acute inhibition of MEK suppresses congenital melanocytic nevus syndrome in a murine model driven by activated NRAS and Wnt signaling
Congenital melanocytic nevus (CMN) syndrome is the association of pigmented melanocytic nevi with extra-cutaneous features, classically melanotic cells within the central nervous system, most frequently caused by a mutation of NRAS codon 61. This condition is currently untreatable and carries a significant risk of melanoma within the skin, brain, or leptomeninges. We have previously proposed a key role for Wnt signaling in the formation of melanocytic nevi, suggesting that activated Wnt signaling may be synergistic with activated NRAS in the pathogenesis of CMN syndrome. Some familial pre-disposition suggests a germ-line contribution to CMN syndrome, as does variability of neurological phenotypes in individuals with similar cutaneous phenotypes. Accordingly, we performed exome sequencing of germ-line DNA from patients with CMN to reveal rare or undescribed Wnt-signaling alterations. A murine model harboring activated NRASQ61K and Wnt signaling in melanocytes exhibited striking features of CMN syndrome, in particular neurological involvement. In the first model of treatment for this condition, these congenital, and previously assumed permanent, features were profoundly suppressed by acute post-natal treatment with a MEK inhibitor. These data suggest that activated NRAS and aberrant Wnt signaling conspire to drive CMN syndrome. Post-natal MEK inhibition is a potential candidate therapy for patients with this debilitating condition
Arduous implementation: Does the Normalisation Process Model explain why it's so difficult to embed decision support technologies for patients in routine clinical practice
Background: decision support technologies (DSTs, also known as decision aids) help patients and professionals take part in collaborative decision-making processes. Trials have shown favorable impacts on patient knowledge, satisfaction, decisional conflict and confidence. However, they have not become routinely embedded in health care settings. Few studies have approached this issue using a theoretical framework. We explained problems of implementing DSTs using the Normalization Process Model, a conceptual model that focuses attention on how complex interventions become routinely embedded in practice.Methods: the Normalization Process Model was used as the basis of conceptual analysis of the outcomes of previous primary research and reviews. Using a virtual working environment we applied the model and its main concepts to examine: the 'workability' of DSTs in professional-patient interactions; how DSTs affect knowledge relations between their users; how DSTs impact on users' skills and performance; and the impact of DSTs on the allocation of organizational resources.Results: conceptual analysis using the Normalization Process Model provided insight on implementation problems for DSTs in routine settings. Current research focuses mainly on the interactional workability of these technologies, but factors related to divisions of labor and health care, and the organizational contexts in which DSTs are used, are poorly described and understood.Conclusion: the model successfully provided a framework for helping to identify factors that promote and inhibit the implementation of DSTs in healthcare and gave us insights into factors influencing the introduction of new technologies into contexts where negotiations are characterized by asymmetries of power and knowledge. Strong Longitudinal Magnetic Fluctuations near Critical End Point in UCoAl: A ^59Co-NMR Study
We report ^59Co-NMR measurements in UCoAl where a metamagnetism occurs due to
enhancement of ferromagnetism by magnetic field. The metamagnetic transition
from a paramagnetic (PM) state to a ferromagnetic state is a first order
transition at low temperatures, but it changes to a crossover at high
temperatures on crossing the critical end pint (CEP) at T_CEP ~ 12 K. The
contrasting behavior between the relaxation rates 1/T_1 and 1/T_2 suggests that
the longitudinal magnetic fluctuation of U moment is strongly enhanced
especially near the CEP. A wide diffusion of the fluctuation from the CEP can

Dissociation constants and thermodynamic properties of amino acids used in CO2 absorption from (293 to 353) K
The second dissociation constants of the amino acids βalanine, taurine, sarcosine, 6-aminohexanoic acid, DL-methionine, glycine, L-phenylalanine, and L-proline and the third dissociation constants of L-glutamic acid and L-aspartic acid have been determined from electromotive force measurements at temperatures from (293 to 353) K. Experimental results are reported and compared to literature values. Values of the standard state thermodynamic properties are derived from the experimental results and compared to the values of commercially available amines used as absorbents for CO 2 capture.
Phenomenological Implications of Deflected Mirage Mediation: Comparison with Mirage Mediation
We compare the collider phenomenology of mirage mediation and deflected
mirage mediation, which are two recently proposed "mixed" supersymmetry
breaking scenarios motivated from string compactifications. The scenarios
differ in that deflected mirage mediation includes contributions from gauge
mediation in addition to the contributions from gravity mediation and anomaly
mediation also present in mirage mediation. The threshold effects from gauge
mediation can drastically alter the low energy spectrum from that of pure
mirage mediation models, resulting in some cases in a squeezed gaugino spectrum
and a gluino that is much lighter than other colored superpartners. We provide
several benchmark deflected mirage mediation models and construct model lines
as a function of the gauge mediation contributions, and discuss their discovery

Online interprofessional education related to chronic illness for health professionals : a scoping review
This scoping review focuses on identifying the quantity and quality of research evidence available with
regard to online interprofessional education related to chronic disease management. Recent advances
have seen the emergence of information communication technology and digital health solutions that
may improve monitoring of and self-management of chronic disease. With the growing chronic disease
burden globally, and the advancement of information communication technology, digital health solutions may improve chronic disease monitoring and self-management. However, health professionals are
slow to utilize this technology in chronic disease management. Online education has the potential to
enhance utilization of digital health solutions across interprofessional healthcare teams. This scoping
review focuses on online interprofessional education and eLearning strategies used to promote engagement and achievement of learning outcomes between health care professionals in chronic disease
management. A systematic search of the literature yielded 3112 papers; 15 studies were included in the
review following an independent screening process. The review found very limited research for online
interprofessional education related to chronic disease so it is not feasible to comment or draw conclusions in relation to its impact on interprofessional learning, student engagement in education or its
impact in practice, services or health outcomes. Research methodology and online eLearning strategies
varied across studies, highlighting the need for further rigorous studies that include consistency in
online interprofessional education strategies, evaluations and study methods
Search for Cold Debris Disks around M-dwarfs
Debris disks are believed to be related to planetesimals left over around
stars after planet formation has ceased. The frequency of debris disks around
M-dwarfs which account for 70% of the stars in the Galaxy is unknown while
constrains have already been found for A- to K-type stars. We have searched for
cold debris disks around 32 field M-dwarfs by conducting observations at lambda
= 850 microns with the SCUBA bolometerarray camera at the JCMT and at lambda =
1.2mm with the MAMBO array at the IRAM 30-m telescopes. This is the first
survey of a large sample of M-dwarfs conducted to provide statistical
constraints on debris disks around this type of stars. We have detected a new
debris disk around the M0.5 dwarf GJ842.2 at lambda = 850 microns, providing
evidence for cold dust at large distance from this star (~ 300AU). By combining
the results of our survey with the ones of Liu et al. (2004), we estimate for
the first time the detection rate of cold debris disks around field M-dwarfs
with ages between 20 and 200 Myr. This detection rate is 13^{+6}_{-8} % and is
consistent with the detection rate of cold debris disks (9 - 23 %) around A- to
K-type main sequence stars of the same age. This is an indication that cold

Advances in ab-initio theory of Multiferroics. Materials and mechanisms: modelling and understanding
Within the broad class of multiferroics (compounds showing a coexistence of
magnetism and ferroelectricity), we focus on the subclass of "improper
electronic ferroelectrics", i.e. correlated materials where electronic degrees
of freedom (such as spin, charge or orbital) drive ferroelectricity. In
particular, in spin-induced ferroelectrics, there is not only a {\em
coexistence} of the two intriguing magnetic and dipolar orders; rather, there
is such an intimate link that one drives the other, suggesting a giant
magnetoelectric coupling. Via first-principles approaches based on density
functional theory, we review the microscopic mechanisms at the basis of
multiferroicity in several compounds, ranging from transition metal oxides to
organic multiferroics (MFs) to organic-inorganic hybrids (i.e. metal-organic

Nontypeable haemophilus influenzae induces sustained lung oxidative stress and protease expression
© 2015 King et al. Nontypeable Haemophilus influenzae (NTHi) is a prevalent bacterium found in a variety of chronic respiratory diseases. The role of this bacterium in the pathogenesis of lung inflammation is not well defined. In this study we examined the effect of NTHi on two important lung inflammatory processes 1), oxidative stress and 2), protease expression. Bronchoalveolar macrophages were obtained from 121 human subjects, blood neutrophils from 15 subjects, and human-lung fibroblast and epithelial cell lines from 16 subjects. Cells were stimulated with NTHi to measure the effect on reactive oxygen species (ROS) production and extracellular trap formation. We also measured the production of the oxidant, 3-nitrotyrosine (3-NT) in the lungs of mice infected with this bacterium. NTHi induced widespread production of 3-NT in mouse lungs. This bacterium induced significantly increased ROS production in human fibroblasts, epithelial cells, macrophages and neutrophils; with the highest levels in the phagocytic cells. In human macrophages NTHi caused a sustained, extracellular production of ROS that increased over time. The production of ROS was associated with the formation of macrophage extracellular trap-like structures which co-expressed the protease metalloproteinase-12. The formation of the macrophage extracellular trap-like structures was markedly inhibited by the addition of DNase. In this study we have demonstrated that NTHi induces lung oxidative stress with macrophage extracellular trap formation and associated protease expression.
