Estimating daily nitrogen dioxide level: Exploring traffic effects
Data used to assess acute health effects from air pollution typically have
good temporal but poor spatial resolution or the opposite. A modified
longitudinal model was developed that sought to improve resolution in both
domains by bringing together data from three sources to estimate daily levels
of nitrogen dioxide () at a geographic location. Monthly
measurements at 316 sites were made available by the Study of
Traffic, Air quality and Respiratory health (STAR). Four US Environmental
Protection Agency monitoring stations have hourly measurements of . Finally, the Connecticut Department of Transportation provides data on
traffic density on major roadways, a primary contributor to
pollution. Inclusion of a traffic variable improved performance of the model,
and it provides a method for estimating exposure at points that do not have
direct measurements of the outcome. This approach can be used to estimate daily

Perceived Racial Discrimination and Nonadherence to Screening Mammography
Objective. We examined whether African American women were as likely as White women to receive the results of a recent mammogram and to self-report results that matched the mammography radiology report (i.e., were adequately communicated). We also sought to determine whether the adequacy of communication was the same for normal and abnormal results. Methods. From a prospective cohort study of mammography screening, we compared self-reported mammogram results, which were collected by telephone interview, to results listed in the radiology record of 411 African American and 734 White women who underwent screening in 5 hospital-based facilities in Connecticut between October 1996 and January 1998. Using multivariate logistic regression, we identified independent predictors of inadequate communication of mammography results. Results. It was significantly more common for African American women to experience inadequate communication of screening mammography results compared with White women, after adjustment for sociodemographic, access-to-care, biomedical, and psychosocial factors. Abnormal mammogram results resulted in inadequate communication for African American women but not White women (PAfrican American women may not be receiving the full benefit of screening mammograms because of inadequate communication of results, particularly when mammography results are abnormal
Neighborhood and Individual Level Socioeconomic Variation in Perceptions of Racial Discrimination
In approaching the study of racial discrimination and health, the neighborhood and individual-level antecedents of perceived discrimination need further exploration. We investigated the relationship between neighborhood and individual-level socioeconomic position (SEP), neighborhood racial composition, and perceived racial discrimination in a cohort of African-American and White women age 40-79 from Connecticut, USA.
Design. The logistic regression analysis included 1249 women (39% African- American and 61% White). Neighborhood-level SEP and racial composition were determined using 1990 census tract information. Individual-level SEP indicators included income, education, and occupation. Perceived racial discrimination was measured as lifetime experience in seven situations.
Results. For African-American women, living in the most disadvantaged neighborhoods was associated with fewer reports of racial discrimination (odds ratio (OR) 0.44; 95% confidence interval (CI) 0.26, 0.75), with results attenuated after adjustment for individual-level SEP (OR 0.54, CI: 0.29, 1.03), and additional adjustment for neighborhood racial composition (OR 0.70, CI: 0.30, 1.63). African-American women with 12 years of education or less were less likely to report racial discrimination, compared with women with more than 12 years of education (OR 0.57, CI: 0.33, 0.98 (12 years); OR 0.51, CI: 0.26, 0.99 (less than 12 years)) in the fully adjusted model. For White women, neither neighborhood-level SEP nor individual-level SEP was associated with perceived racial discrimination.
Conclusion. Individual and neighborhood-level SEP may be important in understanding how racial discrimination is perceived, reported, processed, and how it may influence health. In order to fully assess the role of racism in future studies, inclusion of additional dimensions of discrimination may be warranted
Spatiotemporal Calibration of Atmospheric Nitrogen Dioxide Concentration Estimates From an Air Quality Model for Connecticut
A spatiotemporal calibration and resolution refinement model was fitted to
calibrate nitrogen dioxide (NO) concentration estimates from the Community
Multiscale Air Quality (CMAQ) model, using two sources of observed data on
NO that differed in their spatial and temporal resolutions. To refine the
spatial resolution of the CMAQ model estimates, we leveraged information using
additional local covariates including total traffic volume within 2 km,
population density, elevation, and land use characteristics. Predictions from
this model greatly improved the bias in the CMAQ estimates, as observed by the
much lower mean squared error (MSE) at the NO monitor sites. The final
model was used to predict the daily concentration of ambient NO over the
entire state of Connecticut on a grid with pixels of size 300 x 300 m. A
comparison of the prediction map with a similar map for the CMAQ estimates
showed marked improvement in the spatial resolution. The effect of local
covariates was evident in the finer spatial resolution map, where the
contribution of traffic on major highways to ambient NO concentration

Identification of Predictive Pathways for Non-Hodgkin Lymphoma Prognosis
Despite decades of intensive research, NHL (non-Hodgkin lymphoma) still remains poorly understood and is largely incurable. Recent molecular studies suggest that genomic variants measured with SNPs (single nucleotide polymorphisms) in genes may have additional predictive power for NHL prognosis beyond clinical risk factors. We analyzed a genetic association study. The prognostic cohort consisted of 346 patients, among whom 138 had DLBCL (diffuse large B-cell lymphoma) and 101 had FL ( follicular lymphoma). For DLBCL, we analyzed 1229 SNPs which represented 122 KEGG pathways. For FL, we analyzed 1228 SNPs which represented 122 KEGG pathways. Unlike in existing studies, we targeted at identifying pathways with significant additional predictive power beyond clinical factors. In addition, we accounted for the joint effects of multiple SNPs within pathways, whereas some existing studies drew pathway-level conclusions based on separate analysis of individual SNPs. For DLBCL, we identified four pathways, which, combined with the clinical factors, had medians of the prediction logrank statistics as 2.535, 2.220, 2.094, 2.453, and 2.512, respectively. As a comparison, the clinical factors had a median of the prediction logrank statistics around 0.552. For FL, we identified two pathways, which, combined with the clinical factors, had medians of the prediction logrank statistics as 4.320 and 3.532, respectively. As a comparison, the clinical factors had a median of the prediction logrank statistics around 1.212. For NHL overall, we identified three pathways, which, combined with the clinical factors, had medians of the prediction logrank statistics as 5.722, 5.314, and 5.441, respective. As a comparison, the clinical factors had a median of the prediction logrank statistics around 4.411. The identified pathways have sound biological bases. In addition, they are different from those identified using existing approaches. They may provide further insights into the biological mechanisms underlying the prognosis of NHL
Modeled nitrate levels in well water supplies and prevalence of abnormal thyroid conditions among the Old Order Amish in Pennsylvania
<p>Abstract</p> <p>Background</p> <p>Nitrate is a widespread contaminant of drinking water supplies, especially in agricultural areas. Nitrate intake from drinking water and dietary sources can interfere with the uptake of iodide by the thyroid, thus potentially impacting thyroid function.</p> <p>Methods</p> <p>We assessed the relation of estimated nitrate levels in well water supplies with thyroid health in a cohort of 2,543 Old Order Amish residing in Lancaster, Chester, and Lebanon counties in Pennsylvania for whom thyroid stimulating hormone (TSH) levels were measured during 1995-2008. Nitrate measurement data (1976-2006) for 3,613 wells in the study area were obtained from the U.S. Geological Survey and we used these data to estimate concentrations at study participants' residences using a standard linear mixed effects model that included hydrogeological covariates and kriging of the wells' residuals. Nitrate levels estimated by the model ranged from 0.35 mg/L to 16.4 mg/L N-NO<sub>3</sub><sup>-</sup>, with a median value of 6.5 mg/L, which was used as the cutpoint to define high and low nitrate exposure. In a validation analysis of the model, we calculated that the sensitivity of the model was 67% and the specificity was 93%. TSH levels were used to define the following outcomes: clinical hyperthyroidism (n = 10), clinical hypothyroidism (n = 56), subclinical hyperthyroidism (n = 25), and subclinical hypothyroidism (n = 228).</p> <p>Results</p> <p>In women, high nitrate exposure was significantly associated with subclinical hypothyroidism (OR = 1.60; 95% CI: 1.11-2.32). Nitrate was not associated with subclinical thyroid disease in men or with clinical thyroid disease in men or women.</p> <p>Conclusions</p> <p>Although these data do not provide strong support for an association between nitrate in drinking water and thyroid health, our results do suggest that further exploration of this hypothesis is warranted using studies that incorporate individual measures of both dietary and drinking water nitrate intake.</p
