Prevention of gingival trauma:Oral hygiene devices and oral piercings

Maintaining healthy teeth and soft oral tissues for life is important. Oral hygiene devices and oral piercings can damage the soft oral tissues. This thesis investigates the safety of manual toothbrushes, interdental brushes and rubber bristles interdental cleaners by analysing the gingival abrasion score. The efficacy of the various oral hygiene devices is measured by the use of dental plaque scores and gingivitis scores. In addition, the prevalence, the short- and long-term effects and the incidence of complications associated with oral and peri-oral piercings on hard and soft tissues are investigated in the literature

Klinisch Forum “Oral Piercings, the beauty and the harm

Op dinsdag 23 mei 2017 organiseerde de afdeling Parodontologie ACTA een Klinisch Forum ‘Oral Piercings, the beauty and the harm’. Dit forum vond plaats op de vooravond van de promotie van Nienke Hennequin-Hoenderdos met het proefschrift: ‘Prevention of gingival trauma - oral hygiene devices and oral piercings’. Na afloop van het forum presenteerde Mariëlle Nap van het Ivoren Kruis de nieuwe patiëntenfolder ‘Mondpiercings’. Deze folder werd bijgesteld naar aanleiding van de inhoud van het proefschift. Promotor professor Fridus van der Weijden trad op als moderator van dit forum. Het drukbezochte forum werd bijgewoond door 200 studenten, mondhygiënisten en tandartsen en werd mede mogelijk gemaakt door donaties van de Stichting NVvP en Oral-

Gingival trauma: Tooth brushing and oral piercings

Maintaining healthy teeth and soft oral tissues for life is important. Gingival abrasions are reversible localized epithelial trauma of the gingiva. These soft tissue lesions are caused by oral hygiene procedures and are usually not detected during clinical examinations. One approach to assess gingival abrasions is to distinguish them from the normal gingiva by staining the undamaged gingiva with a commercially available plaque disclosing solution. Soft tissues in the oral cavity have the capacity to recover, but repeated trauma of the gingiva may result in gingival abrasions and/or recessions. Manual toothbrushes with end-rounded filaments cause significantly less gingival abrasions. Another factor that can potentially traumatize soft and hard oral tissues is oral jewelry. Oral piercings are not without risks, considering the serious complications for oral and general health reported in the literature. Both lip and tongue piercings are highly associated with the risk of gingival recession, and tongue piercings are associated with tooth injuries. To prevent the risk of complications, patients should be discouraged from wearing oral or perioral jewelry

Het behoud van gezonde gebitselementen en zachte orale weefsels is belangrijk. Klinisch onderzoek heeft aangetoond dat handtandenborstels met afgeronde borstelharen aanzienlijk minder gingiva-abrasiën veroorzaken dan die zonder afronding. Tapered (taps toelopende) borstelharen zijn een alternatief voor afgeronde borstelharen, maar er is geen sterk bewijs voor de effectiviteit. Voor de interdentale ruimtes, daar waar de tandenborstel niet komt, worden speciale mondhygiënehulpmiddelen geadviseerd. Uit klinisch onderzoek onder gingivitispatiënten bleek dat zowel interdentale ragers als plastic-rubberreinigers gingivitis verminderden na 4 weken gebruik. Plastic-rubberreinigers zorgden voor een significante verbetering van de interdentale bloedingsneiging vergeleken met ragers en ze veroorzaakten minder gingiva-abrasiën. Andere factoren die harde en zachte weefsels kunnen traumatiseren zijn mondpiercings. Mondpiercings dragen is zeker niet zonder risico. Tong- en lippiercings zijn geassocieerd met het ontstaan van gingivarecessies en tongpiercings met fracturen van gebitselementen. Om de risico’s op complicaties te voorkomen, moeten patiënten ontmoedigd worden om mond­piercings te dragen
