    Using the National Early Warning Score (NEWS) outside acute hospital settings: A qualitative study of staff experiences in the West of England

    © 2018 Author(s) (or their employer(s)). Objectives Early warning scores were developed to improve recognition of clinical deterioration in acute hospital settings. In England, the National Early Warning Score (NEWS) is increasingly being recommended at a national level for use outside such settings. In 2015, the West of England Academic Health Science Network supported the roll-out of NEWS across a range of non-acute-hospital healthcare sectors. Research on the use of NEWS outside acute hospitals is limited. The objective of this study was to explore staff experiences of using NEWS in these new settings. Design Thematic analysis of qualitative semi-structured interviews with purposefully sampled healthcare staff. Setting West of England healthcare settings where NEWS was being used outside acute hospitals - primary care, ambulance, referral management, community and mental health services. Participants Twenty-five healthcare staff interviewed from primary care (9), ambulance (3), referral management/acute interface (5), community (4) and mental health services (3), and service commissioning (1). Results Participants reported that NEWS could support clinical decision-making around escalation of care, and provide a clear means of communicating clinical acuity between clinicians and across different healthcare organisations. Challenges with implementing NEWS varied - in primary care, clinicians had to select patients for NEWS and adopt different methods of clinical assessment, whereas for paramedics it fitted well with usual clinical practice and was used for all patients. In community services and mental health, modifications were 'needed' to make the tool relevant to some patient populations. Conclusions This study demonstrated that while NEWS can work for staff outside acute hospital settings, the potential for routine clinical practice to accommodate NEWS in such settings varied. A tailored approach to implementation in different settings, incorporating guidance supported by further research on the use of NEWS with specific patient groups in community settings, may be beneficial, and enhance staff confidence in the tool

    EANM guideline for ventilation/perfusion single-photon emission computed tomography (SPECT) for diagnosis of pulmonary embolism and beyond

    These guidelines update the previous EANM 2009 guidelines on the diagnosis of pulmonary embolism (PE). Relevant new aspects are related to (a) quantification of PE and other ventilation/perfusion defects; (b) follow-up of patients with PE; (c) chronic PE; and (d) description of additional pulmonary physiological changes leading to diagnoses of left ventricular heart failure (HF), chronic obstructive pulmonary disease (COPD) and pneumonia. The diagnosis of PE should be reported when a mismatch of one segment or two subsegments is found. For ventilation, Technegas or krypton gas is preferred over diethylene triamine pentaacetic acid (DTPA) in patients with COPD. Tomographic imaging with V/P-SPECT has higher sensitivity and specificity for PE compared with planar imaging. Absence of contraindications makes V/P-SPECT an essential method for the diagnosis of PE. When V/P-SPECT is combined with a low-dose CT, the specificity of the test can be further improved, especially in patients with other lung diseases. Pitfalls in V/P-SPECT interpretation are discussed. In conclusion, V/P-SPECT is strongly recommended as it accurately establishes the diagnosis of PE even in the presence of diseases like COPD, HF and pneumonia and has no contraindications.Peer reviewe

    Low-coordinate first-row transition metal complexes in catalysis and small molecule activation

    Enforcing unusually low coordination numbers on transition metals with sterically demanding ligands has long been an area of interest for chemists. Historically, the synthesis of these challenging molecules has helped to elucidate fundamental principles of bonding and reactivity. More recently, there has been a move towards exploiting these highly reactive complexes to achieve a range of transformations using cheap, earth-abundant metals. In this Perspective, we will highlight selected examples of transition metal complexes with low coordination numbers that have been used in catalysis and the activation of small molecules featuring strong bonds (N2, CO2, and CO)

    Virtual wards for people with frailty:what works, for whom, how and why – a rapid realist review

    BackgroundVirtual wards (VWs) deliver multidisciplinary care at home to people with frailty who are at high risk of a crisis or in crisis, aiming to mitigate the risk of acute hospital admission. Different VW models exist, and evidence of effectiveness is inconsistent. AimWe conducted a rapid realist review to identify different VW models and to develop explanations for how and why VWs could deliver effective frailty management.MethodsWe searched published and grey literature to identify evidence on multidisciplinary VWs. Information on how and why virtual wards might ‘work’ was extracted and synthesised into context-mechanism-outcome configurations with input from clinicians and patient/public contributors. ResultsWe included 17 peer-reviewed and 11 grey literature documents. VWs could be short-term and acute (1-21 days), or longer-term and preventative (typically 3-7 months). Effective VW operation requires common standards agreements, information sharing processes, an appropriate multidisciplinary team that plans patient care remotely, and good co-ordination. VWs may enable delivery of frailty interventions through appropriate selection of patients, comprehensive assessment including medication review, integrated case management, and proactive care. Important components for patients and caregivers are good communication with the VW, their experience of care at home, and feeling involved, safe, and empowered to manage their condition.ConclusionsInsights gained from this review could inform implementation or evaluation of VWs for frailty. A combination of acute and longer-term VWs may be needed within a whole system approach. Proactive care is recommended to avoid frailty-related crises
