    Ankle reconstruction in type II fibular hemimelia

    Ankle reconstruction prior to limb lengthening for was performed in 13 patients with fibular hemimelia with complete radiological absence of the fibula (type II). There were different degrees of absence of metatarsal rays. The hindfoot deformity was a heel valgus in 12 patients and equinovarus in 1 patient. The patientsā€™ ages ranged from 9 to 26Ā months. Excision of the fibular anlage was performed with lateral subtalar and ankle soft tissue releases to restore the ankle and subtalar joint relationships. In all cases, the fibular anlage ended distally in a cartilaginous lateral malleolar remnant that was fused to the talus in two patients. This fibular remnant was advanced distally and fixed to the tibia with 2 Kirschner wires to recreate an ankle mortise. The period of follow-up ranged from 12 to 38Ā months. All patients had a stable ankle without tendency to valgus deformity or subluxation. The ankle range of movement was a mean of 27.3Ā° plantarflexion (25ā€“30) and 18Ā° dorsiflexion (15ā€“20). Reconstruction of the ankle in type II fibular hemimelia using advancement of the cartilaginous lateral malleolar remnant has produced encouraging results in the short-term but longer follow-up is needed

    Object Pose Estimation Annotation Pipeline for Multi-view Monocular Camera Systems in Industrial Settings

    Object localization, and more specifically object pose estimation, in large industrial spaces such as warehouses and production facilities, is essential for material flow operations. Traditional approaches rely on artificial artifacts installed in the environment or excessively expensive equipment, that is not suitable at scale. A more practical approach is to utilize existing cameras in such spaces in order to address the underlying pose estimation problem and to localize objects of interest. In order to leverage state-of-the-art methods in deep learning for object pose estimation, large amounts of data need to be collected and annotated. In this work, we provide an approach to the annotation of large datasets of monocular images without the need for manual labor. Our approach localizes cameras in space, unifies their location with a motion capture system, and uses a set of linear mappings to project 3D models of objects of interest at their ground truth 6D pose locations. We test our pipeline on a custom dataset collected from a system of eight cameras in an industrial setting that mimics the intended area of operation. Our approach was able to provide consistent quality annotations for our dataset with 26, 482 object instances at a fraction of the time required by human annotators

    Diagnostic Efficacy of Endoscopic Ultrasound Elastography in Differentiating Solid Pancreatic Lesions: A Single-Center Experience

    Background/Aims Endoscopic ultrasound (EUS) has a limited ability to determine the nature of solid pancreatic lesions (SPLs). Most recent ultrasound processors are provided with elastography software, which allows quantification of the tissue hardness. The aim of this study is to evaluate the effectiveness of the elasticity score (ES) and strain ratio (SR) in the differentiation of benign pancreatic lesions from malignant pancreatic lesions. Methods The study had a retrospective design; it included 97 patients with SPLs and 19 patients with inflammatory lesions. The ES and SR were determined during the examination; finally, EUS-guided fine needle aspiration was performed. Results In this 2-year study, 116 patients were enrolled (97 with malignant lesions and 19 with benign lesions). There were 69 men and 47 women. Their median age was 55.9 years. A cut-off point was detected at SR of 7.75 with a specificity of 99.9%, sensitivity of 90.7%, positive predictive value (PPV) of 99.9%, negative predictive value (NPV) of 67.9%, and accuracy of 92.2%. After adding the ES to the SR, the cut-off point at 7.75 resulted in a specificity of 94.6%, sensitivity of 99%, PPV of 98%, NPV of 98.5%, and accuracy of 97%. Conclusions The use of the ES combined with the SR increases the accuracy of differentiation between benign and malignant SPLs and is an effective method for the evaluation of pancreatic masses

    Offloading Safety- and Mission-Critical Tasks via Unreliable Connections

    For many cyber-physical systems, e.g., IoT systems and autonomous vehicles, offloading workload to auxiliary processing units has become crucial. However, since this approach highly depends on network connectivity and responsiveness, typically only non-critical tasks are offloaded, which have less strict timing requirements than critical tasks. In this work, we provide two protocols allowing to offload critical and non-critical tasks likewise, while providing different service levels for non-critical tasks in the event of an unsuccessful offloading operation, depending on the respective system requirements. We analyze the worst-case timing behavior of the local cyber-physical system and, based on these analyses, we provide a sufficient schedulability test for each of the proposed protocols. In the course of comprehensive experiments, we show that our protocols have reasonable acceptance ratios under the provided schedulability tests. Moreover, we demonstrate that the system behavior under our proposed protocols is strongly dependent on probability of unsuccessful offloading operations, the percentage of critical tasks in the system, and the amount of offloaded workload

    The Effectiveness of Intraocular Methotrexate in the Treatment of Posterior Uveitis in BehƧetā€™s Disease Patients Compared to Retrobulbar Steroids Injection

    Aim of Work. To evaluate the efficacy of intravitreal methotrexate (MTX) compared to retrobulbar triamcinolone acetonide (TAA), in controlling posterior segment involvement and inducing remissions among BehƧetā€™s disease (BD) patients. Study Design. This is a cross-sectional nonrandomized comparative study. Patients and Methods. 31 adult BD male patients with a mean disease duration of 5.45 years who presented with bilateral posterior segment involvement were included. Each patient received intravitreal injection of 400ā€‰Ī¼g/0.1ā€‰mL (MTX) for the right eye (Group A) and 1ā€‰mL of retrobulbar 40ā€‰mg/mL TAA for the left eye (Group B). Results. 90% of eyes showed complete improvement of anterior chamber reaction, whereas an improvement in vitreous activity in 77% with no significant differences between both groups (pā‰¤0.1). BCVA improved in 77.4% eyes (Group A) compared to 87.1% (Group B) (pā‰¤0.4). Relapses were noted in 11 eyes (35.5%), in group A, with the mean duration of remission being 19.1 weeks Ā± 2.13 compared to 7.35Ā±2.8 in 20 eyes (64.5%) in group B (pā‰¤0.1). Conclusion. No statistical differences were found between both treatment modalities; however, based on clinical observations, intravitreal MTX may ensure better control of inflammatory reaction and may encourage longer remission as compared to retrobulbar TAA in BD patients
