    A qualitative study exploring why adults with intellectual disabilities and obesity want to lose weight and views of their carers

    Background: Obesity is more prevalent in adults with intellectual disabilities (ID) compared to the general population. Motivations for weight loss may influence engagement with weight management programmes and have only been studied in adults without ID. Aims: To determine reasons given by adults with ID and obesity for seeking weight loss and whether these reasons differ from those of their carers. Methods: Prior to a multi-component weight management intervention, participants were asked “why do you want to lose weight?” Carers were asked their views and these were compared to the answers given by the adult with ID. Responses were themed. The Fisher’s Exact analysis was used to test for any relationship between reasons for seeking weight loss and participants’ level of ID, age, gender and BMI. Results: Eighteen men and 32 women; age 41.6 SD 14.6 years; BMI 40.8 SD 7.5 kg/m2; Level ID Mild (28 %), Moderate (42 %), Severe (22 %), Profound (8 %). Eleven were unable to respond. Six themes emerged; Health; Fitness / Activity / Mobility; Appearance / Clothes; Emotional / Happiness; For Others; Miscellaneous. The most frequent reason given overall and by women was “appearance.” Carers cited “health” most frequently and “appearance” least, rarely agreeing with participants. “Health” was given as a reason more from older adults and those with milder ID. No statistically significant associations were found between reasons for seeking weight loss and BMI age, gender or level of ID but the differing views of adults with ID and their carers were clear. Conclusions: Views of adults with obesity and mild or moderate ID can be collected. The opposing views of adults and their carers may affect motivation for weight loss

    The mercury-filled self-healing fuses developed for this program afford very good protection from circuit faults with rapid reclosure. Fuse performance and design parameters have been characterized. Life tests indicate a capability of 500 fuse operations. Fuse ratings are 150 v at 5, 15, 25 and 50 circuit A. A series of sample fuses using alumina and beryllia insulation have been furnished to NASA for circuit evaluation

    A strong-coupling expansion for the Hubbard model

    We reconsider the strong-coupling expansion for the Hubbard model recently introduced by Sarker and Pairault {\it et al.} By introducing slave particles that act as projection operators onto the empty, singly occupied and doubly occupied atomic states, the perturbation theory around the atomic limit distinguishes between processes that do conserve or do not conserve the total number of doubly occupied sites. This allows for a systematic t/Ut/U expansion that does not break down at low temperature (tt being the intersite hopping amplitude and UU the local Coulomb repulsion). The fermionic field becomes a two-component field, which reflects the presence of the two Hubbard bands. The single-particle propagator is naturally expressed as a function of a 2Ă—22 \times 2 matrix self-energy. Furthermore, by introducing a time- and space-fluctuating spin-quantization axis in the functional integral, we can expand around a ``non-degenerate'' ground-state where each singly occupied site has a well defined spin direction (which may fluctuate in time). This formalism is used to derive the effective action of charge carriers in the lower Hubbard band to first order in t/Ut/U. We recover the action of the t-J model in the spin-hole coherent-state path integral. We also compare our results with those previously obtained by studying fluctuations around the large-UU Hartree-Fock saddle point.Comment: 20 pages RevTex, 3 figure

    Transport on randomly evolving trees

    The time process of transport on randomly evolving trees is investigated. By introducing the notions of living and dead nodes a model of random tree evolution is constructed which describes the spreading in time of objects corresponding to nodes. By using the method of the age-dependent branching processes we derive the joint distribution function of the number of living and dead nodes, and determine the correlation between these node numbers as a function of time. Also analyzed are the stochastic properties of the end-nodes; and the correlation between the numbers of living and dead end-nodes is shown to change its character suddenly at the very beginning of the evolution process.     On the Formation of Galaxy Halos: Comparing NGC 5128 and the Local Group Members

    The metallicity distribution function (MDF) for the old red-giant stars in the halo of NGC 5128, the nearest giant elliptical galaxy, is virtually identical with the MDF for the old-disk stars in the LMC and also strongly resembles the halo MDF in M31. These galaxies all have high mean halo metallicities ( ~ -0.4$) with very small proportions of low-metallicity stars. These observations reinforce the view that metal-rich halos are quite normal for large galaxies of all types. Such systems are unlikely to have built up by accretion of pre-existing, gas-free small satellite galaxies, unless these satellites had an extremely shallow mass distribution (d log N / d log M > -1). We suggest that the halo of NGC 5128 is more likely to have assembled from hierarchical merging of gas-rich lumps in which the bulk of star formation took place during or after the merger stage.Comment: 10 pages, LaTeX, plus 3 figures in separate postscript files; Astronomical Journal, in press for December 200

    Computer Tomography (CT) for head injury: adherence to the National Institute for Health and Care Excellence (NICE) criteria

    Objective Head injury is a common presentation to the emergency department yet adherence to guidelines for management is suboptimal. Our study assesses adherence to National Institute for Health and Care Excellence (NICE) computed tomography (CT) head guidelines at a teaching hospital in the United Kingdom. It critically evaluates the efficacy of interventions. Methods We performed a quality improvement project to improve adherence to NICE CT head scan guidelines following head injury. Data was collected over one-month in 2014, and 2017. Interventions included education of the multidisciplinary team, information sheets, team engagement and tri-annual education sessions for junior doctors. The success of these interventions in the adult population was assessed objectively during the second cycle and subjectively via a junior doctor survey. Results 171 patients were included in the study. Following interventions, there was a statistically significant decrease of 23% in the number of CT heads requested with no clear indication (P = 0.00027). The mean time to scan for the one-hour indications decreased from 73 to 55 minutes. The mean time to report all scans decreased from 89 to 57 minutes. The survey results were encouraging. Conclusion Adherence to NICE guidelines for head injury is inadequate but can be improved by interventions

    Fertility control as a means of controlling bovine tuberculosis in badger (Meles meles) populations in south-west England: predictions from a spatial stochastic simulation model

    A spatial stochastic simulation model was used to assess the potential of fertility control, based on a yet-to-be-developed oral bait-delivered contraceptive directed at females, for the control of bovine tuberculosis in badger populations in south-west England. The contraceptive had a lifelong effect so that females rendered sterile in any particular year remained so for the rest of their lives. The efficacy of fertility control alone repeated annually for varying periods of time was compared with a single culling operation and integrated control involving an initial single cull followed by annually repeated fertility control. With fertility control alone, in no instance was the disease eradicated completely while a viable badger population (mean group size of at least one individual) was still maintained. Near eradication of the disease (less than 1% prevalence) combined with the survival of a minimum viable badger population was only achieved under a very limited set of conditions, either with high efficiency of control (95%) over a short time period (1-3 years) or a low efficiency of control (20%) over an intermediate time period (10-20 years). Under these conditions, it took more than 20 years for the disease to decline to such low levels. A single cull of 80% efficiency succeeded in near eradication of the disease (below 1% prevalence) after a period of 6-8 years, while still maintaining a viable badger population. Integrated strategies reduced disease prevalence more rapidly and to lower levels than culling alone, although the mean badger group size following the onset of control was smaller. Under certain integrated strategies, principally where a high initial cull (80%) was followed by fertility control over a short (1-3 year) time period, the disease could be completely eradicated while a viable badger population was maintained. However, even under the most favourable conditions of integrated control, it took on average more than 12 years following the onset of control for the disease to disappear completely from the badger population. These results show that whilst fertility control would not be a successful strategy for the control of bovine tuberculosis in badgers if used alone, it could be effective if used with culling as part of an integrated strategy. This type of integrated strategy is likely to be more effective in terms of disease eradication than a strategy employing culling alone. However, the high cost of developing a suitable fertility control agent, combined with the welfare and conservation implications, are significant factors which should be taken into account when considering its possible use as a means of controlling bovine tuberculosis in badger populations in the UK
