Pars plana vitrectomy for tractional diabetic macular edema with or without internal limiting membrane peeling
Background: The effectiveness of internal limiting membrane (ILM) peeling in the surgical treatment of tractional diabetic macular edema (DME), although widely examined, remains controversial. This study aimed to assess the efficacy of pars plana vitrectomy (PPV) in the management of tractional DME and to highlight any benefits of additional ILM peeling.
Methods: This was an open-label, prospective, comparative, and interventional study that enrolled 50 eyes with tractional DME that underwent PPV and allocated each to one of two groups: group A consisted of 25 eyes that had no ILM peeling and group B consisted of 25 eyes that underwent ILM peeling. Postoperative assessments of best-corrected distance visual acuity (BCDVA) in the logarithm of minimal angle of resolution (logMAR) notation and central macular thickness (CMT) were performed at 1, 3, and 6 months postoperatively.
Results: At baseline, the two groups were comparable in terms of sex ratios, phakic status, insulin use, coexistence of hypertension, and mean (standard deviation [SD]) age, BCDVA, CMT, duration of diabetes mellitus, and glycosylated hemoglobin (HbA1c) levels. In group A, the mean (SD) BCDVA improved significantly from 0.89 (0.12) logMAR preoperatively to 0.64 (0.24) logMAR (P < 0.001), and the mean (SD) CMT declined significantly from 471.28 (80.83) micrometer to 228.20 (26.45) micrometer (P < 0.001), at the 6-month postoperative assessment. Likewise, in group B, the mean (SD) BCDVA improved significantly from 0.83 (0.10) logMAR preoperatively to 0.58 (0.24) logMAR (P < 0.001), and the mean (SD) CMT decreased significantly from 496.84 (89.82) micrometer to 226.20 (18.04) micrometer (P < 0.001), after 6 months. There were no significant differences between groups A and B in the changes in BCDVA (Delta BCDVA) or CMT (Delta CMT) at 1, 3, and 6 months postoperatively with respect to the baseline values (all P > 0.05). Postoperative complications were comparable between the two groups. A significant negative correlation was detected between the preoperative HbA1c level and BCDVA improvement in all participants (r = - 0.82; P < 0.001).
Conclusions: PPV is an effective treatment for tractional DME. Additional ILM peeling was not significantly associated with functional and anatomical benefits over a short period. Long-term glycemic control plays a role in vision gain after vitrectomy in patients with diabetes. Further long-term studies are required to verify our findings
Histological Study of Exocrine part of Pancreas in Laying hens (Gallus gallus domesticus)
Due to the importance of the pancreas gland and its important role of the digestion process, as well as the economic importance of guinea fowl (Numida meleagris). The present study was conducted to study the pancreas histologically in guinea fowl. Samples were taken from the pancreas of (15) healthy adult males of guinea fowl after slaughtering them and separating the pancreas from its appendages. All samples were subjected to histological techniques. In the current study, four types of stains were used when conducting the histological study, which are the Masson trichrome stain, Heamatoxylin and eosin stain, Gomori’s stain and Periodic Acid-Schiff stain. This current study discovered that the pancreas of guinea fowl was parenchymal tissue surrounded by a thin connective tissue capsule. The microscopic examination exhibited the parenchyma tissue of the pancreas was composted of an exocrine portion and an endocrine portion. The exocrine portion forms the largest area of pancreas and consisted of acini and ducting system. The secretory acini vary in shapes and size as globoid, oval, elongated and amorphous, while the exocrine ducts system consisted of intercalated, intralobular, interlobular, and the two major pancreatic ducts, the current study detected that the guinea fowl (Numida meleagris) exhibits the identical structure as other bird species.
Evaluation of Retinol Level Among Preschool Children, Pregnant and Lactating Women Attending Primary Health Care Centres in Baghdad
Background: Vitamin A deficiency (VAD) is a major public health nutrition problem in the developing world. There have been no studies on this topic in Iraq. This study was designed to evaluate the serum retinol levels of preschool children, pregnant and lactating women. Objectives: The present study is an attempt to estimate the prevalence of vitamin A deficiency among preschool children, pregnant and lactating women attending primary health care centers in Baghdad, in addition to figure out the relation between vitamin A deficiency with some demographical, clinical, variables. Subjects and Methods: The study was conducted during the period from October to December 2009. The sample was comprised of 490 subjects, Lactating women pregnant women and under 6 year's old children attending ten primary health care centers in Baghdad. The data were collected through direct interview; blood samples were taken and analyze for serum retinol (SR) by HPLC analysis and hemoglobin (Hb) level, anthropometric measurement were obtained for the study sample. Results: The study showed that the prevalence of vitamin A deficiency in preschool aged children (below 6 years) was (38.3 %); and that for lactating women and pregnant women were (7.1 %) and (25 %) respectively. Forty percent of pregnant women, (25.8 %) of lactating women and a total of (58.6 %) preschool children were anemic, A correlation coefficient between SR and Hb concentrations was significant (N=490, r=0.533, P<0.0001). Conclusion: Vitamin A deficiency is a public health problem, this study shows that subjects in the 3 groups (preschool children, pregnant and lactating women) are at risk of VAD and anemia; nearly half of them had the co-occurrence of VAD and anemia. A close association between vitamin A deficiency and anemia with a correlation coefficient between SR and Hb concentrations was significant
Microstrip Wireless Antenna Used By Capsule For Stomach Internal Photography
a wireless capsule endoscopy is small device used to examine the digestive system. The big challenge in the development of the capsule is how to get high date rate and also reduce the size of the capsule so micro strip antenna has been used which has several advantages such as small size, light weight, low cost. In this paper, UWB mini polygonal slotted patch antenna has been designed which has overall size of 10 X 10mm and bandwidth of 6GHz (5.5 – 11.5 GHz) .The simulation results for return loss (S11) has -26.32 at 6.6 GHz and voltage standing wave ratio (VSWR) at 1.09. The simulated radiation patterns are Omni-directional radiations over the UWB bandwidth.
Fatigue Life Prediction for an Extrusion Die Based on the Experimental Factors
According to the fact that the practical method for finding the working fatigue life is very expensive and takes a longer time with a huge amount of samples till the failure initiation of the die. So, the present research describes the main purpose of predicting the fatigue life of the die numerically based on the highest loading factors getting from the experimental work, indicating how many numbers of product samples can be produced during the whole life of this designed die depending on these loading or over. This fatigue life analysis and assessment were adopted by ANSYS Workbench 2019 R3 software depending on the loading factors (axial extrusion load and friction temperature difference) affecting the die strength and fatigue life during the experimental extrusion process. These factors were generated according to the variation of extrusion speed and changing product materials. The extrusion die was used to convert a cylindrical bar with (19) mm diameter into a square bar with (13 × 13) mm with an extrusion rate of 1.67. The results showed that the highest stress intensity factor reached about (251.4 MPa) at the critical converge-diverge section due to the die profile design, and the die is safe and has an infinite life reaching up to (10^9 cycles), using Lead or Aluminum products taking the maximum extrusion load of (90 kN) and temperature difference of (2 ℃) at 150 mm/min extrusion speed. While changing the product billet into harder materials, the die life decreased and might fail later
Study the Level and the Fluctuation of Induced Nitric Oxide Synthase in Iraqi Patients with Hypertension
The known NOS enzymes are generally alluded to as 'dimeric'in their dynamic shape, disregarding the required calmodulins (CaMs) which, entirely, mean they are tetramers (of two NOS) monomers related with two CaMs). This review included 56 subjects coordinated ages and sex separated into two groups; 28 hypertensive subjects and 28 healthy subjects as control group. The accompanying investigation was done: INOS was fundamentally higher in patients contrasted with controls while there were no noteworthy contrasts found in egg whites amongst patient and control gathering. Also, the expansion in iNOS in light of the fact that iNOS creates a lot of NO as a guard component because of cytokines and is an essential consider the reaction of the body to assault by parasites, bacterial contamination, and tumor development. It has been reasoned that is critical nappy on perfect weight, since heftiness considered principle variables for coronary illness and solidifying of the veins. There is a positive connection between oxidation comes about because of hypertension and their advancements. The point of this review was to examine the connection between of some oxidative anxiety markers and cardiovascular infections
Exploring the Availability and Use of Agricultural Extension Services by Small-Scale Coffee Farmers in Al-Udein District, Ibb Governorate, Yemen
This research aimed to study the availability of extension services that are provided to the coffee small-scale farmers in the Al-Udein district of Yemen. In addition, it outlines the other extension sources that farmers might depend on for the improvement of their knowledge and skills. The researchers found that the majority of respondents (70.8%) do not have access to any public institutions that provide extension services. Furthermore, the whole respondents (100%) stated that they are highly dependent on their experiences gained from the traditional practices. Moreover, more than (90%) of the respondents moderately depend on their friends and neighbors as a source of knowledge and skills. Furthermore, the researchers pointed out that (100%) of the respondents never depend on classical organizations as sources of agricultural extension knowledge. Additionally, (90%) of the respondents do not, at all, depend on agriculture exhibitions, agriculture films, printed materials, radio or TV programs as sources of knowledge and skills. However, (6.3%) of the respondents said that they are highly dependent on ‘WhatsApp’ as a source of knowledge. Finally, the researchers recommended that it is necessary to enhance the role of public extension services and encourage farmers to use the social media platforms and mobile phones in order to support the exchange of knowledge between extension services in one hand and between farmers themselves in the other hand which will led to enhancing the knowledge exchange process effectively and efficiently
