    Environmental condition and residents’ awareness of the Hannoogawa River, a concrete-lined stream in Higashi-Hiroshima City, Japan

    半尾川(はんのをがわ)は,広島県東広島市中心部の西条地区を北から南に流れるコンクリート三面護岸化された小河川である。この川の環境に関する状況と将来の環境改善に向けた課題を明らかにするために,水質調査,水生生物調査,周辺住民の半尾川に対する認識についてのアンケート調査を実施した。水質調査の結果,BODは1.1~3.7 mg L-1の範囲にあり,やや汚染されていることが示唆された。しかし,トラップによる生物調査では全国水生生物調査で指定されている指標生物8種が確認され,この中には比較的きれいな水の指標生物も含まれていた。住民の意識調査の結果,最近10年以内に転入してきた住民は,それ以前から住んでいる住民にくらべ,半尾川の環境に関心が薄い傾向が認められた。今後,環境改善を進めていくためには,住民の関心を高めることが不可欠と考えられる。The Hannoogawa River in the western part of central Saijo town, Higashi-Hiroshima City, is a concrete-lined stream flowing north to south. To clarify its current environmental state and identify challenges for future environmental improvement, we surveyed the water quality, aquatic animals, and the local residents’ awareness of the stream. The biochemical oxygen demand of the stream water ranged between 1.1 and 3.7 mg L-1, indicating that the water was moderately polluted. However, several aquatic species that prefer oligotrophic water were recorded in a trap survey. An awareness questionnaire survey showed that newer residents who had moved to the area within the last 10 years tended to have little interest in the stream environment as compared with the responses of older residents. The results suggest a need for social outreach to increase residents’ awareness of the stream and thus fulfil the requirements for environmental improvements

    Risk for atrial fibrillation in patients with hypertrophic cardiomyopathy: Association with insulin resistance

    SummaryBackgroundWe undertook a cross-sectional study to test the hypothesis that patients with hypertrophic cardiomyopathy (HCM) who have impaired left ventricular (LV) diastolic function are insulin resistant. We also evaluated the relation between the development of atrial fibrillation (AF) and insulin resistance (IR) in patients with HCM.Methods and resultsEighty-eight patients with HCM (71 men, 17 women) were enrolled in the study. IR was estimated using the homeostasis model assessment (HOMA) index. Echocardiographically determined left atrial (LA) dimension was measured as a marker of LA size. The ratio of trasmitral early LV filling velocity to early diastolic mitral annulus velocity (E/e′) was also measured as a marker of LV diastolic function. Twenty-seven patients (31%) had IR. Multivariate logistic regression analyses showed that independent determinants of AF were increased LA size [odds ratio (OR) 3.5, 95% confidence interval (CI) 1.2–9.8] and impaired LV diastolic function [OR 4.6, 95% CI 1.6–12.8]. The strongest determinant of LA size was the HOMA index (p=0.0005). Similarly, the HOMA index (p=0.0019) was an independent determinant of LV diastolic function.ConclusionIR is highly prevalent among non-diabetic patients with HCM. A possible mechanism by which IR affects the development of AF is mediated through its association with increased LA size or impaired LV diastolic function. IR may be an important underlying mechanism for the genesis of AF in HCM

    Evaluation of Preoperative Magnetic Resonance Cholangiopancreatography in Acute Cholecystitis to Predict Technical Difficulties in Laparoscopic Cholecystectomy

    Magnetic resonance cholangiopancreatography (MRCP) is a non-invasive imaging technique that provides high-quality visualization of the biliary tree, including the gallbladder. This study aimed to evaluate the useful-ness of preoperative MRCP for acute cholecystitis in predicting technical difficulties during laparoscopic chole-cystectomy (LC). A total of 168 patients who underwent LC with preoperative MRCP were enrolled in this study. Patients were divided into two groups according to preoperative MRCP findings: the visualized group (n = 126), in which the entire gallbladder could be visualized; and the non-visualized group (n = 42), in which the entire gallbladder could not be visualized. The perioperative characteristics and postoperative complica-tions of the two groups were retrospectively analyzed. Operation time was longer in the non-visualized group (median 101.5 vs. 143.5 min; p < 0.001). The non-visualized group had significantly more intraoperative blood loss than the visualized group (median 5 vs. 10 g; p = 0.05). The rate of conversion to open cholecystectomy was significantly higher in the non-visualized group (1.6 vs. 9.5%; p = 0.03). In conclusion, patients in the non- visualized group showed higher difficulty in performance of LC. Our MRCP-based classification is a simple and effective means of predicting difficulties in performing LC for acute cholecystitis

    Clinical evaluation of suture materials for transtibial pullout repair of medial meniscus posterior root tear

    Background: There are no recommendations for specific suture materials in transtibial pullout repair of medial meniscus posterior root tears. This study aimed to evaluate the clinical outcomes of transtibial pullout repair of medial meniscus posterior root tears using ultrahigh-molecular-weight polyethylene sutures and suture tape. Methods: We retrospectively reviewed the data of 36 patients (27 women and 9 men, mean age 64.1 years) who had undergone transtibial pullout repair of medial meniscus posterior root tears between November 2018 and December 2019. Two groups of 18 patients each received either two different cord-like sutures or suture tape. Clinical parameters were assessed preoperatively and on second-look arthroscopy (mean postoperative period 12 months). The meniscal healing status was assessed using a previously published scoring system (ranging from 0 to 10), and the incidence rate of suture cut-out was assessed on second-look arthroscopy. Results: All clinical scores significantly improved in both groups, with no significant between-group differences on second-look arthroscopy. The arthroscopic meniscal healing scores significantly differed between sutures (mean 6.7 points) and suture tape (mean 7.4 points; p = 0.044). No significant between-group difference in the suture cut-out rate was observed. Conclusions: This study found no significant differences in the clinical outcomes between ultrahigh-molecular-weight polyethylene sutures and suture tape. Favorable clinical outcomes were obtained using both types of suture; however, the usefulness of suture tape appears to be limited

    Synchronous Total Occlusion of the Celiac Axis and Superior Mesenteric Artery: An Autopsy Case

    Acute mesenteric ischemia (AMI) is often caused by superior mesenteric artery (SMA) embolization. We report a rare case of synchronous celiac axis and SMA embolization in an elderly woman with initially mild abdominal pain. Ultimately, a second contrast-enhanced computed tomography revealed extensive necrosis from the stomach to the transverse colon together with liver ischemia due to hours of occlusion. Multiorgan failure made palliation the only option, and she died the following evening. Autopsy revealed a fragile atherosclerosis-asso-ciated thrombus. Careful examination and repeat diagnostic tests should be performed in patients with mild abdominal symptoms at risk for AMI

    DNA topoisomerase II interacts with Lim15/Dmc1 in meiosis

    Lim15/Dmc1 is a meiosis specific RecA-like protein. Here we propose its participation in meiotic chromosome pairing-related events along with DNA topoisomerase II. Analysis of protein–protein interactions using in vitro binding assays provided evidence that Coprinus cinereus DNA topoisomerase II (CcTopII) specifically interacts with C.cinereus Lim15/Dmc1 (CcLim15). Co-immunoprecipitation experiments also indicated that the CcLim15 protein interacts with CcTopII in vivo. Furthermore, a significant proportion of CcLim15 and CcTopII could be shown to co-localize on chromosomes from the leptotene to the zygotene stage. Interestingly, CcLim15 can potently activate the relaxation/catenation activity of CcTopII in vitro, and CcTopII suppresses CcLim15-dependent strand transfer activity. On the other hand, while enhancement of CcLim15's DNA-dependent ATPase activity by CcTopII was found in vitro, the same enzyme activity of CcTopII was inhibited by adding CcLim15. The interaction of CcLim15 and CcTopII may facilitate pairing of homologous chromosomes
