Assessment of the levels of nitric oxide (NO) and cytokines (IL-5, IL-6, IL-13, TNF, IFN-gamma) in giardiosis
The current study aims to determine the involvement of cellular responses in combating Giardiaintestinalis invasion. The study group consisted of 44 women and 18 men, aged 18–72 years, infected withG. intestinalis. The diagnosis was established based on laboratory investigations (examination of stool, choloscopy,GSA-65). Blood for analysis was collected before antiparasitic treatment and two weeks after treatment termination.The control group consisted of 22 women and 18 men aged 20–45 years. The serum concentrations ofIL-5, IL-6, IL-13, TNF, IFN-g were assayed using a set of Quantikine human. The concentrations of NO in theserum were determined using a set of Total Nitric Oxide Assay. Patients infected with G. intestinalis showeda statistically significant increase in the levels of NO, IFN-g and IL-13. Even the antiparasitic treatment applieddid not reduce the levels of these parameters and only caused a rise in IL-6. Our study showed a lack of acuteinflammatory state in the course of G. intestinalis infection
Assessment of the influence of the inflammatory process on the activation of blood platelets and morphological parameters in patients with ulcerative colitis (colitis ulcerosa)
Ulcerative colitis (colitis ulcerosa) is a non-specific inflammatory bowel disease of unknown etiology. The
symptoms which are observed in the course of ulcerative colitis are: an increase in the number of leukocytes and
blood platelets, an increase in the concentration of IL-6 and anemia. Blood platelets are the key element, linking
the processes of hemostasis, inflammation and the repair of damaged tissues. Activation of blood platelets is connected
with changes in their shape and the occurrence of the reaction of release. P-selectin appears on the surfaces
of activated blood platelets and the concentration level of soluble P-selectin increases in the blood plasma. The aim
of this study was to define whether the increased number of blood platelets in patients with ulcerative colitis
accompanies changes in their activation and morphology. A total of 16 subjects with ulcerative colitis and 32
healthy subjects were studied. Mean platelet count, morphological parameters of platelets and MPC were measured
using an ADVIA 120 hematology analyzer. Concentrations of sP-selectin and IL-6 in serum were marked by
immunoassay (ELISA). MPC, concentration of sP-selectin and IL-6 were significantly higher in subjects with ulcerative
colitis compared to those in the healthy group. There was a decrease of MPV in patients with ulcerative
colitis, which is statistically significant. Chronic inflammation in patients with ulcerative colitis causes an increase in
the number of blood platelets, a change in their morphology and activation. Decreased MPV value reflects activation
and the role blood platelets play in the inflammatory process of the mucous membrane of the colon. A high
concentration of sP-selectin, which is a marker of blood platelet activation, demonstrates their part in the inflammatory
process. The increase in the concentration of sP-selectin correlated positively with the increase in concentration
of IL-6. This is why it may be a useful marker of the activity of colitis ulcerosa. (Folia Histochemica et

Does Toxoplasma gondii Infection Affect the Levels of IgE and Cytokines (IL-5, IL-6, IL-10, IL-12, and TNF-alpha)?
In the study performed in a group of patients infected with
T. gondii, we evaluated Th2 humoral response
(IL-5, IL-6, IL-10) and Th1 cell response (IL-12, TNF-α). The study objective was to assess the effect of
T. gondii on chosen indices of the immune
response. The study involved 52 women infected with
T. gondii (aged 18–42 years) prior to
antiparasitic treatment. In all the patients, we found IgM (index
>0.7) and IgG which exceeded 300 IU/ml. The control
group (C) consisted of 40 healthy women aged 18–46 years. In
the study group (T) and in the control group (C), the levels of
IgE, IL-5, IL-6, IL-10, IL-12, and TNF-α were determined. In our study,
T. gondii patients had twofold higher levels of
IL-5 and IL-6 as compared to healthy subjects, which seems to
confirm the presence of an inflammatory state. We found the level
of IL-10 to be fivefold higher in the course of
toxoplasmosis than in healthy controls. The
levels of IL-12 and TNF-α were comparable to those observed in healthy controls.
The study has revealed that patients infected with
T. gondii show increased production of the humoral
response cytokines, whereas the generation of the cell response
cytokines remains unchanged
IFN-gamma, IL-5, IL-6 and IgE in patients infected with Giardia intestinalis.
The immune system, its cellular and humoral response, is engaged by the host organism to fight against parasitic infections. The study group consisted of 90 patients (58 women and 32 men), aged 18-72 years, infected with G. intestinalis. The diagnosis was established based on laboratory investigations (stool examination, choloscopy, GSA-65). Blood for analysis was collected before (G1), and 2 weeks (G2) and 2 months (G3) after antiparasitic treatment. Control group consisted of 40 healthy subjects (22 women and 18 men), aged 20-45 years. The concentrations of IgE were assayed using a set of VIDAS (bioMerieux) and the concentrations of IL-5, IL-6, IFN-gamma were determined using a set of Quantikine human (R&D Systems). It was revealed that in giardiosis the concentrations of IgE and IL-5 in blood serum were twice as high, the concentration of IL-6 was two and a half times higher and the concentration of IFN-gamma was almost four times higher as compared to healthy controls
Changes in adiponectine concentration in blood serum of obese type 2 diabetes patients in relation to renal function
WSTĘP. Celem pracy była ocena stężenia adiponektyny
u chorych na cukrzycę typu 2 i z otyłością, z prawidłową
lub upośledzoną funkcją nerek w porównaniu
z grupą kontrolną osób z otyłością, bez
cukrzycy i prawidłową funkcją nerek.
MATERIAŁ I METODY. Badano 2 grupy chorych z cukrzycą
typu 2, otyłością i prawidłową funkcją nerek
(grupa A - 16 pacjentów) lub z niewydolnością nerek
(grupa B - 15 pacjentów). Do badania włączono
chorych z przynajmniej 10-letnim wywiadem cukrzycy
typu 2, leczonych insuliną. Jako kryterium
podziału obu badanych grup przyjęto stężenie kreatyniny
wynoszące 1,5 mg/dl. Grupę kontrolną stanowiły
osoby otyłe, bez cukrzycy, z prawidłową
funkcją nerek. U wszystkich oceniano masę ciała,
wskaźnik masy ciała, stężenie HbA1c, kreatyniny, adiponektyny,
profil lipidowy. Analizę statystyczną
przeprowadzono za pomocą testów Manna-Whitneya
do porównania parametrów ilościowych i testu
dokładnego Fishera do oceny częstości występowania cech ilościowych. Oceniano korelacje nieparametryczne
Spearmana pomiędzy wszystkimi
cechami ilościowymi.
WYNIKI. Średnie stężenie adiponektyny w grupie
A wynosiło 77,7 mg/ml, w grupie B - 93,6 mg/ ml,
w grupie kontrolnej - 109,4 mg/ml. Różnice były istotne
statystycznie jedynie w przypadku porównania grupy
A z grupą kontrolną (p = 0,023). Analizując korelacje
stężenia adiponektyny z pozostałymi parametrami,
stwierdzono istotną statystycznie zależność pomiędzy
stężeniem adiponektyny, a masą ciała [r(-)
0,49; p = 0,000] oraz pomiędzy stężeniem adiponektyny
a BMI [r(-) 33; p = 0,018]. Gdy analizowano parametry
oddzielnie dla każdej z grup chorych, istotną
statystycznie korelację stężenia adiponektyny stwierdzano
jedynie w odniesieniu do masy ciała w grupie
kontrolnej [r(-); p = 0,015] i w grupie A [r(-) 0,51; p =
= 0,045]. Korelacja pomiędzy stężeniem adiponektyny
a wartością klirensu kreatyniny była bliska istotności
statystycznej [r(-) 0,47; p = 0,055]. Stężenia
HbA1c były podobne i nie wykazywały istotności statystycznej.
Różnica w stężeniu cholesterolu frakcji
HDL i LDL była istotna statystycznie w grupie chorych
z niewydolnością nerek (B) w stosunku do grupy kontrolnej
(HDL: p = 0,004; LDL: p = 0,035). Różnice
w stężeniu poszczególnych frakcji lipidów w badanych
grupach chorych były nieistotne statystycznie.
W porównaniu z grupą kontrolną częstość występowania
nadciśnienia tętniczego była istotna statystycznie
zarówno w grupie A (p = 0,001), jak i w grupie B (p = 0,005). Istotną statystycznie różnicę w częstości
występowania niewydolności serca stwierdzono
w obu badanych grupach (grupa A - 25%, grupa B
- 66,7%; p = 0,032) i w przypadku porównania grupy
B z grupą kontrolną (p = 0,001).
WNIOSKI. Rozwój niewydolności nerek w przebiegu
cukrzycy powoduje wzrost stężenia adiponektyny
w surowicy krwi. U chorych z cukrzycą typu 2 i otyłością
stwierdza się niższe stężenie adiponektyny
niż u osób otyłych bez cukrzycy. Nie wykazano wpływu
wyrównania metabolicznego cukrzycy na stężenie
adiponektyny.INTRODUCTION. The aim of this work is to assess the
concentration of adiponectine in the blood serum
of obese type 2 diabetes patients with normal and
impaired renal function in comparison to a control
group of obese people without diabetes and with
normal renal function.
MATERIALS AND METHODS. Two groups of obese type
2 diabetes patients and regular renal function
(group A - 16 patients) or renal insufficiency (group
B - 15 patients) were analysed. The research included
patients with at least ten years of recorded type 2
diabetes, treated with insulin. The groups were divided
according to the concentration of creatinine of 1.5
mg/dl. The control group consisted of people with
obesity, but no diabetes, with normal renal function.
They all had their body mass measured, as well as the
concentration of HbA1c, creatinin, adiponectine, lipid
profile. The statistical analysis was performed with
the use of the Mann-Whitney test for quantitative
comparison and Fisher’s exact test for the assessment
of frequency of occurrence of quantitative parameters.
Non-parametric Spearman correlations were
evaluated for all quantitative parameters.
RESULTS. The average concentration of adiponectine in
group A was 77.7 mg/ml, 93.6 mg/ml in group B and
109.4 mg/ml in the control group. The differences were
only statistically relevant between group A and the
control (p = 0.023). The analysis of the correlation
between adiponectine concentration and other
parameters has shown a statistically relevant correlation
between adiponectine concentration and body mass
[r(-) 0.49; p = 0,000] and between adiponectine
concentration and BMI [r(-) 33; p = 0.018].
With the parameters divided into two groups of
patients there was a statistically relevant correlation
only between adiponectine concentration and body mass in the control group [r(-); p = 0.015] and in
group A [r(-) 0.51; p = 0.045]. The correlation
between adiponectine concentration and creatinin
clearance was also close to statistical relevance [r(-)
0.47; p = 0.055]. The concentrations of HbA1c were
similar and showed no statistical relevance. The
difference in the concentration of HDL and LDL
cholesterol was statistically relevant in group B as
compared to the control (HDL: p = 0.004; LDL: p = 0.035). Differences in the concentration of various
lipids were not statistically relevant. In comparison
to the control group the frequency of hypertension
was statistically relevant in both group A (p = 0.001)
and group B (p = 0.005). A statistically relevant difference
in the frequency of circulatory insufficiency is
observed in both groups of patients (group A - 25%,
group B - 66,7%; p = 0.032) and in the comparison
between group B and control (p = 0.001).
CONCLUSIONS. The development of renal
insufficiency in diabetes leads to an increase in
adiponectine concentration in blood serum. Obese
patients with type 2 diabetes show a lower
concentration of adiponectine than obese people
with no diabetes. There is no indication of an
influence of metabolic homeostasis on adiponectine
concentration
Cistatin C as an early marker for renal function loss in type 2 diabetic patients with concomitant albuminuria
BACKGROUND. Type 2 diabetes is the main cause of
the end-stage renal insufficiency needed renal
replacement therapy. Vascular damage turn leads to
diabetic nephropathy leads and progress from onset
of hyperglycemia. Albuminuria is first laboratory
indicator confirm the damage of the renal
glomerulus. Cystatin C is sensitive marker of
a developing renal dysfunction. Aim of the research:
what are the relations between concentration of
cystatin C in blood and albuminuria and HbA1c in
type 2 diabetes?
MATERIAL AND METODS. The research included 68
patients with type 2 diabetes and confirmed
albuminuria. Based on the degree of metabolic
control expressed by the percentage of HbA1c, the
patients were divided into two groups: I group
— 29 patients with type 2 diabetes with HbA1c ≥ 6.1%
to £ 6.5%, with the average duration of diabetes of
5.3 years. II group — 39 patients with type 2 diabetes
with HbA1c > 6.5% to £ 10.0%, with the average
duration of diabetes of 7.2 years. K — the control
group consisted of 21 patients with essential
hypertension and albuminuria, not suffering from
carbohydrates disorders. Concentration cystatin C in
blood was assessed with the ELISA test, the percentage
of HbA1c was measured immunoturbidimetric
method. A Mann-Whitney test was used for the
comparison between the values of the parameters
in the research groups and the control group.
Spearman’s rank correlation coefficient was used for
measuring the relation between the axtent of
albuminuria and the other parameters.
RESULTS. The average concentration of HbA1c in
group II was 7.54% and was connected with
significantly larger average glycaemia (p < 0.001) in
comparison with group I with normal HbA1c
concentration. The highest albumin excretion in urine
and highest average concentration cystatin C in
blood was in group II patients with type 2 diabetes
with HbA1c > 6.5% to £ 10.0%. In group II patients
with type 2 diabetes and abnormal metabolic control
the average concentration of cystatin C was
significantly higher than in the control group patients
with essential hypertension. This difference however
was not statistically significant. Nevertheless
a positive correlation was found between the
concentration of cystatin C in blood and the extent
of albuminuria in research group I patients with type
2 diabetes with HbA1c < 6.5% (p = 0.005).
CONCLUSIONS. The positive correlation between the
concentration of cystatin C and the extent of
albuminuria, found in the research on patients with
type 2 diabetes and normal percentage of HbA1c
(p = 0.005) with shorts average duration of diabetes
about of 5 years, confirms the sensitivity of this
marker even at early stages of renal dysfunction.

niewydolności nerek wymagającej leczenia nerkozastępczego.
Zmiany naczyniowe prowadzące do
powstania cukrzycowej choroby nerek tworzą się
i postępują od początku pojawienia się hiperglikemii.
Albuminuria jest pierwszym laboratoryjnym objawem
potwierdzającym uszkodzenie kłębuszka
nerkowego. Cystatynę C uznaje się za czuły parametr
początkowej dysfunkcji nerek. Celem badania było
wykazanie zależności między stężeniem cystatyny C we
krwi a wielkością albuminurii i odsetkiem hemoglobiny
glikowanej (HbA1c) u chorych na cukrzycę typu 2.
MATERIAŁ I METODY. Badaniami objęto 68 chorych
na cukrzycę typu 2 z potwierdzoną albuminurią. Na
podstawie stopnia wyrównania metabolicznego, wyrażonego
odsetkiem HbA1c, chorych podzielono na
2 grupy: I — 29 chorych na cukrzycę typu 2 z HbA1c
≥ 6,1% do £ 6,5%, z średnim czasem trwania cukrzycy
5,3 roku; II — 39 chorych na cukrzycę typu 2 z HbA1c
> 6,5% do £ 10,0%, z średnim czasem trwania cukrzycy
7,2 roku. Do grupy kontrolnej (K) włączono 21
osób z pierwotnym nadciśnieniem tętniczym, ze
współistniejącą albuminurią, bez zaburzeń gospodarki
węglowodanowej. Stężenie cystatyny C we krwi oznaczano
metodą immunoenzymatyczną (ELISA), a odsetek
HbA1c — metodą immunoturbidymetryczną.
W celu porównania rozkładu wartości oznaczanych
poszczególnych parametrów między badanymi grupami
zastosowano test Manna-Whitneya (p*).
W oznaczaniu zależności między wartością albuminurii
a pozostałymi parametrami wykorzystano
współczynnik korelacji Spearmana (p**).
WYNIKI. Średni odsetek HbA1c w II grupie wyniósł
7,54% i wiązał się z istotnie wyższym średnim stężeniem
glukozy we krwi (p < 0,001), w porównaniu
z I grupą chorych z prawidłowym odsetkiem HbA1c.
Największe wydalanie albumin z moczem oraz najwyższe
średnie stężenie cystatyny C we krwi stwierdzono
w II grupie chorych na cukrzycę typu 2 z HbA1c > 6,5%
do £ 10,0%. W II grupie chorych ze źle wyrównaną
metabolicznie cukrzycą typu 2 średnia wartość cystatyny
C była znacznie wyższa, w porównaniu z grupą
kontrolną chorych z pierwotnym nadciśnieniem tętniczym.
Różnice między otrzymanymi wartościami nie
były jednak istotne statystycznie. Uzyskano dodatnią
korelację między stężeniem cystatyny C we krwi a wielkością albuminurii w I grupie chorych na cukrzycę
typu 2 z HbA1c < 6,5% (p = 0,005).
WNIOSKI. Wykazana w badaniu dodatnia korelacja
między stężeniem cystatyny C a wielkością albuminurii
u chorych na cukrzycę typu 2 z prawidłowym odsetkiem
HbA1c (p = 0,005), z krótszym, około 5-letnim
wywiadem chorobowym, potwierdza wysoką
czułość tego wskaźnika już w początkowych stadiach
rozpoczynającej się dysfunkcji nerek. (Diabet. Prakt.

Role of metabolic disorders and coexisting inflammatory process in etiopatogenesis of vascular endothelial dysfunction in patients with type 2 diabetes
Na świecie obserwuje się dramatyczny wzrost zapadalności i chorobowości na cukrzycę typu 2. Ma ona często bezobjawowy początek; już w momencie rozpoznania u około 50% chorych stwierdza się powikłania narządowe. Utrzymująca się przewlekle hiperglikemia
doprowadza do uszkodzenia naczyń.
Markerem uogólnionego uszkodzenia śródbłonka naczyniowego jest albuminuria. Wpływ na jej występowanie
mają zarówno zaburzenia metaboliczne
jako efekt „glukotoksyczności”, jak i współistniejący stan zapalny. Zahamowanie nadmiernej aktywacji
VEGF, NF-kB, jak i ocena stężenia CRP i IL-6 we krwi u chorych na cukrzycę typu 2 mogą się przyczynić do rozwoju nowoczesnych metod terapeutycznych, mogących doprowadzić do redukcji ryzyka przewlekłych powikłań oraz chorobowości i śmiertelności z przyczyn sercowo-naczyniowych u tych pacjentów. Metabolic disorders have influence to
confirms albuminuria by way of a effect of “glucotoxicity”,
as and coexisting inflammatory process.
Inhibition excessive activation VEGF, NF-kB, as well
as estimate CRP and IL-6 concentration in the blood
in type 2 diabetes patients may in contribute to the
development of modern therapeutic methods, which
can lead to the reduction of the risk of chronic
complications as the prevalence and mortality rate
