    Alpha-foetoprotein and carcinoembryonic antigen in germ cell neoplasms.

    Serum alpha-foetoprotein (AFP) and serum carcinoembryonic antigen (CEA) levels were measured, serially whenever possible, in 70 patients attending the Institute of Radiotherapy, Rotterdam, on account of testicular (65) or ovarian (4) germ cell tumours or, in one case, an endodermal sinus (yolk sac) tumour in the mediastinum. In 15 patients the disease was active; in the others it was in remission. Patients with active disease had raised serum AFP levels which correlated well with disease activity; no patient without evidence of active disease had raised serum AFP levels. None of the patients with active disease was found to have raised serum CEA levels. There was no correlation between serum AFP and CEA levels in patients with germ cell neoplasms, but good correlation between serum AFP levels and disease activity. Serum CEA levels did not correlate with disease activity, and serial determinations would therefore not be useful in monitoring progress in this group of diseases

    Effect of Functional Capacity Evaluation information on the judgment of physicians about physical work ability in the context of disability claims

    Purpose To test whether Functional Capacity Evaluation (FCE) information lead insurance physicians (IPs) to change their judgment about the physical work ability of claimants with musculoskeletal disorders (MSDs). Methods Twenty-seven IPs scored twice the physical work ability of two claimants for 12 specified activities, using a visual analogue scale. One claimant performed an FCE, the other served as a control. Outcome measure was the difference between experimental and control group in number of shifts in the physical work ability for the total of 12 specified activities. Results The IPs changed their judgment about the work ability 141 times when using FCE information compared to 102 times when not using this information (P-value = 0.001), both in the direction of more and less ability. Conclusions The IPs change their judgment of the physical work ability of claimants with MSDs in the context of disability claim procedures more often when FCE information is provide

    Testing facility for hydrogen storage materials designed to simulate application based conditions

    For the daily use of hydrogen storage materials, not only their intrinsic storage properties are important, but also equally important is the performance under practical conditions. Besides the techniques already available for the fundamental characterization of storage materials, there is a growing need to test storage materials under conditions resembling day-to-day use. For that we developed and tested a downscaled hydrogen storage reactor with which it is possible to monitor the hydrogenation behavior under nonideal conditions. Here we present a characterization of the developed reactor setup which enables a fast screening of storage materials. For characterization and calibration purposes, we use the rather well-documented LaNi5–Al alloy as reference. The found experimental results agree well with the properties of LaNi5–Al as reported in literature. Our results show that this reactor setup enables an efficient screening of new developed storage alloys under realistic conditions and is therefore complementary to the already existing characterization setups.ChemE/Chemical EngineeringApplied Science

    An LCA-based evaluation of biomass to transportation fuel production and utilization pathways in a large port’s context

    This study evaluates whether a transition of large ports facilities to biofuel production for mobility improves the environmental performance and satisfies the renewable energy directive (RED) and it is the first LCA study that considers biofuel production from torrefied wood. The systems studied are wood, torrefied wood, and straw pellets circulating fluidized bed gasification for H2, synthetic natural gas, or Fischer–Tropsch (FT) diesel production and use. These systems are evaluated for their global warming, acidification, eutrophication and particulate matter potentials, as well as, for their aggregated environmental performance. The effects of the electricity mix selection and ecoinvent database’s economic allocation are also analyzed. All biomass systems result in a better aggregated environmental performance and benefits for the global warming potential. However, regarding the acidification, particulate matter, and eutrophication potentials, most biomass systems are inferior to the reference systems. Switching to a zero-emission electricity mix offers benefits for all the biomass and fossil-H2 systems and researchers should use databases cautiously. The bio-H2 and FT diesel of wood-based systems show the best environmental performance and satisfy the current and future RED targets. On one hand, the bio-H2 systems result in the largest benefits regarding the global warming potential, and on the other hand, both wood-based FT diesel systems offer overall benefits which concern not only the sustainable target of CO2 emissions reduction, but also the air quality improvement of the broader area as well.</p

    Effective return-to-work interventions after acquired brain injury: A systematic review

    To gather knowledge about effective return-to-work (RTW) interventions for patients with acquired brain injury (ABI). A database search was performed in PubMed, EMBASE, PsycINFO, CINAHL and the Cochrane Library using keywords and Medical Subject Headings. Studies were included if they met inclusion criteria: adult patients with non-progressive ABI, working pre-injury and an intervention principally designed to improve RTW as an outcome. The methodological quality of included studies was determined and evidence was assessed qualitatively. Twelve studies were included, of which five were randomized controlled trials and seven were cohort studies. Nine studies had sufficient methodological quality. There is strong evidence that work-directed interventions in combination with education/coaching are effective regarding RTW and there are indicative findings for the effectiveness of work-directed interventions in combination with skills training and education/coaching. Reported components of the most effective interventions were tailored approach, early intervention, involvement of patient and employer, work or workplace accommodations, work practice and training of social and work-related skills, including coping and emotional support. Effective RTW interventions for patients with ABI are a combination of work-directed interventions, coaching/education and/or skills training.
