Assessment of building operational energy at early stages of design – a monthly quasi-steady-state approach
In this paper a new numerical tool to estimate the operational energy of buildings, in early design stages, is presented. This tool is part of a novel approach for life-cycle analysis based on macro-components developed in the European research project SB_Steel – Sustainable Buildings in Steel. Two early design stages are considered in the scope of the methodology: the concept stage and the preliminary stage. This numerical tool enables to estimate the energy use for space heating, space cooling and domestic hot water production, taking into account (i) the climate; (ii) the use type of the building (e.g. residential, offices and commercial/industrial); (iii) the building envelope characteristics; and (iv) the building services.
The developed algorithm is based on a monthly quasi-steady-state approach, modified for improved accuracy through the calibration of correction factors that depend on the climatic region and the type of building. Good results were achieved, with errors lower than 10% when compared to performance-based approaches such as the use of advanced dynamic methods.
Finally, the case study of a low-rise residential building is presented, in which the results obtained from the simplified methodology are compared with the results from the simulation program EnergyPlus, showing a good agreement between them
Thermal performance and comfort condition analysis in a vernacular building with a glazed balcony
The increase in global environmental problems requires more environmentally efficient construction. Vernacular passive strategies can play an important role in helping reducing energy use and CO2 emissions related to buildings. This paper studies the use of glazed balconies in the North of Portugal as a strategy to capture solar gains and reduce heat losses. The purpose is understanding thermal performance and comfort conditions provided by this passive heating strategy. The methodology includes objective (short and long-term monitoring), to evaluate the different parameters affecting thermal comfort and air quality, and subjective assessments to assess occupants? perception regarding thermal sensation. The results show that the use of glazed balconies as a passive heating strategy in a climate with cold winters is viable. During the mid-seasons, the rooms with balcony have adequate comfort conditions. In the heating season, it is possible to achieve comfort conditions in sunny days while in the cooling season there is a risk of overheating. Regarding indoor air quality, carbon dioxide concentrations were low, but the average radon concentration measured was high when the building was unoccupied, rapidly decreasing to acceptable values, during occupation periods when a minimum ventilation rate was promoted. Hospital Resource Utilization and Treatment Cost of Skeletal-Related Events in Patients with Metastatic Breast or Prostate Cancer: Estimation for the Portuguese National Health System
BACKGROUND:
Skeletal-related events (SREs) occur frequently in patients with bone metastases as a result of breast (BC) and prostate (PC) cancers. They increase both morbidity and mortality and lead to extensive health-care resource utilization.
METHODS:
Health care resource utilization by BC/PC patients with at least one SRE during the preceding 12 months was assessed through retrospective chart review. SRE-treatment costs were estimated using the Portuguese Ministry of Health cost database and analyzed using generalized linear models.
RESULTS:
This study included 152 patients from nine hospitals. The mean (SD) annual SRE-treatment cost per patient was €5963 (€3646) and €5711 (€4347), for BC (n=121) and PC (n=31) patients, respectively. Mean cost per single episode ranged between €1485 (radiotherapy) and €13,203 (spinal cord compression). Early onset of bone metastasis (P = 0.03) and diagnosis of bone metastases at or after the occurrence of the first SRE (P < 0.001) were associated with higher SRE-treatment costs.
CONCLUSION:
These results reveal the high hospital SRE-treatment costs, highlighting the need for early diagnosis and treatment, and identify key factors determining the economic value of therapies for patients with skeletal metastases
Concurrent breast stroma sarcoma and breast carcinoma: a case report
<p>Abstract</p> <p>Introduction</p> <p>Breast cancer is one of the most important health problems in the world and affects a great number of women over the entire globe. This group of tumors rarely presents as bilateral disease and, when it does happen, normally occurs within the same histological type. We report a rare case of concurrent bilateral breast cancer with two different histology types, a breast carcinoma and a breast sarcoma, in a 42-year-old woman referred to our hospital.</p> <p>Case presentation</p> <p>A 42-year-old Caucasian woman admitted to our institute in August 1999, presented with a nodule in the left breast of 3.0 × 2.5 cm, and, in the right breast, one of 1.0 cm, suspected of malignancy and with a clinically negative armpit. Biopsies had revealed invasive mammary carcinoma (right breast) and sarcoma (left breast). She was submitted to bilateral modified radical mastectomy. A histological study showed an invasive mammary carcinoma degree II lobular pleomorphic type with invasion of seven of the 19 excised axillary nodes in the right breast and, in the left breast, a sarcoma of the mammary stroma, for which the immunohistochemistry study was negative for epithelial biomarkers and positive for vimentin. Later, she was submitted for chemotherapy (six cycles of 75 mg/m<sup>2 </sup>5-fluorouracil, epirubicin and cyclophosphamide) followed by radiotherapy of the thoracic wall and axillary nodes on the left. Hormone receptors were positive in the tumor of the right breast, and tamoxifen, 20 mg, was prescribed on a daily basis (five years) followed by letrozole, 2.5 mg, also daily (five years). She presented no sign of negative evolution in the last consultation.</p> <p>Conclusion</p> <p>The risk of development of bilateral breast cancer is about 1% each year within a similar histological type, but it is higher in tumors with lobular histology. In this case, the patient presented, simultaneously, two histologically distinct tumors, thus evidencing a rare situation.</p
Sustainability of bridge maintenance
Bridge maintenance activities are important to consider within sustainable development due to the cost and environmental impact associated with various maintenance activities. Comparisons have been made between different bridge structural forms, based on materials, components and construction method, but less information is available on bridge maintenance activities to help decide a sustainable structural form. Typical maintenance aspects of the predominant forms of bridge structure (i.e. concrete, steel and masonry bridges) were considered in this study to reveal their sustainability in terms of materials, energy, transportation, human health and ecosystems. The results indicate that concrete and steel bridge maintenance activities have an average impact of 42% and 46% compared with 12% for masonry bridge maintenance activities. It is concluded that the component parts of concrete and steel bridges should be revised as they play an integral role in the selection of maintenance options
An integrated MCDM approach to evaluate public transportation systems in Tehran
Public transportation is one of the most important systems in transportation, especially in big and crowded cities. As a result, evaluation of public transportation systems is a strategic decision-making problem for both private and public sections. In this paper, the problem of public transportation passengers in Tehran is addressed and their satisfaction levels are assessed by using passenger satisfaction survey. An integrated MCDM approach is proposed for evaluation of public transportation systems based on Delphi method, group analytic hierarchy process (GAHP) and preference ranking organization method for enrichment of evaluations (PROMETHEE). The proposed model provides more reliable and realistic results and introduces directions for future improvements of public transportation service quality. A sensitivity analysis is applied to investigate the influence of criteria weights on the decision making problem. As a conclusion, the most important public transportation systems in Tehran orderly are: metro, taxi, BRT, bus and van. Therefore, Tehran Municipality and policy makers should encourage and support the previously mentioned systems
Abundância e sazonalidade de cigarrinhas (Hemiptera, Cicadellidae, Cicadellinae) em vegetação herbácea de pomar de laranja doce, no município de Montenegro, Estado do Rio Grande do Sul, Brasil.
O presente estudo teve por objetivo conhecer a flutuação populacional e a constância de cicadelíneos presentes na vegetação herbácea de um pomar de laranja doce (Citrus sinensis (L.) Osbeck), variedade Valência, em Montenegro, RS. O levantamento foi realizado com a metodologia da rede-de-varredura, tendo início em fevereiro de 2001 e finalizando-se em janeiro de 2002. Mensalmente foram coletadas dez amostras, sendo cada uma destas composta por 50 golpes de rede em movimento de avanço sobre a vegetação
herbácea e cultura intercalar nas entrelinhas do pomar (n = 500 golpes/ocasião de coleta). No total foram coletados 928 cicadelíneos,pertencentes a nove espécies: Bucephalogonia xanthophis (Berg, 1879), Diedrocephala variegata(Fabricius, 1775), Hortensia similis(Walker, 1851), Macugonalia leucomelas (Walker, 1851), Parathona gratiosa (Blanchard, 1840), Plesiommata corniculata Young,1977, Sibovia sagata (Signoret, 1854), Sonesimia grossa (Signoret, 1854) e Tapajosa rubromarginata (Signoret, 1855). O período de
maior abundância da comunidade de cicadelíneos foi durante a primavera e o verão. Hortensia similis e M. leucomelas foram as únicas espécies consideradas constantes durante o levantamento. A menor abundância dos cicadelíneos em abril e dezembro provavelmente devese a efeitos causados por fatores abióticos (pluviosidade) e aqueles relativos à estrutura da vegetação (altura das plantas) sobre a

Ticks as potential vectors of Mycobacterium leprae: Use of tick cell lines to culture the bacilli and generate transgenic strains.
Leprosy is an infectious disease caused by Mycobacterium leprae and frequently resulting in irreversible deformities and disabilities. Ticks play an important role in infectious disease transmission due to their low host specificity, worldwide distribution, and the biological ability to support transovarial transmission of a wide spectrum of pathogens, including viruses, bacteria and protozoa. To investigate a possible role for ticks as vectors of leprosy, we assessed transovarial transmission of M. leprae in artificially-fed adult female Amblyomma sculptum ticks, and infection and growth of M. leprae in tick cell lines. Our results revealed M. leprae RNA and antigens persisting in the midgut and present in the ovaries of adult female A. sculptum at least 2 days after oral infection, and present in their progeny (eggs and larvae), which demonstrates the occurrence of transovarial transmission of this pathogen. Infected tick larvae were able to inoculate viable bacilli during blood-feeding on a rabbit. Moreover, following inoculation with M. leprae, the Ixodes scapularis embryo-derived tick cell line IDE8 supported a detectable increase in the number of bacilli for at least 20 days, presenting a doubling time of approximately 12 days. As far as we know, this is the first in vitro cellular system able to promote growth of M. leprae. Finally, we successfully transformed a clinical M. leprae isolate by inserting the reporter plasmid pCHERRY3; transformed bacteria infected and grew in IDE8 cells over a 2-month period. Taken together, our data not only support the hypothesis that ticks may have the potential to act as a reservoir and/or vector of leprosy, but also suggest the feasibility of technological development of tick cell lines as a tool for large-scale production of M. leprae bacteria, as well as describing for the first time a method for their transformation
