Does atrial fibrillation affect plasma endothelin level?
Background: Atrial fibrillation (AF) may result in endocardial endothelium dysfunction.
The main objective of the study was to evaluate the plasma concentration of endothelin-1 (ET-1)
during persistent AF and after sinus rhythm recovery following direct-current cardioversion
and to assess the predictive value of ET-1 in AF patients.
Methods: The study group consisted of 43 patients with persistent AF and normal left
ventricle systolic function who had undergone successful cardioversion. Blood samples were
collected twice: 24 hours before and 24 hours after cardioversion. All patients were also
examined in terms of sinus rhythm maintenance on the 30th day after cardioversion.
Results: There were no differences in ET-1 plasma concentration between the persistent AF
group and the control group (2.6 ± 2.9 fmol/mL vs 2.3 ± 4.5 fmol/mL, NS). Plasma ET-1
levels did not change within 24 hours after successful cardioversion (2.5 ± 2.8 fmol/mL vs
2.6 ± 2.9 fmol/mL, NS). There was no correlation between the baseline plasma levels of ET-1
in patients with persistent AF and sinus rhythm maintenance 30 days after cardioversion.
Conclusions: Persistent AF does not affect plasma ET-1 concentration in patients with
normal left ventricle systolic function and with no symptoms of heart failure. There are no
significant changes in plasma ET-1 level during the 24 hours after cardioversion. (Cardiol J

The performance of triglyceride to high-density lipoprotein cholesterol ratio in acute coronary syndromes using a diagnostic decision tree
Background. Modern modeling techniques, including decision trees, may potentially provide accurate prediction and classification of outcomes, and support the process of clinical decision making. The objective of our study was to evaluate the performance of triglyceride to high-density lipoprotein (TG:HDL-C) ratio in acute coronary syndromes (ACS) presented using a decision tree analysis. Methods. The initial study group consisted of 220 consecutive patients admitted to hospital within the first 6 hours from the onset of chest pain. All these patients met clinical criteria of ACS and were compared with 116 healthy volunteers in a case-control study. Serum was assayed on admission for cardiac troponin I, C-reactive protein, apolipoproteins ApoAI and ApoB, and lipid parameters. Atherogenic lipid ratios: TC:HDL:C, LDL-C:HDL:C and TG:HDL-C were calculated. Results. ACS patients showed almost twice as high median values of TG:HDL-C as controls [2.77 (1.88–4.08) vs. 1.47 (0.99–2.08); p < 0.0001]. The TG:HDL-C ratio was significantly related to the positive history of coronary artery disease, age and lipid parameters, except for LDL-C. The TG:HDL-C ratio was, after age, the most powerful independent predictor and classifier of the occurrence of ACS with the optimal cutoff being 2.28. The performance of TG:HDL-C was superior to other lipid parameters and ratios, and enabled identification of the additional 6 subjects with ACS. Conclusion. The TG:HDL-C ratio is a useful risk marker of the ACS occurrence. Further prospective studies are needed to confirm our findings and clarify the interaction between TG and HDL-C concentrations in ACS patients
Wrodzony zespół wydłuzonego QT - aspekty diagnostyczne
Zespół wydłużonego QT charakteryzuje się zaburzeniami repolaryzacji z wydłużeniem czasu
trwania odstępu QT w zapisie EKG i predyspozycją do groźnych komorowych arytmii (torsade
de pointes) prowadzących do omdleń i nagłych zgonów. Na podstawie badań molekularnych
za przyczynę zaburzeń elektrofizjologicznych występujących w tym zespole uznano defekty
w białkach kanałów jonowych (potasowych i sodowych) błony komórkowej komórek mięśnia sercowego. Dziedziczony autosomalnie dominująco, bez wrodzonej głuchoty zespół Romano-
-Warda jest zbiorem co najmniej 6 genotypów, z których najczęściej występują 3 pierwsze
postacie LQTS1, LQTS2 i LQTS3. W zależności od położenia zmutowanego genu na chromosomie
11 (IKs), 7 (IKr) lub 3 (INa) te 3 genotypy różnią się między sobą zaburzeniami okresu
repolaryzacji ze zmienną morfologią załamka T w zapisach elektrokardiograficznych oraz
czynnikami wyzwalającymi objawową arytmię komorową. W obrazie klinicznym dominują
nawracające utraty przytomności, które w LQTS1 i LQTS2 są najczęściej wywołane stymulacją
układu adrenergicznego (wysiłek, emocje, przestrach, nagły bodziec dźwiękowy). W genotypie
LQTS3 objawowa arytmia komorowa najczęściej występuje w spoczynku lub we śnie.
U 40% pacjentów z wrodzonym zespołem wydłużonego QT przebieg kliniczny jest bezobjawowy.
W postaciach LQTS klinicznie utajonych — wydłużenie odstępu QT, zmiany załamka T oraz
komorowe zaburzenia rytmu serca — mogą występować napadowo, co można zaobserwować
w zapisie Holtera lub w teście wysiłkowym. Często u pozornie zdrowych dzieci i młodzieży
pierwszym objawem może być zatrzymanie krążenia lub nagły zgon. Młodzi pacjenci bez
objawów powinni być objęci leczeniem profilaktycznym ze względu na możliwość występowania
u nich epizodów nagłego zatrzymania krążenia, bez objawów poprzedzających. (Folia

ARCHITECT STAT High Sensitive Troponin I Familiarization Study (FAM) in the Department of Laboratory Medicine, Collegium Medicum, Nicolaus Copernicus University in Bydgoszcz, Poland
Background. International guidelines recommend the use of cardiac troponin assays for early detection of acute myocardial infarction. New high-sensitivity assays along with improved precision and sensitivity, are now widely available, accelerating patient’s diagnosis, treatment and invasive therapy. In this study we evaluated analytical performance of the Abbott ARCHITECT STAT high-sensitive troponin-I immunoassay and its 99th percentile upper reference limit.
Methods. We performed the analytical evaluation of the hs-cTnI assay using Abbott ARCHITECT i2000SR immunoanalyzers. Features of the assay including imprecision, detection limits, linearity of dilution, interferences and method comparisons were assessed, as well as the 99th percentile upper reference limits in a cohort of 427 presumably healthy individuals were established.
Results. Total imprecision ranged from 3.1% to 4.7% and was lowest for the medium controls. The observed limit of blank, limit of detection and limit of quantitation assumed values of 0.1, 1.5 and 4.8 ng/L, respectively. Common interferences, sample dilution and carry over did not affect the hs-cTnI results. Hs-cTnI was detectable in 98% of presumably healthy individuals. The 99th percentile values were age and sex dependent in the presumably healthy, but not in the healthy individuals.
Conclusion. The new high-sensitivity troponin-I assay has improved analytical features and may be a valuable diagnostic tool.
Expression of genes KCNQ1 and HERG encoding potassium ion channels Ikr, Iks in long QT syndrome
Background: The KCNQ1 and HERG genes mutations are responsible for specific types of congenital long QT syndrome (LQT).
Aim: To examine the expression of KCNQ1 and HERG genes that encode potassium channels (rapid and slow) responsible
for the occurrence of particular types of LQT syndrome. The study also attempted to prove that beta-actin is a good endogenous
control when determining the expression of the studied genes.
Methods: The study enrolled six families whose members suffered from either LQT1 or LQT2, or were healthy. Examination
of gene expression was achieved with quantitative PCR (QRT-PCR). Expression of the investigated genes was inferred from
the analysis of the number of mRNA copies per 1 mg total RNA isolated from whole blood. On the basis of KCNQ1 gene
expression profile, the presence of, or absence of, LQT1 could be confirmed.
Results and conclusions: The study revealed a statistically significant difference (p = 0.031) between the number of KCNQ1
gene copies in patients and healthy controls. On the basis of HERG (KCNH2) gene expression profile, patients with LQT2
cannot be unequivocally differentiated from healthy subjects (p = 0.37).

i wolne), odpowiadających za wystąpienie określonego rodzaju zespołu długiego QT (LQTS).
Metody: Do badania włączono 6 rodzin, u członków których zdiagnozowano LQTS1 lub LQTS2, lub zdrowych. Badanie
miało na celu udowodnienie, że beta-aktyna stanowi dobrą kontrolę endogenną przy ustalaniu ekspresji badanych genów.
Do badania ekspresji genów wykorzystano ilościową analizę PCR w czasie rzeczywistym (QRT-PCR). Ekspresję badanych
genów przedstawiono jako liczbę kopii mRNA w przeliczeniu na 1 mg całkowitego RNA izolowanego z krwi pełnej. Dane
zostały wyeksportowane z arkusza Excel do programu analizy danych Statistica V.7.1.
Wyniki i wnioski: Na podstawie profilu ekspresji genu KCNQ1 można potwierdzić występowanie zespołu LQTS1. Badania
wykazały statystycznie istotną różnicę (p = 0,031) między liczbą KCNQ1 kopii genu u osób chorych i zdrowych. Na podstawie
profilu ekspresji genu HERG (KCNH2) chorych z LQTS2 nie można jednoznacznie odróżnić od osób zdrowych (p = 0,37).

Background: Atrial fibrillation (AF) decreases quality of life and significantly increases risk of stroke, congestive heart failure and death. Atrial overload and stretch also result in increased production of natriuretic peptide type A (ANP). The biologically inactive prohormone NT-proANP is released to plasma in the same amounts as ANP but it has higher levels in the blood due to decreased degradation in vivo. In vitro degradation is also slower and NT-proANP may be an interesting alternative for ANP. Aim: To evaluate NT-proANP plasma concentration in patients with persistent AF following successful cardioversion. Methods: The study group consisted of 43 patients with persistent AF and normal left ventricular systolic function, who underwent successful electrical cardioversion (EC). The control group comprised 20 patients with sinus rhythm without a history of AF. Blood samples were collected twice, during visits 24 h before and after EC. All patients were also examined 30 days after the sinus rhythm recovery. The NT-proANP concentration was measured using an immunochemical method (ELISA). Results: Plasma NT-proANP concentration was significantly increased in patients with persistent AF compared to the control group (4.8 ± 2.9 vs. 2.8 ± 1.2 nmol/l, p = 0.004). Plasma NT-proANP level decreased significantly after successful cardioversion (to 3.2 ± 2.4 nmol/l; p 50%), bez objawów klinicznych niewydolności serca, z dobrze kontrolowaną chorobą podstawową (nadciśnienie tętnicze, choroba wieńcowa, cukrzyca), bez przeciwwskazań do KE, u których przywrócono rytm zatokowy. Grupę kontrolną stanowiło 20 osób bez AF w wywiadzie. U chorych wykonano badanie kliniczne, echokardiograficzne i oznaczenia NT-proANP 24 godz. przed KE, 24 godz. po KE oraz w 30. dobie od przywrócenia rytmu zatokowego. Wyniki: Stężenie NT-proANP w osoczu chorych z przetrwałym AF było istotnie większe w porównaniu z grupą kontrolną (4,8 ± 2,9 vs 2,8 ± 1,2 nmol/l, p = 0,004). Stwierdzono istotne zmniejszenie stężenia NT-proANP w osoczu 24 godz. po przywróceniu rytmu zatokowego (3,2 ± 2,4 nmol/l, p < 0,0001). Nie wykazano związku pomiędzy stężeniem NT-proANP w trakcie AF a utrzymaniem rytmu zatokowego w okresie 30 dni po kardiowersji. Wnioski: U chorych z przetrwałym AF oraz prawidłową funkcją skurczową lewej komory stężenie NT-proANP w osoczu, oceniane metodą ELISA, jest istotnie wyższe w porównaniu z grupą osób bez tej arytmii. Peptyd natriuretyczny typu B po przywróceniu rytmu zatokowego u chorych z przetrwałym migotaniem przedsionków - znaczenie kliniczne
Background: Persistent atrial fibrillation (AF) leads to electrical, structural and neurohormonal remodelling of the atria,
including increased plasma B-type natriuretic peptide (BNP) level.
Aim: To assess the clinical value of plasma BNP or NT-proBNP concentrations in patients with persistent AF measured before
and after sinus rhythm restoration following direct-current cardioversion.
Methods: The study group consisted of 43 patients with persistent AF who underwent successful electrical cardioversion. The
mean AF duration was 12.3 weeks. Patients in the study group had no symptoms of heart failure and they had preserved left
ventricular systolic function. Blood samples were collected twice: 24 hours before and 24 hours after electrical cardioversion.
Logistic regression analysis was used to assess the predictive value of BNP and NT-proBNP levels.
Results: Baseline NT-proBNP and BNP levels were increased in patients with persistent AF (290.9 ± 257.2 pg/mL and 148.4 ± 111.4 pg/mL, respectively) compared to a matched control group without AF (47.8 ± 80.6 pg/mL; p = 0.0001 and 74.9 ± 81.7 pg/mL; p = 0.01). Plasma BNP level decreased 24 hours after cardioversion (from 148.4 ± 111.4 to 106.4 ± 74.7 pg/mL;
p = 0.0045) whereas NT-proBNP level did not (from 290.9 ± 257.2 to 262.7 ± 185.6 pg/mL; NS). During an 18-month
follow-up period, 21 (49%) patients remained in sinus rhythm. Neither baseline plasma BNP nor NT-proBNP level predicted
sinus rhythm maintenance.
Conclusions: NT-proBNP and BNP plasma levels are increased in patients with persistent AF. Conversion to sinus rhythm is
associated with a significant decrease in plasma BNP but not NT-proBNP level. Baseline BNP and NT-proBNP levels do not
predict long-term sinus rhythm maintenance. Kardiol Pol 2010; 68, 7: 781-786Wstęp: Migotanie przedsionków (AF) prowadzi do przebudowy elektrycznej, strukturalnej i neurohormonalnej przedsionków.
W wyniku arytmii dochodzi do wzrostu stężenia osoczowego stężenia peptydu natriuretycznego typu B (BNP) oraz
NT-proBNP w osoczu chorych z AF.
Cel: Celem pracy była ocena zmian osoczowego stężenia BNP i NT-proBNP po przywróceniu rytmu zatokowego u chorych
z przetrwałym AF i zachowaną czynnością skurczową lewej komory, a także ocena wartości klinicznej tych pomiarów dla
prognozowania utrzymania rytmu zatokowego po kardiowersji elektrycznej (KE).
Metody: Badaniem objęto grupę 43 chorych z przetrwałym AF, u których przywrócono rytm zatokowy za pomocą KE.
Głównymi kryteriami włączenia do badania były: przetrwałe AF o etiologii niezastawkowej, brak przeciwwskazań do przywrócenia
rytmu zatokowego za pomocą KE, dobra funkcja skurczowa lewej komory w badaniu echokardiograficznym (LVEF
> 50%), brak objawów niewydolności serca, dobrze kontrolowana choroba podstawowa (nadciśnienie tętnicze, choroba
wieńcowa, cukrzyca). Krew do oznaczenia BNP i NT-proBNP pobierano 24 godziny przed KE i po niej. Stężenie NT-proBNP
w osoczu oznaczano za pomocą testu immunoenzymatycznego ELISA, stężenie BNP w osoczu oznaczano metodą immunoenzymatyczną,
używając zestawu AxSYM firmy Abbott. Powtórnej oceny rytmu serca dokonywano na podstawie badania
przedmiotowego i zapisu EKG, po 18 miesiącach od KE.
Wyniki: Stężenia BNP i NT-proBNP w osoczu chorych z przetrwałym AF były istotnie wyższe w porównaniu z odpowiednio
dobraną grupą kontrolną bez wywiadu AF (odpowiednio: 148,4 ± 111,4 pg/ml v. 74,9 ± 81,7 pg/ml; p = 0,01 oraz 290,9
± 257,2 pg/ml v. 47,8 ± 80,6 pg/ml; p = 0,0001). W badanej grupie wykazano silną dodatnią korelację między wiekiem
i wartościami ciśnienia tętniczego a osoczowymi stężeniami BNP i NT-proBNP. Stwierdzono istotne zmniejszenie stężenia
BNP w osoczu, oznaczonego 24 godziny po przywróceniu rytmu zatokowego (148,4 ± 111,4 pg/ml do 106,4 ± 74,7 pg/ml;
p = 0,0045). Natomiast nie wykazano istotnego statystycznie obniżenia stężenia NT-proBNP po przywróceniu rytmu zatokowego
w porównaniu z wartościami sprzed KE (290,9 ± 257,2 pg/ml do 262,7 ± 185,6 pg/ml; p = NS). W 18-miesięcznej
obserwacji 21 (48%) pacjentów utrzymywało rytm zatokowy. W analizie regresji logistycznej nie wykazano związku między
wyjściowym stężeniem BNP i NT-proBNP a utrzymaniem rytmu zatokowego po KE, w obserwacji 1,5-rocznej.
Wnioski: U chorych z przetrwałym AF z zachowaną funkcją skurczową lewej komory wykazano istotnie wyższe stężenia
BNP i NT-proBNP. W 24. godzinie od przywrócenia rytmu zatokowego zaobserwowano istotne obniżenie stężenia BNP,
natomiast nie wystąpiło znamienne statystycznie zmniejszenie stężenia NT-proBNP. Stężenia BNP i NT-proBNP w warunkach
