    Diagnosis of endometrial cancer complicated by morbid obesity—a case report

    A 47 year old morbidly obesity nulligravid female presented to a local hospital with chest pain and shortness of breath, CT of the chest confirmed pulmonary embolism, and endometrial biopsy performed showed grade 1 endometrioid adenocarcinoma with squamous differentiation. CT scan performed demonstrated pelvic, retroperitoneal, inguinal and right axillary lymphadenopathy, and hepatomegaly with grossly unremarkable uterus and ovaries. Due to the burden of disease, axillary and inguinal lymph node biopsy was attempted, however due to depth of nodes, the tissue was not reached and biopsy efforts were abandoned. It required a great deal of discussion with the radiology department to decide which lesion would be least morbid to biopsy. After failed attempts at several methods, the gastroenterologist consulted felt the retroperitoneal lymphadenopathy adjacent to the duodenum was a possibility. The sample was sparse, but confirmed a metastatic adenocarcinoma and ruled out an inflammatory or granulomatous process. Ultimately, this unfortunate woman’s obesity caused her to have a worse outcome, not only due to medical comorbidities, but also with regards to delayed diagnosis and treatment and progression of disease. As the obesity epidemic worsens, this is likely to occur more commonly. It is important to realize the limitations of our consults as all aspects and fields of medicine are limited by morbid obesity

    Toward a microRNA signature of endometrial cancer

    A simple meta-analysis of eight microRNA (miRNA) expression surveys of endometrial cancers reveals a panel of sixteen miRNAs that are significantly over-expressed (n = 15) or under-expressed (n = 1) in at least three surveys. Examination of these miRNAs indicates that they target mRNAs involved in a number of basic cellular processes including the crucial epithelial to mesenchymal transition (EMT) and hypoxia response. The central role played by these miRNAs is reinforced by the demonstration that they are all among the most ancient of all animal miRNAs. This suggests that they are members of a core set of miRNAs dysregulated as part of the carcinogenic cellular reprogramming process

    Cotyledonoid dissecting leiomyoma (CDL) is a rare variant of uterine leiomyoma characterized by intramural dissection within the uterine corpus and a distinctive gross appearance resembling the cotyledons of the placenta. Despite their alarming, sarcomatous appearance both macroscopically and radiographically, these tumors are typically associated with bland histologic findings. Approximately 70 cases of CDL have been described in the literature. This case represents the second described case of CDL in pregnancy, the first in which a pregnancy was carried to term. A 28-year-old primigravid was incidentally found to have a massive, exophytic growth of the uterus on ultrasound with concomitant 14-week intrauterine pregnancy. The pregnancy was monitored with serial growth ultrasounds. She was delivered at 37 weeks via Cesarean section. After being lost to follow-up, the patient presented 2.5 years later with worsening abdominal fullness and persistent uterine mass

    H19 is a maternally expressed non-coding RNA located at chromosome 11p15.5 near the reciprocally imprinted insulin-like growth factor 2 (IGF2) gene. Though the function of H19 is unknown, it is transcribed during embryonic development after which transcription is absent in all but a few tissues including cardiac muscle, breast, ovary, uterus, and placenta. Linking H19, miR-675 and RB1 expression with serous tumors of the endometrium suggests that RB1 suppression may be a differentiating event in serous tumorigenesis

    The relationship between vascularity, p53 gene mutations and distant metastatic disease in epithelial ovarian carcinoma

    Mutations of the p53 tumor suppressor gene are associated with large differences (>7 vessels/HPF) in Microvessel density (MVD) counts between primary and metastatic tumor sites in patients with epithelial ovarian cancer. These data are consistent with models demonstrating p53 mutation functions directly to influence angiogenesis. This information supports continued therapy and research involving angiogenesis inhibitors in patients with ovarian cancer, especially in the setting of increased differences in MVD between primary and metastatic sites
