    Saúde perinatal: metodologia e características da população estudada

    OBJECTIVE: To describe the methodological procedures and some sociodemographic and medical care characteristics of the population studied in a research project on perinatal health developed in the Ribeirão Preto county, SP, in 1994, in order to make historical comparisons with the indicators of mother-child health of a similar study conducted 15 years before in the same municipality. MATERIAL AND METHOD: All the hospital births that occurred in Ribeirão Preto from June 1978 to May 1979 were studied by interviewing the mothers and obtaining data about maternal and paternal habits, social situation of the family, medical care during pregnancy and delivery, duration of pregnancy, and anthropometric and mortality data about the newborn children. A new study was conducted in 1994 using the same methodology to collect data about a sample of 1/3 of the births that had occurred in the municipality in that year (all the hospital births observed during a period of 4 months), and also about the mortality of this group up to one year after the termination of birth data collection. The interviews were held after delivery and infant weight and length were measured immediately after birth. Infant mortality was evaluated by surveying the data referring to all deaths of infants born during the year of the study and up to 1 year after its termination. The data were obtained from the Municipal Health Secretariat to which copies of all death certificates emitted in the registry offices of the county are sent obligatorily. RESULTS: An investigation was made into 3,663 births, 3,579 of which were singletons; of these, 2,846 were from the municipality. Comparison with the data obtained in the previous study showed an increase in the frequency of some indicators such as adolescent mothers, prematurity, low birth weight and cesarian section; on the other hand, there was a 50% decrease in the mortality rate and its components.OBJETIVO: Descrever os procedimentos metodológicos e algumas características demógrafo-sociais e da atenção médica da população estudada em um projeto de pesquisa em saúde perinatal realizado no Município de Ribeirão Preto, SP, Brasil, em 1994, visando a realizar comparações históricas com indicadores de saúde materno-infantil. MATERIAL E MÉTODO: Entre junho de 1978 e maio de 1979 foram estudados todos os nascimentos hospitalares de Ribeirão Preto através de entrevistas com as mães, onde eram obtidos dados sobre hábitos maternos e paternos, situação social da família, atenção médica à gestação e parto, duração da gestação e dados antropométricos e de mortalidade dos recém-nascidos. Em 1994 foi realizado novo estudo com metodologia semelhante, sendo coletados dados de uma amostra de 1/3 dos nascimentos ocorridos no município naquele ano (todos os nascimentos hospitalares observados num período de 4 meses), e também a mortalidade desse grupo até um ano após o encerramento da coleta de dados do nascimento. As entrevistas eram realizadas após o parto, e o peso e comprimento da criança eram aferidos logo após o nascimento. A mortalidade infantil foi avaliada através do levantamento de todos os óbitos de crianças nascidas durante o período do projeto até um ano após seu encerramento. RESULTADOS: Foram analisados 3.663 nascimentos, dos quais 3.579 eram nascimentos únicos; destes, 2.846 eram procedentes do município estudado. Quando comparados com os dados do estudo anterior, verifica-se que houve aumento da freqüência de alguns indicadores como mães adolescentes, prematuridade, baixo peso ao nascer e parto cesáreo; por outro lado, houve diminuição da metade da mortalidade infantil e seus componentes

    Análise de endemismo de táxons neotropicais de Pentatomidae (Hemiptera: Heteroptera)

    The definition of areas of endemism is central to studies of historical biogeography, and their interrelationships are fundamental questions. Consistent hypotheses for the evolution of Pentatomidae in the Neotropical region depend on the accuracy of the units employed in the analyses, which in the case of studies of historical biogeography, may be areas of endemism. In this study, the distribution patterns of 222 species, belonging to 14 Pentatomidae (Hemiptera) genera, predominantly neotropical, were studied with the Analysis of Endemicity (NDM) to identify possible areas of endemism and to correlate them to previously delimited areas. The search by areas of endemism was carried out using grid-cell units of 2.5° and 5° latitude-longitude. The analysis based on groupings of grid-cells of 2.5° of latitude-longitude allowed the identification of 51 areas of endemism, the consensus of these areas resulted in four clusters of grid-cells. The second analysis, with grid-cells units of 5° latitude-longitude, resulted in 109 areas of endemism. The flexible consensus employed resulted in 17 areas of endemism. The analyses were sensitive to the identification of areas of endemism in different scales in the Atlantic Forest. The Amazonian region was identified as a single area in the area of consensus, and its southeastern portion shares elements with the Chacoan and Paraná subregions. The distribution data of the taxa studied, with different units of analysis, did not allow the identification of individual areas of endemism for the Cerrado and Caatinga. The areas of endemism identified here should be seen as primary biogeographic hypotheses.A definição de áreas de endemismo é central aos estudos de Biogeografia Histórica e suas inter-relações são questões fundamentais. Hipóteses consistentes sobre a evolução de Pentatomidae (Hemiptera) na Região Neotropical dependem da acuidade das unidades empregadas nas análises, que no caso de estudos de biogeografia histórica, podem ser áreas endêmicas. Neste trabalho foram estudados os padrões de distribuição de 222 espécies, pertencentes a 14 gêneros de Pentatomidae, com ocorrência predominantemente neotropical, com base em uma Análise de Endemicidade (NDM) a fim de inferir possíveis áreas endêmicas e relacioná-las a áreas previamente delimitadas. A busca por áreas endêmicas foi realizada com quadrículas de 2,5° e 5° latitude-longitude. A análise com base em agrupamentos de 2,5° latitude-longitude permitiu identificar 51 áreas de endemismo, sendo que o consenso destas áreas resultou em quatro agrupamentos de quadrículas. A segunda análise, com quadrículas de 5° latitude-longitude, resultou em 109 áreas de endemismo. O consenso flexível empregado resultou em 17 áreas de endemismo. As análises foram sensíveis à identificação de áreas de endemismo na Mata Atlântica em diferentes escalas. A região Amazônica foi identificada como uma área única no consenso, sendo que a porção sudeste compartilha elementos com as sub-regiões do Chaco e Paraná. Os dados de distribuição dos táxons estudados, com diferentes unidades de análises, não permitiram a identificação de áreas endêmicas para o Cerrado e a Caatinga.     Childhood socioeconomic position, adult socioeconomic position and social mobility in relation to markers of adiposity in early adulthood: evidence of differential effects by gender in the 1978/79 Ribeirao Preto cohort study

    Background: Longitudinal studies drawn from high-income countries demonstrate long-term associations of early childhood socioeconomic deprivation with increased adiposity in adulthood. However, there are very few data from resource-poor countries where there are reasons to anticipate different gradients. Accordingly, we sought to characterise the nature of the socioeconomic status (SES)-adiposity association in Brazil. / Methods: We use data from the Ribeirao Preto Cohort Study in Brazil in which 9067 newborns were recruited via their mothers in 1978/79 and one-in-three followed up in 2002/04 (23–25years). SES, based on family income (salaries, interest on savings, pensions and so on), was assessed at birth and early adulthood, and three different adiposity measures (body mass index (BMI), waist circumference (WC), waist-to-hip ratio (WHR)) ascertained at follow-up. The association between childhood SES, adult SES and social mobility (defined as four permutations of SES in childhood and adulthood: low–low, low–high, high–low, high–high), and the adiposity measures was examined using linear regression. / Results: There was evidence that the association between SES and the three markers of adiposity was modified by gender in both adulthood (P<0.02 for all outcomes) and childhood SES (P<0.02 for WC and WHR). Thus, in an unadjusted model, linear regression analyses showed that higher childhood SES was associated with lower adiposity in women (coefficient (95% confidence intervals) BMI: −1.49 (−2.29,−0.69); WC: −3.85 (−5.73,−1.97); WHR: −0.03 (−0.04,−0.02)). However, in men, higher childhood SES was related to higher adiposity (BMI: 1.03 (0.28,−1.78); WC: 3.15 (1.20, 5.09); WHR: 0.009 (−0.001, 0.019)) although statistical significance was not seen in all analyses. There was a suggestion that adult SES (but not adult health behaviours or birthweight) accounted for these relationships in women only. Upward mobility was associated with protection against greater adiposity in women but not men. / Conclusion: In the present study, in men there was some evidence that both higher childhood and adulthood SES was related to a higher adiposity risk, while the reverse gradient was apparent in women
