    Metabolic syndrome in patients with chronic plaque psoriatic disease: a case-control study from western Uttar Pradesh, India

    Background: Various studies on psoriasis and metabolic syndrome have shown a large variation in their results. An increasing frequency is imposing a substantial burden on the overall health of psoriasis patients that needs to be appropriately foreseen and addressed. Aim of this study was to study various aspects of metabolic syndrome in patients with chronic plaque psoriatic disease in northern Indian region.Methods: A cohort of patients registering for treatment of chronic plaque psoriasis at Dermatology outpatients’ department formed the study population. Detailed history was captured. General physical examination was carried out. A thorough cutaneous examination was undertaken which captured details on type, distribution and arrangement of primary lesions and secondary changes in patients. After overnight fasting, venous blood samples were collected from the subjects and were analysed for serum glucose, triglyceride and HDL-cholesterol. Results: Metabolic syndrome in psoriasis was associated with higher age. Gender wise male preponderance was observed. Among the psoriasis cases, 64% had metabolic syndrome whereas among the control subjects 48% had the condition (p-value 0.158). The mean for serum triglyceride level for psoriasis patients (159.42 mg/dL) was higher than controls (144.25 mg/dL). Forty six percent of cases fulfilled elevated triglycerides ≄150 mg/dl as a criterion of metabolic syndrome, compared to 40% of controls. Conclusions: We observed a higher frequency of metabolic syndrome among psoriasis cases in a northern Indian population. An association of dyslipidemia with psoriasis was also noted. Routine screening of the condition to facilitate early diagnosis and treatment should be undertaken

    Recent Advances in Human Papillomavirus Infection and Management

    Human papillomavirus (HPV) accounts for approximately 4.5% of all cancers which differs at the level of economic development and geographical regions. The life cycle of the HPV is completely dependent on the epithelium differentiation without the involvement of cell death and systemic viremia. Carcinogenesis is the consequence of viral gene expression, dysregulated cell proliferation, and genomic instability. Keratinocytes are the target cell for HPV which act as the physical and immunological barrier. In cervical carcinogenesis, the enhanced level of Th17 infiltration has been observed which increases with the disease progression and is coupled with CCL20 expression in the stromal mesenchymal compartment. IL-6 and M-CSF are known as “switch factors” which are imperative for pro-tumorigenic response in monocytes. Screening of cervical cancer includes three major procedures: cytology, nucleic acid test, and co-testing. For evaluating anal lesions, high-resolution anoscopy is performed which is similar to colposcopy. Prophylactic vaccination is the primary preventive measure to control the HrHPV infection and reduce the burden of HPV-related cancer. The precancerous stage of HPV infection includes excision, ablation, and immunotherapy. Radiotherapy is the acceptable primary treatment for the early stage of anogenital cancer, whereas for the advanced-stage metastatic cancer, palliative therapy is the only option

    An evaluation of quality of life of skin cancer patients after surgery using dermatology life quality index tool

    Background: Quality of life (QOL) has been identified as an important outcome in cancer researches yet the most common malignancy among humans, non-melanoma skin cancer (NMSC), but poorly studied. Aim of the study was to analyze the quality of life of non-melanoma skin cancer patients after surgery using dermatology life quality index inventory (DLQI).Methods: Retrospective cohort of patients operated for non-melanoma skin cancer in last 2 years and paid postoperative 4-month visit formed the study population. Inclusion criteria consisted of subjects operated for non-melanoma skin cancer and paid follow up visits having sufficient physical and mental capacity. Fifty-six subjects fulfilled the selection criteria laid down thus included in this study. Study tools were records of patients, which were obtained from medical records section. If any more information was required, study subjects were contacted.Results: Out of total 56 study subjects, Basal cell carcinomas were found in 91.1% (n=51); squamous cell carcinomas were detected in 7.2% (n=4). Single location wise more lesions were located on the nose 22.1% (n=15) and forehead 17.6% (12). For most patients (75%), the lesion had not been previously treated. 58.9% subjects did not have any other associated co-morbid condition. Lower mean values were observed post-operative i.e. lower DLQI scores were recorded 4 months after surgery in our study which indicates that adverse effects were not very prominent thus preserving quality of life post operatively. Paired sample t-tests revealed a significant effect on DLQI item 1 (p=0.008), item 2 (p=0.043), and item 4 (p=0.003), with scores decreasing (improving QOL) after treatment. The change in total DLQI score demonstrated a trend toward significance, with overall QOL improving after treatment (p=0.024).Conclusions: Previously commonly employed dermatological Quality of life tools demonstrated minimal handicap at initial diagnosis and little change after treatment of nonmelanoma skin cancer. Development of disease-specific instrument is warranted to explore the disease process.

    Clinico-pathological study of skin appendageal tumours from northern India

    Background: The skin appendageal tumours (ATs) encompass a wide variety of tumours clinically presenting as papules and nodules and with histologically distinct features. Early recognition of skin adnexal tumours is very important aspect as far as patient management and prognosis is concerned. The main of the study was to analyse and ascertain clinico-pathological study of skin appendageal tumours in a tertiary care centre.Methods: Retrospective cohort of patients with confirmed diagnosis of AT during 1st January 2013 to 31st December 2015 formed the study population. Study tools were records of the patients such as information from MRD (Medical Record Department) and records from histopathological section i.e. histopathological requisition forms and clinical case sheets. Cases clinically diagnosed as appendageal tumours, but not histologically, were excluded from the study. Results: Finally a total of 48 cases were included in this study. Mean age of onset was 22.7±6.4 years and the duration of complaints 5.34 ± 4.5 years. Benign follicular, benign eccrine, benign sebaceous and benign apocrine tumours were observed to be 48.89%, 42.22%, 2.22% and 6.67% respectively. Regarding distribution of benign and malignant tumours, among the all types of skin appendageal tumours diagnosed, 93.75% (N=45) were benign and 6.25% (N=3) were malignant. Pilomatricoma (54.54%), eccrine acrospiroma (26.31%) and syringocystadenoma papilliferum (67%) were observed as most commonly distributed histopathological types of diagnosed benign follicular, benign eccrine and benign apocrine tumours. Number of benign skin appendageal tumours was found maximum (n=25) in the age group of 26-50 years in both the sexes whereas malignant skin appendageal tumours were observed only (n=3) in the age group of 51-75 years in both the sexes.Conclusions: Findings of this study can be utilized to suspect type of AT thus helping in diagnosis. Profile, pattern and clinical appearance can serve as vital clue though histological confirmation is mandatory to confirm

    Indoor air pollution and asthma in children at Delhi, India

    Introduction: Several studies in developed countries have shown association between indoor air pollution and asthma in children. The present research was undertaken to study this association at Delhi, India. Material and methods: This study took place at Delhi, capital of India. Eight locations based on the source of pollution such as industrial, residential and villages were included. Recording of the demographic profile and clinical examination of each child was conducted at their residence. Indoor SO2, NO2 and SPM (suspended particulate matter) levels were measured by using Handy Air Sampler (Low Volume Sampler). Results: A total of 3104 children were examined of which 60.3% were male and 39.7% were female. 32.4% children were exposed to environmental tobacco smoke. 31.5 % children’s families were using biomass fuels for cooking. History of respiratory symptoms included cough (43.9%), phlegm production (21.9%), shortness of breath (19.3%) and wheezing (14.0%). 7.9% children were diagnosed as having asthma, which was highest in industrial areas (11.8%), followed by residential (7.5%) and village areas (3.9%). The mean indoor SO2, NO2 and SPM levels were 4.28 ± 4.61 mg/m3, 26.70 ± 17.72 mg/m3 and 722.0 ± 457.6 mg/m3 respectively. Indoor SPM was the highest in industrial area followed by residential area and urban village area. Indoor SPM level was significantly (p < 0.001) higher in the asthmatic children’s houses.     Identification and characterization of miRNAome in root, stem, leaf and tuber developmental stages of potato (Solanum tuberosum L.) by high-throughput sequencing

    BACKGROUND: MicroRNAs (miRNAs) are ubiquitous components of endogenous plant transcriptome. miRNAs are small, single-stranded and ~21 nt long RNAs which regulate gene expression at the post-transcriptional level and are known to play essential roles in various aspects of plant development and growth. Previously, a number of miRNAs have been identified in potato through in silico analysis and deep sequencing approach. However, identification of miRNAs through deep sequencing approach was limited to a few tissue types and developmental stages. This study reports the identification and characterization of potato miRNAs in three different vegetative tissues and four stages of tuber development by high throughput sequencing. RESULTS: Small RNA libraries were constructed from leaf, stem, root and four early developmental stages of tuberization and subjected to deep sequencing, followed by bioinformatics analysis. A total of 89 conserved miRNAs (belonging to 33 families), 147 potato-specific miRNAs (with star sequence) and 112 candidate potato-specific miRNAs (without star sequence) were identified. The digital expression profiling based on TPM (Transcripts Per Million) and qRT-PCR analysis of conserved and potato-specific miRNAs revealed that some of the miRNAs showed tissue specific expression (leaf, stem and root) while a few demonstrated tuberization stage-specific expressions. Targets were predicted for identified conserved and potato-specific miRNAs, and predicted targets of four conserved miRNAs, miR160, miR164, miR172 and miR171, which are ARF16 (Auxin Response Factor 16), NAM (NO APICAL MERISTEM), RAP1 (Relative to APETALA2 1) and HAM (HAIRY MERISTEM) respectively, were experimentally validated using 5â€Č RLM-RACE (RNA ligase mediated rapid amplification of cDNA ends). Gene ontology (GO) analysis for potato-specific miRNAs was also performed to predict their potential biological functions. CONCLUSIONS: We report a comprehensive study of potato miRNAs at genome-wide level by high-throughput sequencing and demonstrate that these miRNAs have tissue and/or developmental stage-specific expression profile. Also, predicted targets of conserved miRNAs were experimentally confirmed for the first time in potato. Our findings indicate the existence of extensive and complex small RNA population in this crop and suggest their important role in pathways involved in diverse biological processes, including tuber development

    Utilization of Molecular, Phenotypic, and Geographical Diversity to Develop Compact Composite Core Collection in the Oilseed Crop, Safflower (Carthamus tinctorius L.) through Maximization Strategy

    Safflower (Carthamus tinctorius L.) is a dryland oilseed crop yielding high quality edible oil. Previous studies have described significant phenotypic variability in the crop and used geographical distribution and phenotypic trait values to develop core collections. However, the molecular diversity component was lacking in the earlier collections thereby limiting their utility in breeding programs. The present study evaluated the phenotypic variability for 12 agronomically important traits during two growing seasons (2011–12 and 2012–13) in a global reference collection of 531 safflower accessions, assessed earlier by our group for genetic diversity and population structure using AFLP markers. Significant phenotypic variation was observed for all the agronomic traits in the representative collection. Cluster analysis of phenotypic data grouped the accessions into five major clusters. Accessions from the Indian Subcontinent and America harbored maximal phenotypic variability with unique characters for a few traits. MANOVA analysis indicated significant interaction between genotypes and environment for both the seasons. Initially, six independent core collections (CC1–CC6) were developed using molecular marker and phenotypic data for two seasons through POWERCORE and MSTRAT. These collections captured the entire range of trait variability but failed to include complete genetic diversity represented in 19 clusters reported earlier through Bayesian analysis of population structure (BAPS). Therefore, we merged the three POWERCORE core collections (CC1–CC3) to generate a composite core collection, CartC1 and three MSTRAT core collections (CC4–CC6) to generate another composite core collection, CartC2. The mean difference percentage, variance difference percentage, variable rate of coefficient of variance percentage, coincidence rate of range percentage, Shannon's diversity index, and Nei's gene diversity for CartC1 were 11.2, 43.7, 132.4, 93.4, 0.47, and 0.306, respectively while the corresponding values for CartC2 were 9.3, 58.8, 124.6, 95.8, 0.46, and 0.301. Each composite core collection represented the complete range of phenotypic and genetic variability of the crop including 19 BAPS clusters. This is the first report describing development of core collections in safflower using molecular marker data with phenotypic values and geographical distribution. These core collections will facilitate identification of genetic determinants of trait variability and effective utilization of the prevalent diversity in crop improvement programs
