Constitutive immune function in European starlings, \u3cem\u3eSturnus vulgaris\u3c/em\u3e, is decreased immediately after an endurance flight in a wind tunnel
Life-history theory predicts that animals face a trade-off in energy allocation between performing strenuous exercise, such as migratory flight, and mounting an immune response. We experimentally tested this prediction by studying immune function in European starlings, Sturnus vulgaris, flown in a wind tunnel. Specifically, we predicted that constitutive immune function decreases in response to training and, additionally, in response to immediate exercise. We compared constitutive immune function among three groups: (1) ‘untrained’ birds that were kept in cages and were not flown; (2) ‘trained’ birds that received flight training over a 15 day period and performed a 1-4 h continuous flight, after which they rested for 48 h before being sampled; and (3) ‘post-flight’ birds that differed from the ‘trained’ group only in being sampled immediately after the final flight. A bird in our trained group represents an individual during migration that has been resting between migratory flights for at least 2 days. A bird in our post-flight group represents an individual that has just completed a migratory flight and has not yet had time to recover. Three of our four indicators (haptoglobin, agglutination and lysis) showed the predicted decrease in immune function in the post-flight group, and two indicators (haptoglobin, agglutination) showed the predicted decreasing trend from the untrained to trained to post-flight group. Haptoglobin levels were negatively correlated with flight duration. No effect of training or flight was detected on leukocyte profiles. Our results suggest that in European starlings, constitutive immune function is decreased more as a result of immediate exercise than of exercise training. Because of the recent emergence of avian-borne diseases, understanding the trade-offs and challenges faced by long-distance migrants has gained a new level of relevance and urgency
Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults
Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We
estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from
1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories.
Methods We used data from 3663 population-based studies with 222 million participants that measured height and
weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate
trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children
and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the
individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≥30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference)
and obesity (BMI >2 SD above the median).
Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in
11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed
changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and
140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of
underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and
countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior
probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse
was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of
thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a
posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%)
with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and
obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for
both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such
as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged
children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls
in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and
42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents,
the increases in double burden were driven by increases in obesity, and decreases in double burden by declining
underweight or thinness.
Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an
increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy
nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of
underweight while curbing and reversing the increase in obesit

Prática de atividade física e sedentarismo em brasileiros: resultados da Pesquisa Nacional por Amostra de Domicílios (PNAD) 2008 Practice of physical activity and sedentarism among Brazilians: results of the National Household Sample Survey - 2008
Inquéritos populacionais estão no centro das atividades relevantes para a saúde pública. Atualmente tem-se interesse em compreender aspectos comportamentais influentes na mudança do quadro de saúde individual e coletiva, entre eles a atividade física. O objetivo do presente estudo é apresentar os resultados de prática de atividade física (AF) da Pesquisa Nacional por Amostra de Domicílios (PNAD) em 2008, conforme distribuição regional e características sociodemográficas. O convênio firmado entre o IBGE e o Ministério da Saúde foi responsável pelo levantamento suplementar de saúde. O tamanho de amostra foi de 292.553 pessoas. A prevalência de AF nos níveis recomendados no lazer foi de 10,5%, o mesmo percentual de indivíduos relatando deslocamento ativo para o trabalho. Homens e indivíduos mais jovens foram mais ativos e houve relação direta entre escolaridade e AF no lazer e inversa entre escolaridade e atividade física no deslocamento. Um em cada cinco brasileiros não pratica qualquer AF, e um em cada três assistem, em média, 3h ou mais de televisão por dia. Estes dados visam apoiar as políticas públicas no desenho de estratégias que promovam ações sustentadas de promoção da saúde, especialmente de AF, visando o alcance de resultados que influenciem positivamente na qualidade de vida da população.<br>Population surveys are a key component of the relevant activities for public health. There is growing interest in identifying behavioral aspects which influence individual and collective health, such as physical activity (PA). The aim of this study is to present PA data from the 2008 Brazilian National Household Survey (PNAD) according to regional distribution and the socio-demographic characteristics. A partnership between the Brazilian Institute of Geography and Statistics and the Ministry of Health was responsible for preparing the health supplement of the questionnaire. The sample size was 292,553 individuals. The proportion of subjects practicing PA at the recommended levels in leisure time was 10.5%, exactly the same proportion of subjects reporting active transportation to and from work. Males and younger adults were more active; schooling was directly related to leisure-time PA and inversely associated with transport-related physical activity. One fifth of the Brazilians did not report any physical activity at all and 35.7% watch TV for 3 hours or more, on average, per day. These data may help design public policies aimed at promoting health, in particular through PA, which may positively influence on the quality of life of the Brazilian population
An integrated model of care for inflammatory bowel disease sufferers in Australia: Development and the effects of its implementation
BACKGROUND: Psychological comorbidities are associated with poor outcome and increased healthcare utilization in patients with inflammatory bowel disease (IBD). However, a model of care addressing the biopsychosocial dimension of disease is not routinely applied in IBD. This review describes the development of such a model and the effects of its implementation in a hospital-based cohort of patients with IBD. METHODS: Three different approaches were used: 1) collecting baseline epidemiological data on mental health comorbidities; 2) raising awareness of and targeting mental health problems; 3) examining the effects of the model implementation. RESULTS: High rates of anxiety and depressive symptoms (36% and 13%, respectively) that are maintained over time were identified in IBD patients presenting at a metropolitan teaching hospital. Patients with documented psychological comorbidities were more likely to be hospitalized than those without (odds ratio [OR] = 4.13, 95% confidence interval [CI]: 1.25, 13.61). Improvements in disease activity, anxiety, depression, quality of life, and coping have been noted when cognitive-behavioral therapy (CBT) was provided to patients. A drop in the use of opiates (P = 0.037) and hospitalization rates (from 48% to 30%) in IBD patients has been noted as a result of introduction of the changed model of care. In addition, the mean total cost of inpatient care was lower for IBD patients than controls (US15,236.79] vs. US 53,760.20], P = 0.005). CONCLUSION: Our data to date suggest that an integrated model of care for patients with IBD may yield superior long-term outcomes in terms of medication use and hospitalization rates and reduce healthcare costs.Antonina A.
