    Interprofessional Collaboration in Complex Patient Care Transition: A Qualitative Multi-Perspective Analysis

    Healthcare professionals often feel challenged by complex patients and the associated care needs during care transition. Interprofessional collaboration (IPC) is considered an effective approach in such situations. However, a fragmented healthcare system can limit IPC. This study explored experiences of Swiss healthcare professionals regarding complex patient care transition and the potential of IPC. Professionals from nursing, medicine, psychology, physiotherapy, dietetics and nutrition, social service, occupational therapy, and speech therapy were included. A qualitative between-method triangulation design was applied, with two focus group discussions and ten individual interviews. The combination of different data-collection methods allowed us to explore complex patient care transition and to systematically add perspectives of healthcare professionals from different care settings. Three main themes were identified: (1) Participants described their vision of an ideal complex patient care transition, i.e., the status they would like to see implemented; (2) participants reported challenges in complex patient care transition as experienced today; and (3) participants suggested ways to improve complex patient care transition by IPC. This study highlighted that healthcare professionals regarded IPC as an effective intervention to improve complex patient care transition. It emerged that sustainable implementation of IPC across care organizations is currently limited in Switzerland. In the absence of strong and direct promotion of IPC by the healthcare system, professionals in clinical practice can further promote IPC by finding hands-on solutions to overcome organizational boundaries

    Setting-up a research club for high school students: an engineering educational concept based on increasing both interest and self-efficacy

    In the fall of 2019, the research club changING started as an outreach program in the Cluster of Excellence SE2A. While the cluster deals with interdisciplinary research topics to explore technologies for sustainable and environmentally friendly aviation, the associated research club offers students from the 10th grade onwards the opportunity to gain insights into engineering. The target group here is primarily young women, who are heavily underrepresented in this career field. The research club is experience-oriented and offers high school students the opportunity to explore engineering, its systems, technologies, applications, and social and cultural significance by participating in different projects at various engineering institutes in the Cluster. During their four years of participation in the club, which is voluntary, students are challenged to discover, create, construct, and solve problems. In the process, students learn different engineering concepts and skills. Currently, three batches (75 highschool students) actively participate in the program, accompanied by engineering Bachelor students who serve as mentors. This paper presents the structure of the research club which is based primarily on theories of the emergence and development of individual interests and self-efficacy expectations. A well-known model depicting the development of individual interests is the four-phase model. This model is considered the basis for the structure of the research club program. In this paper, the most important factors in the set-up of the research club and the expected results as well as the many lessons learned are presented

    Charakterisierung und Untersuchung neuer JAK-Inhibitoren zur Behandlung chronisch entzĂĽndlicher Darmerkrankungen

    Durch die Invasion von Bakterien in die Lamina propria des Darmes wird das Immunsystem aktiviert. Diese Aktivierung erfolgt unter anderem durch Zytokine, die durch den JAK/STAT-Signalweg eine Entzündung hervorrufen können, weshalb die JAK Inhibition für chronisch entzündliche Darmerkrankungen eine attraktive Therapiemöglichkeit darstellt. Dabei wurde die bedeutende Rolle der JAK-Proteine in entzündlichen Erkrankungen, wie der rheumatoiden Arthritis gezeigt. Zur Familie der JAK-Proteine zählen JAK1, JAK2, JAK3 und Tyk2. Die Bedeutung von JAK1, JAK2 und Tyk2 in CED wurde bereits beschrieben. Dagegen ist die Funktion von JAK3 in CED bisher nicht hinreichend geklärt. Tofacitinib wurde 2012 als einer der ersten oral verfügbaren JAK-Inhibitoren zur Therapie der rheumatoiden Arthritis in den USA zugelassen. Eine Zulassung in Europa wurde aufgrund der schweren Nebenwirkungen nach Behandlung mit Tofacitinib abgelehnt. Die Nebenwirkungen von Tofacitinib entstehen vermutlich durch die unselektive Inhibition der vier JAK-Proteine. Durch eine gezielte rektale Applikation und durch die Anwendung modifizierter JAK Inhibitoren sollte die systemische Verteilung von Tofacitinib verhindert werden. Hierfür wurden zwei JAK-Inhibitoren zur Anwendung im Kolon synthetisiert. Die Verbindungen unterscheiden sich zusätzlich aufgrund der Selektivität gegenüber den JAK-Proteinen. Die Verbindungen und Tofacitinib als Referenzsubstanz wurden zuerst in vitro in einem intestinalen Karzinomzellsystem charakterisiert. Aufgrund der überzeugenden in vitro Daten wurden die Verbindungen in einem akuten DSS-induzierten Kolitis Modell in der Maus auf ihre Wirkung getestet. Dabei führte weder die Behandlung mit Tofacitinib noch mit den neuen Verbindungen zur Verbesserung der Wundheilung und der klinischen Parameter. Trotz der fehlenden protektiven Wirkung in dem getesteten System könnten die neuen Substanzen Therapiemöglichkeiten bei anderen Erkrankungen, wie Darmkrebs, darstellen

    Image Sensor Nonuniformity Correction by a Scene-Based Maximum Likelihood Approach

    Image sensors come with a spatial inhomogeneity, known as Fixed Pattern Noise or image sensor nonuniformity, which degrades the image quality. These nonuniformities are regarded as the systematic errors of the image sensor, however, they change with the sensor temperature and with time. This makes laboratory calibrations unsatisfying. Scene based nonuniformity correction methods are therefore necessary to correct for these sensor errors. In this thesis, a new maximum likelihood estimation method is developed that estimates a sensor’s nonuniformities from a given set of input images. The method follows a rigorous mathematical derivation that exploits the available sensor statistics and uses only well-motivated assumptions. While previous methods need to optimize a free parameter, the new method’s parameters are defined by the statistics of the input data. Furthermore, the new method reaches a better performance than the previous methods. Specialized developments that include a row- or column-wise and a combined estimation of the nonuniformity parameters are introduced as well and are of relevance for typical industrial applications. Finally it is shown that the previous methods can be regarded as simplifications of the newly developed method. This deliberation gives a new view onto the problem of scene based nonuniformity estimation and allows to select the best method for a given application

    Das Magnetfeld der Erde: Beobachtung und Modellierung von Globalen zu Regionalen Skalen

    The magnetic field of the Earth varies in space and time. Geomagnetism as research area that aims to describe and understand the sources of these variations is supported by two pillars: first, regular high-precision measurements in the global network of magnetic observatories and repeat stations are necessary to register the field and its variations at all. Second, mathematical methods are required in order to extract magnetic field models from this large data set. Methods applied to data offer insights in the mechanisms generating the magnetic field. This thesis covers both subjects. In a first part, following the description of the state of the art in observatory instrumentation, I explain in detail two instruments that have the potential to streamline the classical procedures: The Geomagnetic AUtomated SyStem GAUSS paves the way to automated absolute measurements, up to now only possible manually. The newly developed DI3 technique improves and simplifies the standard manual measurements significantly and thus reduces the requirements placed on observers. The second part deals with the mathematical tools available for geomagnetic field modelling. I focus on harmonic splines that can be derived from the classical approach of spherical harmonics. These base functions are interpolatory and have a localised shape while satisfying Laplaces equation. Hence, they are applicable to fit data regionally or globally. The harmonic splines are used with a data set made of repeat station and observatory measurements from Southern Africa. This region is of special interest because the field intensity is very low and and both spatial and temporal field gradients exist. Subdivided into an analysis of ancient (years 1961-2001) and recent (2005-2009) data, two continuous regional field models SAMS and X-SAMS are derived. From the analysis of the field models, a better understanding of the field behaviour is gained. Finally, the harmonic splines are used in a case study on globally distributed secular variation data. Rotating the data set in a system of coordinates aligned with the dipole axis and modelling it with the harmonic splines reveals the external origin of observed fast variations.Das Magnetfeld der Erde ändert sich in Raum und Zeit. Die Ursachen dieser Änderungen zu beschreiben und zu verstehen ist das Ziel der geomagnetischen Forschung. Sie ruht auf zwei Säulen: Erstens werden regelmäßig hochpräzise Messungen in einem Netz von Observatorien und Säkularpunkten durchgeführt, um das Feld mit seinen Variationen überhaupt aufzuzeichnen. Zweitens sind mathematische Methoden notwendig, um aus dieser Fülle von Daten magnetische Feldmodelle abzuleiten. Werden die Methoden auf die Daten angewandt, kann man Erkenntnisse gewinnen, wie die Felder erzeugt und Variationen hervorgerufen werden. In dieser Arbeit werden beide Herangehensweisen beschrieben. Im ersten Teil stelle ich zwei Instrumente vor, mit deren Hilfe die klassischen Messprozeduren in geomagnetischen Observatorien überholt werden können. Das Geomagnetic AUtomated SyStem GAUSS ebnet den Weg für automatisierte Absolutmessungen, die bisher nur händisch möglich sind. Die neu entwickelte DI3 Technologie verbessert und vereinfacht die manuelle Standardmethode und setzt somit die Anforderungen, die an Observatoren gestellt werden, herab. Der zweite Teil behandelt die mathematischen Methoden, die für geomagnetische Modellierung zur Verfügung stehen. Insbesondere hebe ich harmonische Splinefunktionen hervor, die sich aus dem klassischen Ansatz der sphärisch-harmonischen Analyse ableiten lassen. Diese Basisfunktion haben interpolierenden Charakter und eine lokalisierte Form, wobei sie gleichzeitig die Laplacebedingung erfüllen. Sie können sowohl für regionale als auch für globale Modellierungen eingesetzt werden. Im Anschluss werden die harmonischen Splines auf einen Datensatz angewendet, der aus Messungen in Observatorien und an Säkularpunkten im südlichen Afrika besteht. Diese Region ist geomagnetisch besonders interessant, da die Feldstärke hier verhältnismäßig klein ist und starke räumliche und zeitliche Änderungen beobachtet werden. Die Analyse wird unterteilt in eine Untersuchung älterer und neuerer Daten, aus denen ich die beiden kontinuierlichen Feldmodelle SAMS und X-SAMS ableite. Aus der näheren Untersuchung der Modelle gewinne ich ein besseres Verständnis des geomagnetischen Verhaltens dieser Region. Zum Schluss werden die harmonischen Splines in einem Fallbeispiel benutzt, um Schwankungen in global verteilten Säkularvariationsdaten genauer zu beschreiben. Die Drehung in ein geomagnetisches Koordinatensystem und die Modellierung mit Splinefunktionen deckt auf, dass beobachtete schnelle Variationen durch externe Einflüsse hervorgerufen werden

    Do immigrant-origin candidates attract immigrant-origin voters in party-centred electoral systems? Evidence from Germany

    A burgeoning literature on minority representation asks whether immigrant-origin voters are more likely to vote for candidates of immigrant-origin (CIOs) than for native candidates, thus giving parties incentives to nominate CIOs. At present, however, evidence of such a link comes exclusively from candidate-centred electoral systems. The present study intends to narrow this gap by examining the influence of CIOs on the voting behaviour of immigrant-origin citizens in Germany, a more party-centred electoral environment. An empirical analysis of opinion survey and candidate data from the 2013 Bundestag election suggests that the electoral link between voters and CIOs is considerable. This paper is thus the first one to show that CIOs are a significant factor for the electoral mobilisation of immigrant-origin citizens in a party-centred electoral system
