    Fetomaternal outcome in caesarean section at full dilatation

    Background: To study indications, intraoperative and postoperative complications and fetomaternal outcome in cesarean sections done at full dilatation.Methods: This is a prospective cross-sectional study which was conducted in the department of obstetrics and gynecology, RNT medical college, Udaipur from November 2018 to April 2019. 100 cases of caesarean sections at full dilatation which were performed during this period were analyzed for indications and maternal and fetal morbidity.Results: Among these 100 cesarean sections, majority of cases were in the age group of 21-30 years (46%), booked and  Primigravida(81%).Most common indications were cephalo-pelvic disproportion (27%) and fetal distress (21%). Most commonly baby was delivered either by vertex (44%) or by Patwardhan (31%). Intraoperative complications were higher in terms of hematuria in 41%, Atonic PPH in 35%, uterine incision extension in 28% of cases. In one case bladder injury was noticed. Increased incidence of post-operative febrile illness and wound infection were noted. 44% baby’s required nursery admission, most commonly due to birth asphyxia (16%) and RDS (11%).Conclusions: Cesarean section in the 2nd stage of labor is associated with significantly increased maternal morbidity, Neonatal morbidity and mortality. So proper monitoring during labor and involvement of skilled obstetrician in decision making and delivery is crucial to minimize fetomaternal complications

    Evaluation of thyroid disorders in abnormal uterine bleeding

    Background: Abnormal uterine bleeding (AUB) is a common clinical presentation in gynecology. Alteration in thyroid hormones level has been associated with menstrual disturbances. This study is aimed to know the prevalence of thyroid disorders amongst AUB patients and also the different patterns of menstrual abnormalities associated with thyroid disorders.Methods: 100 Patient of clinically diagnosed AUB were taken from gynecology OPD at RNT Medical College Udaipur from October 2019 to March 2019. All the patients from 19 to 45 age groups presenting with menstrual disturbances were tested for thyroid function by measuring ST3, ST4, and S.TSH. Statistical analysis done by percentage formula.Results: Out of 100 women of AUB, majority were in the age group of 31-40 years (38%). 44% presented with menorrhagia. 65% were euthyroid, 17% had subclinical hypothyroidism, 15% had overt hypothyroidism and 3% were diagnosed as hyperthyroid. Subclinical hypothyroidism, overt hypothyroidism and hyperthyroidism were detected mostly in the age group of 31-40 years. The commonest bleeding abnormalities in hypothyroid patient were oligomenorrhoea while most of the hyperthyroid cases were having menorrhagia.Conclusions: The study concludes that biochemical evaluation of thyroid function is an easy, reliable method and should be made mandatory in all cases of AUB

    The relationship between pregnancy weight gain and birth weight of new born

    Background: Maternal weight gain is one of the most important independent predictors of infant birth weight.  It is stated that under nutrition during pregnancy and low birth weight increase the risk for diabetes and cardiovascular disease in adulthood. Normal weight gain of pregnancy 11kg weight gain should be according to pre-pregnancy BMI.  Patients with BMI > 25 should gain weight of 7kg patient with normal BMI should gain 11kg weights.Methods: This is a hospital based prospective study and was conducted in the Department of Obstetrics and Gynaecology, RNT Medical college Udaipur from November 2018 to March 2019  to find association between maternal weight gain and birth weight. Exclusion criteria included congenital malformations, multiple pregnancy, polyhydraminos, preeclampsia, diabetes, cardiovascular, kidney disease, RH negative pregnancy. Results were calculated by percentage.Results: excessive weight gain during pregnancy increases baby weight. Out of total 100 cases, 88 (88%) cases are Hindus and 12 (12%) cases are Muslims. Out of total 100 cases 72 cases (72%) belong to urban, 28 (28%) belong to Rural area. Out of 100 patients 32 patients gain weight less than 11kg among which 16 newborn have weight less than 2.5kg, 68 patients have weight gain more than 11kg and 67 have baby weight more than 2.5kg.Conclusions: Excessive maternal weight gain during pregnancy increases birth weight. In view of the apparent association between high birth weight and adult adiposity, an advantageous time to initiate obesity prevention efforts may be during pregnancy

    Design and Implementation of 2D Spatial Filter for EEG and MRI Segmentation

    The Electroencephalography (EEG) of brain field continues to be an attractive tool in clinical practice due to its real time depiction of brain function .The aim of this paper is to give a review of digital image segmentation technique .This paper study and implements the different types of 2D spatial filter(weighted, smoothening ,derivative) for EEG segmentation . Paper focuses on developing an automated system to enhance and recover the corrupted EEG signal images and MRI images with the help of 2D spatial filter and it also helps in early and accurate diagnosis of brain tumour. It ensures fast and reliable detection and formal resolution of deformed images by implementing noise addition and removal, edge detection, cropping, histogram adjustment, scale conversion as required by the image.     Changing trends in cesarean delivery: rate and indications

    Background: Caesarean section is a time honoured approach which was introduced in clinical practice as a lifesaving procedure both for the mother and the baby. The study was to compare the cesarean delivery rates over last 7 years and to examine the indications contributing to changed trends, if any.Methods: To compare the rate and indications of cesarean delivery over last 7 years. Data were collected in a retrospective manner from all the deliveries that occurred between January 1 and December 31 in 2009, 2012, 2015, in the Department of Obstetrics and Gynaecology at Jhalawar Medical College, Jhalawar, Rajasthan, India. The rate and indications of primary and repeat cesarean sections were analyzed among the live births to estimate the relative contribution of each indication to the overall increase in rate. Repeat caesarean delivery rate increased from 24.66/1000 live births in 2009 to 46.56/1000 live births in 2015. FD, scar tenderness, NPOL mainly contributed to it.Results: The cesarean delivery rate increased from 149.33/1000 live births in 2009 to 234.03/1000 live births in 2015, with an increase in primary cesarean delivery rate from 124.66/1000 live births in 2009 to 187.46/1000 live births in 2015. Fetal distress, non-progression of labor, obstetric indications contributed to this increase.Conclusions: There is significant increase in the total cesarean rate with primary cesarean due to fetal distress accounting for most of the increase

    Comparative study of oral iron and intravenous iron sucrose for the treatment of iron deficiency anemia in pregnancy

    Background: The aim of this study was to compare the efficacy and safety of iron sucrose with oral iron in the treatment of iron deficiency anemia of pregnancy.Methods: An interventional comparative study was conducted at Jhalawar Medical College, Jhalawar involving 80 pregnant women with iron deficiency anemia from March 2016 to August 2016. Inclusion criteria were gestational age between 24-32 weeks with established iron deficiency anemia, with hemoglobin between 7-10g/dl. Target Hemoglobin was 11 g/dl. In intravenous iron sucrose group iron sucrose dose was calculated from following formula: total iron dose required (mg) = 2.4 x body weight in Kg x (target Hb – Patient’s Hb g/dl) + 500. In oral iron, group patient received ferrous-sulphate 335 mg daily BD. Hb level were reviewed at 2, 4, 6 weeks.Results: Change in Hemoglobin level from baseline significantly higher in IV iron group than oral iron group. In IV iron, group mean value of baseline Hb was 8.07±0.610 g/dl and in oral iron group was 8.48±0.741 g/dl. At the end of 6-week mean hemoglobin in IV iron sucrose was 10.66±0.743 g/dl and in oral iron group was 10.08±0.860 g/dl.Conclusions: Intravenous iron sucrose elevates more Hb than oral iron, with less adverse effects

    Abstract The growing usage of digital platforms has made influencer marketing the most cost-effective marketing tool, especially for products consumed by young digital natives. Among other products, these include fashion and beauty-related items such as cosmetics, whose online marketing has created a particular category of influencers called ‘beauty vloggers.’ By applying the source credibility model, this study investigates whether beauty vloggers’ credible attributes (trustworthiness, expertise, and attractiveness) help consumers build trust towards them and whether followers’ trust leads to desirable marketing outcomes (increased purchase intention). Prior literature used the source credibility model in a fragmented manner. However, this study enhances the holistic framework of social media influencers’ impact on purchase intention by investigating all three credible attributes. It specifically addresses the significance of cultivating trust between beauty vloggers and followers, exploring the impact of vloggers’ credible attributes on consumer purchase intention. Additionally, beauty vloggers periodically engage in parasocial interactions to increase closeness with their audiences, affecting consumer behaviour. This study addresses the limited research on the consequences of parasocial interactions in an emerging market context, examining the impact of beauty vloggers’ repeated interactions on consumer behaviour, contributing to a comprehensive understanding of purchase intention using parasocial interaction as an antecedent. The research was conducted on consumers belonging to the age group of 16–45 who use social media platforms daily. Using structural equation modelling via SmartPLS software, the statistical analysis was done on 357 North Indian respondents. The study explores the influence of beauty vloggers on consumers’ purchasing behaviour in the Indian context. The results demonstrate that beauty vloggers’ credibility traits help in influencing consumer behaviour. The findings also revealed that trust partially mediates the impact of credible attributes of beauty vloggers and consumers’ purchase intention. Additionally, the results show that customers’ purchasing decisions might be influenced by repeated parasocial interactions done by beauty vloggers if they perceive them as credible information sources. The findings of the study contribute to help industrial marketers and practitioners of cosmetic companies use influencer marketing strategically in order to increase market sales
