Seventy years old male with multiple brain damage and intracardiac mass
The case is that of a 70 years old male, ex-smoker, with history of hypertension, who was admitted to the emergency room, with a sudden focal neurological deficit. During hospitalization he suffered additional neurologic deficits. The patient subsequently undergoes a transthoracic echography which showed an 60x50 mm auricular mass. Thoracic CT scan demonstrates pulmonary veins repletion deficits. He died a few days after. The case is discussed in the context of a clinicopathological general session at Son Espases Universitary Hospital
Impact of Wnt/β-Catenin Inhibition on Cell Proliferation through CDC25A Downregulation in Soft Tissue Sarcomas.
The Wnt signaling pathway is an important cellular mechanism for regulating differentiation processes as well as cell cycle events, and different inhibitors of this pathway, for example, PRI-724, are showing promising results in clinical trials for treatment of advanced pancreatic adenocarcinoma or ovarian cancer. Growing evidence suggests that Wnt signaling may also be crucial for tumorigenesis and progression of soft tissue sarcomas (STS), a malignant neoplasm with few therapeutic options at an advanced state. Our study with several STS cell lines and primary cultures shows that inhibition of Wnt/β-catenin signaling with PRI-724 is able to suppress cell viability/proliferation and to increase cell death rates. TCF/β-catenin-mediated transcriptional activity is decreased in treated cells, leading to downregulation of its target genes CCND1 and CDC25A. The latter was critical because its downregulation via siRNA was able to mimic the effect of PRI-724 on cell cycle arrest and cell death induction. An evaluation of NCBI/GenBank data confirmed that CDC25A mRNA is elevated in STS patients. Importantly, PRI-724 in combination with standard STS chemotherapeutics doxorubicin or trabectedin enhanced their antitumoral effect in a synergistic manner according to isobolographic analysis, suggesting that Wnt inhibition through PRI-724 could be a beneficial combination regime in patients with advanced STS.This study was financed by Grupo Español de Investigación en Sarcomas (GEIS) and Fundación Mari PazJiménez Casado. Exploring the role of anti-angiogenic therapies in prostate cancer: results from the phase 3 trial of sunitinib
Prostate cancer is a leading cause of cancer death in men. Despite recent advances in our understanding and treatment of advanced disease, no systemic therapy is curative and new therapies are needed. Targeting angiogenesis is an attractive therapeutic strategy, as angiogenic pathways are upregulated in prostate tumors similar to other malignancies due to imbalance of pro- and anti-angiogenic factors secreted by tumor, endothelial and stromal cells and increased neovasculature. [1] Vascular endothelial growth factor (VEGF) is the most well-characterized pro-angiogenenic factor, with several small molecule inhibitors (sunitinib, sorafenib, pazopanib, axitinib, others), antibodies (bevacizumab) and other drugs that target the VEGF pathway approved and/or in development for the treatment of a wide range of tumor types
Comportamiento de la toxoplasmosis ocular activa en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer" Behavior of the active ocular toxoplasmosis in patients of the "Ramón Pando Ferrer" Cuban Institute of Ophthalmology
To describe the pathological findings in a series of patients diagnosed with ocular toxoplasmosis. METHODS: A case-series, descriptive and observational study was conducted to describe the behavior of the active way of the ocular toxoplasmosis in patients seen in the "Ramón Pando Ferrer" Cuban Institute of Ophthalmology over 2009. A total of 128 were examined who underwent a total laboratory and ophthalmologic examination. RESULTS: The commonest presentation way was as a retinitis and the more involved age-group was that of 20-29 years, with a slight predominance of female sex. The vision decrease and the floaters were the main symptoms of presentation, the clinical diagnosis was made in 106 patients. Inflammation predominated in the 1 retinal zone and involved mainly in a moderate way the vision of patients in the involved eye at diagnosis. In 122 of the 128 treatment was applied consisted in most of the standard scheme of pyrimethamine, sulfadiazine and prednisone. The 43.1 % of involved eyes recovered it normal vision, whereas 34 eyes remained with a severe decrease, most caused by complications including macular scars, which were the more registered. CONCLUSIONS: It is possible to achieve good visual results with an appropriate therapy although it will depend on the involved retinal area, the level of intraocular inflammation and the length of the disease

Reliability and minimal detectable change of three functional tests: forward-lunge, step-up-over and sit-to-stand
[Purpose] To examine the intrasession and intersession reliability and the absolute reliability of three functional dynamic tests—forward-lunge, step-up-over and sit-to-stand tests—using computerized dynamic posturography. [Subjects and Methods] An intra-test and test-retest, repeated measure study was designed. Forty-five healthy subjects twice carried out the forward-lunge test, step-up-over test, and sit-to-stand test on two days, one week apart. The intrasession and intersession reliabilities as judged by the intraclass correlation coefficient (ICC) and the minimal detectable change of the three functional tests were calculated. [Results] Excellent to very good intrasession reliability of the forward-lunge test (ICC range of 0.9–0.8) was found. Very good to good intrasession reliability of the step-up-over test (ICC range of 0.9–0.5) was found and very good intrasession reliability of the sit-to-stand test (ICC range of 0.8–0.7) was found. The minimal detectable change at the 95% confidence level of most of the measures was lower than 30%. [Conclusion] The forward-lunge, step-up-over and sit-to-stand tests are reliable measurement tools
