    Ansiedad en gastroscopias: Comparación de dos intervenciones de enfermería en gastroscopias sin sedación.

    There is a disparity between nurses about the ideal role of nurses in endoscopies without sedation. Some nurses think that providing information to the patient is sufficient to reduce anxiety and improve tolerance and satisfaction, while others believe that behavioral training and positive reinforcement during the procedure are also necessary. The objectives of this study were to test the differences that are produced in the patient’s state of anxiety between the two types of nursing intervention, as well as in the patient’s tolerance and satisfaction. The study included 109 outpatients who had an endoscopy without sedation. They were divided into two groups, the experimental group who received nursing support based on information, behavioral training and positive reinforcement during the procedure, and the control group, who received nursing support based solely on information provided about the procedure. Anxiety was evaluated with a STAI-state test and with psychophysiological parameters at different moments during the process. The data was analyzed with repeated measures of analysis of the variance, which resulted in the following: the STAI score decreased more in the experimental group. Tolerance was greater in the experimental group, patient satisfaction was equal in the two groups, and the difference in the levels of systolic and diastolic blood pressure and heart rate was equal in the two groups. Experimental investigations are useful in nursing to obtain scientific evidence about the ideal clinical practice. It is possible to improve the tolerance of gastroscopy and reduce anxiety due to the procedure, with the intervention of the nurses centered in the cognitive and behavioral aspects of the person.Existe disparidad en las percepciones que las enfermeras de endoscopias  tenemos sobre la intervención de enfermería idónea en gastroscopias sin sedación. Algunas enfermeras  piensan  que el aporte de información es suficiente para reducir la ansiedad, mejorar la tolerancia y satisfacción, mientras  otras defienden que además es necesario  un entrenamiento conductual y un refuerzo positivo durante la prueba.Los objetivos de esta investigación fueron comprobar las diferencias que se producían en el estado de  ansiedad del paciente entre las dos intervenciones de enfermería, así como en la tolerancia y su satisfacción.Se incluyeron 109 pacientes que acudieron vía ambulatoria a realizarse una gastroscopia sin sedación. Se dividieron en dos grupos, el  experimental, con una intervención basada en información, entrenamiento conductual y refuerzo positivo durante la exploración y el control, con una intervención basada en la información. La  ansiedad se evaluó con el test STAI-estado y con  parámetros psicofísicos en diferentes momentos del proceso.Los datos se analizaron con medidas repetidas de análisis de la variancia que  aportaron los siguientes resultados: la puntuación del STAI disminuyó más en el grupo experimental (p=0,035). La tolerancia fue  mejor en el grupo experimental (p= 0,008), la satisfacción del paciente fue  igual en los dos grupos (p=0,5) la diferencia en los valores de tensión arterial sistólica, diastólica y frecuencia cardíaca fue  igual en los dos grupos (p=0,085, p=0,690, p=0,984)Las investigaciones experimentales son posibles en enfermería para obtener evidencias científicas sobre la idónea práctica clínica. Es posible mejorar la tolerancia de la gastroscopia y disminuir la ansiedad debida al procedimiento,  con una intervención de enfermería centrada en el aspecto cognitivo y conductual de la persona

    Transformative learning experience among nursing students with patients acting as teachers: Mixed methods, non-randomized, single-arm study

Design Non-randomized, single-arm study with quantitative before and after measurements and qualitative data. Methods The intervention consisted of five 90-min workshops led by two women living with HIV. Thirty-four nursing students participated, and quantitative and qualitative data were gathered from February to June 2018. We used the Patient-Practitioner Orientation Scale (PPOS) and the KAP questionnaire on HIV/AIDS to collect quantitative data. Results Statistically significant differences were found in the global score for care orientation and its two dimensions, caring and sharing. About the changes resulting from the workshops, the quantitative results—more patient-centred care perception and better attitudes towards people living with HIV—match the qualitative findings in all the aspects studied, except in sharing. Conclusion Incorporating expert patients as teachers in the nursing bachelor's degree resulted in more patient-centred care and improved knowledge, attitudes and practices.     The mastery of the English language is an aspiration of the future professionals who study this language at Ciego de Ávila University of Pedagogical Sciences, however in meeting this goal certain difficulties have hindered it, due to the poor development of vocabulary which is manifested in everyday use.     How the lagged and accumulated effects of stress, coping, and tasks affect mood and fatigue during nurses' shifts

    Nurses experience significant stress and emotional exhaustion, leading to burnout and fatigue. This study assessed how the nurses' mood and fatigue evolves during their shifts, and the temporal factors that influence these phenomena. Performing a two-level design with repeated measures with moments nested into a person level, a random sample of 96 nurses was recruited. The ecological momentary assessment of demand, control, effort, reward, coping, and nursing tasks were measured in order to predict mood and fatigue, studying their current, lagged, and accumulated effects. The results show that: (1) Mood appeared to be explained by effort, by the negative lagged effect of reward, and by the accumulated effort, each following a quadratic trend, and it was influenced by previously executing a direct care task. By contrast, fatigue was explained by the current and lagged effect of effort, by the lagged effect of reward, and by the accumulated effort, again following quadratic trends. (2) Mood was also explained by problem-focused and emotion-focused coping strategies, indicative of negative mood, and by support-seeking and refusal coping strategies. (3) Fatigue was also associated with direct care and the prior effect of documentation and communication tasks. We can conclude that mood and fatigue do not depend on a single factor, such as workload, but rather on the evolution and distribution of the nursing tasks, as well as on the stress during a shift and how it is handled. The evening and night shifts seem to provoke more fatigue than the other work shifts when approaching the last third of the shift. These data show the need to plan the tasks within a shift to avoid unfinished or delayed care during the shift, and to minimize accumulated negative effect

    Knowledge about the best practice guidelines in the nursing degree : A non-randomized post-test design

    Altres ajuts: acords transformatius de la UABAim: To compare knowledge of Nursing Degree students about Best Practice Guidelines when there are included as teaching content in a subject vs knowledge through having the usual internship experience without teaching specific guidelines contents. Design: Non-randomized post-test-only design with a comparison group. Methods: 143 students of the nursing degree at the Autonomous University of Barcelona were recruited. The intervention group received a classroom training in three Best Practice Guidelines with Problem-Based Learning methodology. The comparison group only attended internship, without specific guidelines contents. Knowledge was evaluated with an ad hoc post intervention questionnaire. The information was collected between 2016 and 2018. Results: The average score of knowledge was low, 5.1 out of 10, and differs between guides. The best results were obtained by the students with internships and that had consulted the guides on some occasions. Synchronized effort and leadership in Academia and Healthcare are needed to favour evidence-based practice. The combination of the consultation of the Best Practice Guidelines in theoretical learning combined with the practice, increases the knowledge of the Best Practice Guidelines and will favour the implementation of evidence-based practice.     A novel SPE-UHPLC-DAD method for the determination of fumagillin produced by Aspergillus fumigatus in cell culture media

    [EN]Fumagillin is a biomolecule produced by Aspergillus fumigatus that is gaining relevance due to its connection with invasive aspergillosis. The determination of this molecule might help to understand the propagation of this disease and study its use as a potential biomarker. In spite of the interest of fumagillin in microbiological research, no quantitative method has been developed so far for its determination in cell culture media. In this work, the first validated method for the quantitative analysis of fumagillin in RPMI-1640 is presented. The sample treatment consists of a mixed-mode anion exchange Solid Phase Extraction that effectively removes potential interferences and offered a recovery of 83 ± 7%. The analysis was carried out by Ultra High Performance Liquid Chromatography coupled to Diode Array Detection at 336 nm. The method fulfilled the validation criteria established by EMA and FDA guidelines for bioanalysis (selectivity, carry over, linearity, accuracy, precision, dilution integrity and stability) and offers a limit of quantitation (25 μg·L−1) suitable for its intended use. Indeed, the method was satisfactorily applied to the quantification of the fumagillin produced by three strains of Aspergillus fumigatus with different toxin production capacity.This research was funded by University of the Basque Country (UPV/EHU) (project GIU19/068 and project COLAB20/11) and by Basque Government (grant number IT1362-19).     UNA PROPUESTA PARA LA ENSEÑANZA DEL FRANCÉS A TRAVÉS DE LA MARSELLESA

    La investigación se desarrolló con los estudiantes de quinto año de la carrera de Licenciatura en Turismo de la Universidad de Ciego de Ávila Máximo Gómez Báez con el objetivo de ofrecer una nueva alternativa en la enseñanza del idioma francés incorporando de esta forma un nuevo material a la escasa bibliografía existente para esta especialidad. Se comprobó que los estudiantes poseen un pobre vocabulario en sentido general, así como dificultades con algunos elementos gramaticales particularmente con la conjugación de los verbos franceses en los distintos tiempos verbales, los adjetivos demostrativos  los pronombres posesivos y los adjetivos posesivos. La Marsellesa ofreció la posibilidad de trabajar no solo en aquellos aspectos de la gramática mencionados con anterioridad sino que permitió también enriquecer el vocabulario de los estudiantes, mejorar la ortografía, la pronunciación y contribuir a elevar la cultura general e integral de los futuros profesionales del turism
