    The interplay between cognition, functional and dual-task gait in persons with a vestibular disorder versus healthy controls

    Close links exist between vestibular function and cognition. Dual-task (DT) tests may have ecological validity to assess the impact of daily life cognitive-motor demands in people with vestibular dysfunction (PwVD), functional gait and falls risk. The present paper aimed at building predictive models for functional gait under DT conditions, while clarifying the impact of vestibular dysfunction, individual characteristics, varying task types and motor-cognitive demands. Case-controlled observational study with 39 PwVD and 62 healthy participants. The Functional Gait Assessment (FGA), with and without an additional motor, numeracy, or literacy task, was completed. Multiple linear regression was used to fit models to predict FGA under single and DT performance. Dual task cost (DTC, %) was calculated to assess DT interference on FGA performance using the equation: 100*(single task score-dual task score)/single-task score. Following Bonferroni corrections for multiple comparisons (corrected alpha level of 0.003), PwVD had poorer performance than controls for all FGA conditions (p < 0.001), motor (- 3.94%; p = 0.002) and numeracy (- 22.77%; p = 0.001) DTCs and spatial working memory (p = 0.002). The literacy DTC was marginally significant (- 19.39% p = 0.005). FGA single and DT motor, numeracy, and literacy models explained 76%, 76%, 66% and 67% of the variance respectively for PwVD. Sustained attention, visual memory and sex contributed to all models; short-term visual recognition memory, balance confidence, and migraine contributed to some models. Cognitive performance is impaired in PwVD. Motor, numeracy and literacy tasks impair functional gait performance. Cognitive assessment and FGA with a numeracy or literacy cognitive component should be included within assessment protocols and considered in the provision of targeted interventions for PwVD

    Performance of industrial melting pots in the provision of dynamic frequency response in the Great Britain power system

    As a result of the increasing integration of Renewable Energy Source (RES), maintenance of the balance between supply and demand in the power system is more challenging because of RES’s intermittency and uncontrollability. The smart control of demand is able to contribute to the balance by providing the grid frequency response. This paper uses the industrial Melting Pot (MP) loads as an example. A thermodynamic model depicting the physical characteristics of MPs was firstly developed based on field measurements carried out by Open Energi. A distributed control was applied to each MP which dynamically changes the aggregated power consumption of MPs in proportion to changes in grid frequency while maintaining the primary heating function of each MP. An aggregation of individual MP models equipped with the control was integrated with the Great Britain (GB) power system models. Case studies verified that the aggregated MPs are able to provide frequency response to the power system. The response from MPs is similar but faster than the conventional generators and therefore contributes to the reduction of carbon emissions by replacing the spinning reserve capacity of fossil-fuel generators. Through the reviews of the present balancing services in the GB power system, with the proposed frequency control strategy, the Firm Frequency Response service is most beneficial at present for demand aggregators to tender for.     Power system frequency response from the control of bitumen tanks

    Bitumen tanks were tested to investigate the capability of industrial heating loads to provide frequency response to an electric power system. A decentralized control algorithm was developed enabling the tanks to alter their power consumption in proportion to the variations of grid frequency. The control maintains the normal operation of tanks and causes little impact on their primary function of storing hot bitumen. Field investigations were undertaken on 76 tanks with power ratings from 17 to 75 kW. A model of a population of controlled tanks was developed. The behavior of the tanks was compared between the simulations and the field tests. The model of controlled tanks was then integrated with a simplified Great Britain power system model. It was shown that the controlled tanks were able to contribute to the grid frequency control in a manner similar to and faster than that provided by frequency-sensitive generation

    Common health issues and advised treatments reported in an ultraendurance ocean rowing race

    OBJECTIVES: Ocean rowing is an extreme ultraendurance sport where individuals row unsupported across an ocean. This can mean months at sea in unpredictable and inhospitable conditions. The extreme physical and mental demands associated mean medical issues are common yet under researched, impacting the medical assistance provided. METHODS: The Talisker Whisky Atlantic Challenge provides a framework with safety protocols to create safer environments for athletes to attempt transatlantic crossings. Race logs, which record communication between rowers and safety teams, were anonymised and examined to create cumulative totals of medical issues requiring help and an incident timeline. RESULTS: Duty officers were notified of 65 medical complaints, 32% (n=21) requiring race doctors. Dermatological conditions were most common, accounting for 32% of complaints (n=21) and affecting 24% of rowers. Pressure sores and infections represented 43% and 38% of dermatological complaints respectively. Musculoskeletal injuries equalled 18% (n=12) of complaints, affecting 20% of rowers. Most cases were reported in weeks one and three, with 37% (n=24) of issues and 90% (n=10) of seasickness cases in week one. Hygiene and antibiotics were frequently advised management options. CONCLUSION: Conditions requiring race doctor input represent the most severe cases with the potential to deteriorate. Managing dermatological and musculoskeletal issues is vital throughout the race, while seasickness prophylaxis and education is essential in initial race stages. Infections are cause for concern, with gluteal pressure sores a potential route for secondary infections. Further research is required before evidence-based guidelines for management of health issues in ocean rowers can be introduced

    Common Health Issues Encountered by Ultraendurance Ocean Rowers

    INTRODUCTION: Ocean rowing is an extreme ultraendurance sport in which athletes push themselves to their mental and physical limits while rowing across an ocean. Limited academic attention has meant health issues facing this population are poorly understood. This report provides a descriptive analysis of the injuries and illnesses encountered by ocean rowers at sea and suggests potential preventative measures.METHODS: Retrospective self-reported data were collected from ocean rowers via an online 29-question survey, classified by medical system, and totaled to produce a report of the most frequently encountered symptoms.RESULTS: Seventy-one ocean rowers, accounting for 86 ocean rowing attempts, completed the survey. Dermatologic symptoms formed 52% (n=169) of all reported issues, followed by musculoskeletal injuries (14%; n=45), mental health symptoms (11%; n=36), gastrointestinal symptoms (5%; n=16), and neurologic symptoms (2%). Gluteal pressure sores were the most common dermatologic symptoms (24%; n=40), hallucinations the most common mental health symptoms (69%; n=25), hand and finger issues the most reported musculoskeletal problems (36%; n=16); vomiting (38%) and headaches (50%) were the most common gastrointestinal and neurologic issues, respectively. Seasickness was reported in 42% of expeditions (n=33).CONCLUSIONS: This report presents the physiological, mental, and medical challenges facing ocean rowers. Dermatologic and musculoskeletal issues were most common and varied greatly in severity. Over 90% of reported infections occurred as a dermatologic complaint, demonstrating the importance of preventative measures such as hygiene and wound care. Continued work with a larger population is required to further understand the physiological stress and medical complaints associated with transoceanic rowing.</p
