    Incidence and prognosis of rheumatic carditis in a Belgian hospital. A 10 year review (1965-1974)

    106 cases of first attack of acute rheumatic fever were recorded during a decade (ending in 1974) in a Brussels pediatric clinic. 56 patients were Belgian and 50 were children of immigrants. The incidence of acute carditis was 23% among the former, 46% among the latter (P ≤ 0.01), and 34% among the whole group. The frequency of residual heart disease after a 3-yr antistreptococcal prophylaxis fell to 20% among the whole group.     The incidence of various congenital malformations was established in a series of 1,628 autopsies of children aged from 0 to 15 yr. There were 21.3% still borns, 31.7% new borns less than 8 days old and 47% children from 8 days to 15 yr old. The incidence of congenital malformations was found to be 19.9%; the highest frequency was found in the group of older children (24.7%). In 176 cases (10.8%), one or more cardiac anomalies were present. Extracardiac malformations were found in 198 children (12.1%). The most common abnormalities occurred in the genito urinary system (4.6%), central nervous (3.9%), gastro intestinal (3.1%) and skeletal systems (3.1%). Congenital heart disease was associated with other congenital anomalies in 50 cases (28%). The most common association was found with the genito urinary system: 11.4% of the cardiac cases had a urinary tract abnormality, whereas 26.3% of the urological anomalies were associated with various types of cardiac malformations. The cases in which there was only a septal defect (atrial and or ventricular) had the highest incidence of extra cardiac anomalies.     Pulmonary artery pressure changes in infants with bronchiolitis

    Sequential measurements of both the lesser circulation pressures and the blood gas tensions were made in 13 infants with bronchiolitis. Moderate pulmonary hypertension was present in most subjects but there was no evidence of heart failure. Pulmonary hypertension correlated with hypoxemia and disappeared after high oxygen mixture breathing.     Acquired pulmonary artery aneurysm in an infant

    An aneurysm of the left pulmonary artery developed in an infant during a staphylococcal generalized infection. The successful surgical treatment required a left pneumonectomy.     The changing prognosis of congenital heart diseases in young children

    In our Paediatric Cardiac Unit, 190 infants with congenital heart disease (CHD) were hospitalized from 1963 till 1967 (period A) and 254 were hospitalized from 1972 till 1976 (period B). Fifty-six patients (29%) were operated upon during period A and 111 (44%) during period B, the post-operative survival rates being 55% and 68% respectively. The proportion of one-stage corrective procedures was 25% during period A, and 47% during period B. The overall and cardiac fatality rates were respectively 56% and 41% during period A compared to 36% and 30% during period B. During period B 40% of all deaths occurred during the first month of life in patients not operated upon. The improved results of period B were related to more rapid and more intensive management of critically ill infants, but progress has still to be made during the neonatal period.SCOPUS: ar.jinfo:eu-repo/semantics/publishe

    Intensive care in young children undergoing cardiac surgery
