The “Welcome to Medicare” Visit: A Missed Opportunity for Cancer Screening Among Women?
On January 1, 2005, Medicare began covering a “Welcome to Medicare” visit (WMV) for new enrollees with fee-for-service (FFS) Medicare (Parts A and B). The new benefit was expected to increase demand for mammography and Pap tests among women transitioning onto Medicare. Adherence to chemotherapy guidelines has rarely been studied among large populations, mainly because performance status (PS), a key component in assessing chemotherapy appropriateness, is missing from claims-based datasets. Characterization of soot properties in two-meter JP-8 pool fires.
The thermal hazard posed by large hydrocarbon fires is dominated by the radiative emission from high temperature soot. Since the optical properties of soot, especially in the infrared region of the electromagnetic spectrum, as well as its morphological properties, are not well known, efforts are underway to characterize these properties. Measurements of these soot properties in large fires are important for heat transfer calculations, for interpretation of laser-based diagnostics, and for developing soot property models for fire field models. This research uses extractive measurement diagnostics to characterize soot optical properties, morphology, and composition in 2 m pool fires. For measurement of the extinction coefficient, soot extracted from the flame zone is transported to a transmission cell where measurements are made using both visible and infrared lasers. Soot morphological properties are obtained by analysis via transmission electron microscopy of soot samples obtained thermophoretically within the flame zone, in the overfire region, and in the transmission cell. Soot composition, including carbon-to-hydrogen ratio and polycyclic aromatic hydrocarbon concentration, is obtained by analysis of soot collected on filters. Average dimensionless extinction coefficients of 8.4 {+-} 1.2 at 635 nm and 8.7 {+-} 1.1 at 1310 nm agree well with recent measurements in the overfire region of JP-8 and other fuels in lab-scale burners and fires. Average soot primary particle diameters, radius of gyration, and fractal dimensions agree with these recent studies. Rayleigh-Debye-Gans theory of scattering applied to the measured fractal parameters shows qualitative agreement with the trends in measured dimensionless extinction coefficients. MBM 12: young protoplanetary discs at high galactic latitude
(abridged) We present Spitzer infrared observations to constrain disc and
dust evolution in young T Tauri stars in MBM 12, a star-forming cloud at high
latitude with an age of 2 Myr and a distance of 275 pc. The region contains 12
T Tauri systems, with primary spectral types between K3 and M6; 5 are weak-line
and the rest classical T Tauri stars. We first use MIPS and literature
photometry to compile spectral energy distributions for each of the 12 members
in MBM 12, and derive their IR excesses. The IRS spectra are analysed with the
newly developed two-layer temperature distribution (TLTD) spectral
decomposition method. For the 7 T Tauri stars with a detected IR excess, we
analyse their solid-state features to derive dust properties such as
mass-averaged grain size, composition and crystallinity. We find a spatial
gradient in the forsterite to enstatite range, with more enstatite present in
the warmer regions. The fact that we see a radial dependence of the dust
properties indicates that radial mixing is not very efficient in the discs of
these young T Tauri stars. The SED analysis shows that the discs in MBM 12, in
general, undergo rapid inner disc clearing, while the binary sources have
faster discevolution. The dust grains seem to evolve independently from the
stellar properties, but are mildly related to disc properties such as flaring

U.S. Preventive Services Task Force Recommendations and Cancer Screening Among Female Medicare Beneficiaries
Background: Medicare covers several cancer screening tests not currently recommended by the U.S. Preventive Services Task Force (Task Force). In September 2002, the Task Force relaxed the upper age limit of 70 years for breast cancer screening recommendations, and in March 2003 an upper age limit of 65 years was introduced for cervical cancer screening recommendations. We assessed whether mammogram and Pap test utilization among women with Medicare coverage is influenced by changes in the Task Force's recommendations for screening
Child Psychosocial Adjustment and Parenting in Families Affected by Maternal HIV/AIDS
Child adjustment and parenting were examined in 23 9-through 16-year-old youth from families affected by maternal HIV infection and 20 same-age peers whose mothers were not infected. Children whose mothers were seropositive reported significantly more externalizing problems. Infected mothers reported less age-appropriate supervision/monitoring relative to non-infected mothers. Better mother-child relationship quality and less impairment in parental supervision/monitoring of age-appropriate youth behaviors were associated with fewer externalizing difficulties among the HIV-positive group only. Similarly, only among HIV-infected mothers was refraining from engaging in inconsistent disciplinary tactics associated with lower reports of internalizing and externalizing problems. These data highlight the promise of programs targeting parenting skills to prevent or ameliorate child difficulties

Soot formation, transport, and radiation in unsteady diffusion flames : LDRD final report.
Fires pose the dominant risk to the safety and security of nuclear weapons, nuclear transport containers, and DOE and DoD facilities. The thermal hazard from these fires primarily results from radiant emission from high-temperature flame soot. Therefore, it is necessary to understand the local transport and chemical phenomena that determine the distributions of soot concentration, optical properties, and temperature in order to develop and validate constitutive models for large-scale, high-fidelity fire simulations. This report summarizes the findings of a Laboratory Directed Research and Development (LDRD) project devoted to obtaining the critical experimental information needed to develop such constitutive models. A combination of laser diagnostics and extractive measurement techniques have been employed in both steady and pulsed laminar diffusion flames of methane, ethylene, and JP-8 surrogate burning in air. For methane and ethylene, both slot and coannular flame geometries were investigated, as well as normal and inverse diffusion flame geometries. For the JP-8 surrogate, coannular normal diffusion flames were investigated. Soot concentrations, polycyclic aromatic hydrocarbon (PAH) laser-induced fluorescence (LIF) signals, hydroxyl radical (OH) LIF, acetylene and water vapor concentrations, soot zone temperatures, and the velocity field were all successfully measured in both steady and unsteady versions of these various flames. In addition, measurements were made of the soot microstructure, soot dimensionless extinction coefficient (&), and the local radiant heat flux. Taken together, these measurements comprise a unique, extensive database for future development and validation of models of soot formation, transport, and radiation
Geosynchronous Imaging Fourier Transform Spectrometer (GIFTS) Engineering Demonstation Unit (EDU) Overview and Performance Summary
The Geosynchronous Imaging Fourier Transform Spectrometer (GIFTS), developed for the NASA New Millennium Program (NMP) Earth Observing-3 (EO-3) mission, has recently completed a series of uplooking atmospheric measurements. The GIFTS development demonstrates a series of new sensor and data processing technologies that can significantly expand geostationary meteorological observational capability. The resulting increase in forecasting accuracy and atmospheric model development utilizing this hyperspectral data is demonstrated by the uplooking data. The GIFTS sensor is an imaging FTS with programmable spectral resolution and spatial scene selection, allowing spectral resolution and area coverage to be traded in near-real time. Due to funding limitations, the GIFTS sensor module was completed as an engineering demonstration unit that can be upgraded to flight quality. This paper reviews the GIFTS system design considerations and the technology utilized to enable a nearly two order performance increase over the existing GOES sounder and shows its capability. While not designed as an operational sensor, GIFTS EDU provides a flexible and accurate testbed for the new products the hyperspectral era will bring. Efforts to find funding to upgrade and demonstrate this amazing sensor in space are continuing
