Introduction: Research studies determine that the socio-economic context is a risk factor for malocclusions. The concentration of orthodontists around urban centres leads to a lack of treatment in rural areas. It is important to look for new approaches to providing orthopaedic-orthodontic treatments.Case Description: Patient belonging to a rural community with skeletal class II, malposition, volume anomalies, discrepancy, nasal ventilatory insufficiency, dysfunctional swallowing, postural alteration, low weight, mature delay and perinatal trauma. Treatment is performed using functional orthopaedic appliances, correct jaw relationships, functionally adequate occlusion, functional and postural re-education are achieved.Discussion: Living conditions, geographical isolation, lack of access to health services and cultural particularities are risk factors for malocclusion. Proper diagnostics and quality treatment require the design of specific protocols adapted to the situation. The apparatus used, an endobuccal complex of new stimuli that generates a change in the muscular synergy in the search for reorganizing nervous afferences and regulating oral functions, must be adequate for the development of the treatment according to the characteris-tics of the patient and his context. The planning carried out, the difficulties, challenges and solutions that are transferred to the project and work methodology. The professionals act on a voluntary basis within the framework of the joint work of two nonprofit foundations.Conclusion/Clinical Significance: Rural communities with social vulnerability require specific protocols in the health-disease-care process. Functional orthopaedic appliances were effective in the treatment of jaw disorders. Interdisciplinary work and professional and institutional commitment are important for achieving results.https://onlinelibrary.wiley.com/doi/epdf/10.1111/idj.12444Fil: Gabrielli, Carolina Guadalupe.
