Coronavirus disease 2019: the second wave in Italy
PurposeThis paper describes how Italy addressed the first Coronavirus disease 2019 (COVID-19) wave and analyzes the possible causes of the current second wave.Design/methodology/approachDescriptive analysis of critical points and differences in the containment strategies between the first and the second waves in Italy.FindingsItaly's strict lockdown has been credited with getting the initial major outbreak under control. Furthermore, the way Italy handled the first wave was considered a lesson for other countries. On the contrary, a decentralized and highly bureaucratic political system with low coordination and political conflicts between government, regions and stakeholders led to a relaxation of individual health behaviors, poor and conflicting communication to the general public, poor management of the public transport and the reopening of schools and companies after the summer, that in turn generated the second wave, which is showing signs of becoming worse than the first.Originality/valueThis is a commentary piece

Therapeutic strategies to prevent the recurrence of nasal polyps after surgical treatment: an update and in vitro study on growth inhibition of fibroblasts
Chronic rhinosinusitis with nasal polyps (CRSwNP) is the most bothersome phenotype of chronic rhinosinusitis, which is typically characterized by a Type 2 inflammatory reaction, comorbidities and high rates of nasal polyp recurrence, causing severe impact on quality of life. Nasal polyp recurrence rates, defined as the number of patients undergoing revision endoscopic sinus surgery, are 20% within a 5 year period after surgery. The cornerstone of CRSwNP management consists of anti-inflammatory treatment with local corticosteroids. We performed a literature review regarding the therapeutic strategies used to prevent nasal polyp recurrence after surgical treatment. Finally, we report an in vitro study evaluating the efficacy of lysine-acetylsalicylic acid and other non-steroidal anti-inflammatory drugs (ketoprofen and diclofenac) on the proliferation of fibroblasts, obtained from nasal polyp tissue samples. Our study demonstrates that diclofenac, even more so than lysine-acetylsalicylic acid, significantly inhibits fibroblast proliferation and could be considered a valid therapeutic strategy in preventing CRSwNP recurrence
Perturbation of specific transcripts in peripheral blood mononuclear cells in breast cancer: a case control pilot study
Introduction: Breast cancer (BC) is the most common type of diagnosed cancer worldwide and the leading cause of cancer death in women and it is the second most frequent cancer-causing mortality for women worldwide. Peripheral blood-based biopsy for BC could be a promising tool for risk prediction and diagnosis. In this study, we aimed to evaluate the gene expression profile of PBMCs in Italian patients with BC.
Methods: In this case-control pilot study, we isolated PBMCs from 22 BC patients and 21 healthy controls and evaluated the expression of a panel of 52 target genes related to BC or circadian rhythm by a customized TaqMan Open Array Real-Time PCR panel.
Results: Among the differentially expressed genes, 22 remained unchanged. These unchanged genes are mainly involved in cellular processes, including the circadian clock, cellular responses to stress/stimuli, the immune system, signal transduction, and metabolism. We found a total of 30 significantly de-regulated genes. In particular, 8 genes, including PARP6, IGFR1, EZH2, VEGFA, NOTCH1, CD44, BCAR1, and CD24A, resulted significantly down-regulated in patients with BC
compared to Controls, while 22 genes were significantly up-regulated in BCs patients compared to Controls. We found several already reported BC-associated genes up-regulated in PBMCs of our BC patients, but FOXO3, ARNTL, and ADAM17 emerged as the most strongly up-regulated. The
enrichment pathways analysis highlight that de-regulated genes are mainly involved in the regulation of gene expression and transcription, signal transduction, and immune system response.
Discussion: The results of our pilot study demonstrated that the evaluation of PBMC gene signature could be a valuable tool for primary prevention and early diagnosis of BC in several high-risk settings, thus reducing the global mortality associated with this tumour.
Take-home message: Non-invasive screening programs, particularly those adopted in workplaces, may have a great impact on early diagnosis and good prognosis for BC. Our study provided proof of concept that the development of a screening test based on PBMC-derived gene expression biomarkers could be a viable route
Prevalence and Predictors of Burnout Syndrome among Italian Psychologists following the First Wave of the COVID-19 Pandemic: A Cross-sectional Study
Introduction: The main objective of this study was to assess the prevalence and predictors of Burnout Syndrome (BOS) among Italian psychologists following the first wave of COVID-19 pandemic. As a secondary objective, geographical differences in the prevalence of BOS symptoms were investigated across regional macro-areas in this category of workers. Methods: Using a non-probabilistic convenience sample, four-hundred sixty-eight participants responded to an online survey which included psychometric valid questions from the Maslach Burnout Inventory and The Big Five Inventory-10. Descriptive and inferential statistics were utilized to analyze the data. Results: The overall prevalence of BOS in the study sample was nearly 17%, although no statistically significant differences were noted among volunteer (17.5%) and non-volunteer group (16.2%). Statistically significant differences were found in the Depersonalization (DP) levels. Prevalence of BOS varied across Italian regional macro areas. Neuroticism was positively associated with Emotional Exhaustion (EE) and DP. Agreeableness was negatively associated with EE and DP. Openness was negatively associated with DP. The only personality trait that did not reach any significance level across BOS dimensions was Conscientiousness. Telematic approach was positively associated with DP. Discussion and Conclusions: In Italy, during the COVID-19 pandemic, psychologists who offer services in a volunteering setting are at high risk of developing BOS. Policymakers should develop guidelines for training and prevention programs to contain BOS and preserving the quality of care, through workplace health promotion and occupational health surveillance programs
