    Simulating Dynamical Features of Escape Panic

    One of the most disastrous forms of collective human behaviour is the kind of crowd stampede induced by panic, often leading to fatalities as people are crushed or trampled. Sometimes this behaviour is triggered in life-threatening situations such as fires in crowded buildings; at other times, stampedes can arise from the rush for seats or seemingly without causes. Tragic examples within recent months include the panics in Harare, Zimbabwe, and at the Roskilde rock concert in Denmark. Although engineers are finding ways to alleviate the scale of such disasters, their frequency seems to be increasing with the number and size of mass events. Yet, systematic studies of panic behaviour, and quantitative theories capable of predicting such crowd dynamics, are rare. Here we show that simulations based on a model of pedestrian behaviour can provide valuable insights into the mechanisms of and preconditions for panic and jamming by incoordination. Our results suggest practical ways of minimising the harmful consequences of such events and the existence of an optimal escape strategy, corresponding to a suitable mixture of individualistic and collective behaviour.Comment: For related information see http://angel.elte.hu/~panic, http://www.helbing.org, http://angel.elte.hu/~fij, and http://angel.elte.hu/~vicse

    In Defence of Public Higher Education: Knowledge for a Successful Society (The Alternative White Paper for HE)

    The present Conservative Government, like the Coalition Government that preceded it, has an ideological predisposition towards the market and its supposed benefits to consumers, but appears to have no vision of Higher Education and its benefits to students and to the whole of society. These wider societal benefits can be summarised under three aspects: * educating the next generation of the population * carrying out research to address social and scientific challenges * maintaining an independent platform for research into society and science to facilitate democratic debate. The last of these, sometimes drawn under the umbrella of ‘academic freedom’, is the basis of the historic contract between Universities and the State. We contend, following the UNESCO Recommendation (1997), that academic freedom must be sufficient to guarantee the independence of scientific inquiry, commentary and teaching. Pressures from funding agencies and the state are usually cited as the principal threats to academic independence. The last decade has seen the rise of a third threat, namely an increasing managerial interference in academic life deriving from the introduction of market imperatives. These three societal benefits are interconnected. Without independent research there can be no scientific independence and no cutting-edge teaching. Without a focus on critique and challenge, students may see ‘education’ as a mere process of accumulating ‘facts’ to meet test criteria. A narrow focus on the acquisition of qualifications undermines the education process itself. Employers have criticised graduate recruits for insufficient creativity, of being rote-taught and thus un-adaptable to a modern business subject to rapid technological change. Importantly, critical skills are necessary to meet the challenge of business and for inclusive democratic engagement. The idea of a University that unites these three aspects is undermined by a new model of Higher Education Institution that sees the investment in human capital only as a private benefit. The Government’s White Paper, Success as a Knowledge Economy, and associated legislative programme, consolidate a fee-loan (or debt-finance) model of funding which puts the costs of higher education onto new graduates and future taxpayers, while reducing taxes for current taxpayers – many of whom directly benefit from publicly-supported higher education, or from its wider public benefits. The Government suggests that it is merely replacing direct public funding with one that places the ‘student at the heart of the system’. But it proposes that public funding should be directed towards the realisation of the private benefits of higher education, and it fails to acknowledge the wider public benefits that higher education affords. In truth, the proposals place the market at the heart of the system and subordinate the student as a consumer of higher education, with loans functioning as a voucher to present at a university of choice (providing that the student has the grades required). It is our view that this new funding model is wrong in principle and deficient in practice. The regulatory framework that is being introduced in its wake will undermine the declared aims to improve teaching quality, to enhance social mobility, and to improve access and achievement. The extension of university title to for-profit providers will also threaten the wider public benefits of higher education, by allowing them to compete as single-function institutions, and giving them access to publicly-supported loans for their students without a guarantee of their longer term stability. This will intensify existing competition and encourage a ‘race to the bottom’. Our defence of an alternative vision of Higher Education takes place in the context of a dismal lack of leadership by the various mission groups representing universities in the sector – for example, Universities UK and the Russell Group – and other bodies responsible for the sector. Their willing advocacy of a fee-loan model of funding (to avoid possible cuts) has abdicated their leadership role in a proper debate on the values of public higher education. This failure to defend the values of the very public higher education they are chartered to provide is in marked contrast to representations made by another group. Lobbyists on behalf of for-profit providers are seeking a supposed ‘level-playing field’ in undergraduate degree provision, despite having no track record of success in the UK, a disastrous record in the USA, and no desire to provide any wider public benefit of their existence. The Government’s position is also in marked contrast to public attitudes. The British Social Attitudes Survey (NatCen) has, before and since the introduction of tuition fee changes, regularly asked questions about public attitudes to higher education and inequality. The majority of the population has consistently opposed high levels of student debt, believed that education has a value beyond simply providing the means to a better job, and maintained that inequality in Britain is an obstacle to the fulfillment of opportunities. Perhaps surprisingly, this commitment to the values of publicly-funded higher education is especially marked among those without graduate-level qualifications. Politicians who argue that the latter resent paying taxes to finance education for ‘middle-class people’ should seek evidence for this assertion. But the ‘debate’ among politicians, members of the BIS secretariat and corporate lobbyists over the issue has been remarkable for its superficial, un-evidenced character. It has also been remarkable for the absence of full public debate (Leach 2016). This Alternative White Paper aims to correct this imbalance.     Components of acquisition-to-acquisition variance in continuous arterial spin labelling (CASL) imaging

    <p>Abstract</p> <p>Background</p> <p>Images of perfusion estimates obtained with the continuous arterial spin labelling technique are characterized by variation between single acquisitions. Little is known about the spatial determinants of this variation during the acquisition process and their impact on voxel-by-voxel estimates of effects.</p> <p>Results</p> <p>We show here that the spatial patterns of covariance between voxels arising during the acquisition of these images uncover distinct mechanisms through which this variance arises: through variation in global perfusion levels; through the action of large vessels and other, less well characterized, large anatomical structures; and through the effect of noisy areas such as the edges of the brain.</p> <p>Conclusions</p> <p>Knowledge of these covariance patterns is important to experimenters for a correct interpretation of findings, especially for studies where relatively few acquisitions are made.</p

    Effect of closed endotracheal suction in high-frequency ventilated premature infants measured with electrical impedance tomography

    Objective: To determine the global and regional changes in lung volume during and after closed endotracheal tube (ETT) suction in high-frequency ventilated preterm infants with respiratory distress syndrome (RDS). Design: Prospective observational clinical study. Setting: Neonatal intensive care unit. Patients: Eleven non-muscle relaxed preterm infants with RDS ventilated with open lung high-frequency ventilation (HFV). Interventions: Closed ETT suction. Measurements and results: Changes in global and regional lung volume were measured with electrical impedance tomography. ETT suction resulted in an acute loss of lung volume followed by spontaneous recovery with a median residual loss of 3.3% of the maximum volume loss. The median stabilization time was 8 s. At the regional level, the lung volume changes during and after ETT suction were heterogeneous in nature. Conclusions: Closed ETT suction causes an acute, transient and heterogeneous loss of lung volume in premature infants with RDS treated with open lung HFV

    Evaluation of a joint Bioinformatics and Medical Informatics international course in Peru

    Background: New technologies that emerge at the interface of computational and biomedical science could drive new advances in global health, therefore more training in technology is needed among health care workers. To assess the potential for informatics training using an approach designed to foster interaction at this interface, the University of Washington and the Universidad Peruana Cayetano Heredia developed and assessed a one-week course that included a new Bioinformatics (BIO) track along with an established Medical/Public Health Informatics track (MI) for participants in Peru. Methods: We assessed the background of the participants, and measured the knowledge gained by track-specific (MI or BIO) 30-minute pre- and post-tests. Participants' attitudes were evaluated both by daily evaluations and by an end-course evaluation. Results: Forty-three participants enrolled in the course - 20 in the MI track and 23 in the BIO track. Of 20 questions, the mean % score for the MI track increased from 49.7 pre-test (standard deviation or SD = 17.0) to 59.7 (SD = 15.2) for the post-test (P = 0.002, n = 18). The BIO track mean score increased from 33.6 pre-test to 51.2 post-test (P less than 0.001, n = 21). Most comments (76%) about any aspect of the course were positive. The main perceived strength of the course was the quality of the speakers, and the main perceived weakness was the short duration of the course. Overall, the course acceptability was very good to excellent with a rating of 4.1 (scale 1-5), and the usefulness of the course was rated as very good. Most participants (62.9%) expressed a positive opinion about having had the BIO and MI tracks come together for some of the lectures.     Genomic Prevalence of Heterochromatic H3K9me2 and Transcription Do Not Discriminate Pluripotent from Terminally Differentiated Cells

    Cellular differentiation entails reprogramming of the transcriptome from a pluripotent to a unipotent fate. This process was suggested to coincide with a global increase of repressive heterochromatin, which results in a reduction of transcriptional plasticity and potential. Here we report the dynamics of the transcriptome and an abundant heterochromatic histone modification, dimethylation of histone H3 at lysine 9 (H3K9me2), during neuronal differentiation of embryonic stem cells. In contrast to the prevailing model, we find H3K9me2 to occupy over 50% of chromosomal regions already in stem cells. Marked are most genomic regions that are devoid of transcription and a subgroup of histone modifications. Importantly, no global increase occurs during differentiation, but discrete local changes of H3K9me2 particularly at genic regions can be detected. Mirroring the cell fate change, many genes show altered expression upon differentiation. Quantitative sequencing of transcripts demonstrates however that the total number of active genes is equal between stem cells and several tested differentiated cell types. Together, these findings reveal high prevalence of a heterochromatic mark in stem cells and challenge the model of low abundance of epigenetic repression and resulting global basal level transcription in stem cells. This suggests that cellular differentiation entails local rather than global changes in epigenetic repression and transcriptional activity

    HIF2α reduces growth rate but promotes angiogenesis in a mouse model of neuroblastoma

    <p>Abstract</p> <p>Background</p> <p>HIF2α/EPAS1 is a hypoxia-inducible transcription factor involved in catecholamine homeostasis, vascular remodelling, physiological angiogenesis and adipogenesis. It is overexpressed in many cancerous tissues, but its exact role in tumour progression remains to be clarified.</p> <p>Methods</p> <p>In order to better establish its function in tumourigenesis and tumour angiogenesis, we have stably transfected mouse neuroblastoma N1E-115 cells with the native form of HIF2α or with its dominant negative mutant, HIF2α (1–485) and studied their phenotype <it>in vitro </it>and <it>in vivo</it>.</p> <p>Results</p> <p><it>In vitro </it>studies reveal that HIF2α induces neuroblastoma cells hypertrophy and decreases their proliferation rate, while its inactivation by the HIF2α (1–485) mutant leads to a reduced cell size, associated with an accelerated proliferation. However, our <it>in vivo </it>experiments show that subcutaneous injection of cells overexpressing HIF2α into syngenic mice, leads to the formation of tumour nodules that grow slower than controls, but that are well structured and highly vascularized. In contrast, HIF2α (1–485)-expressing neuroblastomas grow fast, but are poorly vascularized and quickly tend to extended necrosis.</p> <p>Conclusion</p> <p>Together, our data reveal an unexpected combination between an antiproliferative and a pro-angiogenic function of HIF2α that actually seems to be favourable to the establishment of neuroblastomas <it>in vivo</it>.</p

    Newly uncovered physics of MHD instabilities using 2-D electron cyclotron emission imaging system in toroidal plasmas

    Validation of physics models using the newly uncovered physics with a 2-D electron cyclotron emission imaging (ECEi) system for magnetic fusion plasmas has either enhanced the confidence or substantially improved the modeling capability. The discarded &quot;full reconnection model&quot; in sawtooth instability is vindicated and established that symmetry and magnetic shear of the 1/1 kink mode are critical parameters in sawtooth instability. For the 2/1 instability, it is demonstrated that the 2-D data can determine critical physics parameters with a high confidence and the measured anisotropic distribution of the turbulence and its flow in presence of the 2/1 island is validated by the modelled potential and gyro-kinetic calculation. The validation process of the measured reversed-shear Alfveneigenmode (RSAE) structures has improved deficiencies of prior models. The 2-D images of internal structure of the ELMs and turbulence induced by the resonant magnetic perturbation (RMP) have provided an opportunity to establish firm physics basis of the ELM instability and role of RMPs. The importance of symmetry in determining the reconnection time scale and role of magnetic shear of the 1/1 kink mode in sawtooth instability may be relevant to the underlying physics of the violent kink instability of the filament ropes in a solar flare

    Coronary arterial fistulas

    ABSTRACT: A coronary arterial fistula is a connection between one or more of the coronary arteries and a cardiac chamber or great vessel. This is a rare defect and usually occurs in isolation. Its exact incidence is unknown. The majority of these fistulas are congenital in origin although they may occasionally be detected after cardiac surgery. They do not usually cause symptoms or complications in the first two decades, especially when small. After this age, the frequency of both symptoms and complications increases. Complications include 'steal' from the adjacent myocardium, thrombosis and embolism, cardiac failure, atrial fibrillation, rupture, endocarditis/endarteritis and arrhythmias. Thrombosis within the fistula is rare but may cause acute myocardial infarction, paroxysmal atrial fibrillation and ventricular arrhythmias. Spontaneous rupture of the aneurysmal fistula causing haemopericardium has also been reported. The main differential diagnosis is patent arterial duct, although other congenital arteriovenous shunts need to be excluded. Whilst two-dimensional echocardiography helps to differentiate between the different shunts, coronary angiography is the main diagnostic tool for the delineation of the anatomy. Surgery was the traditional method of treatment but nowadays catheter closure is recommended using a variety of closure devices, such as coils, or other devices.
