    Оптимизация антибактериальной терапии у пациентов с эндо-пародонтальными поражениями

    To improve the quality and effectiveness of medical care for patients with inflammatory periodontal diseases, it is necessary to search for new approaches in both diagnosis and treatment. The aim of our study is to determine the effect of the diode laser on the pathogenic microflora of periodontal pockets and root canals in patients with endo-periodontal lesions (EPL). We carried out a comparative assessment of the bacterial contents of the root canals and periodontal pockets by polymerase chain reaction (PCR) before and after treatment with a diode laser (Doctor Smile Simpler, wavelength 980 nm) for 54 patients with EPL. The control group consisted of 56 patients who were treated according to the generally accepted method, including professional oral hygiene, endodontic treatment, and curettage of periodontal pockets. Evaluation criteria were a qualitative assessment of the content of periodontopathogens in the root canal and periodontal pocket before and after and treatment. As a result of our study, we found a statistically significant (p<0.01) reduction in the colonization of periodontal pockets and root canals in patients with EPL after laser decontamination. The high technological effectiveness of the methods used in combination with the low risk of complications at the stages of endodontic and periodontological treatment provides an antibacterial effect and reduces the duration of inflammation. The results of treatment of patients using a diode laser give reason to recommend their use in the dental practice.Для повышения качества и эффективности оказания медицинской помощи пациентам с воспалительными заболеваниями пародонта необходим поиск новых подходов к диагностике и лечению патологии. Целью исследования было изучить влияние излучения диодного лазера на патогенную микрофлору пародонтальных карманов (ПК) и корневых каналов (КК) у пациентов с эндо-пародонтальными поражениями (ЭПП). Проведена сравнительная оценка бактериального содержимого КК и ПК методом полимеразно-цепной реакции (ПЦР) до и после лечения с применением стоматологического диодного лазера (Doctor Smile Simpler, длина волны 980 нм) у 54 пациентов с ЭПП. Группу контроля составили 56 пациентов, которым проводилось лечение по общепринятой методике, включающей профессиональную гигиену рта, эндодонтическое лечение и кюретаж ПК. Критерием оценки явилось качественное содержание пародонтопатогенов в КК и ПК до и после лечения. Выявлено статистически значимое (p<0,01) снижение обсемененности ПК и КК у пациентов с ЭПП после лазерной деконтаминации. Высокая технологичность использованных методик в сочетании с низкими рисками осложнений на этапах эндодонтического и пародонтологического лечения обеспечивает антибактериальный эффект и сокращает продолжительность воспалительных явлений. Результаты лечения пациентов с применением диодного лазера дают основание рекомендовать их использование в практике врача-стоматолога

    Studying abrasive wear of materials for direct and indirect restorations with increased dental abrasion

    Strength characteristics of restoration materials, namely their resistance to abrasive wear, are of great importance in choosing a treatment method for increased dental abrasion. The purpose of this study was to research the abrasive wear of materials for direct and indirect dental restorations in patients with increased dental abrasion by measuring areas of the teeth occlusion using computer occlusiograms. Patients who underwent direct restorations with nanocomposite and indirect restorations with lithium disilicate ceramic restorations as a treatment for increased dental abrasion were underwent occlusiography by using TScan III (Tekscan, USA) 1 month and 24 months after the treatment was completed, to measure the area of teeth occlusion in the IP position (a multi-point occlusal contact position). The degree of dental abrasion of restoration materials provided judging on changing the area of occlusal contacts in the same patient over time and in compared with the area of occlusal contacts in the control group. The results of the study shown that the resistance to abrasive wear of ceramic restorations is in consistent with a tooth enamel, and nanocomposite materials, despite their durability, are prone to abrasion as well. Therefore, indirect ceramic restorations should be in preference in treatment of increased dental abrasion, in which an increased occlusive load is one of the clinical manifestations

    Evaluation of medical records maintenance quality in dental medical organizations of Kazan

    Aim. Improvement of the quality control of dental medical organizations when diagnosing dentoalveolar anomalies and deformities. Methods. A retrospective analysis of an orthodontic patient’s medical record maintenance at the dental clinics of Kazan. Results. Only 30.7% of all the verified documentation were established to be most thoroughly and correctly filled. Information about the obligatory medical insurance policy was indicated only in 2 cards out of all the verified documentation. In all orthodontic patient medical records the fringe benefit category code was absent. The name of the insurance organization in which a patient was unsured, was available only in 2.3% of the cards. Patient’s passport data were available only in 14 (15.9%) cards, and the type of payment for services was registered only in 38 (43.2%). The diagnosis established by a referring medical organization was indicated only in 7 (8%) of the records. When examining the dental rows of patients, their dimensions and apical bases of the jaws, the shape of the dentition were indicated in 40.9% of the documentation. The contact of adjacent teeth was described in 36.4% of all cases, presence of diastema between the upper jaw incisors was represented in 30.7% of the medical charts of orthodontic patients. The diastema between the lower jaw incisors was reflected in 25 (28.4%) medical charts of orthodontic patients. Based on the doctor’s notes in the medical records, it is difficult to understand whether the whole necessary list of diagnostic measures had been performed to the patient in order to confirm this or that orthodontic pathology. Such filling of the medical chart of orthodontic patient at times makes the experts doubt in the correctness of diagnosis of a certain patient. Conclusion. Doctors-orthodontists do not pay enough attention to the collection of anamnesis of the disease, evaluation of the nature of complaints, as well as use of basic and additional methods of patient examination

    A Comparative Cross-Sectional Study Assessing the Psycho-Emotional State of Intensive Care Units’ Physicians and Nurses of COVID-19 Hospitals of a Russian Metropolis

    Working in intensive care units (ICUs) is stressful and potentially leads to various psychoemotional disorders. Today, this issue represents a serious concern to the healthcare sector and affects the quality of healthcare provided. This study aimed to assess and compare the psycho-emotional state in COVID-19 and non-COVID-19 hospitals’ ICU healthcare workers (HCWs). From January to July 2021, we conducted an anonymous cross-sectional web survey of ICU physicians and nurses (N = 1259) of various hospitals in a metropolis with a population of over 10 million people. The statistical distributions of non-COVID-19 ICU HCWs showed the following results: Emotional exhaustion levels (low 14.6%, average 30.8%, and high 54.6%); depersonalization levels (low 11.6%, average 16.5%, and high 71.9%); and reduced personal accomplishment levels (low 23.5%, average 40.3%, and high 36.2%). The statistical distributions of COVID-19 ICU HCWs showed the following results: Emotional exhaustion levels (low 16.5%, average 31.5%, and high 52%); depersonalization levels (low 7.4%, average 9.4%, and high 83.1%); and reduced personal accomplishment levels (low 25.4%, average 45.4%, and high 29.1%). This study found a strong correlation between emotional exhaustion, aggression, and depersonalization in non-COVID-19 ICU HCWs and also found a correlation between their age, aggression, emotional exhaustion, and occupational stress. © 2022 by the authors.
