Infected Pseudoaneurysm of the Superficial Femoral Artery in a Patient with Salmonella enteritidis
Mycotic aneurysms, defined as irreversible dilation of an artery due to destruction of the vessel wall by infection, are rare but are associated with a high risk of rupture if not treated promptly. The case of a healthy 52-year-old smoker who presented with pyrexia, rigors, night sweats and severe right leg pain with swelling is presented. He was diagnosed with a superficial femoral artery mycotic aneurysm, with Salmonella enteritidis as the causative agent. He was treated with high-dose antibiotics, local debridement and autologous reconstruction. A high index of suspicion is needed to make the correct diagnosis in these cases. Prompt surgical intervention and antimicrobial therapy are the cornerstones of treatment to reduce the associated high morbidity and mortality
Continuous regional anaesthesia provides effective pain management and reduces opioid requirement following major lower limb amputation
Objectives: Postoperative stump pain after major lower limb amputation is a significant impediment to the recovery of amputees. The vast majority of patients require opioid analgesics following surgery, which are associated with opioid-related side-effects. Here, we investigate whether intraoperative placement of a peripheral nerve stump catheter followed by continuous infusion of local anesthetic is as effective at pain control as current analgesic practices. If beneficial, this procedure could potentially reduce post-amputation opioid consumption and opioid-related adverse effects.
Methods: A retrospective chart review was conducted of 198 patients over a 4-year period who had undergone a major lower limb amputation for indications related to peripheral vascular disease. Postoperatively, 102 patients received a perineural catheter were compared to 96 patients who did not. The primary outcomes of this study were the amount of morphine equivalents used in the first 72 hours postoperatively and postoperative pain intensity in the first 24 hours.
Results: A total of 198 lower-limb amputations were selected for analyses. Multiple regression analyses indicated that perineural catheter use was associated with a lower cumulative postoperative opioid consumption over the first 72 hours but not postoperative pain scores at 24 hours. Perineural catheter use led to a 40% reduction in opioid use during the first 72 hours postoperatively. Mixed model repeated measures analysis demonstrated that this opioid reduction was consistent over time. Other variables related to total opioid use included age, presurgical chronic pain, pre-surgical opioid use, patient-controlled analgesia.
Conclusions: Continuous perineural infusions of local anesthetic are a safe and effective method for reducing post-amputation opioid analgesic medications after major lower limp amputation.This study was supported by Canadian Society for Vascular Surgery 2013 National Student Research Award
Cyclooxygenase-2 inhibition: effects on tumour growth, cell cycling and lymphangiogenesis in a xenograft model of breast cancer
Cyclooxygenase-2 (COX-2) is associated with poor-prognosis breast cancer. We used a nude mouse xenograft model to determine the effects of COX-2 inhibition in breast cancer. Oestrogen receptor (ER)-positive MCF7/HER2-18 and ER-negative MDAMB231 breast cancer cell lines were injected into nude mice and allowed to form tumours. Mice then received either chow containing Celecoxib (a COX-2 inhibitor) or control and tumour growth measured. Tumour proliferation, apoptosis, COX-2, lymphangiogenesis and angiogenesis were assessed by immunohistochemistry (IHC), Western blotting or Q-PCR. Celecoxib inhibited median tumour growth in MCF7/HER2-18 (58.7%, P=0.029) and MDAMB231 (46.3%, P=0.0002) cell lines compared to control. Cyclooxygenase-2 expression decreased following Celecoxib treatment (MCF7/HER2-18 median control 65.3% vs treated 22.5%, P=0.0001). Celecoxib increased apoptosis in MCF7/HER2-18 tumours (TUNEL 0.52% control vs 0.73% treated, P=0.0004) via inactivation of AKT (median pAKTser473 57.3% control vs 35.5% treated, P=0.0001 – confirmed at Western blotting). Q-PCR demonstrated decreased podoplanin RNA (lymphangiogenesis marker) in the MCF7/HER2-18 – median 2.9 copies treated vs 66.6 control (P=0.05) and MDAMB231-treated groups – median 160.7 copies vs 0.05 control copies (P=0.015), confirmed at IHC. Cyclooxygenase-2 is associated with high levels of activated AKTser473 and lymphangiogenesis in breast cancer. Cyclooxygenase-2 inhibition decreases tumour growth, and may potentially decrease recurrence, by inactivating AKT and decreasing lymphangiogenesis
J/Psi Production in Au-Au Collisions at sqrt(s_NN) = 200 GeV at the Relativistic Heavy Ion Collider
First results on charm quarkonia production in heavy ion collisions at the
Relativistic Heavy Ion Collider (RHIC) are presented. The yield of J/Psi's
measured in the PHENIX experiment via electron-positron decay pairs at
mid-rapidity for Au-Au reactions at sqrt(s_NN) = 200 GeV are analyzed as a
function of collision centrality. For this analysis we have studied 49.3
million minimum bias Au-Au reactions. We present the J/Psi invariant yield
dN/dy for peripheral and mid-central reactions. For the most central collisions
where we observe no signal above background, we quote 90% confidence level
upper limits. We compare these results with our J/Psi measurement from
proton-proton reactions at the same energy. We find that our measurements are
not consistent with models that predict strong enhancement relative to binary

Scaling properties of proton and anti-proton production in sqrt(s_NN) = 200 GeV Au + Au collisions
We report on the yield of protons and anti-protons, as a function of
centrality and transverse momentum, in Au+Au collisions at sqrt(s_NN) = 200 GeV
measured at mid-rapidity by the PHENIX experiment at RHIC. In central
collisions at intermediate transverse momenta (1.5 < p_T < 4.5 GeV/c) a
significant fraction of all produced particles are protons and anti-protons.
They show a centrality-scaling behavior different from that of pions. The
p-bar/pion and p/pion ratios are enhanced compared to peripheral Au+Au, p+p,
and electron+positron collisions. This enhancement is limited to p_T < 5 GeV/c
as deduced from the ratio of charged hadrons to pi^0 measured in the range 1.5

Suppressed pi^0 Production at Large Transverse Momentum in Central Au+Au Collisions at sqrt(s_NN) = 200 GeV
Transverse momentum spectra of neutral pions in the range 1 < p_T < 10 GeV/c
have been measured at mid-rapidity by the PHENIX experiment at RHIC in Au+Au
collisions at sqrt(s_NN) = 200 GeV. The pi^0 multiplicity in central reactions
is significantly below the yields measured at the same sqrt(s_NN) in peripheral
Au+Au and p+p reactions scaled by the number of nucleon-nucleon collisions. For
the most central bin, the suppression factor is ~2.5 at p_T = 2 GeV/c and
increases to ~4-5 at p_T ~= 4 GeV/c. At larger p_T, the suppression remains
constant within errors. The deficit is already apparent in semi-peripheral

Deuteron and antideuteron production in Au+Au collisions at sqrt(s_NN)=200 GeV
The production of deuterons and antideuterons in the transverse momentum
range 1.1 < p_T < 4.3 GeV/c at mid-rapidity in Au + Au collisions at
sqrt(s_NN)=200 GeV has been studied by the PHENIX experiment at RHIC. A
coalescence analysis comparing the deuteron and antideuteron spectra with those
of protons and antiprotons, has been performed. The coalescence probability is
equal for both deuterons and antideuterons and increases as a function of p_T,
which is consistent with an expanding collision zone. Comparing (anti)proton
yields p_bar/p = 0.73 +/- 0.01, with (anti)deuteron yields: d_bar/d = 0.47 +/-

Mid-Rapidity Direct-Photon Production in p+p Collisions at sqrt(s) = 200 GeV
A measurement of direct photons in p+p collisions at sqrt(s)=200 GeV is
presented. A photon excess above background from pi^0 --> gamma+gamma, eta -->
gamma+gamma, and other decays is observed in the transverse momentum range 5.5
< p_T < 7 GeV/c. The result is compared to a next-to-leading-order perturbative
QCD calculation. Within errors, good agreement is found between the QCD

Centrality dependence of charged hadron production in deuteron+gold and nucleon+gold collisions at sqrt(s_NN)=200 GeV
We present transverse momentum (p_T) spectra of charged hadrons measured in
deuteron-gold and nucleon-gold collisions at \sqrts = 200 GeV for four
centrality classes. Nucleon-gold collisions were selected by tagging events in
which a spectator nucleon was observed in one of two forward rapidity
detectors. The spectra and yields were investigated as a function of the number
of binary nucleon-nucleon collisions, \nu, suffered by deuteron nucleons. A
comparison of charged particle yields to those in p+p collisions show that the
yield per nucleon-nucleon collision saturates with \nu for high momentum
particles. We also present the charged hadron to neutral pion ratios as a
