    Accessible lifelong learning at higher education:outcomes and lessons Learned at two different PilotSites in the EU4ALL Project

    [EN] The EU4ALL project (IST-FP6-034778) has developed a general framework to address the needs of accessible lifelong learning at Higher Education level consisting of several standards-based interoperable components integrated into an open web service architecture aimed at supporting adapted interaction to guarantee students' accessibility needs. Its flexibility has supported the project implementation at several sites with different settings and various learning management systems. Large-scale evaluations involving hundreds of users, considering diverse disability types, and key staff roles have allowed obtaining valuable lessons with respect to "how to adopt or enhance eLearning accessibility" at university. The project was evaluated at four higher education institutions, two of the largest in Europe and two mediumsized. In this paper, we focus on describing the implementation and main conclusions at the largest project evaluation site (UNED), which was involved in the project from the beginning, and thus, in the design process, and a medium-sized university that adopted the EU4ALL approach (UPV). This implies dealing with two well-known open source learning environments (i.e. dotLRN and Sakai), and considering a wide variety of stakeholders and requirements.     Single-cell analysis of CD4+ T-cell differentiation reveals three major cell states and progressive acceleration of proliferation

    Background: Differentiation of lymphocytes is frequently accompanied by cell cycle changes, interplay that is of central importance for immunity but is still incompletely understood. Here, we interrogate and quantitatively model how proliferation is linked to differentiation in CD4+ T cells. Results: We perform ex vivo single-cell RNA-sequencing of CD4+ T cells during a mouse model of infection that elicits a type 2 immune response and infer that the differentiated, cytokine-producing cells cycle faster than early activated precursor cells. To dissect this phenomenon quantitatively, we determine expression profiles across consecutive generations of differentiated and undifferentiated cells during Th2 polarization in vitro. We predict three discrete cell states, which we verify by single-cell quantitative PCR. Based on these three states, we extract rates of death, division and differentiation with a branching state Markov model to describe the cell population dynamics. From this multi-scale modelling, we infer a significant acceleration in proliferation from the intermediate activated cell state to the mature cytokine-secreting effector state. We confirm this acceleration both by live imaging of single Th2 cells and in an ex vivo Th1 malaria model by single-cell RNA-sequencing. Conclusion: The link between cytokine secretion and proliferation rate holds both in Th1 and Th2 cells in vivo and in vitro, indicating that this is likely a general phenomenon in adaptive immunity

    Il libro, che pubblica gli atti del convegno internazionale, svoltosi a Roma nel maggio del 2007 nell’ambito della ricerca PRIN-2005 (UniversitĂ  di Roma La Sapienza, Trento, Bari e Urbino), analizza il periodo della cultura tedesca della fine-secolo con particolare attenzione ai personaggi e ai movimenti che si sono collocati negli “spazi intermedi”, intesi non solo e non tanto come zone di confine in senso territoriale, quanto piuttosto come zone di interscambio culturale e disciplinare, come tentativi di “superamento” degli ordini mentali e culturali costituiti. L’ambito cronologico che il gruppo di ricerca prende in esame va da 1888 al 1933. La periodizzazione proposta vuole rinviare a quella serie di movimenti e di pensatori che hanno dato il via alla messa in discussione di quei valori costituiti, dalla cui frammentazione Ăš nata la cultura del “secolo breve”. In questo periodo, a partire dalla Jahrhundertwende, sono state elaborate e sperimentate nella prassi letteraria e artistica, una serie di teorie, una serie di “visioni del mondo” che implicavano tanto una pratica produttiva quanto un ruolo dell’artista e dell’arte quanto una visione piĂč generale dei cambiamenti epocali e dell’epoca moderna. Tali teorie e pratiche artistiche, di vario genere e di vario livello, costituiscono un patrimonio concettuale perchĂ© in esse si trovano sperimentate e a volte anticipate molte questioni che oggi sono di estrema attualitĂ : dal rapporto uomo-macchina al rapporto parola-immagine, dalla definizione degli spazi urbani alla questione della guerra, dalla ricerca della identitĂ  monoculturale alla presa d’atto dell’esistenza dell’estraneo. Queste varie problematiche sono state espresse in termini linguistici nuovi giacchĂ© gli autori si sono posti – teoricamente e praticamente – il problema del linguaggio artistico e ne hanno modificato (a volte radicalmente) i codici espressivi. Il libro di divide in tre parti: I: Per una definizione della Klassische Moderne che presenta l’ampia e animata discussione sulla stessa definizione teorica del movimento artistico-culturale, sia sui termini cronologici di questa nuova periodizzazione. Questa parte raccoglie i contributi di Aldo Venturelli, Helmut Kiesel e Fabrizio Cambi. II: La rivoluzione delle forme, che analizza la produzione delle avanguardie (sia pure definite “marginali” ) e prende in considerazione anche aspetti della produzione musicale, nonchĂ© forme di narratologia e nuove forme di scrittura. Questa seconda parte pubblica i saggi di Mauro Ponzi, Rosmarie Keller, Silvio Vietta, Hans Dieter Zimmermann, Elio Matassi e Sabine Meine. III: La fucina dei nuovi linguaggi che analizza l’innovazione dei linguaggi artistici sull’esempio di autori meno radicali che quelli delle avanguardie storiche. Questi autori, tipici esponenti della Klassische Moderne, hanno cercato di coniugare l’innovazione dei linguaggi con il recupero di alcuni aspetti della tradizione.     Introduction: With the expansion of available Information and Communication Technology (ICT) services, a plethora of data sources provide structured and unstructured data used to detect certain health conditions or indicators of disease. Data is spread across various settings, stored and managed in different systems. Due to the lack of technology interoperability and the large amounts of health-related data, data exploitation has not reached its full potential yet. Aim: The aim of the CrowdHEALTH approach, is to introduce a new paradigm of Holistic Health Records (HHRs) that include all health determinants defining health status by using big data management mechanisms. Methods: HHRs are transformed into HHRs clusters capturing the clinical, social and human context with the aim to benefit from the collective knowledge. The presented approach integrates big data technologies, providing Data as a Service (DaaS) to healthcare professionals and policy makers towards a "health in all policies" approach. A toolkit, on top of the DaaS, providing mechanisms for causal and risk analysis, and for the compilation of predictions is developed. Results: CrowdHEALTH platform is based on three main pillars: Data & structures, Health analytics, and Policies. Conclusions: A holistic approach for capturing all health determinants in the proposed HHRs, while creating clusters of them to exploit collective knowledge with the aim of the provision of insight for different population segments according to different factors (e.g. location, occupation, medication status, emerging risks, etc) was presented. The aforementioned approach is under evaluation through different scenarios with heterogeneous data from multiple sources

    Today's rich digital information environment is characterized by the multitude of data sources providing information that has not yet reached its full potential in eHealth. The aim of the presented approach, namely CrowdHEALTH, is to introduce a new paradigm of Holistic Health Records (HHRs) that include all health determinants. HHRs are transformed into HHRs clusters capturing the clinical, social and human context of population segments and as a result collective knowledge for different factors. The proposed approach also seamlessly integrates big data technologies across the complete data path, providing of Data as a Service (DaaS) to the health ecosystem stakeholders, as well as to policy makers towards a "health in all policies" approach. Cross-domain co-creation of policies is feasible through a rich toolkit, being provided on top of the DaaS, incorporating mechanisms for causal and risk analysis, and for the compilation of predictions

    Differentiation of naĂŻve CD4+ T cells into functionally distinct T helper subsets is crucial for the orchestration of immune responses. Due to extensive heterogeneity and multiple overlapping transcriptional programs in differentiating T cell populations, this process has remained a challenge for systematic dissection in vivo. By using single-cell transcriptomics and computational analysis using a temporal mixtures of Gaussian processes model, termed GPfates, we reconstructed the developmental trajectories of Th1 and Tfh cells during blood-stage Plasmodium infection in mice. By tracking clonality using endogenous TCR sequences, we first demonstrated that Th1/Tfh bifurcation had occurred at both population and single-clone levels. Next, we identified genes whose expression was associated with Th1 or Tfh fates, and demonstrated a T-cell intrinsic role for Galectin-1 in supporting a Th1 differentiation. We also revealed the close molecular relationship between Th1 and IL-10-producing Tr1 cells in this infection. Th1 and Tfh fates emerged from a highly proliferative precursor that upregulated aerobic glycolysis and accelerated cell cycling as cytokine expression began. Dynamic gene expression of chemokine receptors around bifurcation predicted roles for cell-cell in driving Th1/Tfh fates. In particular, we found that precursor Th cells were coached towards a Th1 but not a Tfh fate by inflammatory monocytes. Thus, by integrating genomic and computational approaches, our study has provided two unique resources, a database www.PlasmoTH.org, which facilitates discovery of novel factors controlling Th1/Tfh fate commitment, and more generally, GPfates, a modelling framework for characterizing cell differentiation towards multiple fates

    Obliteration of radical cavities with autogenous cortical bone; long-term results

    <p>Abstract</p> <p>Background</p> <p>To evaluate the long-term surgical outcome(s) in patients who have undergone canal-wall-down operation with mastoid and epitympanic obliteration using autologous cortical bone chips, bone pate and meatally-based musculoperiosteal flap technique.</p> <p>Method</p> <p>Retrospective evaluation of seventy patients operated during 1986–1991 due to a cholesteatoma. An otomicroscopy was performed to evaluate the postoperative outer ear canal configuration with a modified Likert scale (1 – 4). The outer ear canal physical volume was assessed by tympanometry. The hearing outcome and a patient-filled questionnaire were also analyzed.</p> <p>Results</p> <p>The posterior wall results were 1.8 (± 0.9 SD) and the attic region 1.8 (± 0.9 SD) (ns., p > 0.05). These values show either no cavity formation or minor formation of a cavity, with a good functional result. The mean volume of the operated ear canal was 1.7 (± 0.5 SD) ml. The volume of the contralateral ear canal was 1.2 (± 0.3 SD) ml (*** p < 0.0001). A comparison of the current mean ABG to the preoperative mean ABG and to the ABG at one-year postoperatively, 5-years postoperatively or 10-years postoperatively showed no statistical significance (p > 0.05).</p> <p>Conclusion</p> <p>ABG does not significantly change in the long-term. The configuration of the cavity tends to change, however, the obliteration material is stable in the long-term and clinically significant cavitation rarely occurs.</p

    Dendritic cells license regulatory B cells to produce IL-10 and mediate suppression of antigen-specific CD8 T cells

    Regulatory B cells (Bregs) suppress and reduce autoimmune pathology. However, given the variety of Breg subsets, the role of Bregs in the pathogenesis of type 1 diabetes is still unclear. Here, we dissect this fundamental mechanism. We show that natural protection from type 1 diabetes in nonobese diabetic (NOD) mice is associated with increased numbers of IL-10-producing B cells, while development of type 1 diabetes in NOD mice occurs in animals with compromised IL-10 production by B cells. However, B cells from diabetic mice regain IL-10 function if activated by the innate immune receptor TLR4 and can suppress insulin-specific CD8 T cells in a dendritic cell (DC)-dependent, IL-10-mediated fashion. Suppression of CD8 T cells is reliant on B-cell contact with DCs. This cell contact results in deactivation of DCs, inducing a tolerogenic state, which in turn can regulate pathogenic CD8 T cells. Our findings emphasize the importance of DC–Breg interactions during the development of type 1 diabetes
