Semiparametric finite mixture of regression models with Bayesian P-splines
Mixture models provide a useful tool to account for unobserved heterogeneity and are at the basis of many model-based clustering methods. To gain additional flexibility, some model parameters can be expressed as functions of concomitant covariates. In this Paper, a semiparametric finite mixture of regression models is defined, with concomitant information assumed to influence both the component weights and the conditional means. In particular, linear predictors are replaced with smooth functions of the covariate considered by resorting to cubic splines. An estimation procedure within the Bayesian paradigm is suggested, where smoothness of the covariate effects is controlled by suitable choices for the prior distributions of the spline coefficients. A data augmentation scheme based on difference random utility models is exploited to describe the mixture weights as functions of the covariate. The performance of the proposed methodology is investigated via simulation experiments and two real-world datasets, one about baseball salaries and the other concerning nitrogen oxide in engine exhaust
Aetiologic diagnosis of hearing loss in children identified through newborn hearing screening testing
With the implementation of universal newborn hearing screening (UNHS) programmes and early diagnosis and treatment of hearing problems, the need has clearly emerged to implement and carry out a systematic and coordinated protocol for the aetiological diagnosis of permanent hearing impairment (PHI). Within the framework of the Italian Ministry of Health project CCM 2013 “Preventing Communication Disorders: a Regional Program for early Identification, Intervention and Care of Hearing Impaired Children”, it has been decided to consider the problems relative to aetiological diagnosis of child PHI within UNHS programmes. The specific objective was to apply a shared diagnostic protocol that can identify the cause in at least 70% of cases of PHI. For this part of the project, four main recommendations were identified that can be useful for an efficient aetiological diagnosis in children affected by PHI and that can offer valid suggestions to optimise resources and produce positive changes for third-level audiologic centres
Systematic review of the scientific literature on the economic evaluation of cochlear implants in adult patients
A systematic review of the economic literature of cochlear implants (CI) was conducted with the aim of summarizing the results of studies on the cost effectiveness of monolateral and bilateral (sequential/simultaneous) CI in adult patients affected by severe to profound prelingual and postlingual hearing impairment. The literature search was performed using “PubMed MEDLINE” and the Centre for Reviews and Dissemination search engines. Inclusion criteria related to economic evaluation included primary studies published in English language from January 2000 to May 2010 and aimed to quantify costs of CI and compare monolateral CI vs. acoustic prosthesis and bilateral (sequential/ simultaneous) CI vs. monolateral CI in terms of cost per unit of effectiveness. Four articles were identified. The mean direct medical cost of the monolateral CI varied from € 30,026 to € 45,770 in postlingually deafened patients, and the cost of device represented the main cost component. Additional median costs of simultaneous and sequential bilateral CI were, respectively, € 21,831 and € 25,459. The mean direct medical cost of monolateral CI was € 31,942 in prelingually deafened patients. The monolateral CI in postlingually deafened patients represented a cost effective intervention as compared with no implant (€ /QALY varied from € 7,930, € 24,983 to € 33,094). Monolateral CI were not a cost effective intervention for traditional patients with more than 40 years of hearing impairment (€ 64,604/QALY ) or for patients with marginal benefits from using acoustic prosthesis with more than 30 years of hearing impairment (€ 106,267/QALY ). The cost effectiveness of monolateral CI worsened with increasing age (€ /QALY from € 23,439 for patients < 30 years old to € 55,369 for patients > 70 years). Bilateral CI in postlingually deafened patients were less cost effective than monolateral CI (from € 91,943/QALY to € 102,640/QALY ). Monolateral CI were cost effective in prelingually deafened patients (€ /QALY : € 8,096). Given the few economic evaluation studies in literature, future researches are needed to support the cost effectiveness results of CI in adults and to evaluate the cost effectiveness of bilateral CI, as well as to estimate the non-medical direct and indirect cost components
Sub-Nyquist Field Trial Using Time Frequency Packed DP-QPSK Super-Channel Within Fixed ITU-T Grid
Sub-Nyquist time frequency packing technique was demonstrated for the first
time in a super channel field trial transmission over long-haul distances. The
technique allows a limited spectral occupancy even with low order modulation
formats. The transmission was successfully performed on a deployed Australian
link between Sydney and Melbourne which included 995 km of uncompensated SMF
with coexistent traffic. 40 and 100 Gb/s co-propagating channels were
transmitted together with the super-channel in a 50 GHz ITU-T grid without
additional penalty. The super-channel consisted of eight sub-channels with
low-level modulation format, i.e. DP-QPSK, guaranteeing better OSNR robustness
and reduced complexity with respect to higher order formats. At the receiver
side, coherent detection was used together with iterative maximum-a-posteriori
(MAP) detection and decoding. A 975 Gb/s DP-QPSK super-channel was successfully
transmitted between Sydney and Melbourne within four 50GHz WSS channels (200
GHz). A maximum potential SE of 5.58 bit/s/Hz was achieved with an OSNR=15.8
dB, comparable to the OSNR of the installed 100 Gb/s channels. The system
reliability was proven through long term measurements. In addition, by closing
the link in a loop back configuration, a potential SE*d product of 9254
bit/s/Hz*km was achieved
Early care in children with permanent hearing impairment
The implementation of regional protocols for newborn hearing screening and early audiologic diagnosis represent the first step of the entire diagnostic, rehabilitative and prosthetic programme for children with permanent hearing impairment. The maximum benefit of early diagnosis can indeed be obtained only by prompt rehabilitation aimed at fostering the child’s communicative, linguistic and cognitive development. Within the framework of the CMM 2013 project of the Ministry of Health entitled “Preventing Communication Disorders: a Regional Program for Early Identification, Intervention and Care of Hearing Impaired Children”, the problems concerning the promotion of the global development of children with PHI throughan early rehabilitation project based on shared knowledge and scientific evidence. In this project, our specific aim was to define the features and modes of access to a precise and specialised rehabilitation project for the small hearing-impaired child within three months from audiologic diagnosis. Three main recommendations relative to assessment and rehabilitation aspects of early care emerged from the study
When multidisciplinary surgical trans-orbital approaches should be considered to reach the skull base
SUMMARY The transorbital approaches are a group of surgical procedures performed passing through the orbital spaces and aimed to reach deeper areas. This kind of surgery has been proved to be safe and effective in the management of selected lesions of the anterior, middle and infratemporal fossa. The aim of the present study is to perform a review of the literature, in order to draw the reader’s attention on the main features of this kind of surgery, focusing on the anatomical background and the surgical setting; we will also summary the current indications and contraindications to this approach and find out the related complications and the possible alternatives. Even if we consider the transorbital approach as a promising route to the skull base, we underline that there is no better approach over another and the choice must always consider several elements. Furthermore, as for every skull base procedure, a multidisciplinary management is strongly advisable
Bone-Anchored Hearing Aids Fitted According to NAL and DSL Procedures in Adults with Mixed Hearing Loss
BACKGROUND: Bone-anchored hearing aids represent a valid alternative for patients with conductive/mixed hearing loss who cannot use hearing aids. To date, these devices have given good audiological results, thanks to various fitting prescription programs (i.e., National Acoustic Laboratories and Desired Sensation Level). The aim of this study is to compare 2 types of fitting algorithms (National Acoustic Laboratories and Desired Sensation Level) implemented for bone-anchored hearing devices. METHODS: We retrospectively enrolled 10 patients followed at our operative unit, suffering from bilateral symmetrical mixed hearing loss and who underwent bone-anchored hearing aid implantation. All patients experienced each prescriptive procedure, National Acoustic Laboratories and Desired Sensation Level, for 7 months (on average), and they were subjected to audiological tests and questionnaires to evaluate the best program. RESULTS: National Acoustic Laboratories and Desired Sensation Level prescriptions yielded similar results. Desired Sensation Level allowed less amplification of the low frequencies than the National Acoustic Laboratories prescription, and these differences were the only statistically significant. Desired Sensation Level allowed better disyllabic word and sentence recognition scores only in quiet and not in noisy conditions. The subjective questionnaires showed similar results. At the end of the trial sessions, more patients (60%) definitively chose the Desired Sensation Level program for their device. These patients were those with a worse hearing threshold. CONCLUSION: The 2 prescriptive programs allowed similar results although patients with a worse threshold seem to prefer the DSL program. This is the first evaluation of the 2 prescriptive programs, National Acoustic Laboratories versus Desired Sensation Level, for bone conduction devices available in the literature. Further studies are needed to confirm this initial finding
Photonic Combinatorial Network for Contention Management in 160 Gb/s Interconnection Networks based on All-Optical 2x2 Switching Elements
A modular photonic interconnection network based on a combination of basic 2Ă—2 all-optical nodes including a photonic combinatorial network for the packet contention management is presented. The proposed architecture is synchronous, can handle optical time division multiplexed (OTDM) packets up to 160 Gb/s, exhibits self-routing capability, and very low switching latency. In such a scenario, OTDM has to be preferred to wavelength division multiplexing (WDM) because in the former case, the instantaneous packet power carries the information related to only one bit, making the signal processing based on instantaneous nonlinear interactions between packets and control signals more efficient. Moreover, OTDM can be used in interconnection networks without caring about the propagation impairments because of the very short length (< 100 m) of the links in these networks. For such short-range networks, the packet synchronization can be solved at the network boundary in the electronic domain without the need of complex optical synchronizers. In this paper, we focus on a photonic combinatorial network able to detect the contentions, and to optically drive the contention resolution block and the switching control block. The implementation of the photonic combinatorial network is based on semiconductor devices, which makes the solution very promising in terms of compactness, stability, and power consumption. This implementation represents the first example of complex photonic combinatorial network for ultrafast digital processing. The network performance has been investigated for bit streams at 10 Gb/s in terms of bit error rate (BER) and contrast ratio. Moreover, the suitability of the 2Ă—2 photonic node architecture exploiting the earlier mentioned combinatorial network has been verified at a bit rate up to 160 Gb/s. In this way, the potential of photonic digital processing for the next generation broad band and flexible interconnection networks has been demonstrated
