    This paper discusses the expansion of Oceania through a Marshallese indigenous lens as a focal point. It explains that decolonizing methodologies allows reclaiming of space for mental liberation and reassurement of constitutional rights. It highlights similar occurrences of decolonization practices meeting resistance in the 21st century all while strengthening the human right argument that no human deserves any less than their fellow human brothers and sisters. It argues that an indigenous imagery can only be viewed through an indigenous lens where the researches’ level of purity is retained and unfiltered. It nevertheless argues that Marshallese ethnolinguistics reveal the same cultural practices in America, Judeo-Christianity, and Oceania thus dictating the reality that “we are the same not withstanding one stays here and one there (Bedbedjin Bedbedjen, Bedbedjinma wot Kwe)”. It further explains the importance in these similarities and how Marshallese spirituality predates introduced American Judeo-Christianity despite the latter attempting to marginalize the former. It concludes by stating that Marshallese contributions on the global stage are rooted in that culture of love (IaKwe) which is echoed by the custom(s) revealing the significance of Marshallese validation academically, spiritually, economically, & socially to prevent institutionalized discrimination. This paper ends stating that the agency to know one’s self and how one should fit in the world, is a human right in itself and Marshallese are entitled to this sense of self worth through knowing thy self by thy self where real thinking takes place in one’s own mind as we all live our own lives

    A Real-Life Turkish Experience of Ruxolitinib in Polycythemia Vera

    Introduction:Ruxolitinib is a small -molecule inhibitor of the JAK1/2 pathway. This study aimed to reveal the results and side-effect profile of the use of ruxolitinib as a treatment option in polycythemia vera (PV).Methods:A total of 34 patients with PV from 18 different centers were included in the study. The evaluation of the response under treatment with ruxolitinib was determined as a reduction in spleen volume (splenomegaly size: ≥35%) by imaging and control of hematocrit levels (≤45%) compared to baseline.Results:While the number of patients in which a reduction in spleen volume and hematocrit control was achieved was 19 (55.9%) at 3 months of treatment, it was 21 (61.8%) at 6 months. Additionally, while the number of side effects was negatively correlated with the reduction in spleen volume (Spearman’s rho: -0.365, p=0.034), a decrease in the hematocrit level was positively correlated (Spearman’s rho: 0.75, p=0.029). Those without a reduction in spleen volume experienced more constipation (chi-square: 5.988, Fisher’s exact test: p=0.033).Conclusion:This study shed light on the use of ruxolitinib in PV and the importance of splenomegaly on studies planned with larger patient groups

    Comparison of Intraabdominal and Trocar Site Local Anaesthetic Infiltration on Postoperative Analgesia After Laparoscopic Cholecystectomy

    OBJECTIVE: This study aimed to compare the efficacy of local anaesthetic infiltration to trocar wounds and intraperitoneally on postoperative pain as a part of a multimodal analgesia method after laparoscopic cholecystectomies. METHODS: The study was performed on 90 ASA I–III patients aged between 20 and 70 years who underwent elective laparoscopic cholecystectomy. All patients had the same general anaesthesia drug regimen. Patients were randomized into three groups by a closed envelope method: group I (n=30), trocar site local anaesthetic infiltration (20 mL of 0.5% bupivacaine); group II (n=30), intraperitoneal local anaesthetic instillation (20 mL of 0.5%) and group III (n=30), saline infiltration both trocar sites and intraperitoneally. Postoperative i.v. patient controlled analgesia was initiated for 24 h. In total, 4 mg of i.v. ondansetron was administered to all patients. Visual analogue scale (VAS), nausea and vomiting and shoulder pain were evaluated at 1., 2., 4., 8., 12., 24. hours. An i.v. nonsteroidal anti-inflammatory drug (NSAID) (50 mg of dexketoprofen) as a rescue analgesic was given if the VAS was ≥5. RESULTS: There were no statistical significant differences between the clinical and demographic properties among the three groups (p≥0.005). During all periods, VAS in group I was significantly lower than that in groups II and III (p<0.001). Among the groups, although there was no significant difference in nausea and vomiting (p=0.058), there was a significant difference in shoulder pain. Group III (p<0.05) had more frequent shoulder pain than groups I and II. The total morphine consumption was higher in groups II and III (p<0.001 vs p<0.001) than in group I. The requirement for a rescue analgesic was significantly higher in group III (p<0.05). CONCLUSION: Trocar site local anaesthetic infiltration is more effective for postoperative analgesia, easier to apply and safer than other analgesia methods. Morphine consumption is lesser and side effects are fewer; therefore, this method can be used as a part of common practice

    Small-scale fisheries in the southern Black Sea: Which factors affect net profit?

    Small-scale fisheries (SSF) is a local and community-based activity that can be traced back to ancient times, and thus, closely related to the history of humankind. However, large-scale fisheries have grown tremendously, approaching an industrial sector in the last century, due to their socio-economic and political properties, including both national and international aspects. This progress towards industrial-scale fisheries led to the involvement of scientific research, first aiming to improve production efficiency, and then, to protect ecosystems as resources exploited for fisheries activity, by mitigating their adverse impacts. During this evolutionary progress, SSF was usually neglected because of their limited production ability, and thus minimal economic contribution, until the later phase when the protection of ecosystem resources gained sufficient importance. As a result of this, many countries lack data on SSF, undermining efforts for the creation of proper policies for this type of fisheries. The aim of this study was to evaluate the productivity and the effects of some demographic characteristics, boat structures, and some cost (input) items on the net profit of SSF in the Black Sea. The eligible sample for this study consisted of 5575 small-scale fishing boats in the Black Sea. The number of fishers to be surveyed was determined as 315 using the “Simple Random Sampling” method, based on operators of boats < 12 m, i.e., boats in the SSF. Questionnaires were conducted face-to-face with fishers. In this study, it was tested if six parameters were investigated to determine whether they had a significant effect on net profit in SSF. These parameters were: (1) engine power; (2) number of fishing days; (3) boat length; (4) consumption of fuel in fishing; (5) education level of fishers; and (6) overall professional experience of fishers. To do so, Simple Linear Regression Analysis was performed to determine the effect of the data considered as independent variables when the net profit was set as the dependent variable. Atlantic bonito, Sarda sarda (Bloch, 1793); whiting, Merlangius merlangus (Linnaeus, 1758); rapa whelk, Rapana venosa (Valenciennes, 1846); and turbot, Scophthalmus maximus (Linnaeus, 1758) were the most important commercial fish species for small scale fishing. When catch per boat in SSF was evaluated, Kırklareli province ranked first with 97 007 kg, with Atlantic bonito (44 778 kg) being the most common species caught. Samsun had the second-largest catch per boat with 91 761 kg. The total net profit of 303 boats was calculated as €1 794 938 and the mean net profit per boat was €5924. The highest per boat mean net profit (€25 909) was in Kırklareli. According to the results of the study, the number of days at the sea, boat length, engine power, and fuel cost had a significant effect on the net profit while education level and professional experience were not important in productivity. The economically-fragile SSF sector may need some kind of supporting subsidy. It would be beneficial to provide support to the majority of fishers active in the SSF in terms of complementary alternative employment opportunities in the regions where they are located

    Brentuximab vedotin and bendamustine: an effective salvage therapy for relapsed or refractory Hodgkin lymphoma patients

    The prognosis is poor for relapsed or refractory (R/R) classical Hodgkin Lymphoma (cHL) patients. The brentuximab vedotin (Bv) and bendamustine (B) combination has been used as a preferable salvage regimen in R/R cHL patient trials. We retrospectively evaluated response rates, toxicities, and the survival in R/R cHL patients treated with the BvB combination. In a multi-centre real-life study, 61 R/R HL patients received intravenous doses of 1.8 mg/kg Bv on the first day plus 90 mg/m2 B on the first and second days of a 21-day cycle as a second-line or beyond-salvage regimen. Patients’ median age at BvB initiation was 33 (range: 18–76 years). BvB was given as median third-line treatment for a median of four cycles (range: 2–11). The overall and complete response rates were 82% and 68.9%, respectively. After BvB initiation, the median follow-up was 14 months, and one- and two-year overall survival rates were 85% and 72%, respectively. Grade 3/4 toxicities included neutropenia (24.6%), lymphopenia (40%), thrombocytopenia (13%), anaemia (13%), infusion reactions (8.2%), neuropathy (6.5%), and others. The BvB combination could be given as salvage regimen aiming a bridge to autologous stem cell transplant (ASCT), in patients relapse after ASCT or to transplant-ineligible patients with manageable toxicity profiles
