    (1) Aims: To assess the incidence of inflammatory bowel disease (IBD) in Spain, to describe the main epidemiological and clinical characteristics at diagnosis and the evolution of the disease, and to explore the use of drug treatments. (2) Methods: Prospective, population-based nationwide registry. Adult patients diagnosed with IBD-Crohn's disease (CD), ulcerative colitis (UC) or IBD unclassified (IBD-U)-during 2017 in Spain were included and were followed-up for 1 year. (3) Results: We identified 3611 incident cases of IBD diagnosed during 2017 in 108 hospitals covering over 22 million inhabitants. The overall incidence (cases/100,000 person-years) was 16 for IBD, 7.5 for CD, 8 for UC, and 0.5 for IBD-U; 53% of patients were male and median age was 43 years (interquartile range = 31-56 years). During a median 12-month follow-up, 34% of patients were treated with systemic steroids, 25% with immunomodulators, 15% with biologics and 5.6% underwent surgery. The percentage of patients under these treatments was significantly higher in CD than UC and IBD-U. Use of systemic steroids and biologics was significantly higher in hospitals with high resources. In total, 28% of patients were hospitalized (35% CD and 22% UC patients, p < 0.01). (4) Conclusion: The incidence of IBD in Spain is rather high and similar to that reported in Northern Europe. IBD patients require substantial therapeutic resources, which are greater in CD and in hospitals with high resources, and much higher than previously reported. One third of patients are hospitalized in the first year after diagnosis and a relevant proportion undergo surgery

    Outcomes after intensive chemotherapy for secondary and myeloid-related changes acute myeloid leukemia patients aged 60 to 75 years old: a retrospective analysis from the PETHEMA registry

    © 2024 Ferrata Storti Foundation Published under a CC BY-NC license.Treatment options for patients with secondary acute myeloid leukemia (sAML) and AML with myeloid-related changes (AMLMRC) aged 60 to 75 years are scarce and unsuitable. A pivotal trial showed that CPX-351 improved complete remission with/without incomplete recovery (CR/CRi) and overall survival (OS) as compared with standard "3+7" regimens. We retrospectively analyze outcomes of 765 patients with sAML and AML-MRC aged 60 to 75 years treated with intensive chemotherapy, reported to the PETHEMA registry before CPX-351 became available. The CR/CRi rate was 48%, median OS was 7.6 months (95% confidence interval [CI]: 6.7-8.5) and event-free survival (EFS) 2.7 months (95% CI: 2-3.3), without differences between intensive chemotherapy regimens and AML type. Multivariate analyses identified age ≥70 years, Eastern Cooperative Oncology Group performance status ≥1 as independent adverse prognostic factors for CR/CRi and OS, while favorable/intermediate cytogenetic risk and NPM1 were favorable prognostic factors. Patients receiving allogeneic stem cell transplant (HSCT), autologous HSCT, and those who completed more consolidation cycles showed improved OS.     A facility and community-based assessment of scabies in rural Malawi.

    Background Scabies is a neglected tropical disease of the skin, causing severe itching, stigmatizing skin lesions and systemic complications. Since 2015, the DerMalawi project provide an integrated skin diseases clinics and Tele-dermatology care in Malawi. Clinic based data suggested a progressive increase in scabies cases observed. To better identify and treat individuals with scabies in the region, we shifted from a clinic-based model to a community based outreach programme. Methodology/principal findings From May 2015, DerMalawi project provide integrated skin diseases and Tele-dermatological care in the Nkhotakota and Salima health districts in Malawi. Demographic and clinical data of all patients personally attended are recorded. Due to a progressive increase in the number of cases of scabies the project shifted to a community-based outreach programme. For the community outreach activities, we conducted three visits between 2018 to 2019 and undertook screening in schools and villages of Alinafe Hospital catchment area. Treatment was offered for all the cases and school or household contacts. Scabies increased from 2.9% to 39.2% of all cases seen by the DerMalawi project at clinics between 2015 to 2018. During the community-based activities approximately 50% of the population was assessed in each of three visits. The prevalence of scabies was similar in the first two rounds, 15.4% (2392) at the first visit and 17.2% at the second visit. The prevalence of scabies appeared to be lower (2.4%) at the third visit. The prevalence of impetigo appeared unchanged and was 6.7% at the first visit and 5.2% at the final visit. Conclusions/significance Prevalence of scabies in our setting was very high suggesting that scabies is a major public health problem in parts of Malawi. Further work is required to more accurately assess the burden of disease and develop appropriate public health strategies for its control

    Presence of acute and chronic renal failure in patients with paroxysmal nocturnal hemoglobinuria: results of a retrospective analysis from the Spanish PNH Registry

    Paroxysmal nocturnal hemoglobinuria (PNH) is a rare, life-threatening blood disease. With the advent of eculizumab treatment, renal function has substantially improved, although no data from real-world clinical practice are available. An observational, retrospective, multicenter study was conducted in Spain on clinical data obtained from outpatient visits of patients with PNH (Spanish PNH Registry) who had experienced acute (ARF) or chronic (CRF) renal failure. Of the 128 patients registered (April 2014), 60 were diagnosed with classic PNH. Twenty-seven (45.0%) patients with a mean age of 48.5 (±16.2) years had renal failure, ARF or CRF, and were included in this study. Near half of the patients (n = 13; 48.1%) presented with ARF alone, 33.3% (n = 9) had CRF with episodes of ARF, while 18.5% (n = 5) were diagnosed with CRF alone. For patients with diagnosis of PNH and renal failure (n = 27), the median time to the first ARF episode was 6.5 (CI 95%; 2.2, 14.9) years, whereas the median to the diagnosis of CRF was 14.5 (CI 95%; 3.8, 19.2) years after the diagnosis of PNH. Patients with ARF (n = 22) were treated with eculizumab and did not experience new episodes of ARF, except for one patient with sepsis. Of the patients with CRF, two received treatment without experiencing further episodes of ARF. Sixteen patients who completed treatment (11 with ARF and 5 with ARF + CRF) recovered from the episode of ARF or from CRF. Of the remaining patients treated with eculizumab, one patient improved from stages III to II, three patients stabilized without showing disease progression, and one patient progressed from stages III to IV. Treatment with eculizumab in PNH patients has beneficial effects on renal function, preventing ARF and progression to CRF.This study was funded by Alexion Pharma Spain, SL (Barcelona, Spain).Peer reviewe

    Este proyecto pretende fomentar el interés de los alumnos por el aprendizaje y que adquieran los conceptos mínimos de las distintas materias que componen el currículo de ESO. A la vez, se trabajan las destrezas básicas y las técnicas de estudio a través de juegos. Cada grupo elabora un juego de mesa de preguntas y respuestas y diseñan ellos mismos el tablero y las reglas del juego. Estos juegos se intercambian entre los distintos grupos y se usan para repasar los contenidos de las asignaturas. Además se realiza un juego que consiste en tres pruebas individuales de conocimientos y dos pruebas colectivas de investigación que permiten a cada clase ir sumando puntos. La clase que queda primera gana una excursión. El proyecto ha recibido una gran acogida por parte del alumnado que se ha esforzado por trabajar en equipo, y también ha recibido una valoración muy positiva por parte del profesorado que considera que ha incidido positivamente en los resultados académicos de los alumnos. El proyecto adjunta tres CD-ROM con el juego de Historia del Arte de segundo de Bachillerato, Educación Física de primero de Bachillerato y de memoria visual; y un anexo con la lista de materiales elaborados y fotografías del desarrollo de la experiencia.Madrid (Comunidad Autónoma).     Clinical characteristics and outcome of SARS-CoV-2 infection in admitted patients with chronic lymphocytic leukemia from a single European country

    Spain has been one the most affected countries by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the causative agent of coronavirus disease 2019 (COVID-19) pandemic [1, 2]. Patients with chronic lymphocytic leukemia (CLL) could be at risk of more severe COVID-19 clinical forms [3] since they often carry immune perturbations aggravated by treatments used for the disease itself [4]. Two major series on patients with COVID-19 and CLL encompassing different countries and health systems reported heterogeneous factors related to the outcome [5, 6]. Herein, we are presenting the largest series of CLL patients with proved COVID-19 from a single country and Health system.Peer reviewe

    La importancia de conocer el idioma

    Este proyecto consiste en mejorar el uso cotidiano de la lengua castellana, abordándola desde el vocabulario, la ortografía y la compresión lectora en las distintas asignaturas. Para dar mayor globalidad al ejercicio, se trabaja también estos factores en otros idiomas como el inglés y el francés. Los objetivos son desarrollar un vocabulario adecuado a las distintas materias; escribir correctamente; trabajar la comprensión de los textos; aumentar la cohesión y la coordinación entre alumnos y profesores; fomentar la utilización de distintos métodos de enseñanza-aprendizaje; fomentar el interés con actividades motivadoras e innovadoras en la enseñanza curricular; y potenciar el trabajo en grupo e individual. Las actividades llevadas a la práctica son acordes a la edad y a los objetivos propuestos. Se elaboran listados de palabras de ortografía dudosa, se trabajan textos para acceder a su comprensión, se hacen concursos de ortografía, se cuelgan carteles por pasillos y aulas con palabras significativas, se fabrican tarjetas para jugar-aprendiendo en clases y tutorías, y se utilizan programas de ordenador que refuerzan el aprendizaje de vocabulario y ortografía. La metodología pone en activo tanto a profesores como a alumnos. Los primeros se encargan de hacer los listados de palabras y de preparar y dar formato a las pruebas eliminatorias del concurso. Los alumnos se encargan de buscar la información necesaria para definir las palabras, hacer los carteles y exponer las conclusiones. La evaluación consiste principalmente en reuniones de profesores que analizan el desarrollo del proyecto, exponen las dificultades encontradas y establecen modificaciones y correcciones en relación a los ejercicios.
