Final Report of the AFIT Quality Initiative External Discovery Committee
This report summarizes the findings of the Air Force Institute of Technology’s (AFIT’s) Quality Initiative - External Discovery Team. The overarching purpose of the Quality Initiative is to create a detailed, executable investment strategy for modernizing AFIT’s instructional capabilities across five thrust areas. These activities were completed over the course of one year, beginning in June of 2016 and concluding in June of 2017. The data gathered were evaluated and several recommendations for further review were decided upon by the External Discovery Team. The following report briefly covers those recommendations and provides sources from which the recommendations were gleaned. These recommendations are meant to serve as a baseline for ways in which AFIT could begin to program resources to help improve teaching and instruction across the institution as a whole. The data presented here are meant to serve as a compliment to the Internal Discovery Team’s report that focuses on data and feedback gathered from institutions internal to AFIT

The High Flux Isotope Reactor (HFIR) cold source project at ORNL
Following the decision to cancel the Advanced Neutron Source (ANS) Project at Oak Ridge National Laboratory (ORNL), it was determined that a hydrogen cold source should be retrofitted into an existing beam tube of the High Flux Isotope Reactor (HFIR) at ORNL> The preliminary design of this system has been completed and an approval in principal of the design has been obtained from the internal ORNL safety review committees and the US Department of Energy (DOE) safety review committee. The cold source concept is basically a closed loop forced flow supercritical hydrogen system. The supercritical approach was chosen because of its enhanced stability in the proposed high heat flux regions. Neutron and gamma physics of the moderator have been analyzed using the 3D Monte Carlo code MCNP. A 3D structural analysis model of the moderator vessel, vacuum tube, and beam tube was completed to evaluate stress loadings and to examine the impact of hydrogen detonations in the beam tube. A detailed ATHENA system model of the hydrogen system has been developed to simulate loop performance under normal and off-normal transient conditions. Semi-prototypic hydrogen loop tests of the system have been performed at the Arnold Engineering Design Center (AEDC) located in Tullahoma, Tennessee to verify the design and benchmark the analytical system model. A 3.5 kW refrigerator system has been ordered and is expected to be delivered to ORNL by the end of this calendar year. The present schedule shows the assembling of the cold source loop on side during the fall of 1999 for final testing before insertion of the moderator plug assembly into the reactor beam tube during the end of the year 2000

Corrosion monitoring in the UF{sub 6} cylinder yards at the Oak Ridge K-25 Site: FY 1994 report
Depleted uranium hexafluoride (UF{sub 6}) at the U.S. Department of Energy`s K-25 Site at Oak Ridge, Tennessee, has been stored in large steel cylinders that have undergone significant atmospheric corrosion damage over the last 35 years. A detailed experimental program to characterize the corrosion damage was initiated in 1992. Large amounts of corrosion scale and deep pits are found to cover UF{sub 6} cylinder surfaces. Ultrasonic wall thickness measurements have shown uniform corrosion losses up to 20 mils (0.5 mm) and pits up to 100 mils (2.5 mm) deep. Electrical resistance corrosion probes, TOW sensors, and thermocouples have been attached to cylinder bodies. Atmospheric conditions are monitored using rain gauges, relative humidity sensors, and thermocouples. Long-term (16-year) data are being obtained from mild steel corrosion coupons on test racks as well as attached directly to cylinder surfaces. Corrosion rates have been found to be intimately related to the times-of-wetness, both tending to be higher on cylinder tops due to apparent sheltering effects. Data from the various tests are compared, discrepancies are discussed, and a pattern of cylinder corrosion as a function of cylinder position and location is described

Achieving increased spent fuel storage capacity at the High Flux Isotope Reactor (HFIR)
The HFIR facility was originally designed to store approximately 25 spent cores, sufficient to allow for operational contingencies and for cooling prior to off-site shipment for reprocessing. The original capacity has now been increased to 60 positions, of which 53 are currently filled (September 1994). Additional spent cores are produced at a rate of about 10 or 11 per year. Continued HFIR operation, therefore, depends on a significant near-term expansion of the pool storage capacity, as well as on a future capability of reprocessing or other storage alternatives once the practical capacity of the pool is reached. To store the much larger inventory of spent fuel that may remain on-site under various future scenarios, the pool capacity is being increased in a phased manner through installation of a new multi-tier spent fuel rack design for higher density storage. A total of 143 positions was used for this paper as the maximum practical pool capacity without impacting operations; however, greater ultimate capacities were addressed in the supporting analyses and approval documents. This paper addresses issues related to the pool storage expansion including (1) seismic effects on the three-tier storage arrays, (2) thermal performance of the new arrays, (3) spent fuel cladding corrosion concerns related to the longer period of pool storage, and (4) impacts of increased spent fuel inventory on the pool water quality, water treatment systems, and LLLW volume
Psychopathology associated with Alzheimer's disease and related disorders
This study compared the occurrence of psychiatric symptoms in a large group of community-residing participants enrolled in an Alzheimer's disease patient registry who met clinical diagnostic criteria for Alzheimer's disease or multi-infarct dementia, as well as mixed Alzheimer's disease and multi-infarct dementia.
Psychiatric morbidity observed from a psychiatric examination of 514 patients with Alzheimer's disease, 135 patients with multi-infarct dementia, and 86 cases with mixed dementia was analyzed using analysis of variance comparing symptoms across groups and chi-square test for differences in frequency of occurrence. Associations between pairs of symptoms were also reported.
The frequency of occurrence and patterns of psychiatric problems were similar in patients with Alzheimer's disease and multi-infarct dementia. Agitation was the most frequent symptom, followed by depression, apathy, and behavioral disorders. Patients with mixed dementia had significantly more psychopathology.
This study demonstrated that significant psychopathology occurs in individuals with Alzheimer's disease or multi-infarct dementia. The high levels of symptoms in mixed dementia suggest that the two conditions have a synergistic effect on behavioral problems. The prevalence of multiple symptoms is higher than previously reported
Severe family violence and Alzheimer's disease: prevalence and risk factors
This study describes the prevalence of violence and the putative risk factors for violence in 184 Alzheimer patients and their primary caregivers living in the community. Analysis of the severe violence subscale of the Conflict Tactics Scale indicated that 15.8% of patients had been violent in the year since diagnosis. A total of 5.4% of caregivers reported being violent toward the patient. The overall prevalence of violence was 17.4%. The variables most associated with violence were caregiver depression and living arrangement

Are there clinical differences between familial and nonfamilial Alzheimer's disease?
The purpose of the study was to determine whether there are differences in clinical characteristics in two groups of patients with Alzheimer's disease, those reported to have a family history of dementia and those without a family history of dementia.
Using a data set from an Alzheimer's disease patient registry, funded as part of a National Institute on Aging cooperative agreement, the authors made comparisons of sociodemographic and clinical variables in a group of 462 patients with Alzheimer's disease, 172 reported to have at least one first-degree relative with dementia and 290 classified with no family history.
Patients with a presumptive family history differed from those without a family history in two ways: the course of dementia was described as having a fast rather than a slow progression from onset of symptoms to diagnosis, and caregivers reported a higher prevalence of family history of psychiatric disorders. There were no significant differences in age at onset, duration, female gender, aphasia and apraxia, handedness, family history of Down's syndrome, or number of children, brothers, and sisters.
The association of faster course and family history of psychiatric disorders in the patients with a family history of dementia is consistent with the hypothesis of heterogeneity, but the overall results could also be explained by a genetic-environmental model of Alzheimer's disease
