JAK-2 inhibitors and allogeneic transplant in myelofibrosis

7The activation of the JAK1/JAK2 pathway plays a crucial role in the pathogenesis of myelofibrosis. Treatment with the JAK2 inhibitor ruxolitinib demonstrated to reduce splenomegaly and symptoms in patients affected by myelofibrosis, leading to a significant improvement of overall survival in comparison with the supportive therapies. Taking in account this recent therapeutic progress, it is necessary to redefine the role of the allogeneic hematopoietic stem cell transplantation, which has been considered the only curative option for fit myelofibrosis patients up to now. In the era of JAK2 inhibitors, allogeneic transplant is still indicated in patients with intermediate-2 and high-risk myelofibrosis or red blood cell transfusion dependent patients or patients with unfavourable karyotype. There is no direct evidence to recommend which conditioning regimen should be preferentially adopted. Graft failure, relapse and transplant related mortality are still current issues of the allogeneic stem cell transplantation, particularly from unrelated donors. Ruxolitinib can be efficaciously included in the platform of allogeneic transplant. In fact, ruxolitinib treatment for 3-4 months before transplant has demonstrated to reduce spleen and improve performance status in about 30-50% of patients, without impairing the outcome of the subsequent transplant. Ruxolitinib has to stopped the day before conditioning to avoid rebound phenomenon. Urban growth drivers in a Europe of sticky people and implicit boundaries

We investigate urban GDP pc growth across the EU12 using data for functionally defined cities - rather than administrative regions. We test hypotheses on the role of human capital, EU integration and fragmentation of urban government and explore spatial dependence and mechanisms of spatial interaction. Results are acceptable on standard econometric tests without measures of spatial interaction but there is spatial dependence. If variables reflecting spatial adjustment are included, they are statistically significant and eliminate spatial dependence. Not only do the results now provide consistent estimates of parameters, they also support relevant theoretical insights and show national borders are still significant barriers to economic adjustment. People in Europe are sticky so it is unreasonable to assume spatial disparities will disappear. Our findings also imply that cities in Europe form national rather than a single continental system

Is mass loss along the red giant branch of globular clusters sharply peaked? The case of M3

There is a growing evidence that several globular clusters must contain multiple stellar generations, differing in helium content. This hypothesis has helped to interpret peculiar unexplained features in their horizontal branches. In this framework we model the peaked distribution of the RR Lyr periods in M3, that has defied explanation until now. At the same time, we try to reproduce the colour distribution of M3 horizontal branch stars. We find that only a very small dispersion in mass loss along the red giant branch reproduces with good accuracy the observational data. The enhanced and variable helium content among cluster stars is at the origin of the extension in colour of the horizontal branch, while the sharply peaked mass loss is necessary to reproduce the sharply peaked period distribution of RR Lyr variables. The dispersion in mass loss has to be <~ 0.003 Msun, to be compared with the usually assumed values of ~0.02 Msun. This requirement represents a substantial change in the interpretation of the physical mechanisms regulating the evolution of globular cluster stars.Comment: Accepted for publication in The Astrophysical Journa

Our result generalizes the classical characterization of reflexive modules with respect to Morita dualities. The linear compactness notion considered, permits us to obtain finiteness properties of the rings and modules involved
