    A desire for weight loss in season increases disordered eating behaviour risk and energy deficiency in athletes

    Objective: The objective was to explore eating behaviour, body image and energy status in female university team sport athletes.Design: This was a cross-sectional, descriptive study design.Setting: The setting was North-West University, Potchefstroom, South Africa.Subjects: Subjects were top university female field hockey and netball player volunteers, aged 18-30 years (n = 22), and recruited during their sport season.Outcome measures: Athletes completed demographic, health and sport, and body weight questionnaires. The Eating Disorder Inventory and the Three-Factor Eating Questionnaire’s cognitive dietary restraint subscale were used to measure disordered eating behaviour. Body composition was measured with dual-energy X-ray absorptiometry. Estimated energy availability (estEA) was determined from a three-day diet and exercise record.Results: Fifteen (68%) athletes were identified with an increased risk of disordered eating. Sixteen (73%) were dieting. A low estEA (24 ± 12 kcal/kg fat-free mass/day) was found in 59% of the athletes, of whom 85% (11/13) had increased risk of disordered eating. A significantly positive (p-value < 0.05) association was found between cognitive dietary restraint and drive for thinness (r = 0.4) and body weight (r = 0.5). A negative association was found between desired weight loss (r = −0.5), energy intake (r = −0.5) and estEA (r = −0.7).Conclusion: Nonlean build athletes who diet in season are at increased risk of disordered eating behaviour and low energy availability. Keywords: disordered eating, body image, nonlean-build sports, female students, energy statu

    The response of sap flow to pulses of rain in a temperate Australian woodland

    In water-limited systems, pulses of rainfall can trigger a cascade of plant physiological responses. However, the timing and size of the physiological response can vary depending on plant and environmental characteristics, such as rooting depth, plant size, rainfall amount, or antecedent soil moisture. We investigated the influence of pulses of rainfall on the response of sap flow of two dominant evergreen tree species, Eucalyptus crebra (a broadleaf) and Callitris glaucophylla (a needle leaved tree), in a remnant open woodland in eastern Australia. Sap flow data were collected using heat-pulse sensors installed in six trees of each species over a 2 year period which encompassed the tail-end of a widespread drought. Our objectives were to estimate the magnitude that a rainfall pulse had to exceed to increase tree water use (i.e., define the threshold response), and to determine how tree and environmental factors influenced the increase in tree water use following a rainfall pulse. We used data filtering techniques to isolate rainfall pulses, and analysed the resulting data with multivariate statistical analysis. We found that rainfall pulses less than 20 mm did not significantly increase tree water use (P>0.05). Using partial regression analysis to hold all other variables constant, we determined that the size of the rain event (P<0.05, R 2=0.59), antecedent soil moisture (P<0.05, R 2=0.29), and tree size (DBH, cm, P<0.05, R 2=0.15), all significantly affected the response to rainfall. Our results suggest that the conceptual Threshold-Delay model describing physiological responses to rainfall pulses could be modified to include these factors. We further conclude that modelling of stand water use over an annual cycle could be improved by incorporating the T-D behaviour of tree transpiration. © 2007 Springer Science+Business Media B.V

    High levels of genetic structure and striking phenotypic variability in a sexually dimorphic suckermouth catfish from the African Highveld

Uncovering biological diversity to more accurately understand diversity patterns, and ultimately the processes driving diversification, is important not only from an evolutionary perspective but also a conservation perspective. This is particularly pertinent in Africa's rivers in which overall diversity, as well as how it arose, is poorly understood in comparison with lacustrine environments. Here we investigate population divergence in the sexually dimorphic suckermouth catfish species Chiloglanis anoterus (Crass, 1960) from the African Highveld, in which we observe striking variability in exaggerated male caudal fins across its range. As this trait is likely to be indirect evidence for sexual selection by female choice, a mechanism that has been shown to increase species diversity in different taxa, we used an integrated approach to test if current diversity in this species is underestimated. Results based on phylogenetic inference, population genetics and geometric morphometrics indicate that the recognized species C. anoterus represents five distinct lineages that may be considered confirmed candidate species. We suggest that diversification in these highland catfish has been facilitated through geographical isolation in upper river catchments, and that sexual selection through female choice has probably driven variation in male caudal fin morphology. In contrast to the relatively large range size of the currently recognized species (C. anoterus), our findings highlight highly restricted ranges of the lineages identified here, indicating that these highland habitats may harbour higher levels of endemic diversity than previously thought

    Get screened: a pragmatic randomized controlled trial to increase mammography and colorectal cancer screening in a large, safety net practice

    Abstract Background Most randomized controlled trials of interventions designed to promote cancer screening, particularly those targeting poor and minority patients, enroll selected patients. Relatively little is known about the benefits of these interventions among unselected patients. Methods/Design "Get Screened" is an American Cancer Society-sponsored randomized controlled trial designed to promote mammography and colorectal cancer screening in a primary care practice serving low-income patients. Eligible patients who are past due for mammography or colorectal cancer screening are entered into a tracking registry and randomly assigned to early or delayed intervention. This 6-month intervention is multimodal, involving patient prompts, clinician prompts, and outreach. At the time of the patient visit, eligible patients receive a low-literacy patient education tool. At the same time, clinicians receive a prompt to remind them to order the test and, when appropriate, a tool designed to simplify colorectal cancer screening decision-making. Patient outreach consists of personalized letters, automated telephone reminders, assistance with scheduling, and linkage of uninsured patients to the local National Breast and Cervical Cancer Early Detection program. Interventions are repeated for patients who fail to respond to early interventions. We will compare rates of screening between randomized groups, as well as planned secondary analyses of minority patients and uninsured patients. Data from the pilot phase show that this multimodal intervention triples rates of cancer screening (adjusted odds ratio 3.63; 95% CI 2.35 - 5.61). Discussion This study protocol is designed to assess a multimodal approach to promotion of breast and colorectal cancer screening among underserved patients. We hypothesize that a multimodal approach will significantly improve cancer screening rates.     Reporting quality of published reviews of commercial and publicly available mobile health apps (mHealth app reviews): A scoping review protocol

Published by BMJ.Introduction Reviews of commercial and publicly available smartphone (mobile) health applications (mHealth app reviews) are being undertaken and published. However, there is variation in the conduct and reporting of mHealth app reviews, with no existing reporting guidelines. Building on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we aim to develop the Consensus for APP Review Reporting Items (CAPPRRI) guidance, to support the conduct and reporting of mHealth app reviews. This scoping review of published mHealth app reviews will explore their alignment, deviation, and modification to the PRISMA 2020 items for systematic reviews and identify a list of possible items to include in CAPPRRI. Method and analysis We are following the Joanna Briggs Institute approach and Arksey and O\u27Malley\u27s five-step process. Patient and public contributors, mHealth app review, digital health research and evidence synthesis experts, healthcare professionals and a specialist librarian gave feedback on the methods. We will search SCOPUS, CINAHL Plus, AMED, EMBASE, Medline, APA PsycINFO and the ACM Digital Library for articles reporting mHealth app reviews and use a two-step screening process to identify eligible articles. Information on whether the authors have reported, or how they have modified the PRISMA 2020 items in their reporting, will be extracted. Data extraction will also include the article characteristics, protocol and registration information, review question frameworks used, information about the search and screening process, how apps have been evaluated and evidence of stakeholder engagement. This will be analysed using a content synthesis approach and presented using descriptive statistics and summaries. This protocol is registered on OSF (https://osf.io/5ahjx). Ethics and dissemination Ethical approval is not required. The findings will be disseminated through peer-reviewed journal publications (shared on our project website and on the EQUATOR Network website where the CAPPRRI guidance has been registered as under development), conference presentations and blog and social media posts in lay language

    Modern health data practices come with many practical uncertainties. In this paper, I argue that data subjects’ trust in the institutions and organizations that control their data, and their ability to know their own moral obligations in relation to their data, are undermined by significant uncertainties regarding the what, how, and who of mass data collection and analysis. I conclude by considering how proposals for managing situations of high uncertainty might be applied to this problem. These emphasize increasing organizational flexibility, knowledge, and capacity, and reducing hazard

    The challenges of communicating research evidence in practice: perspectives from UK health visitors and practice nurses

    &lt;p&gt;Background: Health practitioners play a pivotal role in providing patients with up-to-date evidence and health information. Evidence-based practice and patient-centred care are transforming the delivery of healthcare in the UK. Health practitioners are increasingly balancing the need to provide evidence-based information against that of facilitating patient choice, which may not always concur with the evidence base. There is limited research exploring how health practitioners working in the UK, and particularly those more autonomous practitioners such as health visitors and practice nurses working in community practice settings, negotiate this challenge. This research provides a descriptive account of how health visitors and practice nurses negotiate the challenges of communicating health information and research evidence in practice.&lt;/p&gt; &lt;p&gt;Methods: A total of eighteen in-depth telephone interviews were conducted in the UK between September 2008 and May 2009. The participants comprised nine health visitors and nine practice nurses, recruited via adverts on a nursing website, posters at a practitioner conference and through recommendation. Thematic analysis, with a focus on constant comparative method, was used to analyse the data.&lt;/p&gt; &lt;p&gt;Results: The data were grouped into three main themes: communicating evidence to the critically-minded patient; confidence in communicating evidence; and maintaining the integrity of the patient-practitioner relationship. These findings highlight some of the daily challenges that health visitors and practice nurses face with regard to the complex and dynamic nature of evidence and the changing attitudes and expectations of patients. The findings also highlight the tensions that exist between differing philosophies of evidence-based practice and patient-centred care, which can make communicating about evidence a daunting task.&lt;/p&gt; &lt;p&gt;Conclusions: If health practitioners are to be effective at communicating research evidence, we suggest that more research and resources need to be focused on contextual factors, such as how research evidence is negotiated, appraised and communicated within the dynamic patient-practitioner relationship.&lt;/p&gt

    This article uses framing theory to examine how activists and trade unions have framed labour’s political agenda in Malaysia. A polity grounded in ethnicity continues to hinder the formation of cross-ethnic collective worker identities and labour politics. However, inclusive popular democratising movements have strengthened in recent years, providing a favourable context for greater emphasis on non-ethnic political action by trade unions. The latter have shifted in this direction, adopting elements of the popular movement’s ‘human rights’ internationalism. Thus, the democratic movement’s frame has influenced that of the trade unions, with implications for framing theory
